Stromal vascular fraction injection to treat intractable radiation-induced rectovaginal fistula

  • Rectovaginal fistula, which can arise after an injury to the vaginal canal or rectum, is a troublesome obstacle for patients' everyday life. In most cases, it can be covered with a local flap, but previous radiation therapy increases the recurrence rate, making it especially difficult to cure. As the application of stromal vascular fraction (SVF) obtained from enzymatically digested autologous adipose tissue has become increasingly common, several reports have advocated its effectiveness for the treatment of refractory wounds. In light of the angiogenic, regenerative characteristics of SVF, it was incorporated as a treatment option in two cases of rectovaginal fistula discussed here. As described in this report, irradiated rectovaginal fistulas in rectal cancer patients were successfully treated with SVF injection, and we suggest SVF as a feasible treatment option for cases of rectovaginal fistula that would otherwise be very difficult to cure.

An Analysis of Observational Environments for Solar Radiation Stations of Korea Meteorological Administration using the Digital Elevation Model and Solar Radiation Model (수치표고모델과 태양복사모델을 이용한 기상청 일사 관측소 관측환경 분석)

  • In order to analyze the observational environment of solar radiation stations operated by the Korea Meteorological Administration (KMA), we used the digital elevation model (DEM) and the solar radiation model to calculate a topographical shading, sky view factor (SVF) and solar radiation by surrounding terrain. The sky line and SVF were calculated using high resolution DEM around 25 km of the solar stations. We analyzed the topographic effect by analyzing overlapped solar map with sky line. Particularly, Incheon station has low SVF whereas Cheongsong and Chupungryong station have high SVF. In order to validation the contribution of topographic effect, the solar radiation calculated using GWNU solar radiation model according to the sky line and SVF under the same meteorological conditions. As a result, direct, diffuse and global solar radiation were decreased by 12.0, 5.6, and 4.7% compared to plane surface on Cheongsong station. The 6 stations were decreased amount of mean daily solar radiation to the annual solar radiation. Among 42 stations, eight stations were analyzed as the urgent transfer stations or moving equipment quickly and more than half of stations (24) were required to review the observational environment. Since the DEM data do not include artifacts and vegetation around the station, the stations need a detail survey of observational environment.

The effective implementation of adaptive second-order Volterra filter (적응 2차 볼테라 필터의 효율적인 구현)

  • In this paper, we propose an efficient method for implementing the adaptive second-order Volterra filter. To reduce computational load, the UCFD-SVF has been proposed. The UCFD-SVF, however, shows deteriorated convergence performance. We propose a new method that initializes the adaptive filter weights periodically on the fact that the energy of the filter weights is slowly increased. Furthermore, we propose another method that the interval for the weight initialization is variable to guarantee the performance and we shows the method gives the better performance under the non-stationary environment through the computer simulation for the adaptive system identification.

  • The conventional indexes for describing the GNSS positioning performance such as satellite visibility, dilution of precision (DOP) and signal to noise ratio is very useful in open sky, however, they are not useful in the land road environment. In this paper new index is suggested for describing the GNSS positioning performance for the road environment. The new index is called Sky View based DOP (SVDOP). SVDOP is derived referring the Sky-View-Factor (SVF). The usefulness is analyzed by comparing the SVDOP and SVF in land road environment after the singularity points are selected and SVDOP is calculated.

Numerical Simulation of Flow Characteristics behind a Circular Patch of Vegetation using a Two-Dimensional Numerical Model (2차원 수치모형을 이용한 원형군락 하류의 흐름특성 수치모의)

  • This paper presents numerical simulations of flow around a circular patch of vegetation using a depth-averaged two-dimensional numerical model which is capable of simulating flow structure in vegetated open channel. In order to account for vegetation effect, drag force terms are included in governing equations. Numerical simulations are conducted with various solid volume fractions (SVF). Flow passes through a circular patch and low velocity region, which is called wake region, is formed downstream of the patch. When SVF is larger than 0.08, a recirculation is observed. The location of the recirculation is moved further downstream as SVF decreases. Von-$K{\acute{a}}rm{\acute{a}}n$ vortex street is developed beyond the wake region due to interaction between two shear layers induced by a circular patch of vegetation. The vortex is developed as SVF is larger than 0.08, and the location of the vortex is consistent with the maximum of turbulence kinetic energy. The location of the peak of turbulence kinetic energy is moved further downstream as SVF decreases.

Efficacy and Safety of Autologous Stromal Vascular Fraction in the Treatment of Empty Nose Syndrome

  • Objectives. Regenerative treatment using stem cells may serve as treatment option for empty nose syndrome (ENS), which is caused by the lack of turbinate tissue and deranged nervous system in the nasal cavity. We aimed to assess the efficacy and safety of the autologous stromal vascular fraction (SVF) in the treatment of ENS. Methods. In this prospective observational clinical study, we enrolled 10 ENS patients who volunteered to undergo treatment of ENS through the injection of autologous SVF. Data, including demographic data, pre- and postoperative Sino-Nasal Outcome Test-25 (SNOT-25) scores, overall patient satisfaction, and postoperative complications, were prospectively collected. Nasal secretion was assessed using the polyurethane foam absorption method, and the levels of biological markers were analyzed in both ENS group and control group using enzyme-linked immunosorbent assay. The SVF extracted from abdominal fat was diluted and injected into both inferior turbinates. Results. Among the 10 initial patients, one was excluded from the study. Subjective satisfaction was rated as "much improved" in two and "no change" in seven. Among the improved patients, the mean preinjection SNOT-25 score was 55.0 and the score at 6 months after injection was 19.5. However, the average SNOT-25 score of nine participants at 6 months after injection (mean${\pm}$standard deviation, $62.4{\pm}35.8$) did not differ significantly from the baseline SNOT-25 score ($70.1{\pm}24.7$, P>0.05, respectively). Among the various inflammatory markers assessed, the levels of interleukin $(IL)-1{\beta}$, IL-8, and calcitonin gene-related peptide were significantly higher in ENS patients. Compared with preinjection secretion level, the nasal secretions from SVF-treated patients showed decreased expressions of $IL-1{\beta}$ and IL-8 after injection. Conclusion. Although SVF treatment appears to decrease the inflammatory cytokine levels in the nasal mucosa, a single SVF injection was not effective in terms of symptom improvement and patient satisfaction. Further trials are needed to identify a more practical and useful regenerative treatment modality for patients with ENS.

