Effects of Head Posture on the Rotational Torque Movement of Mandible in Patients with Temporomandibular Disorders (두경부 위치에 따른 측두하악장애환자의 하악 torque 회전운동 분석)

  • The purpose of this study was to evaluate the effect of specific head positions on the mandibular rotational torque movements in maximum mouth opening, protrusion and lateral excursion. Thirty dental students without any sign or symptom of temporomandibular disorders(TMDs) were included as a control group and 90 patients with TMDs were selected and examined by routine diagnostic procedure for TMDs including radiographs and were classified into 3 subgroups : disc displacement with reduction, disc displacement without reduction, and degenerative joint disease. Mandibular rotational torque movements were observed in four head postures: upright head posture(NHP), upward head posture(UHP), downward head posture(DHP), and forward head posture(FHP). For UHP, the head was inclined 30 degrees upward: for DHP, the head was inclined 30 degrees downward: for FHP, the head was positioned 4cm forward. These positions were adjusted with the use of cervical range-of-motion instrumentation(CROM, Performance Attainment Inc., St. Paul, U.S.A.). Mandibular rotational torque movements were monitored with the Rotate program of BioPAK system (Bioresearch Inc., WI, U.S.A.). The rotational torque movements in frontal and horizontal plane during mandibular border movement were recorded with two parameters: frontal rotational torque angle and horizontal rotational torque angle. The data obtained was analyzed by the SAS/Stat program. The obtained results were as follows : 1. The control group showed significantly larger mandibular rotational angles in UHP than those in DHP and FHP during maximum mouth opening in both frontal and horizontal planes. Disc displacement with reduction group showed significantly larger mandibular rotational angles in DHP and FHP than those in NHP during lateral excursion to the affected and non-affected sides in both frontal and horizontal planes(p<0.05). 2. Disc displacement without reduction group showed significantly larger mandibular rotational angles in FHP than those in any other head postures during maximum mouth opening as well as lateral excursion to the affected and non-affected sides in both frontal and horizontal planes. Degenerative joint disease group showed significantly larger mandibular rotational angles in FHP than those in any other head postures during maximum mouth opening, protrusion and lateral excursion in both frontal and horizontal planes(p<0.05). 3. In NHP, mandibular rotational angle of the control group was significantly larger than that of any other patient subgroups. Mandibular rotational angle of disc displacement with reduction group was significantly larger than that of disc displacement without reduction group during maximum mouth opening in the frontal plane. Mandibular rotational angle of disc displacement without reduction group was significantly larger than that of disc displacement with reduction group or degenerative joint disease group during maximum mouth opening in the horizontal plane(p<0.05). 4. In NHP, mandibular rotational angles of disc displacement without reduction group were significantly larger than those of the control group or disc displacement with reduction group during lateral excursion to the affected side in both frontal and horizontal planes. Mandibular rotational angle of disc displacement without reduction group was significantly smaller than that of the control group during lateral excursion to the non-affected side in frontal plane. Mandibular rotational angle of disc displacement without reduction group was significantly larger than that of disc displacement with reduction group during lateral excursion to the non-affected side in the horizontal plane(p<0.05). 5. In NHP, mandibular rotational angle of the control group was significantly smaller than that of disc displacement with reduction group or disc displacement without reduction group during protrusion in the frontal plane. Mandibular rotational angle of disc displacement without reduction group was significantly larger than that of the disc displacement with reduction group or degenerative joint disease group during protrusion in the horizontal plane. Mandibular rotational angle of the control group was significantly smaller than that of disc displacement without reduction group or degenerative joint disease group during protrusion in the horizontal plane(p<0.05). 6. In NHP, disc displacement without reduction group and degenerative joint disease group showed significantly larger mandibular rotational angles during lateral excursion to the affected side than during lateral excursion to the non-affected side in both frontal and horizontal planes(p<0.05). The findings indicate that changes in head posture can influence mandibular rotational torque movements. The more advanced state is a progressive stage of TMDs, the more influenced by FHP are mandibular rotational torque movements of the patients with TMDs.

