A Study on the Concordance Rate of the Diagnostic Results Among Sasang Constitutional Medicine(SCM) Specialists (사상의학(四象醫學) 전문의(專門醫)의 체질진단(體質診斷)결과 일치도에 관한 연구)

  • 1. Objectives The purpose of this study was to investigate the concordance rate of the diagnostic results among Sasang Constitutional Medicine(SCM) specialists. 2. Methods We carried out two studies in order to investigate the concordance rate of the diagnostic results between SCM specialists. We calculated the kappa value for analyzing concordance rate(using the SPSS program). 1) Study A : We investigated 142 clients who had taken health examinations and diagnosis of Sasang constitution from March 9, 2005 to March 29, 2005 at a health examination center of Kyung-Hee medical center. Every SCM specialist diagnosed 142 clients directly. Each SCM specialist don’t know the other specialist’s diagnosis. 2) Study B : We investigated 176 clients who had taken health examinations and diagnosis of Sasang constitution from January 2005 to March 2005 at the department of Occupational and Environmental medicine of Kyung-Hee medical center. One SCM specialist diagnosed 176 clients directly and the other specialist knowing previous specialist’s diagnosis, diagnosed each person indirectly 3. Results 1) The concordance rate of the diagnostic results among SCM specialists in study A was relatively high(kappa value=0.700). 2) The concordance rate of the diagnostic results among SCM specialists in study B was high(kappa value=0.895). 4. Conclusions We can confirm the concordance rate of the diagnostic results among SCM specialists is relatively high.

  • Objectives This research was proposed to present Clinical Practice Guideline(CPG) for Soeumin Disease of Sasang Constitutional Medicine(SCM): Greater Yin Symptomatology. This CPG was developed by the national-wide experts committee consisting of SCM professors. Methods First, it was performed that search and collection of literature related SCM such as "Dongeuisusebowon", Textbook of SCM, Clinical Guidebook of SCM and Fundamental research to standardize diagnosis of Sasang Constitutional Medicine. And journal search related clinical trial or Human complementary medicine of SCM was performed domestic and overseas. Finally, 1 article was selected and included in CPG for Greater Yin Symptomatology of Stomach Cold-based Interior Cold disease in Soeumin disease. Results & Conclusions CPG of Greater Yin symptomatology in Soeumin Disease include classification, definition and standard symptoms of each pattern. Greater Yin symptomatology is classified into mild and moderate pattern by severity. Greater Yin Symptomatology Mild pattern is classified into Greater Yin Symptomatology accompanied abdominal pain and bowel irritability and Greater Yin pattern accompanied Epigastric stuffiness and fullness. And Greater Yin Symptomatology moderate pattern is classified into Greater Yin pattern accompanied Jaundice, Greater Yin pattern accompanied Edema and Greater Yin pattern by Yin toxin.

Congestive Hyperpsychotic symptomatology severe pattern is classified into intermediate and advanced pattern and Greater Yang disease Reverting Yin pattern.

  • 1. Objectives This research was proposed to find out the chronical concept of etiology in Sasang Constitutional Medicine(SCM). 2. Methods The related contents of etiology were selected in Je-Ma Lee's literatures such as "Donguisusebowon-SaSangchobongyun(東醫壽世保元四象草本卷)"(DSS), "Donguisusebowon-GabObon(東醫壽世保元甲午本)"(DGO), " Donguisusebowon-ShinChukbon(東醫壽世保元辛丑本)"(DSC), and the research was written in order ro find out the chronical change of etiologic concept in SCM. 3. Results and Conclusions The chronical concept of etiology in SCM was as follows : There were comments that the cause of diseases was the imbalanced nature and emotion(性情偏急), the fault of human relationship(人事), and alcohol-sex-wealth-power(酒色財權) in DSS, and the imbalanced nature and emotion, exopathogen(外感), and endopathogen(內觸) in DGO In the last, the causes of disease was set to the imbalanced nature and emotion in DSC.

The Preventive Thoughts of Sasang Constitutional Medicine (사상의학의 예방의학사상)

  • Objectives On the point of modern preventive medicine, Sasang Constitutional Meidicine(SCM) has the thoughts of preventive medicine. Modern preventive medicine has the three dimensional prevention; primary, secondary and tertiary prevention. The preventive thoughts of SCM concerning three dimensional prevention have been investigated in this article. Method Materials and references were collected about the prevention of SCM. Lee Jema's books such as Donguisusebowon, Gyeokchigo and Donguisusebowon Chobonguen were included. Result and Conclusions As the primary prevention of SCM, Regimen according to ages, explanation according to health index as Myeongmaeksilsu(命脈實數), moderation of four ruinous causes such as intoxication, seduction, opulence and authority, adequate social life and control of 4 kinds of emotions(sorrow, anger, joy and pleasure) are included. As the secondary prevention of SCM, explanation according to disease index, fragile Organs and frequently acquired diseases according to constitutions are included. As the tertiary prevention of SCM, rehabilitation for the stroke patients according to constitutions is included. In conclusion, SCM has all of 3 dimensional prevention methods although that might be applied with modern considerate modification.

