The Effects of an Insight-oriented Nursing Program on Schizophrenic Patients' Insight, Positive and Negative Syndromes, and Depression (병식-지향 간호프로그램이 정신분열병 환자의 병식, 양성.음성증상, 우울에 미치는 효과)

  • Purpose: This study examined the effects of an insight-oriented nursing program on schizophrenic patients' insight, positive and negative syndromes, and depression. Method: A non-equivalent control group non-synchronized design was utilized. For this research, a total of 34 subjects were selected at a psychiatric hospital in Daegu during the period ranging from Oct. 1st, 2005 to Feb. 11th, 2006 (experimental group=17, control group=17). Result: After the treatment, between the experimental and control groups, there wasa significant difference in the level of SUMD(t=-4.52, p=.000) and positive syndrome(t=6.97, p=.013), but no significant difference in the negative syndrome(t=-1.81, p=.079) and depression(t=2.23, p=.145). Conclusion: The results of this study show that the insight-oriented nursing program works well for schizophrenic patients. Therefore, it is recommended that this program should be used in clinical practice as an effective nursing intervention for schizophrenic patients.

Are steroids required in the treatment of ganglion impar blockade in chronic coccydynia? a prospective double-blinded clinical trial

  • Background: Ganglion impar blockade is a reliable and effective treatment option used in patients with coccydynia. Our primary objective was to specify the role of corticosteroids in impar blockade. We compared applications of local anesthetic with the local anesthetic + corticosteroid combination in terms of treatment efficiency in patients with chronic coccydynia. Methods: Our study was a prospective randomize double-blind study. The patients were divided into 2 groups after randomization. The first group (group SL) was made up of patients where a corticosteroid + local anesthetic were used during ganglion impar blockade. In the second group (group L) we used only local anesthetic. We evaluated numeric rating scale (NRS) and Beck depression scale, which were employed before the procedure and in 1st and 3rd months after the procedure. Results: Seventy-three patients were included in the final analysis. We detected a significantly greater decrease in NRS values in the 1st month in group SL than in group L (P = 0.001). In the same way, NRS values in the 3rd month were significantly lower in the group with steroids (P = 0.0001). During the evaluation of the Beck test, we detected significantly greater decreases in the 1st month (P = 0.017) and 3rd month (P = 0.021) in the SL group than in the L group. Conclusions: Ganglion impar blockade decreases pain in the treatment of chronic coccydynia and improve depression. Addition of steroids in a ganglion impar blockade is required for treatment response that should accumulate over a long period of time.

Stress, Coping, and Depression in Patients Following Hemopoietic Stem Cell Transplantation (조혈모세포이식 환자의 스트레스, 대처 및 우울)

  • Purpose: This study was done to identify the level of stress, coping, and depression among hemopoietic stem cell transplantation patients who received care in an outpatient center. Method: Data were collected from 81 patients who underwent hemopoietic stem cell transplantation at C University S Hospital between August 2005 and February 2006. Results: Stress and depression were significantly higher following hemopoietic stem cell transplantation in women, and in patients who were worse off financially or who were in bad health. The highest item of stress was 'economic burden for treatment'. There were significantly higher levels of emotion-focused coping among patients who had a spouse and who received motivation from the doctor. The highest item for problem-focused coping was 'try to look at the bright side of life'. The highest items for emotion-focused coping were to 'have faith that treatment will be finished quickly' and 'believe that your situation will improve'. Stress was significantly correlated with depression among these patients. Conclusion: It is necessary to develop nursing interventions to enhance positive coping and to decrease stress and depression among patients who have a hemopoietic stem cell transplantation.

