Perception and Practice level of Korean Medical Doctors on Infection Control and Prevention in Korean Medicine Facilities (한방의료기관 감염관리에 대한 한의사의 인식도와 수행도)

  • Purpose : This study was designed to identify the perception and practice level of infection control among korean medical doctors and to identify factors that may influence the performance and practice level. Methods : Data were collected using the on­line survey method. Seven hundred and eighty four Korean medical doctors(KMD) participated the survey. The study was conducted from December 2018 to January 2019. Results : The results of this study are as follows. 1. Participants who experienced infection risk by needles or sharp instruments were 596(76%). and participants who had experienced blood or body fluid contact with the mucous membrane or skin of the patient during treatment were 226(28.8%) of them. 2. The degree of perception and practice of the infectious guideline was higher in the group over 50 years, in the doctor group, in the group with more than 6 years experience in clinic and in the group who work in the hospital. (p < 0.05) 3. In the performance of the infection control management related to the Korean medical treatment, the practice level of the article 'Discard the remaining needle that used for one patient' was the lowest at 4.02, 'Identify the patient and check the validity period of sterilization of medicines or instruments' was the second lowest in 4.16. 4. Among the contents of "Prevention of Nosocomial infection and Sanitary Safety Guidelines" issued by the Korean Medical Association in 2008, the guideline "Prevention of infection by pathogens such as HIV, MRSA, SARS" were lowest article in the perception and practice level of participants. 5. Regression analysis was performed to find out the factors affecting perception and performance of the participants. The regression model showed significant difference in the regression model of the working years. (p < 0.05) 6. In order to examine the effect of the variables on the perception and practice of the infectious guideline, the mediated effect of the knowledge and education level according to the years of working, age, education degree was found to be significant only in the education degree variable. (p < 0.05) In conclusion : in order to improve the perception and practice of infectious control of Korean medicine doctors, it is necessary to include the contents of infection management as essential education during the continuing education of Korean medicine association.

Comparison of knowledge, cognition and practice level on oral health care of the clients visiting dental clinic in Pusan area (부산지역 일부 치과의원 내원환자의 구강건강 지식 및 인식과 실천수준 비교)

  • Objectives : To propose the data for prevention of oral health problems through assessment on the knowledge, cognition and practice about oral health care of the clients who visit dental clinics. Methods : The subjects were total 400 clients from the four dental clinics in Busan city. Their knowledge, cognition and performances about oral healthcare collected with questionnaire survey from February to March, 2011. The rate and mean compared by chi-square test, t-test, and ANOVA. Results : Mean level of their knowledge, recognition and practice were 72.5, 80.2, and 65.6, respectively. Theses levels were higher in women (p<0.01), in the group of higher interested (p<0.001), immediately treated (p<0.001), correct tooth brushing (p<0.05), toothbrushing educated (p<0.001), take regular scaling (p<0.001), use assistant hygiene items (p<0.001), instead no differences by age and education level. And, the rate of correct toothbrushing was higher as 1.24 (1.03~1.50) times in the group with higher knowledge level, the rate of immediate treatment and regular scaling were higher as 1.35 (1.07~1.70) times, and 2.26 (1.41~3.64) times in the group with higher recognition level, respectively. Conclusions : These results demonstrate that insufficient knowledge and recognition of oral health care are related to a lack of its practice. Though their primary goal of the visits was treatment, the clients' attention needed to raise the comprehensive knowledge and recognition levels for their oral health promotion, especially reach to the performance level rather than simple knowledge acquisition. And more, to achieve the efficient oral health promotion, the importance of early treatment and strengthened efforts for prevention should be emphasized.

The Effect of Knowledge, Attitude and Practice on Noise - induced Hearing Loss (소음에 대한 지식, 태도 및 실천이 청력손실에 미치는 영향)

