구속스트레스에 의한 백서 악하선의 Amylase 분비 변화 (Change of the Amylase Secretion on the Rat Submandibular Gland in the Restraint Stress Condition)

  • 현대인들에게는 다양한 증상과 징후를 나타내는 스트레스성 질환이 증가되고 있는 추세에 있다. 이러한 스트레스 시에 구강 내에는 구강건조이나 구강작열감증후군, 구강점막질환, 구취 등이 호발 된다는 것은 이미 널리 알려진 사실이다. 이때 대부분의 환자에서 공통적으로 나타나는 현상은 타액선의 기능적 및 구조적 변화에 따른 타액성분의 변화이다. 이에 저자는 스트레스가 구강 내 타액선에 어떠한 영향을 미치며, 타액선으로부터 분비되는 ${\alpha}$-amylase에 어떠한 영향을 미치는지를 알아보고자 본 실험을 시행한 결과 다음과 같은 유의성 있는 결과를 얻었다. 1. 구속 스트레스에 의하여 타액선 조직은 선포세포가 공포화되며 변성되었고, 선포간극이 이개되었으며, 도관주위조직도 변성되었다. 2. 구속 3시간 후에는 선포세포가 위축되기 시작하였고, 구속 6시간 후에는 선포 간 이개현상이 관찰되었으며, 구속 72시간 후에는 도관 주위 조직도 변성되기 시작하였고, 점차 심화되어 구속 168시간 후에는 선포와 도관 주위조직이 모두 심하게 변성되었다. 3. 면역조직화학적 검사에서 구속 3시간 후에는 amylase의 발현이 부분적으로 불규칙하게 관찰되었으며, 구속 6시간 후에는 다소 감소되었고, 구속 12시간 후에는 amylase가 부분적으로 응집되었으며, 점차 감소되고 응집되어, 구속 48시간 후에는 전 타액선에 amylase의 응집현상이 심화되었고, 구속 168시간 후에는 amylase의 발현이 매우 감소되었다. 이상의 결과를 바탕으로 정서적인 스트레스를 받으면 타액선이 변성될 수 있으며, 타액선으로부터 분비되는 중요한 소화 효소인 amylase가 변화되어 소화 장애를 일으키거나 구강건강과 관계없이 구취발생에도 영향을 미칠 수 있다고 예측할 수 있어, 향후 amylase의 분비변화에 따른 지속적인 연구가 필요할 것으로 생각된다.

Design and Implementation of Salivary Electrical Stimulator for xerostomia

  • After 40 years of age, the saliva glands are aged and the saliva is not made enough to cause xerostomia symptoms. Side effects such as hypertension medication or diuretics that the elderly take mainly can cause xerostomia syndrome. In addition, autoimmune diseases, diabetes, anemia, depression and other common diseases that cause xerostomia symptoms. If the saliva secretion is insufficient, tooth decay and gum disease are likely to occur, and the digestive ability of the saliva is also reduced due to the lack of amylase, which is a digestive element. Once the degenerated salivary gland is restored to its normal state, it is difficult to recover. In this paper, we give electrical stimulation to the masseter which is in contact with the large pituitary gland, and stimulate the salivary gland to the utmost by using speech recognition using words corresponding to oral gymnastics. Use the STM32F407VG to implement a system to relieve xerostomia.

타액선 신티그라피를 이용한 정상 타액선기능의 정량분석 (Quantitative Analysis of Normal Salivary Gland Function using Salivary Scintigraphy)

