영유아의 아토피피부염 중증도 관련 요인 연구 (Associated Factors with Severity of Atopic Dermatitis in Children)

  • The prevalence of atopic dermatitis (AD) has increased over the past decades. A variety of factors are related to the development of atopic dermatitis, including genetics and environmental factors. The purpose of the study was to examine factors associated with severity of atopic dermatitis in 104 children aged 6-60 months. To investigate the association between severity and other factors children were divided into two groups, mild (n = 62) and severe (n = 42) groups, based on SCORAD index which measures the severity of atopic dermatitis. Results showed that family history, family's smoking, period of having AD and the levels of the serum total IgE were significantly higher in severe group. More than 6 months of breastfeeding and weaning after 6 month were not associated with severity of AD, but the number of food eliminated was associated with severity. Only vitamin C intake was significantly different by severity after adjusting for energy intake, family history, family’s smoking and period of having AD (p = 0.033). There was no association between the severity of atopic dermatitis and growth of children. This study concluded that the severity of atopic dermatitis was associated with family history, family’s smoking, period of having AD, children's serum total IgE, and vitamin C intakes. The result of this study should provide the significant information for better management of AD.

Soybean Fermented with Bacillus amyloliquefaciens (Cheonggukjang) Ameliorates Atopic Dermatitis-Like Skin Lesion in Mice by Suppressing Infiltration of Mast Cells and Production of IL-31 Cytokine

  • The present study was conducted with the aim to investigate the ameliorative effects of a new soybean product (cheonggukjang) fermented with Bacillus amyloliquefaciens SCGB1 (SFBA) in atopic dermatitis (AD) mouse model. Visual evaluation of AD induction in the mice indicated the remarkable control of SFBA in reducing the pathological severity of AD-like skin lesions reported as the SCORAD score of AD clinical symptoms. The results revealed that SFBA reduced dorsal skin and epidermal thickness to a similar extent with prednisolone. Further analysis revealed the dominance of SFBA in restraining mast cell infiltration in the dermis; immunoglobulin-E expression in serum; and TH2 IL-4 cytokine and itch-related IL-31 cytokine in the mice skin and serum. SFBA also suppressed scratching behaviours in mice induced by compound 48/80. Further histological findings also revealed the alleviation of collagen fiber deposition in dermal skin of the AD mice model. These actions of SFBA were examined to be mediated by its suppression of the phosphorylation activation of key signalling molecules such as $NF-{\kappa}B$ and MAPK responsible for the induction of cytokine production. Thus, SFBA can be considered as a promising functional food for managing clinical, histological and immunological spectra associated with AD.

아토피 피부염에서 한약 외용제와 양약 병용 치료의 효과: 체계적 문헌고찰 및 메타분석 (Efficacy of Combination Treatment of Herbal Medicine for External Use and Western Medicine for Atopic Dermatitis: A Systematic Review and Meta Analysis)

  • Objectives: This study was conducted to evaluate the effect of combination treatment of herbal medicine for external use and western medicine for atopic dermatitis (AD). Methods: We searched randomized controlled trials (RCTs) which assess the effect of combination treatment of herbal and western medicine for AD through 8 electronic databases from the start to December 2022. The data synthesis was conducted by using Review Manager (RevMan, ver.5.4.1) and Cochrane risk-of-bias tool was used to evaluate the risk of bias. Results: 13 RCTs were included. The combination treatment group had significantly higher total efficacy rate(p<0.0001) and lower SCORAD score (p<0.00001) than the western medicine treatment group. The adverse event rate was also significantly lower in the combination treatment than the western medicine treatment group (p<0.0001). But there was no significant difference in recurrence rate (p=0.09). Conclusion: This study demonstrates that the combination treatment of herbal and western medicine could be safe and effective for AD. However, due to limits of included studies such as high heterogeneity between the literature and unclear risk of bias, further studies are warranted.