  • In order to evaluate the effects of knowledge, attitude and practice on noise-induced hearing loss or hearing threshold level, questionnaire survey was performed and hearing thresholds of 1 kHz and 4 kHz were measured on 1,040 subjects with workers exposed to noise, safety and health officers. industrial hygienists, analysts rind office workers. The results were as follows ; 1. The following 6 factors were obtained by factor analysis and factor rotation of 30 questionnaire of knowledge, attitude and practice about noise; knowledge of noise (F1), concern of hearing protective devices (F2), concern of noise induced hearing loss (F3), concern of noise level and hearing impairment (F4), concern of noise in workplace (F5) and recognition of noised-induced hearing loss (F6). 56.1% of variance was explained by 6 factors. 2. Significant variables influencing knowledge, attitude and practice about noise were education level and age in F1, personal protective devices (PPE) and education level in F2, age and education level in F3, education level, age and sex in F4, PPE, education level, age and work duration in F5, and work duration and PPE in F6. 3. Hearing thresholds of 4 kHz were significantly higher in workers exposed to noise than that of in the other subjects and tended to be higher in industrial hygienists, safety and health officers and analysts than that of the office workers. 4. Significant variables influencing hearing thresholds of 1 kHz were age, education level, F5 and F6 in workers exposed to noise, and F1 in industrial hygienists. 5. Significant variables influencing hearing thresholds of 4 kHz were age, F6, sex, work duration, F1, F5, F2 and F3 in workers exposed to noise, F1 and age in safety and health officers, and F6, sex and F4 in industrial hygienists. With the above results, it suggested that workers exposed to noise be needed the education of knowledge, attitude and practice about noise in hearing conservation program for the prevention of noise induced hearing loss. Also, it suggested that health managers in workplace be needed countermeasures to prevent hearing loss although they are intermittently exposed to noise.

  • We developed a HPMA Information System to establish the voluntary safety management for the Paint Industry. HPMA System means that manager, supervisor and worker on the process-line find out the hazardous and/or unsafe potentials, make improvement of the field safety by the cooperation with top manager, safety staff and eventually put in the practice. HPMA Information System consists of functions to suggest and evaluate the safety concerns hierarchically from top manager to workers on internet or intranet. We establish the safety management and sharing of the safety information and are able to find the unsafe potentials by the HPMA Information System. Also We are able to mine new safety information and establish accident prevention model by the Safety DATABASE.

  • Incident investigation is regarded as a means to improve safety performance. For the prevention of industrial accidents, measures such as providing safety education, enhancing management interest and participation, establishing a safety management system, and conducting inspection of the work site are necessary. In particular, accident investigation activities, which are an important element of safety management, help to prevent similar accidents, thereby minimizing damage and enhancing work safety. They are critical for understanding business-related incidents and the vulnerabilities and opportunities associated with them. Therefore, it is clear that accident investigation activities are important for accident prevention. The primary focus of many incident investigation processes is on identifying the cause of an event. While considerable research has been conducted on potential accident investigation tools there has been little research on including the views and experiences of practitioners in the accident investigation process. In this study, a questionnaire survey was conducted among safety managers in the domestic manufacturing/construction industry to understand the practice of accident investigation. The investigation pertained to companies' accident investigation systems, the competence of investigators, and the identification and recommendations of the cause of accidents. From the analysis results of accident investigations, investigators' competence, the difficulty level of investigations, and the root causes of accidents were identified from the viewpoint of the participants of the accident investigations. In particular, the development of standardized and simple accident investigation methods and their dissemination to companies were found to be necessary for activating the root cause of accidents. Based on this, it can be used as basic data for the development of root cause analysis investigation techniques that are easily applicable to organizations.

  • In this paper, the systematic problems of safety management in domestic construction industry are presented by the investigation and analysis of construction fatal injuries. In particular, it is discussed that the current construction safety management system by contractors leading is ineffective to prevent the serious accidents caused by inappropriate planning and design and to circulate the safety management cycle without interruption. A couple of ways for improving domestic construction safety management system are suggested in broad perspective. This is done by the analysis of construction safety management system of four advanced industrial countries such as the United States, the United Kingdom, Germany, and Japan and the study of a best practice. Total safety management system by owners leading is recommended to prevent construction accident effectively because this system makes all the parties join to the management system and distribute the responsibilities clearly to each party. This will drop the accident occurrence rate by dragging all the parties' cooperations and activating the total safety management system in an early stage.

  • Purpose: This paper is intended to provide a clauses of scope of nursing practice with nursing act. Method: This was a planning study. The provision of scope of nursing practice is constructed through critical review of literatures and regulations. The validity of the legal definition was tested through expert and staffs in affiliates of Korean Nurses Association review. Result: 'Nursing is an activity that assesses and diagnoses the reaction of an individual, family, and community for health promotion and maintenance, illness prevention and rehabilitation and to provide intervention and evaluate the results. This practices are done through nursing knowledge and skills. The nursing practices include basic nursing services (general hygiene, environment and safety control, emotional and physical comfort, examination and surgery related care, systematic observation and reporting about patients, activity and organ function maintenance), practice of doctor's regimen, consulting and education to patients, reference, collaboration and management with other health personnel, public health activity by regulation. And nursing standards are set by a separate code. Conclusions: The result of this study can be used to offer for nursing act. So, This legal definition will be constantly discussed and extended to reflect actual nursing practice.