  • This study was to investigate normal salivary flow rates and normal indices of Quantitative analysis of salivary scintigraphy. 96 adult volunteers were studied by Questionnaire evaluating salivary conditions and clinical examinations. 35(male 23, female 12, age range 23-31years) that absented subjective and objective symptoms related saliva were classified as normal group. The normal group underwent measurement unstimulated and stimulated salivary flow rates and salivary scintigraphy. The obtained results were as follows: 1. There were not significant in sex differences of unstimulated and stimulated salivary flow rates. The unstimulated salivary flow rate was $0.66{\pm}0.41g/min$, stimulated salivary flow rates was $1.61{\pm}0.69g/min$. 2. As comparing of parameters of salivary scintigraphy, the Uptake ratio(UR), $T_{max}$, $T_{min}$, Maximum accumulation (MA), Maximum secretion(MS) of parotid and submandibular glands were not significant in sex and side-ralated differences. 3. The UR, $T_{max}$, MA, MS of parotid gland were significantly higher than those of submandibular gland; in the parotid gland, UR, $3.67{\pm}0.88$, $T_{max}$, $18.77{\pm}0.43min$, MA, $41.35{\pm}9.22%$, MS, $43.13{\pm}9.13%$; in the submandibular gland, UR, $3.04{\pm}0.10$, $T_{max}$, $18.48{\pm}0.52min$, MA, $36.47{\pm}14.18%$, MS, $36.88{\pm}12.20%$. 4. As classifying of time-activity curve, the most of parotid gland was N-type(97.1%), submandibular gland was observed in order of M-type(67.1%), N-type(21.4%), F-type(11.4%), however, was not observed S-type. 5. As the type of time-activity curve of submandibular gland was more flattened, the UR, $T_{max}$, MA, MS were significantly decresed.

Homer2 regulates amylase secretion via physiological calcium oscillations in mouse parotid gland acinar cells

  • The salivary glands secrete saliva, which plays a role in the maintenance of a healthy oral environment. Under physiological conditions, saliva secretion within the acinar cells of the gland is regulated by stimulation of specific calcium (Ca2+) signaling mechanisms such as increases in the intracellular Ca2+ concentration ([Ca2+]i) via storeoperated Ca2+ entry, which involves components such as Orai1, transient receptor potential (TRP) canonical 1, stromal interaction molecules, and inositol 1,4,5-triphosphate (IP3) receptors (IP3Rs). Homer proteins are scaffold proteins that bind to G protein-coupled receptors, IP3Rs, ryanodine receptors, and TRP channels. However, their exact role in Ca2+ signaling in the salivary glands remains unknown. In the present study, we investigated the role of Homer2 in Ca2+ signaling and saliva secretion in parotid gland acinar cells under physiological conditions. Deletion of Homer2 (Homer2-/-) markedly decreased the amplitude of [Ca2+]i oscillations via the stimulation of carbachol, which is physiologically concentrated in parotid acinar cells, whereas the frequency of [Ca2+]i oscillations showed no difference between wild-type and Homer2-/- mice. Homer2-/- mice also showed a significant decrease in amylase release by carbachol in the parotid gland in a dose-dependent manner. These results suggest that Homer2 plays a critical role in maintaining [Ca2+]i concentration and secretion of saliva in mouse parotid gland acinar cells.

동적 타액선 신티그라피에서 기능적 매개변수 지표를 활용한 정량 분석의 유용성 (Usefulness of the quantitative analysis by using the functional parameter index from dynamic salivary glands scintigraphy)

