A Study on the Side Effects of Using Herbal Medicine Assigned to a Different Constitutional Type (타약수해례(他藥受害例) 연구)

  • 1. Purpose : Sasang constitutional herb medicine is known as specific to his own's constitution. So Other constitutional herb medicine can induce side effects. This study investigates the cause and side effects of other constitutional herb medicine. 2. Methods : Literature study was carried out with the chapters including harm to Other constitutions in the books "Dongyisasangjinryoeuijeon(東醫四象診療醫典)" and "Joeuihak(朝醫學)". And the causes of side effects were studied in terms of herbology and movements of Qi in Sasang constitutional medicine. 3. Results : Herb medicines of Taeeumin's have the dispersing Qi energy, so Soeumin could have side effects mostly and some of Soyangin could do. Herb medicines of Soeumin's have the warming Qi energy, so Soyangin could have side effects like dermatitis or fever. Herb medicine of Soyangin's have cooling Qi energy, so Soeumin could have side effects of digestive system like diarrhea and some of Taeeumin could do. 4. Conclusion : It is thought that herb medicine is safe and less harmful when herb medicine is prescribed and taken according to his own constitution.

The Effects of Medical Nutrition Therapy on Plasma Lipid Levels of Apo E3 genotype hyperlipidemic Patients according to Sasang Constitutions (APo E3 Genotype 고지혈증 환자에서 사상체질에 따른 의학영양치료 혈중 지질 농도에 미치는 영향)

  • The purpose of this study was to investigate the effects of medical nutrition therapy(MNT) on plasma lipid levels of hyperlipidemic patients with apo E3 genotype according to Sasang Constitutions. From March to July, 2001, the 33 hyperlipidemic patients admitted to K University Medical Center were studied. The study subjects were classified according to their Sasang constitutions by QSCC II questionnaire which have been used at K University Oriental Medical Center. The anthropometric assessments, blood analysis, and apo E genetic typing were carried out. Nutrient intake was determined by food record method of food taken during two weekdays and one weekend. The MNT including the instruction for hypolipidemic and hypocholesterolemic diet (step l diet) was performed for 12 weeks. The results are as follow; (1) The mean age of hyperlipidemic patients was $49.91{\pm}8.48$ years. (2) The distributions of Sasang Constitution were 60.6% of Tae-eum, 21.2% of So-yang, and 18.2% of So-eum. The distributions of apo E genotype were 6.5% of apo E2/3, 78.8% of apo E 3/3, and 15.2% of apo E 3/4. (3) The nutrient consumption of the apo E3 subjects before the MNT showed lower calorie, iron, calcium, and vitamin B2 intakes than the RDAs for each nutrients with no significant differences among the constitutions. After 12-week of MNT, only the fat consumption was decreased in the Tae-eum group. The MNT did not change the pattern of food intake. (4) The plasma level of triglyceride, total cholesterol, and LDL-C were not changed after MNT in the three constitutional groups. The level of HDL-C was significantly increased significantly in Tae-eum and So-yang group and the level of homocystein was lowered in Tae-eum group after MNT. It could be concluded that the 12-weeks MNT with hypolipidemic and hypocholesterolemic diet did not change the level of total cholesterol, triglyceride, and LDL-C effectively regardless of Sasang constitutions even though the subjects' dietary intake was improved by MNT.

  • Although the judgment of constitutions is given according to the Sasang Typology in domestic korean medicine hospitals and universities, the unified constitution clinical information has not been accumulated. Therefore, in this paper, based on collection items where oriental doctors and experts have already agreed, we construct the input system which correctly and effectively collects the clinical information of persons of which constitutions are distinguished explicitly. In the future, the input system could be utilized to collect clinical information continuously and will help to search and analyze the constitution clinical information.

  • Childhood obesity leads to adult obesity in case of failure of obesity treatment in childhood. It has been reported recently that the various risk factors that cause childhood obesity may lead to diabetes and arteriosclerotic cardiovascular disease in adulthood, thus the treatment of childhood obesity is more important. Sasang Typology advocated by the Korean traditional medical doctor in the late Joseon Dynasty, Je-Ma Lee (李濟馬), separated into four types of human constitution. It is known that Tae-Eum type (太陰人) is likely to be obese compared with other constitutions. This study examined the effects that influence on indicators related obesity and metabolic diseases through conducting aquatic application program at Tae-Eum type children who are severely obese (BMI > 30) during 12 weeks. Moreover, this study investigated the herbal prescriptions for obesity of Tae-Eum type (太陰人).

