In vivo 3-dimensional Kinematics of Cubitus Valgus after Non-united Lateral Humeral Condyle Fracture

  • Background: Nonunion of lateral humeral condyle fracture causes cubitus valgus deformity. Although corrective osteotomy or osteosynthesis can be considered, there are controversies regarding its treatment. To evaluate elbow joint biomechanics in non-united lateral humeral condyle fractures, we analyzed the motion of elbow joint and pseudo-joint via in vivo three-dimensional (3D) kinematics, using 3D images obtained by computed tomography (CT) scan. Methods: Eight non-united lateral humeral condyle fractures with cubitus valgus and 8 normal elbows were evaluated in this study. CT scan was performed at 3 different elbow positions (full flexion, $90^{\circ}$ flexion and full extension). With bone surface model, 3D elbow motion was reconstructed. We calculated the axis of rotation in both the normal and non-united joints, as well as the rotational movement of the ulno-humeral joint and pseudo-joint of non-united lateral condyle in 3D space from full extension to full flexion. Results: Ulno-humeral joint moved to the varus on the coronal plane during flexion, $25.45^{\circ}$ in the non-united cubitus valgus group and $-2.03^{\circ}$ in normal group, with statistically significant difference. Moreover, it moved to rotate externally on the axial plane $-26.75^{\circ}$ in the non-united cubitus valgus group and $-3.09^{\circ}$ in the normal group, with statistical significance. Movement of the pseudo-joint of fragment of lateral condyle showed irregular pattern. Conclusions: The non-united cubitus valgus group moved to the varus with external rotation during elbow flexion. The pseudo-joint showed a diverse and irregular motion. In vivo 3D motion analysis for the non-united cubitus valgus could be helpful to evaluate its kinematics.

관절염 대상자의 건강상태 조사 - 일개 도.농 복합시를 중심으로 - (A Study on Health Status of Arthritis Clients in Wonju City)

  • The purpose of this study was to investigate the health status and to examine the relationship between health related variables of the clients with arthritis. This descriptive-correlational study was measured physical health status(pain, fatigue, physical disability, joint flexibility) and psycho-social health status(social function disability, self-efficacy, depression). The clients were consisted of 127 arthritis clients who were randomly selected from health center, 2 sub health center and 6 primary health care center in Wonju City. The data was collected from November 2000 to January 2001. The results of the study analyzed using a SPSS win were as follows: 1) The subjects' average age was 64.81 years old(female: 86.6%, male: 13.4%). 2) In physical health status, the average score of fatigue was 63 scores and pain was 69.77. Physical disability score was higher than total disability score. In psychosocial disability, housekeeping management disability score was higher than social function disability score. In psycho-social health status, the average score of self-efficacy was 70.86 and the subjects have higher level of depression, more than 20 scores. 3) On pain sites, almost of subjects complained of a knee joint pain. The waist, wrist, and shoulder joint pain were followed. The numbers of pain site were 8.3 per individuals. 4) The flexibility of knee and shoulder joint were lower than another joints. 5) Correlation with fatigue, pain, physical disability, social disability, self-efficacy and depression was revealed highly significant. We suggest that health promotion program considering our research results should be provided for arthritis clients in community level.

동통과 골성 운동 제한이 있는 주관절에서 후내측 도달법을 이용한 변연 절제 관절 성형술 (Debridement Arthroplasty using Posteromedial Approach in Elbow Joint with Pain and Bony Limitation of Motion)

  • 목적: 동통과 골성 운동 제한이 동반된 주관절에 대해 후내측 도달법을 이용한 변연 절제 관절 성형술의 임상적 결과를 분석하였다. 대상 및 방법: 2005년 3월부터 2008년 3월까지 주관절의 동통과 골성 운동 제한으로 후내측 도달법을 통한 변연 절제 관절 성형술을 시행 받은 16명을 대상으로 하였다. 추시 관찰 기간은 평균 27.6(13~52)개월이였다. 통증에 대해서 Visual Analogue Scale, 술 전과 술 후 굴곡, 신전, 운동범위를 측정하고 기능에 대해서 Mayo Elbow Performance Score (MEPS)를 측정하여 분석, 평가 하였다. 결과: 최종 추시 시 VAS (visual analogue scale )는 술 전 평균 4.5에서 술 후 평균 1.1으로 통계학적으로 유의한 감소(p<0.001 )를 보였다. 관절 운동 범위는 술 전 평균 $61.6^{\circ}$(0~90)에서 술 후 $109.4^{\circ}$(80~120)로 통계학적으로 유의하게 증가하였다(p<0.001). MEPS 역시 술 전 59.4에서 술 후에는 85.6으로 증가하였다(p<0.001). 술 후 혈종이나 주관절 불안정증 같은 합병증은 없었다. 결론: 후내측 도달법을 이용한 변연 절제 관절 성형술은 쉽게 척골 신경에 대한 조작이 가능하고 주관절 내측 측부 인대의 손상을 최소화 할 수 있으며 주관절 전 구획(all compartment)에 광범위한 변연 절제가 가능하여 동통과 골성 관절 운동 제한이 동반된 주관절에서 동통의 감소와 관절 운동 범위의 회복에 도움이 되는 유용한 술식으로 사료된다.

