Promoting the Quarry Workers' Hazard Identification Through Formal and Informal Safety Training

  • Background: The surface mining industry has one of the highest fatality rates among private industries in the United States. Despite recent decreases in the fatality rates of comparable industries, the fatality rate in the surface mining industry has increased. Meanwhile, a lack of safety research in surface mining has hindered efforts to improve safety strategies in the surface mining workplace. Method: This study examined quarry workers' hazard identification skills by conducting a case study of a surface mining facility in the Mid-Atlantic region of the United States. Semistructured interviews were conducted with eight quarry workers who were employed at the mine facility. In addition to the interviews, data were collected through field notes, notes from an expert meeting with safety managers, and site photographs to explore quarry workers' safety behaviors in the workplace. Results: The results showed that quarry workers identified hazards and improved their safety performance by translating safety knowledge learned from training into practice, acquiring hands-on work experience, learning from coworkers, and sharing responsibilities among team members. Conclusion: This study contributes to understanding quarry workers' safe performance beyond what they have learned in safety training to include their interaction with other workers and hand-on experience in the workplace. This study informs practitioners in the surface mining industry to build a safe work environment as they design effective safety programs for employees.

  • Recently, there has been a large social issue about the environmental sector for safe zones from crime. Since 2010, starting with Seoul, most local governments have enacted ordinances related to crime prevention environmental design (hereinafter referred to as CPTED) and are showing active movement. In this study, as part of a method to understand how to use the design elements applied to CPTED, the safety village creation project of deteriorated construction districts in Daegu Metropolitan City was conducted, focusing on the 10 project destinations using CPTED, and the CPTED application elements were evaluated by CPTED. The analysis was conducted according to the basic principles, and the elements related to the architectural plan of the environmental design elements were reviewed. In order to extract the environmental design elements of the project site, the CPTED guideline appropriate for the sector was used based on the standard guidelines for environmental design for crime prevention in Daegu City and the contents of the crime prevention urban design ordinance.

Anatomical study to determine a new approach to treat benign masses located in the anterior condyle region: a case report

  • A 22-year-old male patient presented to the clinic with severe pain in the preauricular area with an inability to completely occlude the jaw. Facial computed tomography and magnetic resonance imaging revealed a well-defined lesion that was tentatively diagnosed as a benign tumor or cystic mass. Surgical approach of a lesion in the condyle is delicate and problematic as many vulnerable anatomical structures are present. There are several methods for surgery in this area. Typically, an extraoral approach is dangerous because of potential injuries to nerves and arteries. The intraoral approach also presents difficulties due to the lack of visibility and accessibility. On occasion, coronoidectomy may be performed. The goal here was to determine an easier and safer new surgical approach to the condyle. We reached the anterior part of the pterygoid plate in the same method as in Le Fort I surgery. From this point, through the external pterygoid muscle, approaching the anterior aspect of the condyle is relatively easy and safe, with minimal damage to the surrounding tissues. Pus was drained at the site, and the lesion was diagnosed as an abscess. Pain and inability to close the mouth resolved without recurrence.

Evacuation Safety Evaluation of High School according to Hydrogen Fluoride Leakage

  • The purpose is to evaluate evacuation safety by simulating the toxic effects of hydrogen fluoride leaks in areas surrounding national industrial complexes and to suggest alternatives for areas that do not satisfy evacuation safety. For human casualties caused by hydrogen fluoride leakage accidents, Available Safe Egress Time (ASET) is calculated by the toxic effects quantified with the Areal Locations of Hazardous Atmospheres (ALOHA), an off-site consequence assessment program. The Required Safe Egress Time (RSET) is calculated through Pathfinder, an evacuation simulation program. Evacuation safety is assessed by comparing ASET and RSET. The ALOHA program was used to evaluate the time to reach AEGL-2 concentration in 12 scenarios. The Pathfinder program was used to assess the total evacuation time of the high school among specific fire-fighting objects. Of the 12 accident scenarios, ASET was larger than RSET in the worst-case scenarios 1 and 9. For the remaining 10 accident scenarios, the ASET is smaller than the RSET, so we found that evacuation safety is not guaranteed, and countermeasures are required. Since evacuation safety is not satisfactory, we proposed to set up an evacuation area equipped with positive pressure equipment and air respirators inside specific fire-fighting objects such as the high school.