A Study on the Reliability and Validity test of the QSCC II + (Revised Questionnaire for the Sasang Constitution Classification) (개정(改定)된 사상체질분류검사지(四象體質分類檢査紙)II의 신뢰도(信賴度)와 타당도(妥當度)에 대한 연구(硏究))

  • 1. Objectives This is a statistical study on the Reliability and Validity of the Revised Questionnaire for the Sasang Constitution classification(QSCC II +). 2. Methods To analyze the reliability; we gathered the results of the QSCC II + of 266 people. Each person took the questionnaire three times, and the interval of the examinations were about one year. With the results, we used stratified analysis to check up the consistency rate of three examinations, and analyzed the tendency of agreement by crosstabs to get the kappa value. For the validity, we gathered the results of the QSCC II + of 587 people who were Taeumin, Soyangin, Soeumin. They took the questionnaire once, and SCM specialists strictly examined to diagnose their Sasang constitutions. We compare their results of the QSCC II + to the diagnostic result of their Sasang constitutions, and analyzed by crosstabs to get the rate of accuracy of the QSCC II +, and the kappa value for the measure of agreements. 3. Results and Conclusions The expected rate to get the same results was 54.5% in Soyangin, 68.1% in Taeumin, 65.7% in Soeumin, and kappa value was $0.311{\sim}0.414$, which means the reliability of the QSCC II + is analytically significant. The accuracy of the results of QSCC II +, compared to the diagnosis of the SCM specialists, was 58.4%, but increased up to 60.8%, and validity also became moderately good(K=0.412) by age limitation. Because in the 70's results of the QSCC II + isn't likely to agree to the diagnosis of the SCM specialists.

Angiotensin Converting Enzyme (ACE) Gene Polymorphism and Sasang Constitution in Patients with Cerebral Infarction (뇌경색 환자의 안지오텐신 전환요소 유전자 다형성과 사상체질)

  • The correlation between angiotensin converting enzyme (ACE) polymorphisms and cerebral infarction (CI) has been controversial. Such controversy may be due to different classifications of cerebrovascular diseases and ethnic differences. I studied the correlation between ACE genotypes and CI patients by case-control study in the Korean population. I also classified CI patients and control group into four types according to Sasang constitutional medicine. Furthermore I investigated the correlation among ACE genotypes, CI and Sasang constitutions. The frequencies of D allele were 0.32 in subjects with CI and 0.40 in the control group without CI (X2=0.128, p=0.720). In patients with CI, the frequency of Taeumins, one of four Sasang constitutional types, was significantly higher than that in controls (X2=15.425, p<0.00l). I did not find any correlation between ACE polymorphism and CI in Koreans. However, there were significant differences in allele frequencies between Koreans and Europeans, while similarities were shown to those of Japanese and Chinese populations.

Study of Premenstration Syndrome of Female College Students According to Sasang Constitution (사상체질에 따른 여대생의 월경전 증후군 양상에 관한 연구)

  • Objectives The purpose of this study was to investigate the premenstrual symptoms according to Sasang constitution and to utilize for understanding the characteristics of Sasang constitution. Methods Thirty one female university students participated in this study and Sasang Constitution and patterns of premenstrual symptoms was determined by Questionnaire for Sasang Constitution Classification (QSCC) II and premenstrual symptoms screening tool, respectively. For statistical analysis, chi-square test, analysis of variance and correlation test has been used. Results There existed significant differences in menstrual symptoms between Sasang constitutions. Taeumin had higher body mass index in severe PMS(Premenstrual Symptoms) group than normal/mild PMS group, and Soyang in and Soeumin are vice versa. Taeumin showed depressed mood and hopelessness, decreased interest in school activities, Soyangin showed decreased interest in home activities, and Soeumin showed decreased interest in social activities. In terms of interfered functional activities, Soyangin had increased feeling overwhelmed or out of control and Taeumin and Soeumin had increased hypersomnia. Conclusion Menstrual symptoms are different among Sasang constitutional types based on PSST(Premenstrual Symptoms Screening Tool). These results may serve as the basis in identifying and treating premenstrual syndromes related to Sasang constitution.

Gene-gene Interaction in Cerebral Infarction Patients : A Study on Relationship Between Apolipoprotein E, ACE Gene Polymorphism and Sasang Constitution

  • Sasang Constitutional Medicine is a major branch of Korean Traditional Medicine. The differences of disease susceptibility to be shown in Sasang constitution may be due to genetic factors. Therefore, I examined interrelationship among cerebral infarction (CI), apolipoprotein E (apo E) gene polymorphism, and Sasang constitutional classification. Apo E is a key protein modulating the highly atherogenic apoB containing lipoproteins and is a candidate gene for the development of coronary artery disease (CAD). The ε2 and/or ε4 alleles were the first to be implicated in premature CAD, which resulted in this polymorphism being extensively studied. I investigated the association between apo E genotype and CI by case-control study in a Korean population. I also classified CI patients and control group into groups according to Sasang Constitutional Medicine. 218 CI patients and 379 controls without CI were examined. Apo E genotype was determined by 8% polyacrylamide gel separation after DNA amplification. A frequency of apo E ε3/ε3 in the apo E genotype distribution was higher in the CI patients compared with that in controls. Also, it was widely known that Taeumin was easily attacked with CI, but there was no association between apo E polymorphim and Taeumin. However, the Taeumin constitution did not enhance the relative risk for CI in the subjects with apo E ε2 and/or ε4 alleles. No differences in the apo E genotypes frequencies were observed in the Taeumin compared with that in the other constitutions. In addition, I investigated whether the DD(deletion/deletion) or ID(insertion/deletion) genotype of angiotensin converting enzyme (ACE) gene, a candidate gene for CI, was associated with CI, Taeumin constitution, and apo E polymorphism. As a result, the frequency of Taeumin constitution was significantly higher in CI patients with both apo E ε3/ε4 and ACE ID/DD genotypes than in the remaining Sasang constitutions. In summary, it was concluded that the apo E polymorphism is a major risk factor for CI in Koreans and the ACE ID/DD genotype enhanced the relative risk for CI in the subjects with apo E ε3/ε4 genotype and Taeumin constitution.