The Effect of Morning Stretching on Depression and Motivation of Rehabilitation for Stroke Patients (뇌졸중 환자의 우울과 재활동기에 대한 아침체조의 적용 효과)

  • Purpose: This study was to examine the effect of morning stretching on depression and motivation of rehabilitation for stroke patients. Method: Design of this study was noneqivalent control pre-test post-test design. Subjects were total 62 patients (Experimental: 30 subjects, Control: 32 subjects) being hospitalized for stroke in a national medical center. Measures were CES-D for depression and PAREMO for motivation of rehabilitation. Data were collected from 1st March to 30th July, 2004. Data have been processed using SPSS/WIN 11.0. Results: The results of this study were as followings: 1) Experimental group was showed lower level of depression than that of control group (t=8.934, P=.000). 2) Experimental group was showed higher motivation of rehabilitation than control group (t=-12.43, P=.000). Conclusion: As a result, it was confirmed that morning stretching which was intervened in this study was one of the nursing intervention that lowers the level of depression and promotes motivation of rehabilitation for stroke patients, that helps to improve their psychological and mental problems.

  • Objectives : The purpose of this study was to investigate the depressive change and predictors of chronic illnesses and to examine whether depressive change predictors are different according to chronic disease types. Methods : Data from the 1st to the 10th year of the Korean Welfare Panel of 5,023 adults over 20 years of age who were treated for chronic diseases for more than three months were examined. We assessed demographic, sociological, and socioeconomic factors including depression predictors of chronically ill patients, and analyzed the data using latent growth modeling and multiple group analysis. Results : Depression symptoms of chronically ill patients gradually decreased, showing a linear relationship across time. In addition, multiple group analysis was performed for each type of chronic disease. Conclusions : We described the phenomenon of depression in chronically ill patients, and discussed the practical and policy interventions required to improve depression in chronically ill patients.

The Relationship between Eating Disorders, Self-esteem and Depression among in College Women (여대생의 섭식장애행동과 자아존중감 및 우울간의 관계)

  • Purpose: The purpose of this study was to investigate the relationship between eating disorder, self-esteem and depression in college women. Method: Participants were 282 college women. Data were collected between Nov. 1st and 30th, 2003, and provide basic data on eating disorder levels in college women and basic data for health control programs. The instrument was a questionnaire consisting of 8 items on general characteristics and weight control, 24 on eating disorders, 10 on self-esteem and 10 on depression. Analysis of the data was done using numbers, percentages, means and standard deviations, t-test and Pearson correlation coefficients. SPSS WIN 10.0 was used in data analysis. Result: The greatest difference for eating disorders was between the group with normal body weight and the group with low body weight (t=-6.94, p=.000). There was a high positive correlation between Body Mass Index and eating disorder (r=.383, p<.01), between eating disorder and depression (r=.161, p<.01). There was a high negative correlation between eating disorder and self-esteem (r=-.196, p<.01), and between self-esteem and depression (r=-.537, p<.01). Conclusion: These results indicate that college women need more education and counseling on dietary concerns. Also, systematic efforts to establish a more health conscious social standard for beauty should be taken. Further empirical and experimental studies are required to investigate factors influencing attitudes towards eating held by college women and to determine variables which affect various specific dimensions of these attitudes.

Menopause and Cognitive Function : Hospitalized Female Patients with Depression (여성 우울증 입원 환자에서 폐경 여부에 따른 인지기능의 차이)

  • Objectives Although forgetfulness is a common complaint among menopausal depressed women, there is still a debate about the relationship between memory impairment and menopause. The aim of this study is to examine whether menopause is related to cognitive decline among women with depressive disorders. We hypothesized that postmenopausal depressed women show generally poorer performance than premenopausal depressed women on various cognitive function tests. Methods With a retrospective chart review, we identified a total of 87 female patients (45 premenopausal patients and 42 postmenopausal patients) who were hospitalized with depressive disorders from 2000 to 2016. Demographic and clinical variables and cognitive test results were compared between two groups. Results Education year is longer in premenopausal group than postmenopausal group whereas clinical characteristics (illness duration, recurrence, and symptom severity) and mean Intelligence Quotient (IQ) were similar between two groups. The postmenopausal group took longer time for Bender-Gestalt Test (BGT) recall, Trail Making Test (TMT)-A, and TMT-B than the premenopausal group. After controlling for age and education, significant difference was remained for BGT recall (p = 0.029). Conclusions Postmenopausal state may be related with decline of visuospatial memory function, in particular, among depressed female patients. Other areas of cognitive function including complex attention, verbal memory, auditory memory, and working memory might be interpreted while considering age and education level.

