장대터널에서의 암반 블록의 수치 모델링을 이용한 터널 안전 시공법 (Safe tunneling method using numerical modeling of rock blocks in long tunnels)

  • 국토의 70%이상이 산악 지형인 우리나라에서는 지형적인 특성으로 터널 건설이 중요하며, 터널 건설의 증가로 안전한 터널 건설이 중요한 문제로 대두되고 있다. 최근, 도로의 고속화와 직선화로 인해 장대터널 건설이 증가하고 있다. 본 연구에서는 장대터널에서의 암반 블록의 수치 모델링을 이용한 터널 안전 시공법을 제안하고, 현장에서 관찰한 불연속면 정보를 근거로 하여 실제 장대터널에 적용했다. 실제 터널현장에 있어서, 암반 블록의 수치 모델링을 이용한 터널 안전 시공법을 위해서 새롭게 개발한 수치해석 프로그램을 사용하여 정확한 키블럭 추출이 가능한 것을 검증하였다. 사용하기 쉬운 사용자 인터페이스를 가지고 있는 본 컴퓨터 시뮬레이션 기법은 암반 블럭의 안정성 계산뿐만 아니라 추가 보강대책공의 설계도 가능하다.

  • Shield TBM tunneling method has been getting the spotlight for urban tunneling. It can be minimized the civil complaint during construction and possible safe tunneling. But the settlement has occurred inevitably due to characteristics of shield TBM equipment. For this reason, the civil complaint can occur in urban areas when tunnel with shallow depth passes through neighboring building or residential area. In this study, the occurrence factors of settlement according to shield TBM tunneling and the tendency of ground settlement by strata condition had analyzed. It is suggested that the practical settlement estimation method and minimizing method of ground settlement under simultaneous backfill grouting condition through measurement results and back analysis data using gap parameter.

  • 최근 국내에서 쉴드TBM을 이용하는 터널공사의 발주가 증가하고 있으나 터널구간의 특성을 고려한 장비제작을 위한 정보의 제공은 체계적이지 못하여, 적합한 장비설계를 하는데 어려움이 많다. 본 기술보고는 외국의 사례검토를 바탕으로 토압식 쉴드TBM의 장비발주를 위한 설계항목과 세부 요구사양의 구성에 대해 국내에서 활용 가능한 내용을 예시로서 제시하였다. 제안하는 내용을 바탕으로 발주자는 장비 발주 시 필요한 전반적인 방향을 제시가능하고, 입찰자는 명확하고 과업조건에 적합한 장비를 설계, 제작하여 민원이 없고, 안전한 토압식 쉴드TBM 터널공사의 활성화에 기여할 수 있을 것으로 기대된다.

  • Tunnelling to bypass major landslide areas is considered as a good and long-term environmentally friendly solution to reduce an existing hazard. In Norway, hundreds of kilometres of tunnels have been constructed in areas prone to landslides and snow avalanches. Although tunnelling is considered as an expensive mitigation strategy for bypassing landslides, analysis indicate that in some cases the cost of building a tunnel can be repaid by savings in driving costs (fuel) alone over a period of 5-10 years due to reduced driving distances. The other benefits of constructing tunnels in landslide areas include savings in time and increased safety. The Norwegian Method of Tunnelling (NMT) is considered safe, efficient and cost effective compared to other tunnelling techniques. Some aspects of NMT, which are considered safe and cost efficient, are presented. The application of updated rock support techniques, including reinforced ribs of shotctrete (RRS), which is a key component of the Norwegian Method of Tunnelling (NMT), is highlighted.

Natural Orifice Transluminal Endoscopic Surgery and Upper Gastrointestinal Tract

  • Since the first transgastric natural orifice transluminal endoscopic surgery was described, various applications and modified procedures have been investigated. Transgastric natural orifice transluminal endoscopic surgery for periotoneoscopy, cholecystectomy, and appendectomy all seem viable in humans, but additional studies are required to demonstrate their benefits and roles in clinical practice. The submucosal tunneling method enhances the safety of peritoneal access and gastric closure and minimizes the risk of intraperitoneal leakage of gastric air and juice. Submucosal tunneling involves submucosal tumor resection and peroral endoscopic myotomy. Peroral endoscopic myotomy is a safe and effective treatment option for achalasia, and the most promising natural orifice transluminal endoscopic surgery procedure. Endoscopic full-thickness resection is a rapidly developing natural orifice transluminal endoscopic surgery procedure for the upper gastrointestinal tract and can be performed with a hybrid natural orifice transluminal endoscopic surgery technique (combining a laparoscopic approach) to overcome some limitations of pure natural orifice transluminal endoscopic surgery. Studies to identify the most appropriate role of endoscopic full-thickness resection are anticipated. In this article, I review the procedures of natural orifice transluminal endoscopic surgery associated with the upper gastrointestinal tract.

  • Excavation of long tunnels by shielded TBMs is a safe, fast, and efficient method of tunneling that mitigates many risks related to ground conditions. However, long-distance tunneling in great depth through adverse geological conditions brings about limitations in the application of TBMs. Among various harsh geological conditions, squeezing ground as a consequence of tunnel wall and face convergence could lead to cluttered blocking, shield jamming and in some cases failure in the support system. These issues or a combination of them could seriously hinder the performance of TBMs. The technique of excavation has a strong influence on the tunnel response when it is excavated under squeezing conditions. The Golab water conveyance tunnel was excavated by a double-shield TBM. This tunnel passes mainly through metamorphic weak rocks with up to 650 m overburden. These metamorphic rocks (Shales, Slates, Phyllites and Schists) together with some fault zones are incapable of sustaining high tangential stresses. Prediction of the convergence, estimation of the creeping effects and presenting strategies to overcome the squeezing ground are regarded as challenging tasks for the tunneling engineer. In this paper, the squeezing potential of the rock mass is investigated in specific regions by dint of numerical and analytical methods. Subsequently, several operational solutions which were conducted to counteract the challenges are explained in detail.