소아 아토피피부염 환자의 부증상 빈도 및 특징에 대한 연구 (A study of the frequency and characteristics of minor clinical manifestations in children with atopic dermatitis)

  • 목 적 : 국내 소아 아토피피부염 환자의 부증상의 빈도 및 연령과 중등도에 따른 차이를 분석하여 아토피 피부염의 진단 및 치료에 도움이 되고자 본 연구를 시행하였다. 방 법 : 2007년 4월부터 12월까지 순천향대학교 병원 소아알레르기 호흡기센터에 내원하여 아토피피부염으로 진단 받은 106명 환아를 대상으로 하였다. 대상 환자들은 첫 내원시 3명의 소아과 전문의에 의해 아토피피부염 부증상이 평가 되었으며 Hanifin과 Rajka의 부증상 항목과 2001년 아토피피부염 국제진단기준을 바탕으로 한 20가지 항목을 작성하였다. 대상 환자들은 2세 미만과 2세 이상으로 연령군을 분류하고 SCORAD 점수를 이용하여 중증도를 분류하였으며 원인 항원 특이 IgE 항체를 측정하여 아토피군과 비아토피군으로 분류하였다. 결 과 : 아토피피부염 환자에서 부증상으로 피부건조증(78.3%), 환경이나 감정요인에 의한 악화(43.4%), 태선화(35.8%), 눈 주위 색소 침착(34.0%), 이개 균열(33.0%), 피부 감염(31.1%)이 높은 빈도를 보였다. 2세 이상인 군에서 눈주위 색소 침착(P=0.01), 목주름(P=0.01), 태선화(P=0.001) 항목에서 통계학적으로 유의하게 높은 빈도를 보였고중증 아토피피부염에서 피부 건조증(P=0.04), 피부 감염(P=0.03), 모공 각화증(P=0.02), 반복적인 결막염(P=0.02), 눈 주위 색소침착(P=0.001), 안면 피부염(P=0.001), 태선화(P=0.001), 손/발 습진(P=0.04) 항목이 높은 빈도를 보였다. 아토피군에서 안면 피부염(P=0.01)과 태선화(P=0.04) 항목이 유의하게 높은 빈도를 보였다. 결 론 : 국내 소아 아토피피부염 환아에서 흔히 관찰되는 부증상의 빈도 및 나이, 중증도에 따른 차이를 확인하였고 국내 소아 아토피피부염 진단 및 중증도 평가에 있어 부증상의 몇몇 항목이 유의하게 활용될 수 있는 가능성을 제시할 수 있었다.

아토피피부염 환자의 개인별 증상 기록에 대한 순응도 연구 (Study on Compliance of Personal Health Record Application in Patients with Atopic Dermatitis)

  • Objectives : The purpose of this study is to evaluate clinical utilization by measuring compliance with the use of mobile health applications (AtopyPHR developed in a previous study) for patients with atopic dermatitis. Methods : Based on the AtopyPHR and the input period and frequency survey results for each symptom item, a scenario for measuring compliance was derived. The study period was 4 weeks. Participants installed AtopyPHR app and Telegram app on their smartphones, conducted user training on the app, and recorded symptoms using the app for 4 weeks. At the 2nd and 4th week visits, the AtopyPHR data recorded by the user can be viewed on the web page and used for medical decision. Compliance was analyzed by the date the symptoms were recorded. Results : There were 28 participants, all (100%) were compliant, and the compliance was 96.8. The patients were 1 to 18 years old, and the average age was 8.2±5.7 years, 10 males and 18 females. The actual date of participation in recording symptoms was 28.6±0.56 on average. Compared to Week 1, compliance decreased at Week 2, and Week 4 had the highest compliance. Daily check, daily emotion, stool/urine/sleep, and meal management showed high compliance, SCORAD and quality of life were higher than required to record. Conclusions : AtopyPHR was effective in compliance. The results of this study could be used to collect personal health data in daily life through the AtopyPHR, improving participant compliance. It is considered to be meaningful because it measured the compliance with the symptom record actually recorded using the mobile app rather than a questionnaire. This study may be useful not only for personal health care but also for medical decisions, as opinions are given by experts who treat atopic dermatitis.