The Change and Relationship between Maternal Role Strain and Husband's Support in First-time Mothers with regard to their Employment Status (취업유무에 따른 초산모의 배우자 지지와 역할긴장간의 관계 및 변화)

  • This study attempted to determine the relation between the maternal role strain and their husbands' support in employed and nonemployed first-time mothers, and how it changed over time. A descriptive, longitudinal and comparative study design was conducted to collect and analyzed the data at three times(at 3-4days, at 4-6wks and at 3-month postpartum) regarding the change of maternal role strain and their husbands' support in employed and nonemployed first-time mothers. The subjects were 133 first-time mothers who delivered vaginally at K university hospital in Deagu, Korea from Dec., 1994 to Oct., 1995. Among these 58 were employed and 75 were nonemployed first-time mothers. This instruments used to data collection in this study were by Hobbs(1968 : The Difficulty Index for First-time Parents), Staffensmeier(1982: Transition Difficulty Measure), Tayer(1976: Emotional Support from Husband), Lee, Hae Kyoung(1992 : Physical Support from Husband). The analysis of data was done with SAS/PC program which included percentage, $x^2$-test, Pearson correlation, Repeated Measure ANOVA, Time Contrast Analysis and t-test. The results were as follows : 1. There was a significant negative correlation only at 3-4days(r=-.19, P=.0265) and at 4-6wks postpartum(r=-.18, P=.0392) between maternal role strain and husband's support of the first-time mothers. Thus, the more support from husband, the less maternal role strains the first-time mothers felt. Therefore, the 1st hypothesis that "the more husband's support the first-time mother felt at all the time of testing, the less maternal role strains they got" was partially supported at 3-days and 4-6 weeks postpartum. There was no correlation between maternal role strain and t husband's support of the employed first-time mothers at al testing times(at 3-4days postpartum : r=-.95, P=.9548, at 4-6wks postpartum : r=-.0960, P=.4733 and at 3-month postpartum : r=-.05, P=7306). On the contrary, the unemployed first-time mothers felt less maternal role strain when they received more support from their husband at 3-4 days postpartum(r=-.31, P=.0073) and at 4-6wks postpartum(r=-.23, P=.0490). 2. There was no difference of maternal role strain between two groups with regards to employment status(F=.97, P=.3270). But the maternal role strains of two groups were changed differently each other over time(F=3.89, P=.00234). Therefore, the 2nd hypothesis that "there was the difference in the maternal role strains with regard to employment status and over time" was rejected. 3. There was no difference in husband's support between the employed and the nonemployed first-time mothers(F=3.06, P=.0826). But there was a significant interaction between employment status and over time(F=3.64, P=.0267), so the support from husband of the employed and the unemployed first-time mothers was changed differently each other over time. The support from husband of the employed first-time mothers was lowered significantly and continuously at 4-6wks (F=5.20, P=.0263) and at 3-month postpartum(F=6.47, P=.0137) than at 3-4 days postpartum. On the contrary, there was no difference in change of husband's support of the employed first-time mothers between the 3-4 days and the 4-6wks postpartum(F=1.70, P=.1962) and between the 3-4 days and the 3-month postpartum(F=.21, P=.6513). Mean husband's support of the nonemployed first-time mothers was raised at 4-6wks postpartum than at 3-4 days postpartum but lowed at 3-month postpartum. The support form husband of both groups was tending downwards at 3-month postpartum than at 4-6wks postpartum, but the one between two groups was changed differently each other over time. The husband's support of the employed first-time mothers(M${\pm}$SD=64. 26${\pm}$8.63) was higher than the one of the nonemployed first-time mothers(M${\pm}$SD=59.16${\pm}$11.11) (t=-2.98, P=.0035), so the 3rd hypothesis that "there was the difference in the husband's support with regard to employment status and over time" was supported. On the basis of these conclusions, the following suggestion is proposed. This study examined the change and relationship between maternal role strain and husband's support in the employed and the nonemployed first-time mothers. So the further study regarding the comparison between the employed and the nonemployed multiparas is necessary.

