Esthetic and functional surgery and reconstruction after oral cancer ablation (임상가를 위한 특집 3 - 심미-기능적인 구강암 수술과 재건)

  • Oral cancer ablation surgery results in tissue defects with functional loss. Accompanying neck dissection results in facial nerve weakness and dysmorphic changes. To minimize the complications after oral cancer surgery, accurate dissection without damaging facial nerve and vital structures are mandatory. Marginal mandibular branch of facial nerve should be dissected or contained in the superficial layer of deep cervical fascia to minimized facial palsy after operation. Reconstruction after cancer ablations is routine procedures and free flap reconstruction is the most commonly used. Radial forearm free flap is the most versatile flap to reconstruct soft tissue defects and it is easy to design according to the defect size and shape. However, donor site scar and secondary skin graft from thigh result in unesthetic and cumbersome wounds. Double layered collagen graft in the donor site could reduce secondary donor site for skin graft. In conclusion, oral and maxillofacial surgeon should know the exact anatomy of the face and neck during neck dissection. Radial forearm free flap is most versatile flap for soft tissue reconstruction and double collagen graft can reduce postoperative scar and there is no need for secondary skin graft.

TREATIMENT OF ANKYLOGLOSSIA USING Z-PLASTY TECHNIQUE: A CASE REPORT (설유착증 환자에서의 Z-Plasty를 이용한 치료증례)

  • Ankyloglossia, or tongue-tie, is a congenital condition which occurs as a result of fusion between the tongue and the floor of the mouth. Ankyloglossia often results in malocclusion with an anterior "open bite" deformity, early prognathism, swallowing problem, speech disorder, and periodontal problem. Generally lingual frenectomy is used for treatment of ankyloglossia, but incomplete operation and simple frenectomy may produce a scar contracture resulting in a more deformed ankyloglossia than was present initially. The Z-plasty is used for the correction of scar contractures and the replacement of missing tissue and this procedure is ideally suited for the treatment of an ankylosed frenum. Most authors advise postponement of any decision for surgical correction of tongue-tie until the age of 4 years, unless the child is having much difficulty with sucking or swallowing. We treated 4 patients with ankyloglossia using Z-plasty technique. As a result, we found out that it was effective for correction of movement limitation of tongue, prevention of relapse. Further, periodic check ups are needed for evaluation of relapse, improvement of speech, and other functions of the tongue.

Perineal Reconstruction with the Perineal Perforator Based Island Flap (회음 천공지 기저 도서형 피판을 이용한 회음부 재건)

  • Perineal area is composed of compact structures of urogenital organs and anus requiring a more sophisticated selection of flap and reconstruction. For achieving better outcome then conventional flap surgery, we use the perineal perforator based island flap for its reconstruction. After locating the perforator by Doppler, the flaps were designed according to the defect or expected vaginal orifice. The flaps were elevated bilaterally as island pattern. Finally defect or neovagina was reconstructed with inconspicious linear scar hidden in the inguinal crease. Five cases were performed with the perineal perforator based island flap. There were 3 cases of vulvar cancer, 1 case of transsexualism, and 1 case of ambiguous genitalia because of congenital adrenal hyperplasia. Operative results were satisfactory with good contouring and less prominent donor scar, when they were compared with other flap reconstructions such as latissimus dorsi perforator flap, groin flap, gracilis myocutaneous flap etc. The perineal perforator based island flap is highly recommended with the advantages of easy flap elevation, good rotation arc, and appropriate flap thickness for contouring. Compared with other conventional flaps, it can be selected as a good option for moderate defect of perineal area.

