The change of the Mechanoreceptor of Posterior Cruciate Ligament after Posterior Cruciate Ligament Injury in the Rabbit (토끼에서 후방십자인대 손상 후 후방십자인대의 기계적 수용기의 변화)

  • Purpose : To assess the change in the number of mechanoreceptors of posterior cruciate ligament(PCL) after transecting posterior cruciate ligament of the rabbit. Materials and Methods : We selected 10 skeletally-matured New Zealand white rabbits and one of the two posterior cruciate ligaments was transected and the other underwent sham-operation on each rabbit. Afterward, PCLs with synovium were obtained at femoral attach area in both knees in 20 weeks postoperatively, but in one of the cases, the transected PCL did not heal and had to be excluded from this study. Consequently, histologic examination of the ligament for the remaining 9 cases was done after being prepared with modified gold chloride stain. Results : The study showed a little bit more decrease in the number of mechanoreceptors in the transected group compared with that in the sham-operated group, but the number was statistically negligible. Conclusions : It is considered that the knee joint with injured PCL may still maintain proprioception since the decrease in the number of mechanoreceptors is not considerable, but further study on biomechanical role and function of mechanoreceptor of healed PCL is required.

Effect of Pressure Taping between Tibia and Fibula on Pain, ROM and Strength in Athletes diagnosed with High Ankle Sprain (원위경비인대결합 손상 선수의 경·비간 압박테이핑 적용이 통증, 관절가동범위, 근력에 미치는 영향)

  • This study was conducted to identify the effects of pressure taping between tibia and fibula of High Ankle Sprain athletes on pain, Range of Motion(ROM), and strength and to provide basic data for rehabilitation programs. The subjects of the study were conducted with a total of 10 athletes except for four who gave up who were diagnosed with high ankle sprain, or who were diagnosed with ankle sprain but their physical examinations proved positive. The results showed no significant differences in pain(Visual Analog Scale, VAS). The ROM was significantly increased in inversion(IV) and eversion(EV) in both groups. The Isometric strength was significantly improved in IV(0°, 7°, 14°) and EV(0°) in Taping Group(TG). When taping was applied to athletes with injury to the High Ankle Sprain, ROM and muscle strength improved at the same pain level.

  • This study was carried out to investigate the current trends in skiing-related research from existing literature in the field of kinematics, measurement sensor and computer simulation. In the field of kinematics, research is being conducted on the mechanism of ski turn, posture analysis according to the grade and skill level of skiers, friction force of ski and snow, and air resistance. In the field of measurement sensor and computer simulation, researches are being conducted for researching and developing equipment using IMU sensor and GPS. The results of this study are as follows. First, beyond the limits of the existing kinematic analysis, it is necessary to develop measurement equipment that can analyze the entire skiing area and can be deployed with ease at the sports scene. Second, research on the accuracy of information obtained using measurement sensors and various analysis techniques based on these measures should be carried out continuously to provide data that can help the sports scene. Third, it is necessary to use computer simulation methods to clarify the injury mechanism and discover ways to prevent injuries related to skiing. Fourth, it is necessary to provide optimized ski trajectory algorithm by developing 3D ski model using computer simulation and comparing with actual skiing data.

Retrospective Study of Traumatic Dental Injuries among Children Aged 0 - 15 Years in Wonju (원주세브란스기독병원 응급실로 내원한 0 - 15세 어린이의 치과적 외상에 관한 후향적 분석)

  • This study was designed to evaluate the age, gender, location of trauma, etiology, injury site, types of treatment, elapsed time after trauma, and arrival time of children who visited trauma center of Wonju Severance Christian Hospital. Records of a total of 841 patients who were 0 - 15 years old and received care in the period from March 2011 to October 2015 at the Trauma Center, Wonju Severance Christian Hospital were analyzed. This study showed that traumatic dental injuries were more common in boys and patients between 0 - 3 years old. Under 6 years old, fall was the most common etiology and home was the most common place of trauma. However, fall decreased, and sports and etc increased largely in etiologic factors over 6 years old. Besides, home decreased, and road and kindergarten school increased largely in the place of trauma. Etiology and location of trauma were statistically influenced by the age (p < 0.05). The most commonly affected injury sites were maxillary incisors and lips. The most patients visited trauma center between 18 - 24 o'clock (53.3%), and the least patients visited between 0 - 6 o'clock (4.6%). 51.5% of patients visited the trauma center within 1 hour of sustaining trauma, and 26.8% and 11.5% of patients visited between 1 - 2 hours and 2 - 3 hours respectively. The most common treatment of traumatic dental injuries was observation, and the second most common treatment was suture. Traumatic dental injuries in children exhibit specific epidemiological features according to children's gender, age, and other conditions. These result from combination of social, developmental, and physiologic factors.