Treat-to-Target Strategy for Asian Patients with Early Rheumatoid Arthritis: Result of a Multicenter Trial in Korea

  • Background: To evaluate the therapeutic benefits of the treat-to-target (T2T) strategy for Asian patients with early rheumatoid arthritis (RA) in Korea. Methods: In a 1-year, multicenter, open-label strategy trial, 346 patients with early RA were recruited from 20 institutions across Korea and stratified into 2 groups, depending on whether they were recruited by rheumatologists who have adopted the T2T strategy (T2T group) or by rheumatologists who provided usual care (non-T2T group). Data regarding demographics, rheumatoid factor titer, anti-cyclic citrullinated peptide antibody titer, disease activity score of 28 joints (DAS28), and Korean Health Assessment Questionnaire (KHAQ) score were obtained at baseline and after 1 year of treatment. In the T2T group, the prescription for disease-modifying antirheumatic drugs was tailored to the predefined treatment target in each patient, namely remission (DAS28 < 2.6) or low disease activity (LDA) ($2.6{\leq}DAS28$ < 3.2). Results: Data were available for 163 T2T patients and 162 non-T2T patients. At the end of the study period, clinical outcomes were better in the T2T group than in the non-T2T group (LDA or remission, 59.5% vs. 35.8%; P < 0.001; remission, 43.6% vs. 19.8%; P < 0.001). Compared with non-T2T, T2T was also associated with higher rate of good European League Against Rheumatism response (63.0% vs. 39.8%; P < 0.001), improved KHAQ scores (-0.38 vs. -0.13; P = 0.008), and higher frequency of follow-up visits (5.0 vs. 2.0 visits/year; P < 0.001). Conclusion: In Asian patients with early RA, T2T improves disease activity and physical function. Setting a pre-defined treatment target in terms of DAS28 is recommended.

Final Impact of Anti-Vascular Endothelial Growth Factor Treatment in Age-related Macular Degeneration (연령관련황반변성 환자에서 항혈관내피성장인자의 치료 방법과 재정 영향 분석)

  • Purpose: To evaluate the effects of anti-vascular endothelial growth factor (VEGF) treatment on the healthcare-related finances of patients with age-related macular degeneration. Methods: Changes in health care financing due to newly introduced benefit standards were predicted over the coming 5-year period (2018-2022). We also analyzed the financial impact of scenarios in which agents similar to anti-VEGF, such as the over-licensed drug bevacizumab, were introduced. For this purpose, the future number of patients receiving anti-VEGF treatments was estimated for various scenarios based on National Health Insurance Corporation claims data followed by an estimate of the financial burden. Results: In the case of age-related macular degeneration, the current standard of care (14 times in a lifetime) was maintained in scenario 1. In 2018, the insurance budget for the coming 5-year period was estimated at approximately 440.3 billion won. The insurance cost for that period was estimated at approximately 560.1 billion won under the revised standard of December 2017 (scenario 2). For scenarios wherein, after 2020, similar treatments (scenario 3) and bevacizumab (scenario 4) were introduced, the estimated health insurance costs were 521 billion won and 419.7 billion won, respectively. Conclusions: Health insurance costs are projected to increase substantially due to the elimination of the 14 time pay standard; however, the actual budget will only moderately increase, due to new limitations of visual acuity ${\leq}0.1$ or in case of scarring/atrophic lesions. Clinically similar agents and bevacizumab could be considered as alternatives to anti-VEGF treatment for age-related macular degeneration.

A Study on the Evaluation of Distribution Stability of Refrigerated Rice Cake Using Natural Antibiotics (천연항균제를 사용한 냉장떡류의 유통 안정성 평가연구)

  • Natural extract in liquid phase was adjusted to 0, 0.25, 0.5, 1, 2, and 4% concentration to check microbial changes and to measure 4, 8, $12^{\circ}C$ for refrigeration temperature. In the case of grapefruit extract, the microbial safety was maintained at all the concentrations at $4^{\circ}C$ storage, but the antimicrobial activity was maintained at $12^{\circ}C$ storage and at $8^{\circ}C$ and 21 days storage. In the case of grape seed extract, only the 4% of the culture at $8^{\circ}C$ satisfied the requirement of safety of food distribution for the last 21 days, and the safety criterion was satisfied only at 4% concentration at $12^{\circ}C$ for 18 days. Complex Scutellaria baicalensis extract showed the total number of microbial cells treated by concentration. It was confirmed that microbial flow safety was maintained at low temperature ($4^{\circ}C$). However, at $8^{\circ}C$ and $12^{\circ}C$, Exceeded the distribution limit. When polylysine was applied to brown rice cake, it showed activity in all groups except $4^{\circ}C$, but these properties were not observed at $8^{\circ}C$ and $12^{\circ}C$. At a concentration of 0.5% or more of chitosan, the growth of the microorganism is suppressed by the 21st day very stably, and a similar tendency is observed at 8 and $12^{\circ}C$, so that it may be an antimicrobial material that inhibits microorganisms. At the first day, the distribution standards for general bacterial counts were exceeded.Ethyl-pyruvate showed that microorganism safety was maintained at $4^{\circ}C$ and 1% concentration, and food safety was stable even at 2 or 4%. Glycine showed very good and stable distribution stability at $4^{\circ}C$. However, at $8^{\circ}C$ and $12^{\circ}C$, the shelf life of 14 days could not be maintained as with the addition of other antimicrobial active substances.

