A STUDY ON CORMORBID PSYCHOPATHOLOGY AND PARENTING ATTITUDE IN CHILDREN AND ADOLESCENTS WITH ATOPIC DERMATITIS (아토피피부염 소아 청소년에서의 정신병리와 양육태도 연구)

  • The purpose of this study was to examine the nature of psychopathology and psychosocial environments of children with Atopic Dermatitis(AD) and their relationship to skin symptoms. Seventy outpatients with AD(mean age 9.94, range 4-15 years) and their mothers were subjected to psychiatric interview and parent- and self-report questionnaire(CBCL, MBRI, CDI, Spielberger's State-Trait Anxiety Inventory for Children(STAI), Family Impact questionnaire). Disease severity was assessed in three dimensions(skin extent, disease activity, and subjective symptom) by SCORAD index. Sixteen(22.9%) of our sample were suspected to have behavioral-emotional problems and they were different from non-problem group on depression, trait-anxiety, maternal controlling attitude and negative appraisal of children. While skin inflammation activity was correlated with only trait-anxiety and depression, subjective skin symptoms were correlated with many psychosocial factors such as depression, state-and traitanxiety, family-impact, social competence, internalized and externalized behavioral problem, sexual problem, and mother's positive appraisal of children). We concluded that children with AD are highrisk group for behavioral-emotional problems, and these problems along with maternal negative parenting attitude can affect their subjective symptom experience.

Retrospective Study about the Effectiveness of a long-term Korean Medicine Treatment on 1035 Atopic Dermatitis Patients (아토피피부염 환자 1035명의 장기간의 한의학적 치료효과에 대한 후향적 연구)

  • Objective : The purpose of this study is to report the effectiveness of a long-term Korean Medicine Treatment on 1035 atopic dermatitis patients Methods : Objective SCORAD Index(OSI) is used for measuring the degree of atopic dermatitis with photos of total 1035 atopic dermatitis patients who has visited oriental clinic from January 9, 2011 to October 31, 2013. Then, OSI was estimated according to gender, age, period of treatment and intensity. Results : 1. The ratio of male to female was 44.4 : 55.6. All patients were constituted with 26.5% of 0~11 year-old group, 15.7% of 12~18 year-old and 57.9% of 19 and more year-old. 2. The ratio of OSI improvement was significantly higher in women than in men after 3 month treatment, but there was no significant difference after 6, 9 and 12 months. 3. The OSI improvement of 0~11 year-old group was significantly higher than the other groups after 3 month treatment. But at the other periods of treatment, there was no significantly difference of the improvement among 3 different groups of age. 4. After 3, 6, 9 and 12 month treatment, the score of OSI was significantly lower than before treatment. After 12 month treatment, the score of OSI was lowest compared to the other periods of treatment. 5. The final OSI score of 87.7% patients was decreased compared to the initial. 6. All patients had been divided with 1.93% of the mild, 22.7% of the moderate and 75.4% of the severe by OSI intensity at initial visit. At final visit, the composition of patients was changed to 14.6% of the mild, 53.7% of the moderate and 31.7% of the severe. 7. The mild patients had got 42.56% of OSI improvement at the final. And the moderate had got 37.72% of OSI improvement at the final. 8. The severe patients had got 36.76% of OSI improvement at the final. And 89.2% of the severe had got decreased final OSI score compared to the initial. Conclusion : There was a definite decreasing of OSI score on the significant rate of Atopic dermatitis patients who had been treated by Korean medicine treatment at least for 3 months.

