Clinical Practice Guideline for Taeeumin Disease of Sasang Constitutional Medicine: Esophagus Cold-based Exterior Cold (Wiwansuhan-pyohan) disease (태음인체질병증 임상진료지침: 표병)

  • Objectives This research was proposed to present Clinical Practice Guideline(CPG) for Taeeumin Disease of Sasang Constitutional Medicine(SCM): Esophagus Cold-based Exterior Cold (Wiwansuhan-pyohan) disease. This CPG was developed by the national-wide experts committee consisting of SCM professors. Methods First, collection and organization of literature related to SCM such as Donguisusebowon, Text book of SCM, Clinical Guidebook of SCM and Fundamental research to standardize diagnosis of Sasang Constitutional Medicine was performed. Secondly, journals related to clinical trial or Human complementary medicine of SCM were searched. Finally, 7 articles were selected and included in CPG for Esophagus Cold-based Exterior Cold (Wiwansuhan-pyohan) disease. Results & Conclusions The CPG of Esophagus Cold-based Exterior Cold (Wiwansuhan-pyohan) disease in Taeeumin Disease include classification, definition and standard symptoms of each pattern. Esophagus Cold-based Exterior Cold (Wiwansuhan-pyohan) disease consists of two aspects : Esophagus-Cold (Wiwanhan) and Esophagus-Cold Lung-Dry (Wiwanhan-paejo) symptomatology. Esophagus-Cold (Wiwanhan) symptomatology is classified into mild and moderate pattern by severity. Mild pattern of Esophagus-Cold (Wiwanhan) symptomatology is classified into Supraspinal Exterior (Baechu-pyo) initial and Wheezing-Dyspnea (Hyocheon) pattern. Moderate pattern of Esophagus-Cold (Wiwanhan) symptomatology is classified into Cold-reversal (Hanguel) and Cold-reversal (Hanguel) advanced pattern. And Esophagus-Cold Lung-Dry (Wiwanhan-paejo) symptomatology is classified into severe and critical pattern by severity. Severe pattern of Esophagus-Cold Lung-Dry (Wiwanhan-paejo) is classified into Dry-Cold (Johan) pattern and Dry-Cold (Johan) advanced pattern. Critical pattern of Esophagus-Cold Lung-Dry (Wiwanhan-paejo) symptomatology consists of Dry-Cold (Johan) intense pattern (Eumhyeol-mogal handa pattern).

Chest-Heat(Hyunggyeok-yeol) symptomatology Mild pattern is classified into Chest-Heat congested(Hyunggyeok-yeol) initial pattern and Chest-Heat congested(Hyunggyeok-yeol) advanced pattern. And Chest-Heat congested (Hyunggyeok-yeol) moderate pattern is classified into Clear Yang Failure of Stomach(Weguck-cheongyang Bulsagnseung) pattern (Upper wasting-thirst(Sangso) pattern), Clear Yang Failure of Large Intestine (Daejang-cheongyang Bulsangseung) pattern (Middle wasting-thirst (Jungso) pattern).

Results & Conclusions The CPG of Lessor-Yang Wind-Injury (Soyang-sangpung) Symptomatology in Soyangin Disease include classification, definition and standard symptoms of each pattern. Lessor-Yang Wind-Injury (Soyang-sangpung) Symptomatology is classified into mild and moderate pattern by severity. Lessor-Yang Wind-Injury (Soyang-sangpung) Symptomatology mild pattern is classified into initial pattern and advanced pattern. Lesser-Yang Wind-Injury (Soyang-sangpung) symptomatology moderate pattern is classified into Chest-binding (Gyeolhyoong) pattern and Chest-binding (Gyeolhyoong) advanced pattern.

Overview on the Sasang Constitutional Abdominal Examination based on Experienced Prescriptions in the Donguisusebowon (동의수세보원(東醫壽世保元) 경험방에 근거한 사상체질별 복진의 문헌연구)

  • Researches of Sasang Consitutional Medicine(SCM) have been conducted in many ways, especially for objective diagnosis methods of SCM. Despite of these efforts, relationship between SCM and abdominal examination is rarely known. Therefore 7 books related to abdominal examination and arranged specific abdominal examination descriptions in each books based on Experienced prescriptions(經驗方) of Donguisusebowon(東醫壽世保元) or Longevity and Life Preservation in Eastern Medicine were collected. In result, So-eumin and So-yangin show opposite descriptions to each others in abdominal examination. So-eumin has stuffiness and rigidity below heart, lower abdominal fullness and cramp. In contrast, So-yangin has fullness in the chest and hypochondrium, chest bind, lower abdominal numbness and lower tension. Strong abdominal surface tension could be palpated in Taeum-in's abdominal examination. Advantages of Abdominal Examination are notified recently, thanks to easiness of performing, objectiveness and well-adaptation to Theory of Traditional Eastern Medicine. These advantages could contribute to researching diagnosis of SCM with theory consensus of SCM specialists and practical trials.

