DeNitrification-DeComposition (DNDC) Improvement through Model Coupling and Sub-model Development Considering Agricultural Land Use and Future Climate Change

  • Climate change is the biggest concern of the $21^{st}$ century. Greenhouse gas (GHG) emissions from various sectors are attracting attention as a cause of climate change. The DeNitrification-DeComposition (DNDC) model simulates GHG emissions from cropland. To study future GHG emissions using this simulation model, various factors that could change in future need to be considered. Because most problems are from the agricultural sector, DNDC would be unable to solve the factor-changing problem itself. Hence, it is necessary to link DNDC with separate models that simulate each element. Climate change is predicted to cause a variety of environmental disasters in the future, having a significant impact on the agricultural environment. In the process of human adaptation to environmental change, the distribution and management methods of farmland will also change greatly. In this study, we introduce some drawbacks of DNDC in considering future changes, and present other existing models that can rectify the same. We further propose some combinations with models and development sub-models.

  • Purpose: This study was conducted to find the change of knowledge, attitude, and practice on musculoskeletal diseases after preventive exercise program. Method: The 169 subjects who worked at an electronic parts manufacturing company in Seoul were participated in 11 week exercise program from February 14, to April 29, 2005. Result: The change of complaint rate of musculoskeletal symptoms based on NIOSH diagnosis standard for each body part was not statistically significant. The change of knowledge, attitude and practice on musculoskeletal diseases after exercise program was statistically significant. The change of knowledge after exercise program was higher in the group of participation days(17-32 days), work department(electronic components dept., mold manufacturing dept., and condenser dept.). The change of attitude after exercise program was higher in the group of work department(electronic components dept.) and job position(employee). The change of practice after exercise program was higher in the group of participation days(17-32 days), and job position(employee). Conclusion: The continuous and regular exercise program will help to prevent musculoskeletal diseases.

Neurometabolic Improvement in Parkinson's Disease after Stereotactic Functional Neurosurgery by Follow-up MR Spectroscopy

  • PURPOSE - To investigate neurometabolism from the brain destructive lesions and striatal putamen-pallidus regions to the clinically worst side in patients with Parkinson's disease after stereotactic functional neurosurgery. METHODS - Using proton magnetic resonance spectroscopy ($^1$H MRS), fifteen patients (7 males and 8 females; mean age 56.5 years; age range 43-67 years) with Parkinson's disease (PD) were studied to measure N-acetylaspartate (NAA), creatine (Cr), choline-containing compounds (Cho) and lactate (Lac) levels on the neurosurgical lesions of thalamus, globus pallidus and striatal putamen-pallidus regions in a brain. RESULTS - Brain destructive lesion and striatal putamen-pallidus region in PD compared with controls were highly and significantly related to NAA/Cho ratios reduction, respectively (P =0.002, P =0.04), but showed no difference from the same regions of PD prior to neurosergery (P =0.06, P =0.77). Increased lactate peaks at 1.3 ppm were present in all the cerebral lesions, and these resonances were confirmed at a long TE =136 ms, indicating that these signals distinguished from lipids. CONCLUSIONS - Our results suggest that NAA/Cho ratios may provide as a neurometabolite marker for neurochemical changes in brain surgical lesion, and the ratios might be related to functional change of neuropathophysiological status in the striatal putamen-pallidus region of PD. Increase of lactate signals, being remarkable in surgical lesions, could be consistent with a common consequence of surgical necrosis. Therefore, MR spectroscopy could be a sensitive diagnostic tool in monitoring neurometabolic changes in PD with neurosurgical treatment.

웨이브렛 변환을 이용한 스트레스 심전도 분석 알고리즘의 개발 (Development of a Stress ECG Analysis Algorithm Using Wavelet Transform)

  • 본 논문에서는 스트레스 심전도를 분석함에 있어서 가장 중요한 파라미터인 ST 세그먼트를 측정하기 위해서 웨이브렛 변환을 이용하여 Wavelet Adaptive Filter(WAF)와 QRS콤플렉스 검출 알고리즘을 설계하였다. WAF는 웨이브렛 변환부와 적응필터부로 구성되어 있으며, 웨이브렛 변환부에서는 웨이브렛 함수를 이용하여 입력되는 심전도 신호를 저주파 대역과 고주파 대역으로 각각 j=-7레벨까지 분할하고, 적응필터부에서는 웨이브렛 변환에 의해 분할된 신호중 j=-7레벨의 저주파 대역 신호를 주입력으로 사용하여 필터링 한다. QRS 콤플레스는 합산신호를 구성한 후 문턱치를 RR간격에 변화에 따라 변화시키면서 검출하였으며, 합산신호는 웨이브렛 변환에 의해 QRS 콤플렉스의 주파수 성분이 포함되어 있는 고주파 대역의 신호를 더하여 구성하였다. WAF는 표준 필터와 일반적인 적응필터와 성능을 비교하였으며, 잡음제거 특성과 신호왜곡도 측면에서 비교필터에 비해 우수한 성능을 보였다. QRS 콤플렉스 검출성능을 평가하기 위해서 MIT/BIH데이터베이스를 이용하여 기존의 QRS 검출 알고리즘들의 검출 방법과 비교하였으며, 웨이브렛에 의한 합산신호를 이용할 경우에 99..67%로써 더 좋은 검출성능을 보였다. 또한 측정된 ST세그먼트의 정확도를 비교.평가를 위하여 European ST-T 데이터베이스와 실제 임상데이터를 이용하였으며 심박수의 변화에 따라 적응적으로 ST세그먼트를 측정할 수 있었다.

