Dip Estimation for Overhead Transmission Conductor using Catenary Angle (카테너리 각도를 이용한 가공송전도체의 이도 추정)

  • Under normal operating condition, the conductor of overhead transmission line could be always hold it's clearance within the safety margin that is specified in the line design guide of power company. Hence it may be very important to measure/or monitor the dip of the conductor, when building a new line, re-tensioning for an aged conductor, or monitoring dynamic line rating to maximize power capability. In this paper, we suggest a new method to estimate the dip and tension by catenary angle of the conductor. Since most conductors in overhead transmission lines show typical catenary curves, it can be uniquely determined the catenary curve for the conductor from the catenary angle at tower. Based on the catenary curve, the dip or horizontal tension can be easily estimated. Through some simulation and simple experimental results, it is verified that the suggested method can be effectively applied to measure/or monitor conductor dips and tensions in the overhead transmission lines.

Analysis of Recent Trends in Clinical Research Publications on Acupuncture and Moxibustion Studies (침구 임상연구 논문동향 분석)

  • Objective : This study was aimed to review the trend of clinical research articles of acupuncture and moxibustion from the year of 1995 to 2006. Methods : The retrieving and analyzing period were from January 1995 to July 2006. Articles were collected by retrieving Pubmed database. Search terms were as follows; 'Acupuncture', 'Acupuncture & Meridian', 'Moxibustion'. After reviewing articles by our pre-defined criteria, Correlation was assessed qualitatively between contents of the final selected articles and those of Korea Institute of Oriental Medicine. Results : The number of acupuncture research articles have increased with higher rate since the year of 2000. In the field of medical devices, the articles were sparsely existed. The percentage of clinical acupuncture research articles was 75%. However, in the category of Korean investigators' researches, it was 46%. Publications by Korean investigators were concentrated to several specific journals of which their impact factor were below 1 point. Conclusion : There needs more qualifying studies in acupuncture studies which are represented by impact factor in Journal Citation Reports. It is necessary to research and develop the medical devices quantatively rating the outcome of acupuncture trials. The secondary research, such as, systematic reviews, meta-analysis, would be a key approach to evaluate the effectiveness and safety of acupuncture treatments. Moxibustion research would be noticeable in regard to its historical effectiveness and its variable indications.

Clinical Efficacy and Safety of Yukgunja-tang for Gastroesophageal Reflux Disease: A Systematic Review and Meta-Analysis

  • Objectives: The purpose of this meta-analysis was to evaluate the effects of Yukgunja-tang(YGJT, Rikkunshito, Liujunzi) on Gastroesophageal Reflux Disease(GERD). Methods: Sixteen electronic databases were used to search for studies published through March 2019, and a randomized controlled study was conducted to study the effects of YGJT or modified YGJT on GERD. Study quality was assessed using the risk bias tool provided by Cochran, and data analysis was performed using Review Manager 5.3.0 software Results: Two-hundred and forty articles were initially searched, and 13 studies that satisfied the study criteria were evaluated qualitatively; 11 of the 13 were included in the meta-analysis. In the two studies, the effects of YGJT and a placebo were compared. Meta-analysis showed that YGJT significantly improved FSSG (Frequency Scale for the Symptoms of GERD) scores, but not GSRS (Gastrointestinal Symptom Rating Scale) scores (FSSG: MD -2.40, 95% CI [-5.31, 0.51], p=0.11, GSRS: MD 0.05, 95% CI [-0.12, 0.22], p=0.55). Meta-analysis of nine studies comparing the efficacies of YGJT and conventional medicine showed that YGJT had a significant clinical effect (OR 5.72; 95% CI [3.41, 9.61]; I2 p<0.00001). Conclusion: This study suggests that YGJT effectively relieves the symptoms of GERD. Unfortunately, owing to the small sample sizes, limitations of several methodological qualities, we believe large-scale clinical studies with less bias will provide evidence of qualitative improvement.

  • This study aims to suggest the detailed evaluation criteria based on performances for rock slopes. Using the previous research result, final evaluation items are proposed considering characteristics and similarities of each evaluation item. Weight for each evaluation item is deducted using AHP method, verification for suggested evaluation criteria is conducted based on correlation analysis. The research results as follows. All evaluation items have a high statistical correlation with final evaluation result(safety rating). Especially, items of the "rockfall", "ground deformation", "discontinuity characteristic", "instable lithology" were shown the highest in relative correlation coefficient(R), It is judged that items and weight presented in this study well reflect characteristics of rock slopes.