  • Objectives This study was aimed to develop the clinical practice guidelines, which focuses on Sasang Constitutional Medicine symptomology. By developing clinical practice guidelines, we should present prevention, diagnosis, treatment and assessment of Sasang Constitutional Medicine. Methods We used the method of University of Michigan Health System Guidelines model. Electronic databases including English, Korean databases were systematically searched for clinical articles investigating Sasang Constitutional Medicine symptomology up to March 2010. And "Donguisusebowon-ShinChukbon" was basically used to develop clinical guidelines. Results and Conclusions 12,694 Articles were reviewed. Only 25 articles described adequate methods of this study. Using clinical practice guidelines, we can manage th quality of medicine, emphasis treatment appropriacy and educate patients and doctors. Finally We can get the Sasang Constitutional Medicine standardization and socioeconomic benefit.

Study on the Sasang Type Diagnosis Using Objective Biopsychological Measures (객관적인 생리심리 지표를 사용한 사상체질 진단 연구)

  • Introduction The previous study showed that the psychological Sasang Personality Questionnaire (SPQ) and physical Body Mass Index (BMI) were clinically useful for predicting Sasang types diagnosed with certified clinical specialist and QSCCII. The purpose of current study was to examine its clinical usefulness using large sized clinical sample. Methods The age, sex, height, weight, circumference of waist and hip, Sasang Personality Questionnaire (SPQ), and Sasang type of 2,049 participants listed on the Korean Medicine Data Center were acquired. The anthropometrics of BMI, Ponderal Index (PI), Waist-Hip Ratio (WHR), and Basal Metabolic Rate (BMR) were calculated. The discriminant analysis using SPQ subscales, BMI, age and sex were performed to predict Sasang types. And, the ANCOVA with age and sex as covariate was also used to examine differences among diagnosed and predicted Sasang type groups in biopsychological features. Results The discriminant analysis showed 82.0% of percentage correctly predicted. And, the biopsychological characteristics of predicted Sasang type groups were significantly similar to those of diagnosed Sasang type groups. In both predicted and diagnosed Sasang type groups, the anthropometric measures were in increasing order of So-Eum, So-Yang, and Tae-Eum types, and the psychological measures were in increasing order of So-Eum, Tae-Eum, and So-Yang types as shown in previous studies. Discussion and Conclusion The well validated biopsychological variables of SPQ and BMI showed theoretical and clinical usefulness, and were found to be useful for diagnosing Sasang types in clinical setting. The current study might be useful for the East-West integrative medicine and evidence-based teaching.

Clinical Study on the Characteristics of Back Skin of the Hand According to Sasang Constitutions (사상체질별 손등 피부 특성에 대한 임상 연구)

  • The methodology of constitution diagnosis is main concern in sasang constitutional medicine, but highly reliable method is not yet established. So we statistically analyzed the relationship between the constitution and characteristic factors of skin such as thickness, stiffness, roughness, and touch-feeling, using 1079 clinical data registered to SCIB(Sasang constitution Information Bank), and the following results are obtained: The skin of Taeeumin are more thick or more stiff or more rough than other constitutions. The skin of Soeumin are more soft or more feeble or more feeble-refined than other constitutions. The skin of Soyangin are more slippery or more thin or more slippery-refined than other constitutions. The differences written above were proved to be meaningful enough by Chi-square test.

A Study on the Children's Characteristics by Sasang Constitution Analysed with PSSC-2004 (사상체질음성분석기를 이용한 사상체질별 소아 특성 연구)

  • Objective : The purpose of this study was to distribute children by Sasang constitution through voice analysis, and to investigate the difference of physical and physiological characteristics in constitutions. Material and Methods : The subjects were 380 children aged between 5 and 6 who had been attending kindergartens. In April 2006, children were examined to make a diagnosis of Sasangin by PSSC-2004. Their parents responded the questionnaire on the children's characterist as eating habit, digestion, defecation, urination and perspiration. Results : Sasang constitution composed of 55.0% Soeumin, 1.8% Soyangin and 43.2% were Taeumin, and there were no significant physical and physiological differences in those groups. Conclusion : In this study, we tried to figure out some physical and physiological characteristics differences in Sasang constitution by using PSSC-2004. Statistically, however, there were no remarkable differences and we needed more specific studies.

  • This paper proposes Sasang constitution classification through speech signals analysis values and comparison. For this, this paper wishes to propose Taeumin classification method of output values signals that comes out speech signal analysis to connect with process classification of Soeumin through skin diagnosis by first step in the whole system configuration to provide for objective index of Sasang constitution. First of all, these characteristic of voices wish to extract phonetic elements that each Sasang constitution groups' clear features. Also, we wish to classify Taeumin through constitution groups' difference and similarity on the basis of results value. Finally, the effectiveness of this method is verified through the experiments.