A Clinical Study for the effect of cosmetic cream containing Samwhangsejegamibang Extracts on Atopic dermatitis patients (삼황세제 가미방 화장품의 아토피 피부염 환자에 대한 임상적 연구)

  • Objective : The purpose of this study is to investigate the effect of cosmetic cream containing Samwhangsejegamibang Extracts on Atopic dermatitis patients. Methods : A total of 51 patients who visited Wonkwang Oriental Medical Center from April 8th, 2011 to October 8th, 2011 were included in this study. In this study, the patients were treated with Samwhangsejegamibang (experimental group) and placebo group. We observed scorad index, IGA, moisture content of skin, oil content of skin, wrinkled skin and hematological examination for 4 weeks. Results : 1. Comparison of changes in Scorad Index, depending on the perspective, the experimental group showed a statistically significant decline than the placebo group. 2. Comparison of change in IGA, depending on the perspective, the experimental group showed a statistically significant decline than the placebo group. 3. Comparison of change in moisture content, oil content of skin and wrinkled skin, both the experimental group and the placebo group showed level elevation but there was no statistically significant change. 4. Comparison of change in IgE and Eosinophil, both the experimental group and the placebo group showed no statistical significance. Conclusions : Considering the above experiments, it is suggested that cosmetic cream containing Samwhangsejegamibang extracts have a positive effect on atopic dermatitis patients.

A Clinical Study for the Efficacy and Safety of Functional Cosmetics Containing peptide Scolopendrasin-I isolated from the Scolopendra subspinipes mutilans in Patients with Dry Skin due to Mild Atopic Dermatitis (아토피성 피부로 건조함을 가진 대상자에 대한 왕지네로부터 분리한 펩타이드 Scolopendrasin-I 함유 기능성 화장품의 기능성 및 안전성을 평가하기 위한 임상적 연구)

  • Objectives : The purpose of this study is to confirm the efficacy and safety of "Functional cosmetics containing peptide Scolopendrasin-I isolated from the Scolopendra subspinipes mutilans" on dry skin due to mild atopic dermatitis. Methods : A total of 24 patients who visited Semyung Oriental Medical Center from November 1st, 2018 to December 20th, 2018 were included in the study. In this study, the patients were treated with Functional cosmetics containing peptide Scolopendrasin-I isolated from the Scolopendra subspinipes mutilans. For 4 weeks of gross examination, instrumental assessment were made before and after the study to evaluate how well the products for treatment group in recovering the dry skin barriers by mild atopic dermatitis. Results : 1. In the primary endpoint, Skin Hydration showed a statistically significant increase in treatment group in baseline, 2 weeks and 4 weeks. 2. In the primary endpoint, Transepidermal Water Loss(TEWL) showed a statistically significant decrease in treatment group between Baseline and 4 weeks. 3. In the secondary endpoint, Skin pH showed a statistically significant decrease in treatment group between baseline and 4 weeks. 4. To evaluate the safety of the products for the human body, Adverse events, SCORAD Index Assessment were conducted; There were no severe adverse events during this study. And SCORAD Index showed a statistically significant decrease in treatment group in baseline, 2 weeks and 4 weeks. Therefore, it is suggested that products, if used for certain period, should be safe for the human body. Conclusions : According to the above experiments, it is suggested that "Functional cosmetics containing peptide Scolopendrasin-I isolated from the Scolopendra subspinipes mutilans" should be effective for dry skin due to mild atopic dermatitis.

Anti-oxidative and anti-inflammatory effects of Danpitang in RAW 264.7 cell (단피탕(丹皮湯) 추출물의 항산화 및 항염증 효과 연구)

  • Objectives : The purpose of this study is to investigate the anti-oxidative and the anti-inflammatory effects of Danpitang(DPT) extract in RAW 264.7 macrophages. Methods : The macrophage cell line RAW 264.7 cells were used and MTT assay was performed to measure the cell viabilities at the various concentrations of DPT($50-400{\mu}g/m{\ell}$). Nitric oxide(NO) was measured in LPS-induced RAW 264.7 cells. Expressions of iNOS, COX-2, $TNF-{\alpha}$, $IL-1{\alpha}$, $IL-1{\beta}$ and IL-6 were also performed by real-time PCR. Protein expression of iNOS and COX-2 was confirmed by western blot. The anti-oxidant activities of DPT was measured by DPPH radical scavenging activity. Results : 1. There was no cytotoxicity in RAW 264.7 cells treated with DPT compared to the control. 2. DPT treated group significantly inhibited NO production compared to the LPS treated group. 3. DPT treated group significantly decreased mRNA expressions of iNOS, COX-2, $TNF-{\alpha}$, $IL-1{\alpha}$, $IL-1{\beta}$ and IL-6 compared to the LPS treated group. 4. To evaluate the safety of the products for the human body, Adverse events, SCORAD Index Assessment were conducted; There were no severe adverse events during this study. And SCORAD Index showed a statistically significant decrease in treatment group in baseline, 2 weeks and 4 weeks. Conclusions : According to the above results, DPT can be used as a therapy in various anti-inflammatory skin diseases.