아토피 피부염의 한약 치료에 대한 무작위대조군연구 중심의 연구 동향 - CNKI를 중심으로 (Research Trends of Randomized Controlled Trials on Herbal Medicine Treatment for Atopic Dermatitis-Focusing on CNKI)

  • The purpose of this study was to analyze the research trends of randomized controlled trials on herbal medicine treatment for atopic dermatitis in China for the last 5 years. We searched for randomized controlled trials with the intervention of herbal medicine for the treatment of atopic dermatitis in the CNKI (China National Knowledge Infrastructure) from January 2014 to December 2018. For the screening of the paper, we used '特应性皮炎' and '异位性皮炎' which mean atopic dermatitis and search was limited to three areas within Medicine & Public Health: Traditional Chinese Medicine, Traditional Chinese Medicinal Herbs, Combination of Traditional Chinese Medicine with Western Medicine. Among the 136 searched studies, we selected a total of 34 studies and analyzed a year of publication, subject characteristics, study design and intervention, prescribed herbal medicine and herbs, pattern identification, evaluation criteria, and outcomes. Longmu decoction (龍牡湯) and Polia Sclerotium (茯笭) was the most frequently prescribed medicine and herb. The most commonly used pattern identification was Blood deficiency and Wind-dryness (血虛風燥), and among them, the most frequently prescribed herb is Rehmanniae Radix (生地黃). In most studies using the total effectiveness and SCORAD index as an outcome measure, the herbal medicine treatment group showed statistically better results than the control group. As a result of the safety assessment, the herbal medicine treatment group was reported having significantly fewer side effects compared to the control group. Hence, it was confirmed that the intervention including herbal medicine had a significant effect on atopic dermatitis. This study would be able to provide the basis of clinical research on atopic dermatitis and applied to the treatment of atopic dermatitis.

다빈도 피부질환에 대한 국내외 한의학 논문에서 평가도구 사용현황에 대한 고찰 (A Review of Assessment Tools in Traditional Medicine on the High Frequent Skin Diseases)

  • Objectives : The purpose of this study is to establish the basis of the assessment tools that can be used for the diagnosis and assessment of skin diseases by reviewing the present status of the assessment tools used in domestic and overseas traditional medicine papers for the past five years. Methods : This study is based on analysis of papers on clinical trials about atopic dermatitis, psoriasis, acne, urticaria published from 1st July 2013 to 30th June 2018. The papers were searched from Oriental Medicine Advanced Searching Integrated System(OASIS), Korean Traditional Knowledge Portal(KTKP), Pubmed, MEDLINE. Results : 40 articles on atopic dermatitis, 34 articles on psoriasis, 26 articles on acne and 10 articles on urticaria were selected. Among them, 38 articles(90%), 20 articles(85%), 22 articles(85%) and 5 articles(50%) used assessment tools in order. Conclusions : SCoring Atopic Dermatitis(SCORAD), objective SCORing of Atopic Dermatitis index(OSI), Eczema Area and Severity Index(EASI) were used in studies on atopic dermatitis. In the case of psoriasis, The Psoriasis Area and Severity Index(PASI) was used in most papers. In the study on acne, most of Korean papers used Korean Acne Grading System(KAGS), but in the overseas papers, various assessment tools were used. In the case of urticaria, the rate of use of the assessment tool was the lowest, but the assessment tool used was unified by Urticaria Activity Score(UAS). Since skin diseases can cause stress and a decrease in quality of life, the quality of life should be taken into account when evaluating treatment effects.

아토피 피부염 환자의 지방산 섭취와 혈중 지방산 조성 및 면역 지표에 관한 연구 (Study of Dietary Fatty acids, Blood Fatty Acid Composition, and Immune Parameters in Atopic Dermatitis Patients)

