Objective: Several muscles surrounding neck are vital not only for neck motion, but for upper extremities motions as well. Neck pain would affect neck and shoulder disability. The Neck Disability Index (NDI), Shoulder Pain and Disability Index (SPADI), and Visual Analogue Scale (VAS) are increasingly used to evaluate treatment effectiveness after chronic neck pain. The purpose of this study was to analyze the correlation of neck pain, shoulder pain, and quality of life in patients with chronic neck pain. Design: Cross-sectional study. Methods: Forty-three patients with neck pain participated in this study. Participants were instructed to place a mark on each item of three clinical measures that best represented their experience of his/her neck and shoulder problem over the last week. The outcome measures of the study were NDI, SPADI, and VAS for neck and shoulder pain and disability. Results: The NDI was significantly correlated with the VAS-Neck, SPADI-Pain, SPADI-Disability, SPADI-Total, and VAS-Shoulder (p<0.05). The VAS-Neck was significantly correlated with the VAS-Neck, SPADI-Pain, SPADI-Disability, SPADI-Total, and VAS-Shoulder (p<0.05). The SPADI-Total was significantly correlated with the NDI, VAS-Neck, SPADI-Pain, SPADI-Disability, and VAS-Shoulder (p<0.05). The SPADI-Pain was significantly correlated with the NDI, VAS-Neck, SPADI-Disability, SPADI-Total, and VAS-Shoulder (p<0.05). The SPADI-Disability was significantly correlated with the NDI, VAS-Neck, SPADI-Total, SPADI-Pain, and VAS-Shoulder (p<0.05). The VAS-Shoulder was significantly correlated with the NDI, VAS-Neck, SPADI-Pain, SPADI-Disability, and SPADI-Total (p<0.05). Conclusions: Neck disability in patients with chronic neck pain should be considered with disability and pain of the shoulder.
Purpose: The purpose of this study was to investigate the correlation between shoulder pain and disability index (SPADI) with participation in badminton and to find the effects of badminton on the shoulder joint. Methods: A questionnaire was answered by 143 people with shoulder pain and disability in badminton clubs located in Busan, Gyeongsangnam-do. Out of the 143 subjects (91 males, 52 females), about half (72 people) were over 40 years old, with 71 people over 50 years old. The questionnaire contained 27 questions involving demographic participation characteristics, playing characteristics, and SPADI. Data analysis was performed using the SPSS statistics program to conduct frequency analysis, cross analysis, the t-test, and one-way analysis of variance. Results: The correlation between badminton career, frequency of play, and SPADI was statistically significant. There was a significant increase in SPADI scores for players over 40 years old. The over 40 year old group with less than 5 years of badminton had a significantly increase in SPADI score. The over 50 year old group showed a significantly lower SPADI score compared with the over 40 group. Conclusion: We suggest that beginner badminton players learn correct posture and badminton skills from professionals, participate regularly, and maintain an appropriate intensity/frequency of badminton.
Background: The prevalence of neck pain and neck dysfunction is high in general population. However, there is little literature on the relationship and factors affecting neck pain and neck dysfunction. Objective: To investigate the correlation between neck and shoulder pain, headaches, neck and shoulder dysfunction, and sleep quality in adults with chronic neck pain, and factors affecting neck pain and neck dysfunction. Design: Cross-sectional study Methods : The sample included 114 subjects, who had complained of chronic neck pain for more 12 weeks. We conducted a Pearson's correlation between neck and shoulder pain, headaches, neck and shoulder dysfunction and sleep quality and a regression analysis of the related variables, thereby analyzing factors affecting neck pain and neck dysfunction. Results : In the present study, in adults with chronic neck pain, neck pain was positively correlated with the Neck Disability Index (NDI), Shoulder Pain and Disability Index (SPADI)-Pain score, and SPADI-Total score (p<.05). The NDI was positively correlated with neck pain, SPADI-Pain score, and SPADI-Total score, as well as with Pittsburgh Sleep Qulity Index(PSQI-K) (p<.05). Among the factors affecting neck pain, shoulder disability as assessed by the SPADI was a significantly associated with neck pain, while shoulder pain and shoulder disability determined by the SPADI were identified as significant variables among the factors affecting neck disability. Conclusion : These results indicated that as neck pain worsened, shoulder pain and neck and shoulder dysfunction also increased, which suggested that shoulder disability affected neck pain. In addition, as the neck dysfunction increased, neck pain and shoulder pain and shoulder dysfunction increased, and sleep quality deteriorated, which suggested that shoulder pain and shoulder disability affected neck disability.