Triathlon-Related Overuse Injury and Medical Issues (트라이애슬론의 과사용 부상과 의학적 위험요인에 대한 고찰)

  • As the nature of triathlons is competition in three successive sports, triathletes experience elevated levels of stress on the body that are absent in single-sport athletic events, and consequently there are more potential medical problems to prepare for. Triathletes can also experience problems such as hypothermia, heat illness, excessive exposure to ultraviolet radiation, musculoskeletal injuries and trauma, immunosuppression, and haemolysis. Depending on the potentiality of such above-listed problems occurring in any given race, race organizers will prepare preventative measures and treatments accordingly. Olympic distance is not the only triathlon racing distance. Sprints, which are normally around half the Olympic distance, are common distances, as well as Long (2 km swim, 80 km bike, 20 km run), Ironman (3.8 km swim, 180 km bike, 42 km run), and ultra-distance events varying in length. Races of longer duration normally result in a higher percentage of athletes experiencing the above-mentioned medical problems, as well as inducing additional health risks such as hyponatraemia. Minimizing the occurrences of serious health issues is possible through the following means: carefully preparing for the probable race-day weather conditions; proper management and organization of the race; preparing an extensive water-safety and ground-course safety plan; distributing necessary nutrition along the course; and stressing the importance for athletes to have proper knowledge concerning race nutrition, biomechanical technique, physical preparation, and utilization of safe equipment. While risks of competing in triathlon are many, the instances of such medical problems are not excessive, and the triathlon has a reputation of being a reasonably safe sport as long as athletes with high risk take added precautions.

A RECENT 5-YEAR RETROSPECTIVE STUDY ON NASAL BONE FRACTURE (비골 골절에 대한 최근 5년간의 후향적 연구)

  • Purpose: This study was performed to investigate the incidence, types of fracture, treatment, associated fracture and complications in patients with nasal bone fracture. Materials and methods: Clinical examination, patient's records and radiographic images were evaluated in 230 cases of nasal bone fractures who were treated at the Department of Oral and Maxillofacial Surgery, Chonnam National University Hospital for recent 5 years; from January 2002 to December 2006. Results: 1. The age of patient was ranged from 4 to 77 years (mean age=36.6 years); Males was 75.7% (n=174), and females 24.3% (n=56). 2. The cause of the nasal bone fracture in this study was a fall or slip down (28.8%, n=66), sports accident (26.0%, n=60), fighting (21.3%, n=49), traffic accident (9.6%, n=22), industrial trauma (7.8%, n=18), and the others (6.6%, n=15). 3. For the patterns of fracture, simple fracture without displacement occured in 10.4% (n=24). Simple fracture with displacement without septal bone fracture was found in 49.5% (n=114). Simple fracture with displacement in company with septal bone fracture showed in 32.6% (n=75). Commiuted fracture with severe depression was presented in 7.4% (n=17). 4. The reduction the displaced nasal bone was carried out in 2 to 10 days (mean 6.8 days) after the injury. 5. Nasal bone fracture associated with Le Fort I fracture (6.5%, n=6.5), Le Fort II fracture (7.4%, n=17), Le Fort III fracture (1.3%, n=3), NOE fracture (13.9%, n=32), ZMC fracture (17.4%, 40), maxillary bone fracture (8.3%, n=19), orbital blow-out fracture (15.7%, n=36), frontal bone fracture (1.3%, n=3) and alveolar bone fracture (10.9%, n=25). 6. The major type of treatment method was closed reduction in 90% (n=207), open reduction in 3% (n=7), and observation in 7% (n=16). 7. There were some complications such as ecchymosis, hyposmia, hypo esthesia and residual nasal deformity which are compatible. Open rhino-plasty was conducted for 3 patients who had residual nasal deformity. Conclusions: These results suggest that most of nasal bone fractures are occurred physically active aged groups(age 10-49 years) and could be treated successfully with closed reduction at 7 days after the injury.