Determination of Cytoplasmic Male Sterile Factors in Onion Plants (Allium cepa L.) Using PCR-RFLP and SNP Markers

  • We have developed a polymerase chain reactionrestriction fragment length polymorphism (PCR-RFLP) marker that can distinguish male-fertile (N) and male-sterile (S) cytoplasm in onions. The PCR-RFLP marker was located in a chloroplast psbA gene amplicon. Digesting the amplicons from different cytoplasm-containing varieties with the restriction enzyme MspI revealed that N-cytoplasm plants have a functional MspI site (CCGG), whereas the S-cytoplasm plants has a substitution in that site (CTGG), and thus no MspI target. The results obtained using this PCR-RFLP marker to distinguish between cytoplasmic male sterile factors in 35 onion varieties corresponded with those using a CMS-specific sequence-characterized amplified region (SCAR) marker. Moreover, the PCR-RFLP marker can identify N- ot S-cytoplasms in DNA sample mixtures in which they are in up to a 10-fold minority, indicating that use of the marker has high diagnostic precision. We also demonstrated the usefulness of the SNP detected in the psbA gene for high-throughput discrimination of CMS factors using Real-time PCR and a TaqMan probe assay.

Preliminary Results of Topical Mitomycin Application in Laryngotracheal Stenosis (후두기관협착증에 있어서 mitomycin 국소 도포 : 예비결과)

  • Restenosis frequently develops with granulation and overgrowth of scar following surgical treatment for laryngotracheal stenosis. Various methods such as stenting or CO2 laser application have been used to prevent restenosis, but they were seldom unsatisfactory. Mitomycin is an antineoplastic antibiotics derived from Streptomyces caespitosus; it inhibits fibroblast proliferation and acts as an alkylating agent to inhibit DNA synthesis. This study was desinged to evaluate effectiveness and determine indications of usage of topical mitomycin for laryngotracheal stenosis as a nonsurgical means of reducing postoperative granulation and scar tissue formation. Patients and Method : A retrospective study was performed on eight cases of laryngotracheal stenosis with topical mitomycin application. The author analyzed clinical outcomes of operative treatment with topical mitomycin. Patients underwent laryngotracheal reconstruction, endoscopic granulation removal, or bronchoscopic bougienage followed by topical application of mitomycin (0.4 mg/$m{\ell}$, 4minuntes) on the lesion intraoperatively. Result : Overall success rate of decannulation was 38% ($\frac{3}{8}$). Successful decannulation was possible in 75% ($\frac{3}{4}$) of laryngeal stenosis patients, 75% ($\frac{3}{4}$) of children, 60% ($\frac{3}{5}$) of the patients without previous surgery, and 75% ($\frac{3}{4}$) of bronchoscopic bougienage. Conclusion : The topical application of mitomycin in laryngotracheal stenosis was effective in untreated pediatric laryngeal stenosis which underwent bronchoscopic bougienage. Our results show that the topical mitomycin application for laryngotracheal stenosis could be a effective adjuvant treatment.

Steroid Injection on Facial Hypertrophic Scar: Report of 3 Cases (안면부 비후성 반흔에 스테로이드 주사: 증례보고)

  • Traumatic lacerations are common in the orofacial region as a result of accidents. Due to the frequent movement of the skin around the mandible, scars in that area are more likely to widen or become hypertrophic. Treatment of facial laceration was performed on three patients and followed by regular check-up. It was discovered that the scars have become hypertrophic, so steroid injections were used. The results were satisfactory with the decrease in sizes and hardnes of the scar. Facial scarring is a particularly distressing phenomenon and has always been a challenge to treat as the scars are more likely to widen or become hyphertrophied due to the frequent movement of the muscle in the facial area. We confirmed that the positive effect of steroid on hypertrophic scars. So, we suggest that proper treatment and periodic follow-up, adjuvant treatments especially steroid injection is necessary in patients with lacerations.

