Risk Assessment in the UK Health and Safety System: Theory and Practice

  • In the UK, a person or organisation that creates risk is required to manage and control that risk so that it is reduced 'So Far As Is Reasonably Practicable (SFAIRP).' How the risk is managed is to be determined by those who create the risk. They have a duty to demonstrate that they have taken action to ensure all risk is reduced SFAIRP and must have documentary evidence, for example a risk assessment or safety case, to prove that they manage the risks their activities create. The UK Health and Safety Executive (HSE) does not tell organisations how to manage the risks they create but does inspect the quality of risk identification and management. This paper gives a brief overview of where responsibility for occupational health and safety lies in the UK, and how risk should be managed through risk assessment. The focus of the paper is three recent major UK incidents, all involving fatalities, and all of which were wholly avoidable if risks had been properly assessed and managed. The paper concludes with an analysis of the common failings of risk assessments and key actions for improvement.

  • In order to enhance the safety of the international aviation industry, the International Civil Aviation Organization has recommended establishing an operational foundation for systematic and integrated collection, storage, analysis and sharing of aviation safety data. Accordingly, the Korea aviation industry also needs to comprehensively manage the safety data which generated and collected by various stakeholders related to aviation safety, and through this, it is necessary to previously identify and remove hazards that may cause accident. For more effective data management and utilization, a standard structure should be established to enable integrated management and sharing of safety data. Therefore, this study aims to propose the framework about how to manage and integrate the aviation safety data for big data-based aviation safety management and shared platform.

  • This paper summarized the safety diagnosis method in the field of durability among the structural safety suggested in the safety diagnosis manual of the reconstruction project and analyzed the safety diagnosis case. It is expected to be a reference material for safety diagnosis in the field of durability by pointing out cases that are wrong during existing safety diagnosis and cases where diagnostics are likely to make errors.

지진계수를 고려한 제방의 사면안정에 관한 연구 (A Study on the Slope Stability of Embankment in Consideration of Seismic Coefficient)

  • This study was performed to investigate the minimum safety factor of embankment in consideration of seismic coefficient by the psuedo-static analysis The variables were cohesion, the internal friction angle, angle of slope, height of seepage, height of embankment, depth of replacement The results obtained were compared with those by Fellenius method, simplified Bishop method and Janbu method. The results were summarized as follows: 1.The increasing rate of the minimum safety factor with the increasing of cohesion appeared larger in Fellenius method and Bishop method than in Janbu method. And that with the increasing of the internal friction angle appeared the lowest value in Janbu method. The minimum safety factor was influenced larger on the internal friction angle than on cohesion. 2.The variation of the minimum safety factor with the height of seepage at 0m and 5 m was nearly similar to Fellenius method, Bishop method and Janbu method. On the other hand, it was decreased suddenly at 25 m. 3.The minimum safety factor with the height of embankment was decreased remarkably under 10 m with the increasing of seismic coefficient. But, it was decreased slowly more than 10 m. As the height of embankment was low, the influence of cohesion appeared larger. 4.In heigher case of the depth of replacement, the phenomenon of reduction of the minimum safety factor appeared remarkably with seismic coefficient increased. And in lower case of the depth of replacement, the minimum safety factor was similar in Fellenius method and Bishop mehtod. But it appeared larger in Bishop method and Janbu method than in Fellenius method with the depth of replacement increased. 5.As the cohesion and the internal friction angle were large, the phenomenon of reduction of the minimum safety factor with the increasing of seismic coefficient appeared remarkably. Also, the influence of seismic coefficient in minimum safety factor appeared larger with the soil parameter increased. 6.When the seismic coefficient was considerated, investigation of the structural body on the slope stability appeared profitably in Fellenius method and Janbu method than in Bishop method.