Objectives : To observe the effect of acupuncture treatment in chronic shoulder pain patients. Methods : 36 voluntary patients were randomly assigned to an acupuncture treatment group(E Group, n=18) and a control group(C Group, n=18). The E Group patients received acupuncture treatment on $LI_{15}$, $TE_{14}$, $GB_{21}$ and Master Dong's acupuncture points, Shin-gwan and Gyun-joong, twice a week for four weeks. The C Group patients received no treatment. All patients in both groups were instructed to practise self exercise in their daily lives. Evaluations were made at baseline and after four weeks of study. The Constant Shoulder Assessment(CSA), Shoulder Pain and Disability Index(SPADI) and the patient's subjective pain was measured by Visual Analogue Scale(VAS). The obtained data was analyzed. Results : The E Group showed significant(p<0.05) improvement in CSA, SPADI and VAS after four weeks of treatment. The C Group showed significant(p<0.05) improvement in CSA, but the change of SPADI and VAS was insignificant(p>0.05). CSA and SPADI of E Group significantly(p<0.05) improved compared to the C Group, but the difference of VAS change in the two groups was insignificant(p>0.05). Conclusions : Four weeks of acupuncture treatment significantly improved CSA, SPADI and VAS. The improvement of CSA and SPADI was significant(p<0.05) compared to untreated patients.
Objectives : The purpose of this study is to investigate the clinical effects of low level laser therapy on shoulder pain. Methods : From October 1, 2009 to November 14, 2009, we applied 15 participants who consented to an assignment had a shoulder pain to low level laser therapy(LLLT). We treated LLLT 8 times on both upper trapezius, levator scapular, pectoralis, suboccipital, rhomboid, rotator cuff tender point for 2 weeks. To evaluate the efficiency of low level laser therapy, we used questionnaire of shoulder pain symptom, questionnaire of a day living discomfort scale, shoulder pain and disability index(SPADI) and pressure pain threshold(PPT) at before treatment and after treatment 4 times and 8 times. Results : There was a significant difference in questionnaire of shoulder pain symptom, questionnaire of a day living discomfort scale, SPADI and PPT after 8th treatment. Conclusions : Low level laser therapy for shoulder pain is effective to improve patients' symptoms measured with questionnaire of pain and a day living discomfort score, SPADI and PPT significantly.
Objective: This study aimed to elucidate the effects of thoracolumbar fascia release (TLFR) on the degree of pain and disability in patients with shoulder pain. Design: Randomized control trial. Methods: Thirty subjects with shoulder pain participated in this study. They were allocated to TLFR group (n=15) and manual physical therapy (MPT) group (n=15). Shoulder pain and disability index (SPADI) and the score on the visual analogue scale (VAS) were measured before and after TLFR. Results: In the TLFR group, the degree of shoulder pain as indicated by SPADI measured after the intervention significantly differed from that before the intervention (p<0.05); moreover, in the MPT group, the degree of shoulder pain was significantly lower (p<0.05). The data of the 2 groups before the intervention significantly differed from those after the intervention (p<0.05). SPADI significantly differed within the groups (p<0.05), but not between the groups. The sum of SPADI did not differ significantly between the groups. The VAS scores of shoulder pain measured before the intervention significantly differed from those measured after the intervention (p<0.05) in the both groups. After the intervention, shoulder pain decreased significantly in the TLFR group as compared to that in the MPT group. Conclusions: TLF release was effective in reducing shoulder pain. The results of this study can be applied in clinical practice for TLFR performed to reduce shoulder pain. Further studies will need to be performed to elucidate the effects of TLFR on functional recovery.
Purpose: This study was to investigate the correlation between pain, disability and quality of life among adolescents and office workers with neck and shoulder pain. Methods: Twenty-four subjects (mean age=24.92 years, SD=3.94, range=20-37) with neck and shoulder pain participated in this study. The outcome measures of the study were visual analogue scale (VAS), neck disability index (NDI), shoulder pain and disability index (SPADI), short form-36 (SF-36) for neck and shoulder pain and disability, quality of life. Pearson's correlation and Speaman's rank correlation were used to measure the association between VAS and NDI, SPADI, SF-36. Results: The VAS showed a positive correlation between weak to moderate with the NDI and SPADI, but no significantly correlated (${\rho}=0.34$, ${\rho}=0.25$). The moderate positive correlation and significant correlation were observed between NDI and SPADI (${\rho}=0.43$, p<0.05). The PCS of SF-36 was significantly negative correlated with the VAS (${\rho}=-0.24$), NDI and SPADI (${\rho}=-0.63$, ${\rho}=-0.59$, p<0.05). Conclusion: The pain and disability of neck and shoulder has closely relevance. And, the pain and disability of neck and shoulder has a negative impact on the quality of life.