Modified Suturing Techniques in Carotid Endarterectomy for Reducing the Cerebral Ischemic Time

  • Objective : Carotid endarterectomy (CEA) is an effective surgical procedure for treating symptomatic or asymptomatic patients with carotid stenosis. Many neurosurgeons use a shunt to reduce perioperative ischemic complications. However, the use of shunting is still controversial, and the shunt procedure can cause several complications. In our institution, we used two types of modified arteriotomy suture techniques instead of using a shunt. Methods : In technique 1, to prevent ischemic complications, we sutured a third of the arteriotomy site from both ends after removing the plaque. Afterward, the unsutured middle third was isolated from the arterial lumen by placing a curved Satinsky clamp. And then, we opened all the clamped carotid arteries before finishing the suture. In technique 2, we sutured the arteriotomy site at the common carotid artery (CCA). We then placed a curved Satinsky clamp crossing from the sutured site to the carotid bifurcation, isolating the unsutured site at the internal carotid artery (ICA). After placing the Satinsky clamp, the CCA and external carotid artery (ECA) were opened to allow blood flow from CCA to ECA. By opening the ECA, ECA collateral flow via ECA-ICA anastomoses could help to reduce cerebral ischemia. Results : The modified suture methods can reduce the cerebral ischemia directly (technique 1) or via using collaterals (technique 2). The modified arteriotomy suture techniques are simple, safe, and applicable to almost all cases of CEA. Conclusion : Two modified arteriotomy suture techniques could reduce perioperative ischemic complications by reducing the cerebral ischemic time.

Identification of an effective and safe bolus dose and lockout time for patient-controlled sedation (PCS) using dexmedetomidine in dental treatments: a randomized clinical trial

  • Background: This study investigated a safe and effective bolus dose and lockout time for patient-controlled sedation (PCS) with dexmedetomidine for dental treatments. The depth of sedation, vital signs, and patient satisfaction were investigated to demonstrate safety. Methods: Thirty patients requiring dental scaling were enrolled and randomly divided into three groups based on bolus doses and lockout times: group 1 (low dose group, bolus dose 0.05 ㎍/kg, 1-minute lockout time), group 2 (middle dose group, 0.1 ㎍/kg, 1-minute), and group 3 (high dose group, 0.2 ㎍/kg, 3-minute) (n = 10 each). ECG, pulse, oxygen saturation, blood pressure, end-tidal CO2, respiratory rate, and bispectral index scores (BIS) were measured and recorded. The study was conducted in two stages: the first involved sedation without dental treatment and the second included sedation with dental scaling. Patients were instructed to press the drug demand button every 10 s, and the process of falling asleep and waking up was repeated 1-5 times. In the second stage, during dental scaling, patients were instructed to press the drug demand button. Loss of responsiveness (LOR) was defined as failure to respond to auditory stimuli six times, determining sleep onset. Patient and dentist satisfaction were assessed before and after experimentation. Results: Thirty patients (22 males) participated in the study. Scaling was performed in 29 patients after excluding one who experienced dizziness during the first stage. The average number of drug administrations until first LOR was significantly lower in group 3 (2.8 times) than groups 1 and 2 (8.0 and 6.5 times, respectively). The time taken to reach the LOR showed no difference between groups. During the second stage, the average time required to reach the LOR during scaling was 583.4 seconds. The effect site concentrations (Ce) was significantly lower in group 1 than groups 2 and 3. In the participant survey on PCS, 8/10 in group 3 reported partial memory loss, whereas 17/20 in groups 1 and 2 recalled the procedure fully or partially. Conclusion: PCS with dexmedetomidine can provide a rapid onset of sedation, safe vital sign management, and minimal side effects, thus facilitating smooth dental sedation.

Hair Diameter Variation in Different Vertical Regions of the Occipital Safe Donor Area

  • Background Little is known concerning hair diameter variation within the safe donor area for hair transplantation surgery. Thicker or thinner hair may be needed, depending on the recipient area, hairline design, and the purpose of surgery. Methods Twenty-seven patients (7 men and 20 women; mean age, 28 years; range, 20-47 years) were included in this study. The midoccipital point was used as the reference point on the horizontal plane at the upper border of the helical rim. The target area width was 15 cm (7.5 cm to the right and left of the reference point) and the height was 8 cm (2 cm above and 6 cm below the reference point). The study area was divided horizontally into 3 5-cm sections (A, B, C) and vertically into 4 2-cm sections (1-4), creating a total of 12 zones. Ten anagen hairs were randomly obtained from each zone and their diameters were measured. Results Hair diameter in the 4 vertical sections varied significantly, gradually decreasing from sections 1 (superior) to 4 (inferior) in all 3 horizontal sections (A, B, and C). Conclusions Our results suggest that sections 1 and 2 of the occipital safe donor area would be useful for obtaining thicker hair, such as in procedures to treat male- and female-pattern hair loss, whereas hair from zones 3 and 4 could be useful for transplantation surgery requiring thinner hair, such as eyebrows, eyelashes, and female hairline correction. Our results may be clinically valuable for planning hair transplant surgery and choosing the optimal donor region.