골프 클럽의 길이 변화에 따른 준비 자세의 변화 (Changes of Setup Variables by the Change of Golf Club Length)

  • To know the proper setup posture for the various clubs, changes of setup variables according to the change of golf club length was investigated. Swing motions of three male low handicappers including a professional were taken using two high-speed videocameras. Four clubs iron 7, iron 5, iron 3 and driver (wood 1) were selected for this experiment. Three dimensional motion analysis techniques were used to get the kinematical variables. Mathcad and Kwon3D motion analysis program were used to analyze the position, distance and angle data in three dimensions. The variables divided into three categories 1) position and width of anterior-posterior direction 2) position and width of lateral direction 3) angles and evaluated based on the theories of many good golf teachers. Major findings of this study were as follows. 1.The stance (distance between ankle joints) was increased as the length of the club increased but the increasing width was not large. It ranges from 5cm to 10cm and professional player showed small changes. 2. Forward lean angle of trunk was decreased (more erected) as the length of the club increased. It ranges from 30 degrees for iron7 to 25 degrees for driver. 3. Angle between horizontal and right shoulder were increased as the length of the club increased. It ranges from 10 degrees to 20 degrees and professional player showed small changes. 4. Anterior-posterior position of the shoulders were located in front of the foot for all clubs and the difference between the shoulder and knee position was decreased as the length of the club increased. 5. Anterior-posterior position of grip (hand) was located almost beneath the shoulders (2.5cm front) for iron7, but it increased to 10cm for the driver. This grip adjustment makes the height of the posture increased only 5cm from iron7 to driver. 6. Lateral position of grip located at 5cm left for the face of iron7, but it located at the right side (behind) for the face of driver. 7. Lateral position of the ball located at the 40%(15cm) of stance from left ankle for iron7 and located at the 10% (5cm) of stance for driver. 8. Head always located at the right side of the stance and the midpoint of the eyes located at the 37% of stance from the right ankle for all clubs. This means that the axis of swing always maintained consistently for all clubs. 9. Left foot opened to the target for all subject and clubs. The maximum open angle was 25 degrees. Overall result shows that the changes of the setup variables vary only small ranges from iron7 to driver. Paradoxically it could be concluded that the failure of swing result from the excessive changes of setup not from the incorrect changes. These findings will be useful for evaluating the setup motion of golf swing and helpful to most golfers.

  • Many studies have reported that housework done using household appliances may affect biomechanical characteristics of the musculoskeletal system. The purpose of this study was to investigate the effects of housework done using a pulsator washing machine on joints and muscles. We calculated joint torques and muscle strengths on the basis of an experimental/virtual three-dimensional motion analysis for six healthy females using the pulsator washing machine at different heights (H) and depths (D) of the inside drum (H (mm){\times}D (mm), Trial1: $962.5{\times}609.4$, Trial2: $962.5{\times}624.4$, Trial3: $982.5{\times}644.4$, Trial4: $995.5{\times}642.4$, Trial5: $1015.5{\times}677.4$). The joint torques and muscle strengths tended to be considerably different during torso flexion in the sagittal plane for all the trials. The maximum joint torques for the thoracic vertebra, left ankle, and right shoulder measured in Trial4 were significantly higher than those measured in the other trials (p < 0.05); in addition, those for the thoracic vertebra and both ankles measured in Trial5 were significantly higher than those measured in the other trials (p < 0.05). The maximum muscle strengths for the left trapezius muscle and both tibialis anterior muscles measured in Trial5 were significantly higher than those measured in the other trials (p < 0.05). These results indicate that housework done using a pulsator washing machine may affect joint torques and muscle strengths, and these effects are dependent on the height and/or depth of the inside drum of the pulsator washing machine.

