A Case Study of SIL Analysis for Single Station Controller in Nuclear Power Plant Based on IEC 61508 (IEC 61508에 기반한 원자력 발전소용 안전 등급 제어기의 SIL 분석에 대한 사례연구)

  • Purpose: It is not easy to suggest a quantitative data related to safety analysis. The objective of this paper is to propose a method of Safety Integrity Level (SIL) analysis and to suggest a SIL analysis result for single station controller in nuclear power plant based on IEC 61508. Methods: The Failure Modes and Effects Diagnostic Analysis (FMEDA) and average probability of failure on demand (PFD) are used for SIL assessment. Results: A SIL of single station controller is evaluated 4 by a reliability analysis results and PFD. Conclusion: A SIL analysis method and result for single station controller based on IEC 61508 are proposed in this paper. It can applicable for a manufacturer data in safety-related system.

  • Accident analysis models were developed to improve the construction site safety and case studies was conducted. In 2016, 86% of fatality accidents occurred due to simple unsafe acts. Structure related accidents are less frequent than the non structure related causes, but the number of casualties per accident is two times higher than non structure one. In the view of risk perception, efforts should be given to reduce accidents caused by low frequency - high consequence structure related causes. In case of structure related accident, structural safety inspection and management (including quality), ground condition management / inspection technology, and provision of risk information delivery system in case of non structure related accident were proposed as a solution. In analysis of relationship between safety related stakeholder, the main problem were the lack of knowledge of controller and player, loss of control due to duplicated controls, lack of communication system of risk information, and relative position error of controller and player.

  • As a domestic aviation industry has been growing continuously, the air traffic volume of en-route has increased rapidly to 2,300 a day. According to developing air navigation system including Performance Based Navigation(PBN), the manner of flights has diversified. Consequentially task of air traffic service has been increasing more and more and the organization of Air Traffic Service(ATC) established one Air traffic management Office(ATMO), extends to 3 Regional organization(Seoul, Busan, Jeju) and a number of air traffic controller exceeds 600. The purpose of this research is going to investigate the relationship between organization's safety climate and air traffic controller's safety behavior, In pursuing above, previous studies related to co-worker and supervisor's safety climate were examined for literature review. Based on this previous studies, research model was constructed. Hypothesis was verified by effect. Data from 209 samples was employed for final survey. The main results show that co-worker and supervisor's safety climate were meaningful factors to effect perceived Safety Behavior and safety knowledge, safety motivation were significantly and positive related to safety compliance.

  • In this study, GEC, VVVF train is mounted in the cab on the Improvement of Brake Controller checks were carried out.   • In a power plant, the utility operates controllers include safety program that has performed several stages verification to prevent accidents in preparation for accidents, or to stably operate in accident. This paper describes the virtualization technology so that the verified binary operating system and application program can operate on the controller processor used in the power plant safety control facility. The technology applied to this virtualization processor uses commercial tools to implement the essential components for the operation of the safety-grade controller processor module, such as command interpreters and analyzers, and the virtualization platform was developed in a Linux-based operating system using the Imperas Tool. In addition, it was checked whether the implemented virtual processor module can normally interpret and execute binary-type instructions.

  • Since the release of safety standard IEC 61508 which defines functional safety of electronic safety-related systems, SIL(Safety Integrity Level) certification for railway systems has gained lots of attention lately. In this paper, we propose a new design technique of the computer board for train control systems with high reliability and safety. The board is designed with TMR(Triple Modular Redundancy) using a certified SIL3 Texas Instrument(TI)'s TMS570 MCU(Micro-Controller Unit) to guarantee safety and reliability. TMR for the control device is implemented on FPGA(Field Programmable Gate Array) which integrates a comparator, a CAN(Controller Area Network) communication module, built-in self-error checking, error discriminant function to improve the reliability of the board. Even if a malfunction of a processing module occurs, the safety control function based on the proposed technique lets the system operate properly by detecting and masking the malfunction. An RTOS (Real Time Operation System) called FreeRTOS is ported on the board so that reliable and stable operation and convenient software development can be provided.

  • Programmable electronic interlocking system plays key role in railway operation and is closely related to railway accidents, in which the programmable electronic controller of interlocking system may become sources. Redundant digital controllers are adopted as the interlocking controllers to prevent the accidents from the controllers being out of order. The redundant digital controllers being fault tolerant are realized through dual or triplex controllers. In this paper, we calculated safety and availability of the redundant digital controllers using Markov models, demonstrated key part to determine the availability ana the safety.

Overhead Crane Controllers and Ergonomic Problems in Industrial Field (산업현장의 천장크레인 조작장치 사용 현황과 인간공학적 문제점)

  • Overhead cranes are the most widely used and have been identified as one of the main causes of fatal accidents. Although structural, mechanical and electrical safety standards for overhead cranes have been established to some extent, ergonomic standards related to crane controllers are rarely established. As a result, many crane accidents are reported to be caused by operator's error. To investigate the actual use of crane controllers in the industrial field, we surveyed 82 overhead cranes in 15 workplace and interviewed workers operating them. This study has presented the ergonomic problems related to the crane controllers in the field. As a result of investigation, it was found that the direction control signs, the sign language, and the button layout on the pendant switches and remote controllers are not standardized, which can cause a human error. To reduce human errors in overhead crane operation, ergonomic standards for controllers should be reflected on the government legislation or standards.

  • Autoregressed Controller, which have trend algorithm, seeks to minimize variability by transferring the output variable to the related process input variable, while multivariate process control techniques seek to reduce variability by detecting and eliminating assignable causes of variation. In the case of process control, a very reasonable objective is to try to minimize the variance of the output deviations from the target or set point. We also investigate algorithm with relevant Shewhart chart, Theoretical control charts, precontrol and process capability. To help the people who want to make the theoretical system, we compare the main techniques in "a study on the relation between multivariate process control techniques and trend algorithms".

A Study on the Safety Assessment and Damage Pattern of Water Purifier Compressors (정수기용 압축기의 안전성 평가 및 소손 패턴 분석에 관한 연구)

  • The purpose of this study is to provide basic data for the safety assessment of a water purifier when water leaks due to inappropriate maintenance and the examination of the cause of accidents related to the leak. Due to its inspection and management by non-specialists, if a leak occurs in a water purifier with the water level controller being inclined, it may result in the failure of the compressor, power supply line, PCB, etc. The analysis of the thermal diffusion pattern of water purifier compressors using a thermal image camera shows that its maximum temperature was approximately $80^{\circ}C$. In addition, its operating current was a maximum of 13 A and the system's operating current was approximately 1.7 A after the compressor was charged. It was found that the housing type power cable cover of the compressor had the effect of preventing electric shock but has poor flame resistance. Furthermore, the performance of the overload protector, PTC relays, etc., was excellent but they have potential for problems as metallic terminals were exposed, resulting in the potential of a safety related accident. The terminals and their surface damaged by the tracking showed a trace of carbonization and the resistance between terminals was measured to be approximately $8{\Omega}$. In addition, while the tracking was proceeding, the fuse and circuit breaker installed for system protection did not operate.