Characteristics of Clinical Trials in Korea for Atopic Dermatitis - Focus on ClinicalTrials.gov Registered Clinical Trials - (식약처 승인 아토피 피부염 의약품 국내 임상 시험의 특성 - ClinicalTrials.gov 등록 임상시험을 중심으로-)

  • Objective : This study summarized the characteristics of clinical trials for atopic dermatitis medicines approved by the Ministry of Food and Drug Safety(MFDS). This study may be a reference for the design of clinical trials of atopic dermatitis herbal medicine treatment which may be carried out later. Method : The characteristics of the clinical trial were analyzed for clinical trials registered with ClinicalTrials.gov, CRIS, and the Korea Health Industry Development Institute among the clinical trial approval statuses posted on the website of the MFDS. Result : 1. Clinical trial drugs were developed in various formulations such as oral medicines, injections, dermatologic agents, and similar proportions. Relatively little clinical trials were found for herbal medicine. 2. In the control evaluation test, most of the treatments for the control group were performed with placebo using Vehicle. 3. In most clinical trials, one intervention group was in the form of a parallel assignment with only one treatment. 4. The age of the subjects was 11 out of 28 studies including minors, and clinical trials targeting minors were also found to be significant. 5. In the case of atopic dermatitis, the cases of subacute chronic or atopic dermatitis more than 6 months or more than 1 year were often used. 6. Most clinical trials were divided into mild to moderate atopic dermatitis or moderate to severe atopic dermatitis. The SCORAD index, EASI, IGA, BSA, and NRS were used as the evaluation criteria. 7. Regulations for the drugs used prior to the trial period for the treatment of atopic dermatitis vary somewhat from one clinical trial to another. 8. IGA was used most often as a primary efficacy tool, and SCORAD index, EASI, and NRS were also used.

A Case of Adult Otitis Media with Effusion Accompanied by Atopic Dermatitis (아토피피부염을 동반한 성인 삼출성 중이염 치험례)

  • Objectives : This study is to report a case of otitis media with effusion accompanied by atopic dermatitis improved by Korean medicine monotherapy. Methods : A 31-year-old woman patient visited our clinic for aural fullness, hearing loss, and tinnitus of the left ear on August 14th, 2021. She also had abdominal pain, diarrhea, and general weakness because of side effects of antibiotics. Besides, she had nasal congestion, rhinorrhea, and dry skin. Through otoscopy, her tympanum was fully filled with yellow effusion. She only took Korean medicine therapy including acupuncture and herbal medicine weekly. The treatment continued for about 4 months(total 14 visits) until December 4th, 2021. Otoscopy, numerical rating scale(NRS), scoring atopic dermatitis(SCORAD) index were used to assess the symptoms. Results : On August 30, 2021, her intestinal symptoms improved. The left tympanum was recovered to normal condition as the treatment continued. The effusion disappeared on October 16th, 2021. On November 6th, 2021, peritympanic blood congestion also almost alleviated. Subjective symptoms including aural fullness, hearing loss, and tinnitus disappeared on October 16, 2021. Nasal symptoms and dry skin also improved. The SCORAD index dropped from 15 to 4. Conclusions : These results suggests that otitis media with effusion can be sufficiently managed by Korean medicine therapy. Further, active application of Korean medicine therapy to common otolaryngological diseases is necessary.