위장 비율동성 당뇨환자에서 족삼리(足三里)($ST_{36}$) 전침처치의 위전도 조절 영향 사례군 연구 (Assessment of Regulatory Effect of Electro-acupuncture at Jogsamni($ST_{36}$), on Electrogastrography in Diabetic Patients with Gastric Dysrhythmia;A Case Series Study)

  • Objectives : This study was to investigate the regulatory effect of electro-acupuncture on Jogsamni ($ST_{36}$) in bradygastric condition or gastroparesis which is a neuropathic complication of type 2 diabetes mellitus(T2DM), assessed using 4-channel electrogastrography (EGG). Methods : It was a case series study. Each patient was given electro-acupuncture(EA) one point, Jogsamni($ST_{36}$) or Gyeonu($LI_{15}$) bilaterally for thirty minutes with 2Hz bipolar square wave frequency and moderate intensity of stimulation. With attaching electrodes around stomach region throughout pre-acupuncture, acupuncture, and post-acupuncture session, these parameters were produced; percent rate of bradygastria power compared to sum of power in all kind of gastric rhythm(% bradygastria), dominant frequency(DF) and dominant power(DP). Parameter was analysed per each channel's variable change from pre-acupuncture session by paired t-test. Results : There was statistically significant decrease in % bradygastria parameters on one of the $ST_{36}$ treated patients from pre-acupuncture session and to post-acupuncture session(P=0.015, P=0.008 respectively). A marginal significant decrease of DP was shown in the other $ST_{36}$ treated patient from pre-acupuncture session through post-acupuncture session(P=0.049). Combined data of two $ST_{36}$ Treated patients showed that there was significant decrease of %bradygastria from pre-acupuncture session to acupuncture session(P=0.020), and was decrease of DP from pre-acupuncture session to post- acupuncture session(P=0.020). Conclusions : EA at $ST_{36}$ resulted in statistically significant decrease in %bradygastria and DP for dysrhythmic condition in T2DM gastroparesis patients. Considering this limited results, further study will be needed to elucidate the effect of multiple-acupuncture on gastric myoelectrical activities.

Effect of coadministration of enriched Korean Red Ginseng (Panax ginseng) and American ginseng (Panax quinquefolius L) on cardiometabolic outcomes in type-2 diabetes: A randomized controlled trial

  • Background: Diabetes mellitus and hypertension often occur together, amplifying cardiovascular disease (CVD) risk and emphasizing the need for a multitargeted treatment approach. American ginseng (AG) and Korean Red Ginseng (KRG) species could improve glycemic control via complementary mechanisms. Additionally, a KRG-inherent component, ginsenoside Rg3, may moderate blood pressure (BP). Our objective was to investigate the therapeutic potential of coadministration of Rg3-enriched Korean Red Ginseng (Rg3-KRG) and AG, added to standard of care therapy, in the management of hypertension and cardiometabolic risk factors in type-2 diabetes. Methods: Within a randomized controlled, parallel design of 80 participants with type-2 diabetes (HbA1c: 6.5-8%) and hypertension (systolic BP: 140-160 mmHg or treated), supplementation with either 2.25 g/day of combined Rg3-KRG + AG or wheat-bran control was assessed over a 12-wk intervention period. The primary endpoint was ambulatory 24-h systolic BP. Additional endpoints included further hemodynamic assessment, glycemic control, plasma lipids and safety monitoring. Results: Combined ginseng intervention generated a mean ± SE decrease in primary endpoint of 24-h systolic BP (-3.98 ± 2.0 mmHg, p = 0.04). Additionally, there was a greater reduction in HbA1c (-0.35 ± 0.1% [-3.8 ± 1.1 mmol/mol], p = 0.02), and change in blood lipids: total cholesterol (-0.50 ± 0.2 mmol/l, p = 0.01), non-HDL-C (-0.54 ± 0.2 mmol/l, p = 0.01), triglycerides (-0.40 ± 0.2 mmol/l, p = 0.02) and LDL-C (-0.35 ± 0.2 mmol/l, p = 0.06) at 12 wks, relative to control. No adverse safety outcomes were observed. Conclusion: Coadministration of Rg3-KRG + AG is an effective addon for improving BP along with attaining favorable cardiometabolic outcomes in individuals with type 2 diabetes. Ginseng derivatives may offer clinical utility when included in the polypharmacy and lifestyle treatment of diabetes. Clinical trial registration: Clinicaltrials.gov identifier, NCT01578837;