Path Model for the Family and School Environment Affecting Adolescent Suicidal Ideation Mediated by Depression (우울증을 매개로 한 청소년의 자살구상에 관한 가족 및 학교환경의 경로분석 모델)

  • This study tests the path model which examines how the variables of past and present family dynamics, school adjustment, filed life events affect the adolescents'depression which in tum influencing their suicidal ideation. In the path model, the exogenous varialbes were negative parenting experience in childhood, maladjustment history of the family (e.g., alcohol, divorce), present parent-adolesecent communication, school adjustment, filed life events were used, whereas the adolescent depression was used as the mediated variable. The endogenous variable was the adolescent suicidal ideation. The respondents of the survey were 635 adolescent who were 1st and 2nd grade of high school located in Seoul and Busan. Using SPSS for Windows, the path analysis was done for male and female adolescents separately, since female adolescents were found to report significantly higher degree of depression and suicidal ideation then were male adolescents. The results of the path analysis were as followings. First, for both the male and female adolescents, depression was the strongest predictor of their suicidal ideation. Also, school adjustment strongly affected the adolescents'depression as well as their suicidal ideation. Meanwhile, filed life events indirectly influenced the suicidal ideation only through the depression. Second, for the male adolescents, negative parenting experience in childhood affected both the depression and suicidal ideation, while the parent-adolescent communication directly affected the suicidal ideation. Third, for the female adolescents, maladjustment family history and negative parenting experience did not affect their depression, but the suicidal ideation. Meanwhile, parent-adolescent communication was a significant predictor of female adolescents'suicidal ideation as well as their depression. In conclusion, discussion on different path of male and female adolescents'suicidal ideation was provided.

The Relationship between Elderly Poverty and Depression Trajectories (노년기 빈곤궤적과 우울궤적의 관계 연구)

  • The aim of this study was to investigate both poverty and depression among older adults, focusing on the relationship of these two trajectories. For expanding the understanding about elderly poverty and depression, the study measured the longitudinal patterns of various transition in these two variables. The data for the study is 1st to 9th waves (2006-2014) of Korea Welfare Panel Study (KoWePS), and 4,431 older adults were used for the final analysis. For data analysis, Semi-parametric group-based modeling and Dual trajectory model were selected. The main results of this study were followings; First, The trajectory groups were identified: non-poverty, decrease poverty, increase poverty, remain high-poverty, chronic poverty groups and 4 trajectories of depression: stable, remain low-depression, risk of depression, chronic depression groups. Second, the study was tried to anticipate the longitudinal transition of poverty and depression status, and investigate the concurrent relationship in these two variables. It turned out that the stable poverty status led the stable depression, and vice versa. Based on these result, this study for elderly welfare were discussed to reduce risk for poverty and depression.

Point Prevalence of Anxiety and Depression among Cancer Inpatients in a General Hospital : A Preliminary Study for the Development of Distress Management Strategies in Cancer Patients (일 종합병원 입원 암환자의 불안과 우울증상의 시점 유병률 : 암 환자 디스트레스 중재 전략을 위한 예비 연구)

  • Objectives: This study aimed to examine the point prevalence of anxiety, depression and the associated risk factors in hospitalized patients with cancer. Methods: A total of 114 patients with cancer who admitted to the Cancer Center in St. Mary's Hospital were enrolled for a day. The patients were asked to complete Hospital Anxiety and Depression Scale(HADS). Sociodemographic and cancer related clinical variables were also evaluated. Results: The point prevalence of anxiety and depression was 10.5% and 34.2%, respectively, as assessed with HADS. Logistic regression models revealed that distant metastasis was significantly associated with anxiety in patients with cancer and previous chemotherapy and Low socioeconomic status were significantly associated with depression. Conclusions: Our results indicate that more than one third of patients with cancer suffer from either anxiety or depression. In addition, previous chemotherapy as well as low socioeconomic status and distant metastasis were associated risk factors of anxiety and depression, respectively. These findings suggest the need for screening and systematized psychosocial intervention for psychological distress in hospitalized patients with cancer.