Efficacy and Safety of a Biodegradable Hyaluronic Acid Microneedle Patch for Dry Skin in Atopic Dermatitis : A Single-blinded, Split-body, Randomized Controlled Trial (아토피피부염 피부건조 증상 개선에 대한 용해성 마이크로니들 패치의 유효성 및 안전성 평가 임상연구)

  • 목적 : 마이크로니들 패치는 피부장벽의 주요 구성성분인 각질층을 관통할 수 있어 일반적인 경피를 통한 약물 전달 수단보다 더 효율적으로 약물을 전달할 수 있다는 장점이 있어 다양한 분야에서 응용되고 있다. 본 임상연구는 아토피피부염 환자의 피부 건조 증상에 대하여 용해성 히알루론산 마이크로니들 패치와 외용제를 병행하여 용해성 마이크로니들 패치의 유효성과 안전성을 평가하였다. 방법 : 본 연구는 단일 기관, 연구자 주도, 평가자 눈가림, 인체 분할 연구로 설계된 무작위 배정 임상시험으로, 피부건조 증상이 있는 아토피피부염 환자 20명을 대상으로 임상연구를 수행하였다. 한 명의 피험자에서 2곳의 시험 병변을 선택한 후, 무작위 배정으로 각각 시험 병변과 대조 병변을 결정하여 2주 동안 매일 양측 병변에 모두 아로마 크림을 도포한 다음, 시험 병변에만 용해성 히알루론산 마이크로니들 패치를 추가로 부착하였다. 결과 : 1차 유효성 평가지표인 local scoring atopic dermatitis(L-SCORAD) index는 치료 후에 시험 병변이 대조 병변보다 유의성 있게 호전되었다. 2차 유효성 평가지표 중 피부 건조감 VAS와 피부 수분 함유량도 시험 병변이 대조 병변보다 유의성 있는 개선을 보였다. Investigator's Global Assessment scale, 소양감 VAS, 경피수분손실량에 있어서는 시험 병변과 대조 병변 모두 치료 후에 유의성 있게 호전되었으나, 두 병변 간의 차이에는 유의성이 없었다. 이상 반응은 나타나지 않았으므로 안전성도 확인되었다. 결론 : 용해성 히알루론산 마이크로니들 패치와 외용제 병행치료는 외용제만 사용하였을 때보다 L-SCORAD index 및 피부 건조감 VAS, 피부 수분 함유량에 있어서 아토피피부염의 증상을 유의성 있게 개선시켰다.

A Case Study of a Patient with Atopic Dermatitis improved by Yangdokbaekho-tang (양독백호탕(陽毒白虎湯)으로 개선된 아토피 피부염 환자 치험 1례)

  • Objectives The purpose of this case study was to report the main effect of Yangdokbaekho-tang to a Soyangin atopic dermatitis patient. Methods This patient was treated by Yangdokbaekho-tang, topical herbal extract, moxa and acupuncture according to the result of Sasang constitutional diagnosis. Results and Conclusions The patient's subjective symptoms of atopic dermatitis and general condition were improved after using Yangdokbaekho-tang in spite of patient's loose stool. The SCORAD index (Scoring Atopic Dermatitis Severity Index) which was used to evaluate the therapeutic effect of Sasang constitutional treatment on atopic dermatitis was also decreased from 86.3 to 42.8 without using anti-histamine or steroid medications.

  • 본 임상연구는 임상연구 모집 공고를 통하여 모집된 대상자 중 Atopic Dermatitis Research Group (2005) 한국인 아토피 피부염 진단기준에 따라 아토피 피부염으로 판정되고 현재 약물(한약, 양약)치료나 외용제 치료를 하고 있지 않은 만 5$\sim$50세의 아토피 피부염 환자를 대상으로 하였다. 피험자들을 이중맹검, 블록무작위배정하여 시험군과 대조군으로 나누어 6주간의 시험기간 동안 육안적 평가, 혈액학적 검사, 피부측정 장비를 통한 기기적 평가를 시험 전후에 실시하여, 시험군에 사용된 제품이 대조군 사용된 제품에 비하여 얼마나 효과적으로 아토피 피부염으로 손상된 피부 장벽을 회복하는지 관찰하였다. 본 임상시험을 통해 'Atomento solution & cream'이 손상된 피부장벽을 회복시키고 피부 보습력을 증가시켜 아토피 피부염에 효과적이었으며, 인체에 이상반응이나 부작용 없이 안전하게 사용될 수 있음을 확인하였다.