Background: Stretch-oriented home exercise programs are often used as treatments for patients with adhesive capsulitis; however, there is lack of research on home exercise programs to strengthen rotator cuffs. Objects: The aim of this study is to investigate the effect of home exercise programs for rotator cuff strengthening on pain, range of motion (ROM), disability level, and quality of life in patients with adhesive capsulitis. Methods: Twenty-two patients with adhesive capsulitis volunteered to participate in this study. The subjects were randomly divided into an experimental group (n = 11) and control group (n = 11). For the experimental group, manual therapy and home exercise programs for rotator cuff strengthening were applied for 6 weeks; for the control group, only manual therapy was applied for 6 weeks. Shoulder pain (quadruple visual analogue scale, QVAS), ROM, disability (shoulder pain and disability index-Korean version, SPADI), and quality of life (world health organization quality of life scale-Korean version, WHOQOL-BREF) were evaluated at baseline, after 3 weeks, and after 6 weeks of intervention. The changes in the measurement variables were analyzed by using repeated measure analysis of variance. Results: Significant differences were observed between the experimental group and control group in the QVAS; SPADI-pain scores; SPADI-disability scores; SPADI-total scores; flexion, abduction, internal and external rotation ROM of the glenohumeral joint; and WHOQOL-BREF total, overall, physical health, and psychological scores. All groups displayed statistically significant improvements as observed in the QVAS, SPADI, flexion, abduction, external and internal rotation ROM of the glenohumeral joint, and WHOQOL-BREF. Conclusion: Home exercise programs for rotator cuff strengthening had a positive impact on shoulder pain, shoulder ROM, disability level, and quality of life in patients with adhesive capsulitis. Therefore, we propose the use of home exercise programs for rotator cuff strengthening in the exercise rehabilitation of patients with adhesive capsulitis.
Purpose: This study examined the effect of proprioceptive neuromuscular facilitation (PNF) rehabilitation exercise on postoperative range of motion (ROM), pain, and function of breast cancer patients. The purpose of this study was to provide fundamental data regarding the use of PNF rehabilitation exercise for patients with breast cancer. Methods: Fourteen patients who underwent surgery due to breast cancer were randomly divided into an experimental group (n=7) that performed PNF rehabilitation exercise and a control group (n=7) that performed general rehabilitation exercise. Both groups performed the respective exercises for 30 minutes, five times a week for 4 weeks. For the measurement of ROM, the range of shoulder abduction was measured using a clinometer smartphone application. The shoulder pain and disability index (SPADI) was used to measure the level of pain and functional activity. A paired t-test was conducted to compare within-group changes before and after the PNF rehabilitation exercise. Differences between the experimental group and control group were analyzed by an independent t-test. For all tests, the level of statistical significance was set at ${\alpha}=0.05$. Results: After the exercise, there was a significant within-group change in the ROM and SPADI in both the experimental group and control group (p<0.01). There was also a significant between-group difference in the ROM and SPADI after the intervention (p<0.05). Conclusion: Rehabilitation exercise is generally applied as a treatment for patients with breast cancer after surgery and is relatively effective. The application of PNF rehabilitation exercise may be useful in such patients, considering its effects on ROM improvement, pain reduction, and functional enhancement.
Background : Patients with frozen shoulder have pain and limited range of motion (ROM) at the affected shoulder. Shoulder pain of these patients usually decreases with recovery of range of motion. The aim of this study is to identify effects of static stretching and hold-relax on recovering ROM and reducing pain of patients with frozen shoulder. Methods : In this study, 15 patients with frozen shoulder who were only treated with physical therapy were recruited. Patients who received any surgical procedure or hydrotherapy were excluded. The subjects were divided randomly into 3 groups; control(n=5), static stretching(n=5) and hold-relax(n=5) groups. Patients were treated with stretching and the total number of treatments was 12 times. The ROM was measured at baseline, after fourth, eighth and final treatments. Shoulder pain and disability index(SPADI) scores were assessed before and after doing all treatments. Repeated measures ANOVA was used to analyze ROM change within each group. Changes of ROM between groups was analyzed by one way ANOVA followed by post hot analysis(Tukey's HSD). SPADI score within each group analyzed using Wilcoxon signed ranks test. A p-value<0.05 was considered statistically significant. Study Design : Repeated measures design. Result : There were significant differences of ROM in both treatment groups except internal rotation of static stretching group. ROM was significantly increased in treatment groups compared with control group, but the difference of ROM change between two treatment groups did not differ significantly. SPADI scores for treatment groups significantly decreased compared with pre-SPADI score. Conclusion : The result suggests that hold-relax can be more effective than static stretching to recover range of motion and alleviate shoulder pain on patients with frozen shoulder.