해상풍력발전단지의 대체통항로 통항안전성 평가에 관한 연구 (Study on the Vessel Traffic Safety Assessment for Routeing Measures of Offshore Wind Farm)

  • 본 연구에서는 대정해상풍력단지 설치 해역과 인근수역을 이용하는 선박교통량과 해상교통흐름의 패턴을 분석하고 단지 조성 후 합리적인 대체통항로 지정에 따른 교통량을 예측하였다. 또한 예측된 교통량을 근거하여 통항로별 통항안전성을 평가하고 검토함으로써 선박의 안전운항에 필요한 제반조건 및 대책을 제시하였다. 풍력단지 설치 해역과 인근수역의 해상교통흐름 패턴을 분석한 결과 총 8가지의 교통흐름으로 분류되었고, 연간 교통량은 8,975 척으로 예측되었다. 이를 근거로 단지 조성 후 4가지의 대체 통항로 지정에 따른 교통량을 예측하였다. 예측된 교통량과 SSPA의 동력선충돌모델을 이용하여 통항안전성을 평가한 결과 충돌 및 침로이탈확률에 관한 국내 안전기준 $10^{-4}$ 이하를 만족하므로 계획된 대체통항로가 유용함을 확인하였다.

Applying Fire Risk Analysis to Develop Fire-safe Modular Walls: Guidance to Material Selection, Design Approach and Construction Method

  • For the past decade, South Korea had experienced catastrophic building fires, which resulted in consider-ably high number of casualties. This motivated research to develop fire-safe wall assemblies. In this study Fire Risk Analysis (FRA) is conducted as part of the project designing phase to ensure fire safety of the final product. Traditional approach was to consider fire performance at the end of the designing stage, when PASS/FAIL fire test results are required to be submitted to the Authority Having Jurisdiction (AHJ). By applying a fire risk analysis to guide the designing phase, overall fire safety of a wall assembly can be achieved more systematically as conducting FRA allows designers to clearly identify elements that are more vulnerable to fire and simply replace them with other practical options. Severity of fire risk is determined by considering the fire hazards of a wall assembly such as the exterior layer, insulation, vertical connectivity, and external ignition sources (e.g., photovoltaic panels). Frequency of fire risk is assessed based on the factors affecting fire likelihood, which are air cavity and fire-stopping applied in the design, and random design changes occurring during on-site construction. Fire risk matrix is proposed based on these fire risk factors and efforts to reduce the fire risk level associated with the wall assembly are given by systematically assessing the fire risk factors identified from fire risk analysis. Current study demonstrates how fire risk analysis can be applied to develop fire-safe walls by reducing the relevant fire risks- both severity and frequency.

주변 환경요소를 고려한 자전거 도로 설계 개선 및 정보제공에 관한 연구 (A Study on the Design Improvement and Information Service of Bicycle Road Considering Environmental Factor)

  • 이 연구의 목적은 자전거 이용과 관련하여 주변의 환경요소를 고려한 안전하고 편리한 자전거 도로 설계 개선 및 정보제공방안에 대하여 연구하는데 있다. 이 연구에서는 기존 자전거 도로 설계 및 시공사례 분석을 통하여 현장여건을 반영한 자전거 도로 설계 방안을 수립하고 자전거 도로의 유형별 교통사고 유형 분석, 현황측량, 현지조사 등을 통하여 안전하고 편리한 자전거 도로 건설을 위한 자전거 도로의 유형별 최적 설계방안을 제시하였다. 안전한 자전거 도로 설계를 위해서는 주변환경 요소를 고려한 제원, 폭원, 곡선반경, 오르막차로 등에 대한 자전거 도로 최적설계와 안전표지 안전시설 분리시설 설치를 통하여 교통수단별 이용자간의 통행분리가 이루어져야 함을 알 수 있었다. 또한 안전한 자전거 도로 설계를 위해서는 최소한의 자전거 이용자의 주행 공간 및 자전거 도로 간의 연결성이 확보되어야 하며 자전거 이용자의 활성화를 위해서는 경사, 노선정보, 편의시설위치, 대중교통 연계정보 등 자전거도 도로의 안전 및 편의관련정보를 제공하여야 할 것으로 판단된다.