Changes in Dermatomal Somatosensory Evoked Potentials according to Stimulation Intensity and Severity of Carpal Tunnel Syndrome

  • Objective : To investigate the change of latency of cervical dermatomal somatosensory evoked potential (DSEP) according to stimulation intensity (SI) and severity of carpal tunnel syndrome (CTS). Methods : Stimulation sites were the C6, C7, and C8 dermatomal areas. Two stimulation intensities $1.5{\times}$sensory threshold (ST) and $2.5{\times}ST$ were used on both normal and CTS patients. Results : In moderate CTS, the latencies of C6 and C7 DSEP during $1.5{\times}ST$ SI and those of C7 DSEP during $2.5{\times}ST$ SI were significantly delayed compared with the values of normal subjects. Significant correlation between the latency of C7 DSEP of $2.5{\times}ST$ stimulation and the median sensory nerve conduction velocity was observed. Conclusion : We suggest that these data can aid in the diagnosis of cervical sensory radiculopathy using low stimulation intensity and of those who have cervical sensory radiculopathy combined with CTS patients.

Three-dimensional analysis of pharyngeal airway change of skeletal class III patients in cone beam computed tomography after bimaxillary surgery

  • Introduction: To evaluate the 3-dimensional changes in the pharyngeal airway of skeletal class III patients after bimaxillary surgery. Materials and Methods: The study sample consisted of 18 Korean patients that had undergone maxillary setback or posterosuperior movement and mandibular bilateral sagittal split osteotomy setback surgery due to skeletal class III malocclusion (8 males, 10 females; mean age of 28.7). Cone beam computed tomography was taken 1 month before and 6 months after orthognathic surgery. Preoperative and postoperative volumes of the nasopharyngeal, oropharyngeal, and laryngopharyngeal airways and minimum axial areas of the oropharyngeal and laryngopharyngeal spaces were measured. Moreover, the pharyngeal airway volume of the patient group that had received genioplasty advancement was compared with the other group that had not. Results: The nasopharyngeal and laryngopharyngeal spaces did not show significant differences before or after surgery. However, the oropharyngeal space volume and total volume of pharyngeal airway decreased significantly (P<0.05). The minimum axial area of the oropharynx also decreased significantly. Conclusion: The results indicate that bimaxillary surgery decreased the volume and the minimum axial area of the oropharyngeal space. Advanced genioplasty did not seem to have a significant effect on the volumes of the oropharyngeal and laryngopharyngeal spaces.

편측 안면신경 마비를 동반한 Eagle 증후군 치험례 (Eagle's Syndrome Presenting Unilateral Facial Nerve Palsy: Case Report)

  • Eagle's syndrome is the manifestation of elongated styloid process that is not a common entity in dental clinics. Without the knowledge of this syndrome misdiagnosis can be made and therefore differential diag-nosis with other diseases of oral and maxillofacial area is required. The symptoms of elongated styloid process are cervical pain, foreign body sensation, dysphagia and pharyngeal pain. A 27 years old male vis-ited our hospital. He had cervical pain with unilateral facial nerve palsy. Along with meticulous clinical examinations, 3D-CT, Plain radiographic studies were used to make the diagnosis. In computed tomogra-phy, Lt. styloid process was elongated and fractured. Patient have undergone surgical resection of elongat-ed styloid process and have been carefully observed. Characteristically, preoperative facial nerve palsy showed rapid postoperative recovery. The other clinical symptoms that were present in the initial visit were diminished. The purpose of this article is to report a case of Eagle's syndrome presenting unilateral facial nerve palsy that was treated with transoral surgical approach with literature review.

Evaluation of SPECT Analysis in Patients with Transient Global Amnesia

  • Objectives: This study investigated alterations in regional cerebral blood flow (rCBF) in patients with transient global amnesia (TGA) using statistical parametric mapping 99 (SPM99). Methods: Noninvasive rCBF measurements using 99mTc-ethyl cysteinate dimer (ECD) SPECT were performed on 8 patients with TGA who have ongoing symptoms and 17 age matched controls. The relative rCBF maps in patients with TGA and controls were compared. Results: In patients with TGA, significant decreased rCBF was found along the L superior temporal extending to L parietal region of the brain and L thalamus. There were areas of increased rCBF in the R temporal, R frontal region and R thalamus. Conclusion: We could demonstrate decreased perfusion in left cerebral hemisphere and increased perfusion in right cerebral hemisphere in patients with TGA using SPM99. The imbalanced change of rCBF between bilateral cerebral hemisphere in patients with TGA might suggest that imbalanced neuronal activity between the bilateral hemispheres may have strong relationship to the pathogenesis of the TGA. For quantitative SPECT analysis in TGA patients, we recommend SPM99 rather than the ROI method because of its definitive advantages.