Study on the Characteristics of Ordinary Symptoms in Overweight and Obesity Patients according to Sasang Constitution (사상체질에 따른 과체중 및 비만 환자의 소증(素證) 특성 비교)

  • Objectives: This study was aimed to find out the characteristics of ordinary symptoms between the control and patient groups, diagnosed as overweight or obesity, in Soyang, Taeeum, and Soeum Constitutions and among those 3 groups. Methods: As a prospective cross-sectional study based on medical records, 9213 patients, who visited Kyung Hee University Medical Center from May in 2007 to June in 2010, were included. To diagnose the constitution, Revised Questionnaire for the Sasang Constitution Classification II and examinations by oriental medical doctors who majored in Sasang Constitution Medicine were performed. Based on the World Health Organization criteria for Asian obesity assessment, body mass index (BMI) was classified into 3 groups; more than 25 as obesity, 23 to 25 as overweight, and less than 23 as normal. Ordinary symptoms questionnaire, which consists of 46 questions in 8 categories, were given to the subjects. One-way ANOVA test in continuous variables and chi-square test in categorical variables were carried out to analyze statistical significance. Results and Conclusions: 1) In general characteristics, we could find out the significant differences in age, sex, height, weight, BMI, waist circumference, blood pressure, total cholesterol, triglyceride, low density lipoprotein cholesterol, high density lipoprotein cholesterol, fasting blood sugar, and hemoglobin A1c among the 3 Constitutional groups. 2) Soyang Constitution of overweight and obesity has tendency to have higher appetite, like cool or cold water, defecate more, swear more, dislike heat, and have warmer or hotter extremities in the ordinary symptoms, compared with the control group. 3) Taeeum Constitution of overweight and obesity has tendency to have higher appetite, like cool or cold water, defecate more, swear more, dislike heat, and have warmer or hotter extremities in the ordinary symptoms, compared with the control group. 4) Soeum Constitution of overweight and obesity has tendency to have higher appetite and less fatigue in the ordinary symptoms, compared with the control group. 5) Soeum Constitution showed the slightest increase in appetite, preference about room temperature water, less number of feces, and feeling of chill and warmer extremities in ordinary symptoms, compared with Soyang and Taeeum Constitutions. Taeeum Constitution showed the distinct increase in appetite and sweating in ordinary symptoms, compared with Soyang and Soeum Constitutions.

Study for correlation between MMPI results and Sasang constitutions, in out-patients of Oriental Neuropsychiatry (내원 환자의 MMPI 군집분석과 사상체질과의 상관성 연구)

  • Objective : For investigating correlation among diagnostic types, MMPI results and Sasang constitutions, in out-patients of Oriental Neuropsychiatry. Method: We got the following result by comparing the out-patients' MMPI and QSCCII from 2000/12/1 to 2001/12/30 at Sanbon Hospital of Oriental Medicine, Wonkwang University. Result and Conclusion: 1. A specific character of population for study's objects was that female (128, 68.4%) was twice bigger that the male(59, 31.6%). And their education levels were 52 of college graduate (27.8%), 40 of high school graduate (21.4%), and 14 of middle school graduate (14 %). A distribution of the occupation, there are 62 of labors (33.2%), 62 of white collars (33.2%), 30 of sales (16%), 16 of students (8.6%). Their marrige statuses were 144 of married (77%), 31 of single (16.6%), 4 of divorced (2.1%), and 1 of bereavement (0.5%). 2. In the physical constitutions there were 70 of Soeumin(37.4%), 56 of Soyangin(29.9%) 55 of Taeumin (29.4%), 6 of others (3.2%). A differentiation with their diagnosis, there were 55 of palpitations (29.4%), 45 of depression (24.1%), 18 of women's diseases (9.6%), 14 of insomnia (7.5%), 13 of headache (7%), 8 of pain (4.3%), 8 of hwabyung (4.3%), 7 of psychosis (3.7%), 4 of dizziness (2.1%), and 15 of others(8%). 3. A result of groupling MMPI, we could classify into 4 different groups. Group 1 is a profile of trasition in a neurosis. Group 2 is a proflie of a mental disease. Group 3 is a proflie of $'{\wedge}'$ type neurosis. And group 4 has normal profile. 4. Regarding each group with physical constitution and a distribution of diagnosis, Group 1 is found out 36.4 % of Soeumin, 34.10 % of palpitations. And group 2 showed 43.6% of Soyangin, 25.6% of depression. Group 3 showed 46.4% of Soyangin, 31.90% of palpitations. Group 4 showed the most in 34.3% of Soyangin, and 31.40% of palpitations 5. A result of the study using Chi-square test is that there is no correlation between the 4 different groups and the Sasang physical constitution.