  • The prevalence of atopic dermatitis (AD) continues to rise in industrialized countries related to Western lifestyle, including dietary habits, especially imbalance of intake of dietary fatty acids. The purpose of this study was to evaluate the dietary fatty acids and the assess the blood fatty acid composition and immune parameters in AD patients. AD (n = 50) patients and gender ${\cdot}$ age matched healthy controls (HC) were studied in case-control clinical trail. Current fatty acids intake status was determined by 3-day food record method. Blood sample were collected from 30 subjects in each group and blood fatty acid composition and immune parameters were analysed. AD patients consumed less PUFA and their n-6/n-3 PUFA ratio was higher than that of HC. Both the ratios of PUFA and MUFA were positively correlated with SCORAD in AD patients (p < 0.05). In the AD patients, there were abnormalities in the fatty acid composition of the RBC and WBC, SFA being significantly high and most n-3 PUFA being significantly low. Moreover, both the ratios of EPA and DHA in WBC were negatively correlated with dietary n-6/n-3 PUFA ratio in AD patients (p < 0.05). Serum total IgE and IL-4 levels of AD patients increased significantly compared with the levels of HC (p < 0.01). Ratios of monocyte and eosinophil in WBC of AD patients increased significantly compared with the levels of HC including total WBC count (p < 0.01), and ratios of Iymphocyte and basophil in WBC of AD patients decreased significantly compared with the levels of HC (p < 0.05). Moreover, the ratios of eosinophil in WBC were positively correlated with dietary P/M ratio (p < 0.05), and the ratios of monocyte in WBC were positively correlated with n-6/n-3 PUFA ratio (p < 0.05) in AD patients. This results indicated that AD patients had significantly high intake of dietary n-6/n-3 PUFA compared with HC. Imbalance of intake of dietary fatty acids affected fatty acid compositions in the RBC and WBC, and these lead to immune imbalance and grow worse of AD.

아토피피부염 개인건강기록 앱 개발에 관한 연구 (Study on Development a Personal Health Record Application of Atopic Dermatitis in Korean Medicine)

  • Objective : The purpose of this study is to develop a personal health record(PHR) application of atopic dermatitis in Korean Medicine(KM). Methods : We have identified the items necessary to provide an PHR application that helps to record and manage the symptoms of an atopic dermatitis in KM. We also derived the symptom collection process and method and applied it to the application. Results : In this study, the types of symptoms collected for atopic dermatitis were derived. Symptoms include daily check, stool/urine/sleep, daily emotion, meal management, symptom photographs, SCORAD, quality of life, progress check, original symptom, pediatric health check, weakness check, and subjective symptoms. The recording cycle can be divided into the first, daily, weekly, specific day, and subjective. We developed the PHR application of atopic dermatitis in KM by deriving the type of symptoms and symptom recording process. The app organized menus into dashboards, checklists, daily checks, and health records. Conclusions : We developed a PHR application for atopic dermatitis by deriving symptom collection items of atopic dermatitis and developing symptom collection process and collection technique. The app does not make an accurate diagnosis of atopic dermatitis symptoms, but it helps facilitate symptom collection and helps to identify or predict a person's health condition. It can also be used for medical treatment through sharing symptoms with Korean medicine. Patients are able to communicate in both directions on a daily, weekly, self-aware basis, at the request of a doctor, to record their symptoms and use them for medical purposes. If the doctor asks for a progress check to refer to the next examination, it can be written through the system. This manages atopic dermatitis in daily life and can be used in the clinical field.

아토피 피부염 증례 연구 분석 - 한약 처방을 중심으로 - (Analysis of Case Studies of Treating Atopic Dermatitis - focusing on Korean Herbal Medicine Used in Cases -)

  • Objectives : The purpose of this study is to investigate the use of herbal medicine for the treatment of atopic dermatitis in cases and to find the general tendency of herbal medicine treatment and to establish the primary treatment direction of Atopic dermatitis. Methods : In the domestic databases, Oriental medicine Advanced Searching Integrated System(OASIS), Korean Traditional Knowledge Portal(KTKP), National Discovery for Science Leader(NDSL), Research Information Sharing Service(RISS), we selected among the papers published in the last 20 years using search terms related to "Atopic dermatitis & Cases". Reports based on Sasang constitutional medicine and Six meridian pattern identification were excluded and a total of 21 papers were finally selected. Results : 113 herbal medicines were retrieved from 21 papers. The most commonly used herbs are Scutellariae Radix(黃芩), Rehmanniae Radix(地黃), and Glycyrrhizae Radix et Rhizoma(甘草). Atractylodis Rhizoma(蒼朮), Akebiae Caulis(木通), Smilacis Rhizoma(土茯?), Paeoniae Radix(芍藥) used mostly when Oozing exists, Trichosanthis Radix (瓜蔞), Asparagi Radix(天門冬), Persicae Semen(桃仁), Carthami Flos(紅花) used mostly when Dryness, Lichenification, and Pigmentation exist. The average score of SCORAD index was improved after Herbal medicine treatment. Conclusion : Through this study, we could find out the tendency of herbal medicine to treat Atopic dermatitis along with Symptoms.