  • One of the most important elements of the management of chemical accident is threat zone estimation of fires, explosions and toxic gas dispersion based on chemical releases. The threat zone estimation is going to be standard of emergency response for the first defender and base line data of off-site risk assesment (hereinafter referred to as "ORA") and risk management plan (hereinafter referred to as "RMP"). Generally, ALOHA form EPA(U.S.) and Kora(from KOREA MINISTRY OF ENVIRONMENT) has been used for the off-site consequence analysis in Korea. However it is hard to predict accurate consequences rapidly in case of emergency. Hydrochloric acid is a multipurpose raw material used in many industrial applications such as chemical, metal and food industries. It is usually treated in concentrations from 10 ~ 35 %, and release accident have occurred frequently. In this study, we have developed a simplified estimating method and equation to calculate threat zone easily in case of emergency due to release accident of hydrochloric acid.

산업안전보건교육 기관의 운영에 따른 교육 만족도 조사결과 사례 보고 (Case Report on the Survey Results of Educational Satisfaction According to the Operation of Occupational Safety and Health Training Institute)

  • Objectives: This case report aims to contribute to the enhancement of training quality for occupational accident prevention by conducting surveys on customers' satisfaction with the training course management by the Occupational Safety and Health Training Institute (OSHTI) of KOSHA. Methods: Surveys were conducted through phone calls, customer service documents, and questionnaires from January 1, 2018 to December 31, 2018. Results: The surveys showed an answer rate of 97.36% and handling rate of 97.47% in 2018, an increase of 1.15% compared to 2017. The number of monthly inbound calls in 2018 was 5,902, rising 0.10% year-on-year, and the average inbound calls per day in 2018 was 289, a decline from 291 the year before. The number of provisions of customer service in 2018 was 68,952, increasing 1.89% year-on-year. The number of inquiries on the training curriculum was 58,744 in 2018, an increase of 3.98% compared to the 56,498 recorded in 2017. Inquiries on job training centers were the most common, numbering 27,114 (39.32%), followed by e-learning 18,470 (26.79%) and expert courses 13,160 (19.09%). Of the 149 answers to the customer service survey, 'Nothing to complain about' accounted for 86 (56.72%) and 'Diversifying training time and programs' numbered 22 (14.77%). Conclusions: Customer satisfaction in 2018 increased compared to that in 2017. However, there is a need to reflect the demands of customers for diversifying training time and programs, offer practice-centered training, and collect opinions on providing information in order to maintain high quality training course management.

심해 시추와 관련된 국내 해양 환경 및 안전 관련법 개선에 대한 입법론적 연구 (A Study on Legislation for the Improvement of the Marine Environment and Safety Act for Deep Sea Drilling)

  • 이 연구는 국내 대륙붕 주변에서 진행되는 심해 시추의 개요 및 현황에 대하여 시추의 개념 및 종류, 대륙붕 개발의 연혁 및 현황을 중심으로 검토하였고, 시추와 관련된 해양 환경, 안전법상 적용의 범위, 문제점 등에 대하여 법적으로 분석하였다. 그리고 이 연구는 해저광물자원의 체계적인 개발을 위하여 영국의 '해양구조물(안전관리절차)규정' 기초로 별도의 법률 신설 방안 및 해양환경관리법, 해사안전법, 석유광산안전규칙에 대한 개선안을 입법론적으로 제안하였다. 특히 해외 해양플랜트와 관련된 사고 사례를 기초로 국내 인력에 대한 국제인증교육에 준하는 교육훈련 및 평가 제도의 구축의 필요성을 강조하였다.

재난안전 분야 핵심 학술지 탐색 및 네트워크 분석 연구 (Study on Determining Core Journals and Network Analysis in the Field of Disaster & Safety)

  • 최근 발생하는 재난의 형태는 복합적이며 대형화되어가고 있는 추세이다. 예고 없이 발생하는 재난에 효과적으로 대비하고 대응하기 위해서는 기존에 활용되는 정형화된 재난안전정보 이외에 재난안전 분야의 학술정보를 활용하는 것이 매우 중요하다. 본 논문에서는 한국과학기술인용색인DB와 재난안전정보 공유 플랫폼 기술개발 R&D 사업에서 개발한 재난안전정보 분류체계를 활용하여 재난안전 분야 학술지를 식별하고 논문 콘텐트의 데이터 식별 및 전처리를 통해 인용관계 네트워크, 공저 네트워크, 키워드 네트워크를 구축하고 분석하였다. 연구 결과, 정보 분석 단위별로 구축된 네트워크를 기반으로 재난안전유형별 국내 및 해외 핵심 학술지, 주요 연구기관, 주요 키워드와 개별 정보간의 연결 구조가 자세히 파악되었으며 사례별로 분석 결과를 제시하였다.