Yang Depletion symptomatology is classified into severe and critical pattern by severity. Yang Depletion Symptomatology severe pattern is classified into initial phase pattern and intermediate phase pattern. And Yang Depletion Symptomatology critical pattern is classified into advanced phase pattern.

Results & Conclusions The CPG of Yin-Depletion (Mangeum) Symptomatology in Soyangin Disease include classification, definition and standard symptoms of each pattern. Yin-Depletion (Mangeum) Symptomatology is classified into severe and critical pattern by severity. The severe pattern of Yin-Depletion (Mangeum) Symptomatology is classified into the pattern of Heat-related diarrhea accompanied by headache (Sinyeol-dutong Mangeum) and the advanced pattern of Heat-related diarrhea accompanied by headache (Sinyeol-dutong Mangeum). The critical pattern of Yin-Depletion (Mangeum) Symptomatology is classified into the pattern of Cold-related diarrhea accompanied by the abdominal pain (Sinhan-bocktong Mangeum) and the advanced pattern of Cold-related diarrhea accompanied by abdominal pain (Sinhan-bocktong Mangeum).

A Study on Clinical Status for Development of Clinical Practice Guidelines for Sasang Constitutional Medicine Symptomatology (사상체질병증 임상진료지침 개발을 위한 임상 현황 조사연구)

  • Objectives We surveyed clinical status of Sasang Constitutional Medicine (SCM) for preparation of adaptation of Clinical Practice Guidelines for Sasang Constitutional Symptomatology. Methods We emailed Google survey form to Doctors of Korean Medicine registered the Association of Korean Medicine on 10/13/2020 and 10/21/2020 and closed survey 10/23/2020. We got a data of 654 cases, removed a case of error, analyzed data of 620 responses by frequency analysis Results and Conclusions Out of 620 respondents, 499 have answered that they refer to or make use of SCM. SCM is mainly applied in the field of treatments using Herbal Medicine. Doctors of Korean Medicine participated in survey of SCM, they think Diseases of digestive system are the most effectively treated by the methods of SCM. Sasang Constitutional Symptomatology most frequently observed are Soeumin Congestive Hyperpsychotic symptomatology, Soyangin Chest-Heat congested Symptomatology, Taeeumin Liver Heat-based Interior Heat disease. The diseases that are mainly applied by SCM are 'Diseases of digestive system' for Soeumin, 'Diseases of digestive system' for Soyangin, 'Diseases of respiratory system' for Taeeumin and 'not applicable' for Taeyangin. The important diagnostic methods of SCM are Inspection-listening and smelling examination-Inquiry-Palpation and survey. In the majority of cases, Prescription of Herbal medicine is used by adding or subtracting some herbal medicine from the original prescription suggested in the oriental medical classics and Saam acupuncture method is the most frequently used for the acupuncture therapy. Diet therapy is the most frequently used for the method of prevention and care.

Esophagus Cold-based Exterior Cold (Wiwansuhan-pyohan) disease consists of two aspects : Esophagus-Cold (Wiwanhan) and Esophagus-Cold Lung-Dry (Wiwanhan-paejo) symptomatology. Esophagus-Cold (Wiwanhan) symptomatology is classified into mild and moderate pattern by severity. Mild pattern of Esophagus-Cold (Wiwanhan) symptomatology is classified into Supraspinal Exterior (Baechu-pyo) initial and Wheezing-Dyspnea (Hyocheon) pattern. Moderate pattern of Esophagus-Cold (Wiwanhan) symptomatology is classified into Cold-reversal (Hanguel) and Cold-reversal (Hanguel) advanced pattern. And Esophagus-Cold Lung-Dry (Wiwanhan-paejo) symptomatology is classified into severe and critical pattern by severity. Severe pattern of Esophagus-Cold Lung-Dry (Wiwanhan-paejo) is classified into Dry-Cold (Johan) pattern and Dry-Cold (Johan) advanced pattern. Critical pattern of Esophagus-Cold Lung-Dry (Wiwanhan-paejo) symptomatology consists of Dry-Cold (Johan) intense pattern (Eumhyeol-mogal handa pattern).

And Chest-Heat congested (Hyunggyeok-yeol) moderate pattern is classified into Clear Yang Failure of Stomach(Weguck-cheongyang Bulsagnseung) pattern (Upper wasting-thirst(Sangso) pattern), Clear Yang Failure of Large Intestine (Daejang-cheongyang Bulsangseung) pattern (Middle wasting-thirst (Jungso) pattern).