토조실험과 수치해석을 이용한 막장면 그라우팅 DSM공법의 안정성 검토 (Inspecting Stablity of DSM method with Grouting on Tunnel Face using Chamber Test and Numericlal Analysis)

  • 도심지에서는 고도의 경제성장과 더불어 증가되는 교통량을 수용하기 위하여 도로와 철도 신설에 따른 지하터널 공사가 활발히 진행되고 있다. 지하공간 개발에서는 토사터널 구간이 많을 수 있고 이에 따른 막장 및 천단부 안정성을 고려한 여러 공법들이 개발 적용되고 있다. 그 중 최근에 적용예가 많은 공법으로 DSM(divided shield method)를 들 수 있는데, 이는 Messer Shield 공법에 근간을 두고 있으며, Messer Shield 공법의 장점을 흡수하고 문제점을 개선하여 안정성 및 시공성을 크게 개선한 비개착 특수터널 공법이다. 이 연구에서는 DSM 공법에서 터널 막장면 그라우팅이 터널의 시공성 향상에 미치는 영향을 대형 토조와 수치해석을 수행하여 검토하였다. 실제도로 크기의 1/2인에 해당하는 터널을 모사하기 위한 토조를 제작하여 실험 수행하였다. 또한 MIDAS GTS를 사용하여 수치해석 분석을 통해 DSM모사 토조의 지반 거동과 비교하였다. 터널 표면 침하와 안정성을 단계별 굴착을 통해 측정하였으며, 수치해석과 비교분석하였다. 연구 결과 그라우팅을 통한 막장 안정을 지표의 침하뿐만 아니라 터널 각 부재의 응력은 모두 안정성 범위에 들어가 있었으며, 이를 통하여 DSM 공법의 기술을 개선하고 터널의 안정성 및 시공성 향상을 기대할 수 있는 기초자료를 확보하였다.

  • 서울지하철 7, 9호선에 적용한 쉴드TBM은 소음 및 진동, 막장면의 안정성 확보 측면에서 발파공법에 비해 매우 안전한 공법이다. 또한, 쉴드TBM은 지반조건이 연약한 경우 효과적이다. 그러나 지반조건과 토피, 지하수 등의 여건에 적합하지 않은 장비를 적용할 경우 장비고장, 잦은 굴진중단, 특히 예상치 못한 불량한 지반을 통과하게 될 경우 터널 굴진 중 트러블이 발생할 수 있다. 본 논문은 일본 쉴드TBM 굴착중의 사고 및 트러블에 대한 사례를 정리하였다. 이중 대표적인 트러블 사례를 이용하여 지반변위 형상에 대한 실제발생 경향을 파악하였으며 수치해석을 통해 역해석결과와 상호비교하였다. 이를 토대로 막장 지보압을 산정할 경우 수치해석 적용시의 오차를 줄이는 방안을 제안하였다.

The pedicled anterolateral thigh flap for trochanteric pressure sore reconstruction: Technical notes to optimize surgical outcomes

  • Background The pedicled anterolateral thigh (ALT) flap has become more popular for the reconstruction of soft-tissue defects in neighboring areas. Nonetheless, few studies in the literature have explored the use of this flap for trochanteric ulcer reconstruction. The aim of our study is to present the author's experience of utilizing the ALT flap, with a focus on technical elements regarding the flap design and the tunneling method to maximize the reach of the flap. Methods The medical records of patients who received pedicled ALT flaps for the reconstruction of trochanteric pressure sores were retrospectively reviewed. The patients' demographic data, operative details, and postoperative complications were evaluated. Results Between October 2018 and December 2019, 10 consecutive patients (age range, 13-45 years) underwent 11 pedicled ALT myocutaneous flaps for trochanteric pressure sore reconstruction. Each flap was designed around the most distal cutaneous perforator that was included in the proximal third of the skin paddle. The flaps ranged in size from 11×6 to 14×8 cm. The ALT flap was transposed through a lateral subcutaneous tunnel in five patients, while the open tunnel technique was used in six patients. All flaps survived, and no vascular compromise was observed. Conclusions The pedicled ALT flap is a safe and reliable option for reconstructing trochanteric pressure sores. An appropriate flap design and a good choice of the tunneling method are crucial for successful flap transposition.

Umbrella Arch 공법에 의한 터널 천단부 보강시 주변 지반의 거동에 관한 연구 (A Study on the Ground Movement around Tunnel Reinforced by Umbralla Arch Method)

  • Soil and rock improvement and reinforcement techniques are applied to achieve safe tunnel excavation in difficult geological conditions. The Umbrella Arch Method(UAM), one of the auxiliary techniques, is used to reduce ground permeability and improve stabtility of the tunnel by inserting a series of steel pipes into ground around the crown inclined to the longitudinal axis of the tunnel. Additionally, multi-step grouting is added through the steel pipes. UAM combines the advantages of a modern forepoling system with the grouting injection method. This technique has been applied in subway, road and utility tunneling sites for the last few years in Korea. This paper presents the results of analysis of the case studies on ground movements associated with UAM used in the Seoul Subway line 5 constructon site. Improvement of tunnel stability and decrease of ground settlement expected with pipe insertion are also discussed. Finally, the method to minimize ground settlements caused by NATM tunnelling are suggested.