Design of a 60 Hz Band Rejection FilterInsensitive to Component Tolerances (부품 허용 오차에 둔감한 60Hz 대역 억제 필터 설계)

  • In this paper, we propose a band rejection filter (BRF) with a state variable filter (SVF) structure to effectively remove the influence of 60 Hz line frequency noise introduced into the sensor system. The conventional BRF of the SVF structure uses an additional operational amplifier (OPAMP) to add a low pass filter (LPF) output and a high pass filter (HPF) output or an input signal and a band pass filter. Therefore, the notch frequency and the notch depth that determine the signal attenuation of the BRF greatly depend on the tolerance of the resistors used to obtain the sum or difference of the signals. On the other hand, in the proposed BRF, since the BRF output is formed naturally within the SVF structure, there is no need for a combination between each port. The notch frequency of the proposed BRF is 59.99 Hz, and it can be confirmed that it is not affected at all by the tolerance of the resistor through the Monte Carlo simulation results. The notch depth also has an average of -42.54dB and a standard deviation of 0.63dB, confirming that normal operation as a BRF is possible. Also, with the proposed BRF, noise filtering was applied to the electrocardiogram (ECG) signal that interfered with 60 Hz noise, and it was confirmed that the 60 Hz noise was appropriately suppressed.

Automatic Phonetic Segmentation of Korean Speech Signal Using Phonetic-acoustic Transition Information (음소 음향학적 변화 정보를 이용한 한국어 음성신호의 자동 음소 분할)

  • This article is concerned with automatic segmentation for Korean speech signals. All kinds of transition cases of phonetic units are classified into 3 types and different strategies for each type are applied. The type 1 is the discrimination of silence, voiced-speech and unvoiced-speech. The histogram analysis of each indicators which consists of wavelet coefficients and SVF (Spectral Variation Function) in wavelet coefficients are used for type 1 segmentation. The type 2 is the discrimination of adjacent vowels. The vowel transition cases can be characterized by spectrogram. Given phonetic transcription and transition pattern spectrogram, the speech signal, having consecutive vowels, are automatically segmented by the template matching. The type 3 is the discrimination of vowel and voiced-consonants. The smoothed short-time RMS energy of Wavelet low pass component and SVF in cepstral coefficients are adopted for type 3 segmentation. The experiment is performed for 342 words utterance set. The speech data are gathered from 6 speakers. The result shows the validity of the method.

Observation and Analysis of Long and Short-wave Radiation According to Types of Summer Shelters (하계 그늘쉼터 유형별 장·단파복사 관측과 해석)

  • In this study, we analyzed the relationship between five factors: long-wave radiation, short-wave radiation, cloudiness, SVF and summer shelters. In the previous study, we recognized the correlation between single building SVF and long-wave radiation. Furthermore, this study attempted to confirm the relationship at the summer shelter with high solar radiation blocking rate. The observations are as follows. ① Cooling in summer shelters was not the effect of temperature but the effect of radiation reduction due to short-wave radiation shielding. ② In the case of the canopy tent with low heat capacity, the long-wave radiation was observed to be 16.7% higher per hour than the comparison control point due to the increase in surface temperature. ③ The long-wave radiation increase rate was different according to SVF, but showed very similar pattern according to the material characteristics of the summer shelters. ④ Passive Cooling effect on the type of summer shelters are determined by the size of the total long and short-wave radiation at that point.

Performance Improvement of Automatic Speech Segmentation and Labeling System (자동 음성분할 및 레이블링 시스템의 성능향상)

  • Database segmented and labeled up to phoneme level plays an important role in phonetic research and speech engineering. However, it usually requires manual segmentation and labeling, which is time-consuming and may also lead to inconsistent consequences. Automatic segmentation and labeling can be introduced to solve these problems. In this paper, we investigate a method to improve the performance of automatic segmentation and labeling system, where Spectral Variation Function(SVF), modification of silence model, and use of energy variations in postprocessing stage are considered. In this paper, SVF is applied in three ways: (1) addition to feature parameters, (2) postprocessing of phoneme boundaries, (3) restricting the Viterbi path so that the resulting phoneme boundaries may be located in frames around SVF peaks. In the postprocessing stage, positions with greatest energy variation during transitional period between silence and other phonemes were used to modify boundaries. In order to evaluate the performance of the system, we used 452 phonetically balanced word(PBW) database for training phoneme models and phonetically balanced sentence(PBS) database for testing. According to our experiments, 83.1% (6.2% improved) and 95.8% (0.9% improved) of phoneme boundaries were within 20ms and 40ms of the manually segmented boundaries, respectively.