의료기관의 위험도 분석 조사 - 지역공공의료원을 중심으로 (A Study on the Hazard and Risk Analysis of Hospital in Korea - Focused on Local Medical Centers)

  • The purpose of this study is to analyse the hazard risk by examining the magnitude and severity of each type of hazard in order to mitigate and prepare for disasters in medical facilities. Methods: The hazard risk analysis for hazard types was surveyed for team leaders of medical facilities. The questionnaire analyzed data from 27 facilities, which were returned from 41 Local Medical Centers. Results: When looking at the 'Risk' by category type of hazard, the influence of health safety and fire/energy safety comes first, followed by natural disaster, facility safety, and crime safety. On the other hand, as for 'Magnitude', facility safety and crime safety come first, followed by health safety, fire/energy safety, and natural disasters. Most of the top types of disaster judged to have high hazard in medical facilities are health types. The top five priorities of hazard in medical facilities, they are affected by the geographical and industrial conditions of the treatment area. In the case of cities, the hazard was found to be high in the order of infectious disease, patient surge, and wind and flood damage. On the other hand, in rural areas, livestock diseases and infectious diseases showed the highest hazard. In the case of forest areas, the hazard was high in the order of wildfire, fire accident, lightning, tide, earthquake, and landslide, whereas in coastal areas of industrial complexes, the hazard was high due to fire, landslide, water pollution, marine pollution, and chemical spill accident. Implications: Through the research, standards will be established for the design of hospitals with disaster preparedness, and will contribute to the preparation of preemptive measures in terms of maintenance.

중앙집중식 컨트롤러 고소작업차의 고소리프트의 구조안정성 평가 (Centralized Controller High-altitude Work Car Elevations Lift Structure Safety Assessment)

  • 본 연구는 선행 연구로 진행되었던 수직상승형 고소작업차의 개발을 위한 구조해석 및 원격 제어를 포함한 중앙집중식 컨트롤러 및 유압식 상승리프트 개발에 관한 사후 연구로 진행되었다. 기존에 연구를 진행하면서 개발하였던 고소리프트의 구조 변경을 통하여 안전성 검토를 실시하였다. Solidworks로 3D 모델링을 진행하였으며, Hypermesh S/W를 통하여 유한요소의 모델을 생성하였다. 또한, 평가를 위한 작업차의 적재 환경은 작업 테이블에 위치별(전체, 상좌, 상우, 하좌, 하우)로 적재량이 250 kg인 조건, 작업 테이블에 위치별(전체, 상좌, 상우, 하좌, 하우)로 적재량이 600kg인 조건으로 하여 구조해석을 진행하여 다방면으로 안전성을 검토하였다. 그 결과 허용 적재량 250 kg과 초과 적재량 600 kg에 대해서 최대응력이 발생하는 부품을 제외한 결과를 살펴보면(Case-11 제외), 응력 수준은 항복강도 이하임을 알 수 있다. Case-11의 경우에는 최대응력이 발생하는 부품을 제외한 후에도 상단의 안전바 중앙 지지부에서 항복강도를 초과하는 영역이 존재하지만, 최대응력이 발생하는 부품과 같이 전체 구조물의 안전성 측면에는 영향을 미치지 않을 것으로 사료된다. 변위(처짐) 결과를 살펴보게 되면, 모든 경우에 있어서 동일하게 테이블에서 최대 처짐이 발생하는 것을 알 수 있으며, 250 kg과 600 kg 모두 처짐이 발생하는 경향은 동일한 것으로 나타났다.
