Risk Management for Ammonia Unloading and Storage Tank Facility (암모니아 입하 및 저장시설에서의 위험도 관리)

  • A lot of hazardous materials have been used for product processing and utility plant. Many accidents including toxic release, fire and explosions occur in the ammonia related facility and plant. Various safety and environment management program including PSM, SMS, ORA etc. are being implemented for risk management and accident prevention in the production industry. Also much study and research have been carried about risk assessment of accident scenario in the academic and research area. In this paper, firstly risk level was assessed by using a typically used KORA program and LOPA PFD method for the selected ammonia unloading and storage facility. And then risk reduction measures for the risk assessed facility were studied in 3 aspects and some measures were proposed. Those Risk Reduction measures are including a leak detection and emergency isolation, water spray, dilution tank, dike and trench, scattering protection in hardware impovement aspect, and a applicable risk criteria, conditional modifier for existing LOPA PFD, alternative supporting modeling program in risk estimation methodology aspect, and last RBPS(Risk Based Process Safety) program, re-doing of process hazard analysis, management system compliance audit in managerial activity aspect.

  • LOSA is a flight safety program that analyses human errors in normal operations. Trained pilot observers monitor the normal flights at the observer seat. LOSA is a proactive non jeopardy data collection tool using threat and error management(TEM) as a framework. With the analysis of crew behaviors through LOSA with The LOSA collaborative(TLC), the airlines can identify the behaviors of the crew during normal operations. The major objective of LOSA is to measure how the crew manage threats, errors and undesired aircraft deviations in the cockpit on day to day operations. The airlines are able to set up effective TEM training with practical six generation Crew recourse management(CRM) with data of error from LOSA instead of theoretical CRM courses. The Airlines can use TEM as an integral part of a Safety Management System(SMS) and uses monitoring and cross-checking skills in the flight operations to manage threats and errors effectively when we know the errors we make in the cockpit on daily operation. The result of LOSA indicates that the error detection rate should be enhanced since around the half of the errors went undetected. The areas which should be focused for enhancing the error detection are monitor, cross-check, the management of workload, automation and taxiway/ runway to manage errors effectively.

  • Korea is topographically and meteorologically prone to natural disasters. In addition to extraordinary weather, localized heavy rain is a irregular occurrence. Korea are frequently damaged by rain, wind, flood, etc. Recently, slope failures are disastrous when they occur in mountainous area adjoining highways. The accidents associated with slope failures have increased due to rapid urbanization of mountainous area. Therefore, the inspection of slope is conducted to maintain highway safety as well as road function. We developed basic function and risk management of a partial shape on highway slopes based on the internet. It is important for quantitative risk evaluation of highway slopes.

  • ICAO's Universal Safety Oversight Audit Programme (USOAP) was initially launched in January 1995, in response to widespread concerns about the adequacy of aviation safety oversight around the world. The recent reduction in aircraft accidents and effective role that is evaluated on the basis of these results, and in 2013 the existing 'snapshot approach' to 'regular monitoring system (USOAP-Continuous Monitoring Approach)' was converted to. ICAO aviation safety assessment of the state in today's international community 'aviation safety credibility' as objective indicators to judge the enormous impact on the aviation industry, the state is not satisfactory, especially if the results of the evaluation and expansion of code-share airline ban, reduced international air transit passengers, including premium increases business and economic penalties should.   • NIR(New Inspection Regime, hereinafter NIR) relating to the PSC(Port State Contorl, hereinafter PSC) has implemented in Asia-Pacific region under Tokyo-MOU from last January 2014. The most noticable difference between NIR and existing TF(Target Factor, hereinafter TF) is to evaluate company performance and to conduct PSC with flexible inspection interval on the basis of Ship Risk Profile; High Risk Ship, Standard Risk Ship and Low Risk Ship.