Safety and Management of Toxicity Related to Aflibercept in Combination with Fluorouracil, Leucovorin and Irinotecan in Malaysian Patients with Metastatic Colorectal Cancer

  • Background: Between October 2012 and February 2015, 25 patients with metastatic colorectal cancer (mCRC) (mean age, $57.0{\pm}12.1years$) were granted access to aflibercept via the Aflibercept Named Patient Program at four centers. Materials and Methods: Here we reported the initial experience of aflibercept / FOLFIRI in combination. We evaluated treatment-related adverse events (AEs), progression-free survival (PFS) and overall survival (OS). Results: The majority of the patients experienced gastrointestinal toxicity (grade 1-2), with diarrhea (52%), mucositis (52%), and nausea/vomiting (20%) being largely observed. Neutropenia (16%) and febrile neutropenia (8%) were common grade 3-4 hematological events. Aflibercept-related toxicity was managed as per practice guidelines. No grade 5 event was reported. Median PFS was 6.12 months (95% CI, 4.80-7.20) and OS was 12 months (95% CI, 9.80-14.18). The partial response (PR), stable disease (SD), and progressive disease (PD) rates were 25% (95% CI: 23.4-27.0), 37.5% (95% CI: 31.6-43.3), and 37.5% (95% CI: 22.5-52.5), respectively. Conclusions: Aflibercept/FOLFIRI can be administered safely in a second line setting to Malaysian patients with mCRC, as the AEs experienced were generally reversible and manageable. The safety and efficacy outcomes were consistent with those observed in Western populations.

Analysis on the Responsibility and Exemption Clause of COLREG Rule 2 (국제해상충돌예방규칙 제2조에 따른 책임과 면책에 관한 분석)

  • The Marine Accident Investigation and Tribunal System is intended to provide a credible solution to prevent the recurrence of similar accidents. When a marine accident occurs, the Korea Maritime Safety Tribunal seeks to find its root causes through an analysis of what provoked the accident. It also contributes to the development of safety policies or practices by making a decision based on the findings. However, if the decision presented as the root cause of a marine accident is ambiguous or unclear, it may be difficult to achieve its intended goal. Hence, if we read some of the decisions of the Maritime Safety Tribunal, it is selective to directly apply the cause of an accident as a source of the measures that can prevent its recurrence. A typical example of this is the expression: "when a seafarer neglects ordinary practice of seaman." The term "ordinary practice of seaman" has been criticized for being used in some decisions like a master key where it is not easy to determine which specific rules or regulations were violated or blame the involved seafarers. Such term is present in Article 2 of the International Regulations for Preventing Collisions at Sea 1972. For the proper use of the term, this paper seeks to compare and establish the concepts of "ordinary practice of seaman" and the duty of care by providing a systematic interpretation of the original text. In addition, the duty of care was reviewed from the perspective of administrative, civil, and criminal laws. Furthermore, relevant legal precedents were reviewed and presented in the study. Accordingly, it is expected that the term "ordinary practice of seaman" would be properly used in decisions that contribute to the prevention of the recurrence of similar marine accidents.

A Study on the Problems of elementary School Safety and its Improvement in Korea (우리나라 초등학교 학교안전의 문제점 및 개선방향에 관한 연구)

  • This research aims to orientation for new ways to improve school safety environment composition : The first study was the survey of various literature and status on primary school safety has been established in residential developments in urban areas were investigated and analyzed. This study was constructed as follows : The first chapter of the study is composed with the purpose, contents and configuration of study, definitions of terms, and consists of a limitation of such studies. And in chapter 2, we define such concepts, the range of school safety incidents. In Chapter 3, the nation's school safety situation was investigated, And in the fourth chapter introduced the practice of foreign school safety plan, Chapter 5 was to provide direction for future school safety and accident prevention plan. Including 12 new elementary school in Dongbaek district and Dongtan New-Town 20 new elementary schools in the metropolitan area of New-City were examined for safety issues, such as the environment of school, school zones, school premises form compositions and its outdoor and indoor facilities. This study was aimed at some of the nation's metropolitan New Town Elementary School. Therefore, there is a limit to this generalization. Also planned construction safety plan elements for the indoor facilities are further studies are needed for this hayeoteumeuro excluded. In addition, the architectural planning elements of the school safety needs additional studies is excluded.