  • This study is in order to promote agricultural products sold in the rural village festivals direct transaction marketplace and enabled by a comprehensive user evaluation of the rural village festivals direct transaction marketplaces in universal design perspective, provide a design plan for your care and comfort of a rural village festival Direct Deals Marketplace there is a purpose. Thus, redefining the principles of universal design suitable for direct transactions rural village festival marketplaces, and to do this by reclassifying the design elements of the farmstand-type in a framework was developed 30 questions to evaluate items for Universal Design Evaluation. And, the selection of three villages along the rural village festivals scale and conducted a survey of Universal Design Rating. As a result, the overall assessment was positive for B villages of rural village festival marketplaces direct transactions, there was a significant difference with the other village. In particular, there was a notable difference in the fairness, efficiency, promotional, safety and hygiene. And, there was a significant difference in design elements for directing atmosphere by selling space, product display, hygiene. This paper presents the design suggestions about a rural village festivals farmers' market aspects of universal design for improvement of the marketplace as following, 1) placement for differentiation the markerplace and around sites in the festival place, 2) to build the operating systems of sustainable management for arrangement and organization during the festival, 3) to install the device for maintaining agricultural products freshness 4) to separate the sale of local specialty products and general food products, and 5) to locate the suitable space considering the visitors tour route in the festival place for large-scale festival.

Systematic Review of Clinical Research on Daegunjoong-tang for Improvement of Gastrointestinal Motility after Surgery for Gastrointestinal Cancer (소화기암 수술 후 위장관 운동성 개선을 위한 대건중탕의 효과에 대한 임상연구의 체계적 문헌고찰)

  • Objectives: This study aimed to evaluate the efficacy of Daegunjoong-tang for improvement of gastrointestinal motility after surgery due to gastrointestinal cancer by analyzing the existing clinical research. Methods: Clinical studies about Daegunjoong-tang for improvement of gastrointestinal motility after surgery due to gastrointestinal cancer were identified in a literature search using the search term "Daikenchuto AND cancer AND ileus." The studies were analyzed in terms of design, inclusion and exclusion of participants, intervention, control, outcomes, and results. Results: Nine articles were identified in the literature search. Four trials included colon cancer participants with colectomy, and three studies included gastric cancer with total gastrectomy. The intervention in each case was Daegunjoong-tang, and most interventions were made by a pharmaceutical company with a Good Manufacturing Practice facility. The most frequently used control was a placebo. The methods were diverse, including measuring gastrointestinal function, motility, quality of life, symptom scores with a numeric rating scale, and blood tests. Safety was investigated by recording adverse events. Conclusions: Some issues were discovered by reviewing the existing clinical research about Daegunjoong-tang for improvement of gastrointestinal motility after surgery for gastrointestinal cancer. These results will be utilized as evidence for using Daegunjoong-tang in clinical practice and designing a clinical trial for Korean patients.

Analysis of Water Loss Rate and Irrigation Efficiency in Irrigation Canal at the Dong-Jin District (동진지구 관개용수로의 손실률 및 관개효율 분석)

  • The purpose of this study is to evaluate the paddy irrigation efficiency using real-time water level monitoring data and intermittent irrigation model in Gimjae, Dong-Jin irrigation district. For this study, the real-time water level data in Gimjae main canal and other secondary canals were collected from 2012 to 2014 and converted to daily discharge using rating curve in each canal. From intermittent irrigation model in paddy, irrigation water requirement was estimated and irrigation efficiency was calculated. The average amount of irrigation water supply per unit irrigation area was 1,011 mm in Gimjae main canal for 12,749 ha irrigation area, 1,011 mm in the secondary canal of upper region and 1,470 mm in the secondary canal of lower region. The median irrigation loss was 43 % in Gimjae main canal, 25 % in secondary canal of upper region and 35 % in the secondary canal of lower region. The larger irrigation area is, the irrigation loss rates tend to decrease in secondary canals. Monthly median irrigation losses in upper region were 10 (June) - 40 % (September) and those in lower region were 25 (May) to 40 % (April, June, August, and September). The results of canal management loss can be available as the basic data for irrigation water management and estimating guideline of optimal irrigation water supply to improve agricultural water use efficiencies.