Meta-analysis of Intravitreal Injection of Anti-vascular Endothelial Growth Factors for Diabetic Macular Edema (당뇨황반부종에서 항혈관성장인자의 유효성과 안전성: 네트워크 메타분석)

  • Purpose: Intravitreal aflibercept, ranibizumab, bevacizumab, and dexamethasone are the most widely used drugs in the treatment of diabetic macular edema (DME). The aim of this study was to compare the efficacy and safety of anti-vascular endothelial growth factors and dexamethasone for the treatment of DME. Methods: There were nine previous systematic reviews on this topic; we updated these high-quality reviews. Seven studies were added to two studies following a literature search. Efficacy outcomes were 1) average improvement in visual acuity, 2) proportion of patients who experienced an improvement in vision (an increase in best-corrected visual acuity (BCVA) of ${\geq}15$ in the Early Treatment Diabetic Retinopathy Study [ETDRS]), and 3) proportion of patients who experienced worsening vision (a decrease in BCVA of ${\geq}15$ in the ETDRS). Safety outcomes included systemic adverse events and ocular-related adverse events. Results: The mean difference in the BCVA for ranibizumab versus bevacizumab treatment was 0.16 (95% confidence interval [CI]: -0.02, 0.34), and that for ranibizumab versus aflibercept was -0.08 (95% CI: -0.26, 0.10). The mean difference in the change of BCVA for aflibercept versus ranibizumab was -0.20 (95% CI: -0.40, -0.01), and that for aflibercept versus bevacizumab was -0.34 (95% CI: -0.53, -0.14). Other efficacy outcomes showed similar trends, and there was no significant difference between treatments. There was also no significant difference in both systemic and ocular adverse events rates between the treatments. Conclusions: In DME patients, the efficacy of aflibercept was found to be higher with respect to BCVA changes compared with ranibizumab or bevacizumab. However, there were no significant difference in terms of visual acuity improvement or visual acuity of more than 15 letters, nor in terms of anti-vascular endothelial growth factors (as a safety outcome).

Effects of Growth Traits on Reproductive Traits for Swine in Korea (종돈의 성장형질이 번식형질에 미치는 영향)

  • A Total of 48,101 performance records of sows for Yorkshire and Landrace breeds were collected from swine breeding farms in Korea from 2001 to 2008. A general ingredient analysis included the fixed effects of breed, parity, year, season, and farm. For the number of heads per 1st parity analysis by each growth traits, the data of 48,101 heads was used to analyze growth traits group. In the general ingredient analysis, the results showed high significance except for lean percentage by season (p<0.05). Average daily gain of Landrace breed ($640.48{\pm}0.749g$) was better than that of Yorkshire breed ($624.22{\pm}0.608g$), and the backfat thickness of Yorkshire breed ($13.44{\pm}0.030mm$) was thicker than that of Landrace breed ($12.50{\pm}0.037mm$). For the number of born alive and number of stillborn by growth traits for each breed, number of born decreased after test end day of 161 to 165 day, and average daily gain of 620 g to 640 g and the highest number of born appeared at the backfat thickness of 13 mm to 14 mm for yorkshire breed. In case of Landrace breed, number of born was the highest, and the number of stillborn increased together with average daily gain. The number of born was high when backfat thickness was less than 11 mm. The number of born trended to decrease when backfat thickness increased.