Kinematic and Dynamic Analyses of Human Arm Motion

  • Purpose: Determining an appropriate path is a top priority in order for a robot to maneuver in a dynamically efficient way especially in a pick-and-place task. In a non-standardized work environment, current robot arm executes its motion based on the kinematic displacements of joint variables, though resulting motion is not dynamically optimal. In this research we suggest analyzing and applying motion patterns of the human arm as an alternative to perform near optimum motion trajectory for arbitrary pick-and-place tasks. Methods: Since the motion of a human arm is very complicated and diverse, it was simplified into two links: one from the shoulder to the elbow, and the other from the elbow to the hand. Motion patterns were then divided into horizontal and vertical components and further analyzed using kinematic and dynamic methods. The kinematic analysis was performed based on the D-H parameters and the dynamic analysis was carried out to calculate various parameters such as velocity, acceleration, torque, and energy using the Newton-Euler equation of motion and Lagrange's equation. In an attempt to assess the efficacy of the analyzed human motion pattern it was compared to the virtual motion pattern created by the joint interpolation method. Results: To demonstrate the efficacy of the human arm motion mechanical and dynamical analyses were performed, followed by the comparison with the virtual robot motion path that was created by the joint interpolation method. Consequently, the human arm was observed to be in motion while the elbow was bent. In return this contributed to the increase of the manipulability and decrease of gravity and torque being exerted on the elbow. In addition, the energy required for the motion decreased. Such phenomenon was more apparent under vertical motion than horizontal motion patterns, and in shorter paths than in longer ones. Thus, one can minimize the abrasion of joints by lowering the stress applied to the bones, muscles, and joints. From the perspectives of energy and durability, the robot arm will be able to utilize its motor most effectively by adopting the motion pattern of human arm. Conclusions: By applying the motion pattern of human arm to the robot arm motion, increase in efficiency and durability is expected, which will eventually produce robots capable of moving in an energy-efficient manner.

가공의치(架工義齒)에 작용(作用)하는 Stress에 관(關)한 광탄성학적(光彈性學的) 분석(分析) (Photoelastic Stress Analysis of Fixed Partial Dentures)

  • The purpose of this study was to investigate stresses in the various components of fixed partial dentures restoring the posterior teeth of the lower jaw, and to measure quantitatively the effects of certain modifications in structural design on the stresses in the restorations using two-dimensional photoelasticity. Two-dimensional photoelastic methods were used in this study. Several models of fixed partial dentures were constructed. Shoulder less margins and anatomic occlusal reduction were incorporated in Model 1. Rounded shoulders and flat occlusal reduction were incorporated in Model 2, while Model 3 was a cantilever fixed partial denture. Other similar fixed partial dentures were constructed with V and U notches deliverately included in the region of the fixed joints for comparative reasons. The birefringent materials used in this study were PSM-1 and PSM-5 in standard sheets. PSM-1 was used for constructing the substructure, and PSM-5 was used in making the components of the fixed partial dentures. The two materials were used in the construction of composite photoelastic models. Improved artificial stone was used to represent dental cement in luting the composite photoelastic models. Static loading procedures were used at preplanned sites to represent occlusal loads in the mouth. 35 mm color and B/W film were used to record isochromatics in accordance with photoelastic procedures. Data reduction was performed using the grid method, which helped in, the mathematical integration procedure (Shear difference method) to separate the principal stresses. The results were as follows. 1. Fixed partial dentures do not function in bending as a symmetrical beam. Alternate areas of tension and compression were demonstrated when multiple contact loading was used. 2. The weakest part in posterior fixed partial dentures is the fixed joint. 3. (1) Models I and modified Model I were loaded on the pontic using a 50 pound vertical static load. The shear stress near the posterior fixed joint in Model 1 (U notches) was+129.4 p.s.i., and at the same fixed joint in modified Model 1 (V notches) was+239.4 p.s.i. The concentration of stress in fixed joint was reduced by 50% when U notches replaced the V notches. (2) Modified Model 2 was loaded using a multiple contact loader at a total load of 125 pounds. The difference between the principal stresses (${\sigma}_1-{\sigma}_2$), shear stress, at the V notches was+600 p.s.i., and at the U notches was+3l7 p.s.i. The shear stress was reduced by 50% when U notches replaced the V notches. V-grooves at the fixed joints should be avoided, and should be replaced by regular shaped U-grooves. 4. Cantilever fixed partial dentures had much higher stresses at the fixed joint than fixed partial dentures that were attached at both ends.