  • 동적 타액선 신티그라피는 비침습적인 방법으로 타액선의 기능을 평가하는데 널리 사용되어 왔다. 본 연구에서는 타액선 신티그라피의 시간-방사능 곡선에서 다양한 기능적 매개변수지표를 측정하고 구강건조증 환자 그룹과 건강한 Control 그룹과 비교를 통해 유용성을 알아보고자 한다. 2014년 12월부터 2015년 2월까지 구강 건조증으로 내원하여 동적 타액선 신티그라피를 시행한 환자 그룹 30명과 건강한 Control 그룹 10명을 대상으로 하였다. 검사방법은 370 MBq의 $^{99m}Tc$-과산화테크네슘을 정맥으로 순간 주사 후 약 30분간 동적 타액선 신티그라피를 연속 촬영하였으며, 주사 후 20분째에 최타 자극제로 비타민C 파우더를 이용하였다. 정량분석은 이하선과 악하선의 시간-방사능 곡선을 통해 기능적 매개변수 지표를 획득하고, 통계 프로그램을 이용하여 구강 건조증 환자와 건강한 Control 그룹과 비교하였다. 동적 타액선 신티그라피에서 이하선과 악하선의 기능적 매개변수 지표인 섭취율(Uptake Ratio, UR), 최대 방사능 시간(Time at Maximum counts, Tmax), 최소 방사능 시간(Time at Minimum counts, Tmin), 최대 축적율(Maximum Accumulation, MA), 축적속도(Accumulation Velocity), 최대 배출율(Maximum Secretion, MS), 최대 자극 배출율(Maximum Stimulation Secretion, MSS), 배출속도(Secretion Velocity, SV)를 각각 측정하였다. 구강 건조증 환자와 건강한 Control 그룹간의 이하선과 악하선의 기능적 매개변수 지표 비교에서는 최소 방사능 시간을 제외한 모든 지표에서 건강한 Control 그룹이 환자 그룹에 비하여 유의성 있게 높게 측정되었으며, 시간에 따른 변화량도 Control 그룹이 환자 그룹에 비하여 유의성 있게 변화량이 크게 측정 되었다. 동적 타액선 신티그라피에서 다양한 기능적 매개변수 지표를 이용한 정량분석은 구강 건조증 환자의 타액선 기능을 평가하는데 객관적이고 표준화된 정보를 제공해 줄 것으로 사료된다.

Comparative Morphological Study on Parotid and Submandibular Salivary Glands in Ovariectomized Rats

  • Background: Estrogen deficiency affects the structure and function of the salivary glands in women, leading to a decrease in salivary secretion and a change in the composition of saliva. Previous studies on changes in the salivary glands that cause estrogen deficiency have reported only partial results for the parotid and submandibular glands, and there are few comparative morphological studies of histological changes between the parotid and submandibular glands in ovariectomized rats (OVX) leading to estrogen deficiency. This study aimed to analyze the histopathological and histochemical changes in the parotid and submandibular salivary glands causing estrogen deficiency by using OVX, and to discuss the mechanism on these changes. Methods: The parotid and submandibular glands from sacrificed control and OVX groups were fixed with cold 4% paraformaldehyde in phosphate buffer (pH 7.2). The tissues were dehydrated using a series of graded ethyl alcohol and embedded in paraffin. For histopathological analysis, sections cut to a thickness of 6 to 7 ㎛ were stained with hematoxylin and eosin (H&E). For histochemical analysis, Periodic acid-Schiff (PAS), Alcian blue (AB, pH 2.5), and PAS+AB (pH 2.5 and pH 1) staining was performed. Results: Histopathological analysis of OVX tissue showed that the parotid and submandibular salivary glands were broadly and clearly separated and divided into lobes. In OVX, acinar and ductal cells with condensed polymorphic or pyknotic nucleus, which are presumed to be characteristic of apoptotic cells, and degenerated cells with lipid deposition in cytoplasmic granules and ruptured membranes were increased. Histochemical analysis of OVX, confirmed an increase in the number and acidification of acinar secretory granules. Conclusion: Histopathological and histochemical changes and the effects of estrogen deficiency are more evident in the submandibular salivary gland than in the parotid gland.

Why Should We Consider Potential Roles of Oral Bacteria in the Pathogenesis of Sjögren Syndrome?