Correlation between body alignment and foot pressure in high School Baseball Players (고등학교 야구선수들의 신체정렬과 족저압 분포 간의 상관관계 분석)

  • This study was undertaken to provide correct posture guidance and reference material for sports injury and injury prevention programs, by analyzing body alignment and plantar pressure distribution in high school baseball players. Totally, 32 subjects were enrolled for the study. Body alignment was measured from the trunk imbalance, kyphotic angle, lordotic angle, pelvic tilt, and pelvic torsion. Foot pressure was measured from the left maximum pressure, right maximum pressure, left weight, and right weight. The mean and standard deviation of the measured factors were calculated and tabulated, and the correlation between body alignment and foot pressure distribution was analyzed using Pearson's correlation analysis. All statistical significance levels were set at 0.05. Body alignment and foot pressure distribution show a positive correlation between the lordotic angle of lumbar spine and the left maximum pressure. There was no correlation between trunk imbalance, kyphotic angle, lordotic angle, pelvic tilt, pelvic torsion, and the left·right weight distribution and the left·right maximum pressure of each foot. Results of this study determined that the increase of the lordotic angle of lumbar spine results in more force on one side of the foot, thereby increasing the maximum pressure. We believe our data could be a reference for exercise programs on physical alignment and plantar pressure distribution of athletes.

Spur-like Lesion on the Lateral Tibial Condyle - A Sign of Chronic ACL tear - (경골 외과의 골극 유사 병변 - 만성 전방 십자 인대 파열의 징후 -)

  • Purpose : Authors experienced cases of chronic ACL tear with spur-like lesion on the tibial condyle which is different from the lateral capsular sign and degenerative change and evaluated the significance of 'spur-like lesion' in relations with chronic ACL injury. Material and Method : We have 5 patients with spur-like lesion on the lateral tibial condyle in simple radiogram. The location, shape and size of the lesion were studied using radiogram and MRI. Cause of injury, associated injury and chronicity of the ACL tear were analyzed. All 5 patients were male, and mean age was 33.8 $(17\~46)$ years. Result : The spur-like lesion was located from 3.8 mm(avg.) below the articular surface of the lateral tibial condyle to the apex of the fibular head and protruded laterally or inferolaterally from just posterior to the Gerdy's tubercle with a round or sharp-end triangular shape. Average length was 6 mm and average width ortho base was 9.2 mm. The cause of injury were sports jnjury 4 cases and traffic accident in one. The chronicity of the ACL tear was average 10.7(8 months$\~$23 years) years and medial meniscus tear was shown in all cases and lateral meniscus tear in three. Conclusion : We suggest that a patient who has a history of trauma with spur-like lesion on the lateral tibial condyle of the knee is expected to have chronic ACL tear.

  • Tricuspid regurgitation after blunt chest trauma is rarely seen in the emergency department. A 19-year-old patient visited our emergency department with chest discomfort after collision with his brother while skiing. Recently, Skiing as a winter sports has become popular with the Korean people, so there is an increasing tendency for patients with diverse traumas associated with ski accidents to visit the emergency department. From simple abrasions or contusions to deadly injuries with unstable vital signs, we are seeing many kind of injuries in the emergency department. We present the case report of a patient with tricuspid regurgitation after a blunt chest trauma during the skiing.

Effects of lower extremity stability by kinesio taping method in elite speed skating athletes' one-leg jumping (엘리트 빙상 선수들의 외발점프 훈련 시 키네시오 테이핑요법이 하지관절 안정성에 미치는 영향)

  • The purpose of this study was to investigating the effect of taping knee by testing the difference on kinetic variables of lower extremity when speed skating athletes jump on one leg. The results were as follows. The height of jumping after taping was higher, but the vertical height was not different according to taping. On take-off(TO), the horizontal and anterior-posterior maximum impulse force were decreased while the vertical maximum impact force was increased after taping. On landing(LD), the anterior-posterior maximum impulse force was decreased but the horizontal and vertical maximum impulse force were increased. TO, the impulse showed low after taping and the impulse dropped largely LD. The knee's moment of extension, eversion were reduced after tapping TO. LD, the flection moment of knee was decreased, but the inversion moment was increased after tapping. This study implies that the knee tapping helps injury prevention and performance enhancement, sports medicine convergence are needed.