테니스 포핸드 스트로크 동안 오픈스탠스 조건에 따른 3차원 운동학적 분석 (3-D Kinematic Analysis According to Open Stance Patterns During Forehand Stroke in Tennis)

  • Recently among several tennis techniques forehand stroke has been greatly changed in the aspect of spin, grip and stance. The most fundamental factor among the three factors is the stance which consists of open, square and closed stance. The purpose of this study was to investigate the relations between the segments of the body, the three dimensional anatomical angle according to open stance patterns during forehand stroke in tennis. For the movement analysis three dimensional cinematographical method(APAS) was used and for the calculation of the kinematic variables a self developed program was used with the LabVlEW 6.1 graphical programming(Johnson, 1999) program. By using Eular's equations the three dimensional anatomical Cardan angles of the joint and racket head angle were defined 1. In three dimensional maximum linear velocity of racket head the X axis showed $11.41{\pm}5.27m/s$ at impact, not the Y axis(horizontal direction) and the z axis(vertical direction) maximum linear velocity of racket head did not show at impact but after impact this will resulted influence upon hitting ball It could be suggest that Y axis velocity of racket head influence on ball direction and z axis velocity influence on ball spin after impact. the stance distance between right foot and left foot was mean $74.2{\pm}11.2m$. 2. The three dimensional anatomical angular displacement of shoulder joint showed most important role in forehand stroke. and is followed by wrist joints, in addition the movement of elbow joints showed least to the stroke. The three dimensional anatomical angular displacement of racket increased flexion/abduction angle until the impact. after impact, The angular displacement of racket changed motion direction as extension/adduction. 3. The three dimensional anatomical angular displacement of trunk in flexion-extension showed extension all around the forehand stroke. The angular displacement of trunk in adduction-abduction showed abduction at the backswing top and adduction around impact. while there is no significant internal-external rotation 4. The three dimensional anatomical angular displacement of hip joint and knee joint increased extension angle after minimum of knee joint angle in the forehand stroke, The three dimensional anatomical angular displacement of ankle joint showed plantar flexion, internal rotation and eversion in forehand stroke. it could be suggest that the plantar pressure of open stance during forehand stroke would be distributed more largely to the fore foot. and lateral side.

Rapid upper limb assessment와 3차원 동작 분석을 활용한 치석제거 자세교정 교육의 효과 (Effect of posture correction training in dental scaling using rapid upper limb assessment and 3D motion analysis)

  • Objectives: The purpose of this study was to investigate the change in the posture of dental hygiene students and clinical dental hygienists when implementing dental scaling before and after posture correction training using the rapid upper limb assessment (RULA) method and 3D motion analysis. Methods: Thirty-two healthy volunteers performed dental scaling to remove artificial calculus on dental manikin. The movement and angle of the joints were verified by RULA and 3D motion analysis during the procedure. The subjects were also photographed for 1 minute during the procedure for 10 minutes while the calculus was removed. After the removal of the calculus, the subject and the instructor checked the video together. Posture correction training was conducted by the instructor so that the subject could perform the calculus removal operation in the correct posture. Artificial calculus of the adjacent teeth was then removed for the same period of time, and the change in posture was reviewed. Results: The total score of the posture change using RULA was $5.72{\pm}0.58$ before training and $4.31{\pm}0.10$ after training, showing a significant decrease after training (p<0.001), and upper arm, lower arm, wrist position, neck and waist position showed significant decrease after training. The three-dimensional motion analysis showed significant differences according to the criteria measured at all measurement sites except the left shoulder (p<0.05) Conclusions: It was confirmed through RULA and 3D motion analysis that postural correction training using calculus removal images was effective, and that correct postural education is essential to preventing musculoskeletal diseases caused by removal of calculus.

  • The purpose of this research is to study doll's symbolism representing our social culture and the characteristics of clothing design. The ball joint doll was selected, that are pretty much resembled to human body in terms of facial expression, body proportion, joint movement, etc. Results of research are summarized as follows: the ball joint doll is indicated a doll whose joints are free to move with a round shaped joint, which originated by Yochilya simon, who adopted German Hans Bellmer's method.'rho ball joint doll, first, there appeared state of human isolation due to the material civilization and industrial revolution, and trend of adhering to one's own world made human possess dolls. Second, according to the doll play became a representing play culture among Kidult culture, dolls, that are resembled with human being, were recognized as personalized objects. Third, along of expanding digital culture that makes easy to exchange information via Internet, so, ball joint doll market is growing fast, for which various levels of mania are being formed. Take a look at the doll fashion in modern society. In accordance to form of Look, it is largely categorized Young Casual look, Office look, Ethnic look and Military look. When take a look at design specialty, young casual look was presented as a comfortable silhouette and also made of elastic materials and expressed vivid bright tones. Office look image was expressed as a suit of jacket with skirt, and cotton wool polyester materials and acro-matic colors. Ethnic look was represented by Japanese traditional dress and Chinese traditional clothing. The Military look was generally emphasized army uniforms of straight silhouette and shoulder straps.