  • Sjögren syndrome (SS) is a chronic autoimmune disorder that primarily targets the salivary and lacrimal glands. The pathology of these exocrine glands is characterized by periductal focal lymphocytic infiltrates, and both T cell-mediated tissue injury and autoantibodies that interfere with the secretion process underlie glandular hypofunction. In addition to these adaptive mechanisms, multiple innate immune pathways are dysregulated, particularly in the salivary gland epithelium. Our understanding of the pathogenetic mechanisms of SS has substantially improved during the past decade. In contrast to viral infection, bacterial infection has never been considered in the pathogenesis of SS. In this review, oral dysbiosis associated with SS and evidence for bacterial infection of the salivary glands in SS were reviewed. In addition, the potential contributions of bacterial infection to innate activation of ductal epithelial cells, plasmacytoid dendritic cells, and B cells and to the breach of tolerance via bystander activation of autoreactive T cells and molecular mimicry were discussed. The added roles of bacteria may extend our understanding of the pathogenetic mechanisms and therapeutic approaches for this autoimmune exocrinopathy.

Treatment of intractable parotid sialocele occurred after open reduction-fixation of mandibular subcondylar fracture

  • A sialocele is a subcutaneous cavity containing saliva, most often caused by facial trauma or iatrogenic complications. In subcondylar fractures, most surgeons are conscious of facial nerve injury; however, they usually pay little attention to the parotid duct injury. We report the case of a 41-year-old man with a sialocele, approximately $5{\times}3cm$ in size, which developed 1 week after subcondylar fracture reduction. The sialocele became progressively enlarged despite conservative management. Computed tomography showed a thin-walled cyst between the body and tail of the parotid gland. Fluid leakage outside the cyst was noted where the skin was thin. Sialography showed a cutting edge of the inferior interlobular major duct before forming the common major duct that seemed to be injured during the subcondylar fracture reduction process. We decided on prompt surgical treatment, and the sialocele was completely excised. A duct from the parotid tail, secreting salivary secretion into the cyst, was ligated. Botulinum toxin was administrated to block the salivary secretion and preventing recurrence. Treatment was successful. In addition, we found that parotid major ducts are enveloped by the deep lobe and extensive dissection during the subcondylar fracture reduction may cause parotid major duct injury.

전이성 선암종을 포함한 경부하마종(Plunging ranula)의 치험례 (A CASE REPORT OF PLUNGING RANULA WITH METASTATIC ADENOCARCINOMA)

  • The plunging ranula is a kind of ranula that goes over the mouth floor to the neck and other adjacent tissue. Sublingual gland is gently accepted as origin of plunging ranula. Plunging ranula develops commonly because of rupture of sublingual gland duct by trauma and extravasation of salivary secretion to the adjacent tissue. It is not true cyst so that there is no epithelium. And it consisted with thin connective tissue, inflammation cell infiltration and salivary secretion. Left without treatment, it can grow into the 10 cm more huge lesion. This report is a case of 73 years old female who was diagnosed as plunging ranula with review of literature. She presented 5 cm submandibular swelling at first. When surgery was delayed because of patient's condition, the lesion grew into the l2cm huge size. We performed excision of sublingual gland, submandibular gland and plunging ranula and had a good result without recurrence.

  • This research aims to study the changes In gastrointestinal function attributed to aging In human. The thresholds for recognition and detection of flavors became elevated and salivary gland acinar cells decreased in the old age. But most esophageal function remained relatively Intact. Although gastric emptying time has been slowed with aging, the total intestinal transit time did not differ. Atropic gastritis due to H. pylori in old man decreased secretion of acid and Intrinsic factor and absorbability of calcium and iron. Pancreatic secretion is droned in older persons. Prevalence of gallstones rised with age. Liver size and portal blood flow decreased significantly with age. Mucosal surface area has been reported to be slightly diminished in the aging man. Glucose transporters decreased and Insulin tolerance Increased. Absorption of aromatic amino acid is diminished with age. Dietary protein In that aging human increased fecal nitrogen excretion. Vitamin A tolerance increased. Vitamin D receptor concentration decreased and resistance to 1,25-(OH)2D3 action increased. Permeability of aging small Intestine Increased. Zinc balance dirt not differ Copper absorption appeared not to be significantly affected by age. Neurotensin secretion decreased thus slowed colonic peristaltic movements and Intestinal mucosal growth.