Biocompatible Material Design Minimizing Hypertrophic Injury and Treatment Effects using a Mini-pig (비대성 흉터를 최소화하는 생체적합성 재료설계와 미니돼지에 대한 치료 효과)

  • Recently, biomedical-grade texture material gauze has often been used to treat wounds. At this time, it is difficult to remove scratches and pushed gauze; if you remove it with force, the tissue may separate and bleeding may occur again. In this study, we studied a method to apply medical-grade silicone material. Similar to the research result that hypertrophic wounds reduce the thickness of scar marks. Through mini-pig experiments, we evaluated the effects on scar treatment. The test results showed that the silicone cover layer applied to the wound site had a sealing effect on the wound area, skin temperature, and histopathological examination. In conclusion, gel treatment utilizing a biocompatible substance had the effect of minimizing hypertrophic scars.

Effect of Benzalkonium Chloride on Percutaneous Absoption of Antisense Phosphorothioate Oligonucleotides

  • The effect of benzalkonium chloride on skin permeability of partially modified antisense phosphorothioate oligonucleotides (PS-ODN), which are designed as scar formation inhibitor, was investigated using Franz Diffusion Cell. When the concentration ratio of PS-ODN-quarternary ammonium salt complex is more than 1:100, the apparent partition coefficient (APC) of each complex was increased in the following order; tetraphenyl phosphonium chloride (TPP) < cetyltrimethyl ammonium bromide(CTAB) < benzalkonium chloride (BZ). The permeability of PS-ODN through the rat skin increased in the presence of BZ. The fluxs of PS-ODN with BZ were increased by addition of Pluronic F 68 or Triton X-100 to phosphate buffered saline (PBS), respectively. When the mole ratio of PS-ODN to BZ is 1:10, the fluxs penetrated of PS-ODN with BZ was greatest. The increase of the permeability in the presence of BZ might be due to the formation of lipophilic ion-pair complex between PS-ODN and BZ. By regulation of mole ratio of PS-ODN to BZ, the development of topical dosage forms using PS-ODN as scar formation inhibitor will be possible with minimal systemic exposure.

Lubricating Characteristics of Diesel Fuel (경유의 윤활특성)

  • The reduction of sulfur content in the diesel fuel has caused the poor lubricity of diesel fuel in the distributor type injection pumps of diesel engines that use the diesel for lubrication of their moving parts. To investigate the reason for poor lubricity of low sulfur diesel fuels, the wear scar diameters by HFRR (High Frequency Reciprocating Rig) were measured on the diesel fuels from Korean markets and the results were compared with their physical and chemical properties. Also, the lubricity change and the improvement effects on lubricity additives for the ultra low sulfur diesel fuel, were made experimentally, that will be regulated to a maximum of 0.005 wt% from about 2005 were evaluated. As a result, a good correlation was found between the wear scar diameter and the polyaromatic compound which includes heterocyclic compound in the diesel fuel. It was also found that the content of polyaromatic compound including heterocyclic compound was affected by the amount of desulfurization treatment fraction. And the lubricity additives with ester base were more effective than that with acid base on the ultra low sulfur diesel fuel. Therefore, it is suggested that the factors affecting the lubricity stated above should be taken into account to improve the lubricity property of the diesel fuel in the refining process.

Case Study of Atrophic Acne Scar Treatment with Re-cell Therapy (리셀테라피를 이용한 위축성 여드름 흉터 치료 11례)

  • Objective: Acne scarring is the most common sequela after dermal inflammation caused by acne. Many methods are currently used to treat acne scarring; doctors using traditional Korean medicine have also attempted to treat acne scarring in a new way that was developed from traditional methods. This study examines the effectiveness of re-cell therapy.Method: Eleven patients with various types of acne scars were treated by re-cell therapy. We used the global acne scarring classification (GASC) and ECCA (from the French “echelle d’evaluation clinique des cicatrices d’acne”), both of which are generally used to measure and grade the degree of acne scarring.Results: As a result of the treatment, the conditions of all patients were conspicuously improved in both scales. The GASC scores of the patients changed from grade 19.6±10.4 to grade 4.9±3.3 (p=0.001), and the ECCA scores changed from grade 120.9±28.7 to grade 41.8±18.3.Conclusion: Re-cell therapy showed effectiveness in treating acne scarring and could be applied in clinical cases.