A Case Report of Symptom Improvement after lobectomy in Male Breast Cancer with Lung Metastasis Patient Treated with Korean Medicine based Integrative Cancer Treatment (남성 유방암 폐전이 환자의 폐절제술 후 한의기반 통합암치료로 증상 호전에 대한 증례보고)

  • Objective: The purpose of this study is to report improvement of symptoms after lobectomy of male breast cancer lung metastasis treated with Korean Medicine based Integrative Cancer Treatment (ICT). Methods: A male left breast cancer patient diagnosed with metastasis on lung at July 2019. After Video assisted thoracic surgery (VATS) left lower lobe (LLL) lobectomy and En bloc wedge resection the patient visited the Daejeon korean medicine hospital of Daejeon university East West Cancer Center (EWCC) to treat operation-site (op-site) pain, dysphagia, anorexia with Korean Medicine Treatment. The patient was treated with Korean Medicine based ICT for an approximately 20 days. The clinical outcomes were measured by National Cancer Institute Common Terminology Criteria for Adverse Event (NCI-CTCAE), Numeral rating scale (NRS) and Eastern Cooperative Oncology Group (ECOG). The safety of treatment was verified by blood tests. Results: After treatment, op-site pain was improved from NRS 9 to 6, dysphagia and anorexia were relieved from NRS 9 to 2. And ECOG score of the patient was improved from grade 2 to 1. Conclusion: This case study suggests that Korean Medicine based ICT may help to improve post operative sequelae in metastatic lung cancer patient.

Continuous Epidural Infusion of Bupivacaine with Tramadol for Post-Cesarean Analgesia (제왕절개술후 통증치료시 지속적 경막외 국소마취제와 Tramadol의 병용투여의 효과)

  • Background: Tramadol administered epidurally is known to have one-thirtieth the potency of morphine for treatment of pain following abdominal surgery. We designed a prospective, randomized, controlled study to evaluate the analgesic efficacy and safety of combined epidural infusion of bupivacaine and tramadol with 2-day infusor as ompared to bupivacaine and morphine combined epidural infusion. Methods: Sixty healthy women scheduled for Cesarean delivery were assigned randomly in double- blind fashion: Group 1 (n=20) were given a mixture of morphine 10 mg(1 ml), 0.5% bupivacaine 40 ml and normal saline(NS) 40 ml; Group 2(n=20) a mixture of tramadol 300 mg(6 ml), 0.5% bupivacaine 40 ml and NS 54 ml; Group 3(n=20) or a mixture of tramadol 500 mg(10 ml), 0.5% bupivacaine 50 ml and NS 50 ml, of continuous dose via epidural route following 1% lidocaine 6 ml as bolus dose for 48 hours postoperatively. We evaluated the analgesic efficacy and side effects of these three groups using visual analogue pain scale (VAPS) and verbal rating scale (VRS). Results: VAPS of group 1 and 3 were lower than group 2, and VAPS of group 1 was lower than group 3(12, 24, 36, 48 hours). VRS of group 1 and 3 were lower than group 2 (12, 24, 36 hours). There were incidences of pruritus was 16 patients in group 1. Conclusions: Tramadol does possess the analgesia effect of morphine, but has the added analgesia following increment. Further research to determine the most effective administration method and reguired dosage of tramadol is further needed.

Comparison of clinical efficacy in epidural steroid injections through transforaminal or parasagittal approaches

  • Background: The transforaminal (TF) epidural steroid injection (ESI) is suggested as more effective than the interlaminar (IL) route due to higher delivery of medication at the anterior epidural space. However, serious complications such as spinal cord injury and permanent neural injury have been reported. The purpose of this study is to evaluate and compare the clinical effectiveness, technical ease, and safety of the TF and parasagittal IL (PIL) ESI. Methods: A total of 72 patients were randomized to either the PIL group (n = 41) or the TF group (n = 31) under fluoroscopic guidance. Patients were evaluated for effective pain relief by the numerical rating scale (NRS) and Oswestry Disability Index (ODI) (%) before and 2 weeks after the ESI. The presence of concordant paresthesia, anterior epidural spread, total procedure time, and exposed radiation dose were also evaluated. Results: Both the PIL and TF approach produced similar clinically significant improvements in pain and level of disability. Among the 72 patients, 27 PIL (66%) and 20 TF (64%) patients showed concordant paresthesia while 14 (34%) and 11 (36%) patients in the same respective order showed disconcordant or no paresthesia. Radiation dose and total procedure time required were compared; the PIL group showed a significantly lower radiation dose ($30.2{\pm}12$ vs. $80.8{\pm}26.8$ [$Cgy/cm^2$]) and shorter procedure time ($96.2{\pm}31$ vs. $141.6{\pm}30$ seconds). Conclusions: ESI under fluoroscopic guidance with PIL or TF approach were effective in reducing the NRS and ODI. PIL ESI was a technically easier and simple method compared to TF ESI.