Studies on the Evaluation of Kenaf as a Bulking Agent in Livestock Composting (가축 분뇨의 퇴비화 수분 조절제로서 Kenaf의 유용성 평가)

  • Hibiscus cannabinus L is a plant in the Malvaceae family. Kenaf was seeded at June 1st in 2010 and harvested at November 18th and dried and evaluated worth as a bulking agent for livestock composting. Harvested and dried Kenaf was divided into the bast, core and leaf. All materials were grinded by hammer mill and the moisture absorption, moisture evaporation, pH, volume weight and C/N ratio were measured. Kenaf was higher water absorption and evaporation ability than those of sawdust and chaff. The pH values of Kenaf were pH $2.8{\pm}0.01$ - $4.34{\pm}0.02$, which is lower pH value than those of sawdust (pH $5.28{\pm}0.01$) and chaff (pH $6.3{\pm}0.02$). The C/N ratio of Kenaf showed 649 of core, 204 of bast and 70 of leaf, which were lower than sawdust (789.1) but higher than chaff (132). In volume weight test, the materials were divided by particle size of Kenaf, named as group A(${\geq}4cm$), B(${\leq}4cm$, ${\geq}0.25cm$) and C(${\leq}0.25cm$). The volume of weight of group A and B for core, bast and leaf showed similar, but group C showed higher than those of sawdust and chaff. Especially, the volume weight of group C for leaf was 5 times higher than those of sawdust and chaff. This study suggested the possibility of using Kenaf as a bulk agent for composting of livestock manure. This is considered that strengthen the competitiveness of farmers through reducing the cost, prevention of environmental pollution caused by livestock manure and environmentally friendly processing of livestock manure.

Influences of role conflict, workload, secondary traumatic stress and self-efficacy on burnout of social workers working with trauma survivors (외상생존자와 일하는 사회복지사의 소진에 대한 역할갈등, 업무부담과 이차적 외상 스트레스 및 자기효능감의 영향)

  • The purpose of this study is to examine the relationship between job characteristics such as role conflict, workload, and self-efficacy, secondary traumatic stress(STS), and burnout of social-workers who are working with survivors of child abuse, spouse abuse and sexual assault. While burnout has been studied over two decades all over the world, secondary traumatic stress(STS) is a less familiar term in the field of social work. Studies explaining the relationship between job-demands, STS, and burnout for social workers working with violence victims lack integrative insights across agencies serving children and adults. Thus, we examine whether the relationships between change based on wheter the survivor is a child or an adult. Two hundred thirty three social workers in Korea who work with violence victims participated in the survey. The data were analyzed with Amos 7.0. Results indicate that STS plays a key role in explaining social worker burnout across the agencies, and Child Protective Service workers were found to have the highest STS symptoms. The pathways to social worker burnout proved to be different based on who you served. Theoretical and practical implications of these findings were discussed.

A Study on Improvement of Laws regarding Welfare for the Aged (노인복지 관련법제의 발전방향)

  • Korea is expected to become an 'aged society' with more than 14 percent of the public aged 65 years or more by 2018. The rapid aging is giving rise to various problems within the society along with falling birthrate in a short period of time. In this context, the role and function of laws on welfare for the aged must be particularly emphasized. Also the Senior Citizens Welfare Act is of great importance as it provides social welfare service on the basis of functional connection with social insurance and public assistance. First, this paper looks into the history of laws related to welfare for the elderly such as the Senior Welfare Act, the Act on Long-term Care Insurance for Senior Citizens and the Basic Old Age Pension Act as well as the findings of earlier studies. In the second place, it will break down such laws by main components aiming to examine details of the laws and questions raised regarding them and to seek ways to achieve improvement with an emphasis on health care, old age income security, housing welfare(assisted living facilities), job security for the aged. The Senior Welfare Act offers substance of social welfare service for the elderly. Income security, health and medical care, welfare measures through long-term care and assisted living facilities, social participation by working are the key elements and all of them should be closely associated to ensure citizens get sufficient public support in their old age. For this purpose, the Senior Welfare Act is under a normative network with laws such as Act on Long-term Care Insurance for Senior Citizens and Basic Old Age Pension Act. Current laws on welfare for the aged including Senior Welfare Act are not sufficiently responsive to the aged society of the 21st century. Income security combined with decent social participation, health and medical care closely connected with long-term care system, efficient expense sharing between government and local government, enhancement of effectiveness of welfare measures can be considered as means to improve current welfare system so that the elderly can enjoy their old age with dignity and respect.
