  • Hiccups, also known as singultus, are spasmodic involuntary contractions of respiratory muscles that shorten respiration. The characteristic sound is caused by rapid closure of the glottis. In oriental medicine, the mechanism of hiccups is "reversed flow of Ki". Hiccups have been associated with neoplasm, infections, seizures, diabetes. renal failure, alcohol ingestion, various drugs, and ischemic events of the myocardium or central nervous system. We present a case of chronic hiccups after cerebral infarction that was successfully treated using moxibustion therapy.

Study of Gastric Electrical Activity and Clinical Characteristics in Patients with Gastroparesis or Non-gastroparetic Functional Dyspepsia in Eastern Traditional Medicine (한방병원에 내원한 위마비 및 비위마비성 기능성 소화불량증 환자에 대한 위 운동성 및 임상적 속성에 관한 연구)

  • Little is known concerning the pathophysiology and symptomatology of gastric dysmotility of patients who have sought treatment in eastern traditional medicine. The aim of this study was to investigate gastric electrical activity and clinical characteristics between gastroparesis and non-gastroparetic functional dyspepsia(FD) and assess the necessity to distinguish them for eastern traditional medical treatment. 60 patients were surveyed by questionnaire. Gastric electrical activity was recorded and analyzed using electrogas-trography. Patients were divided into gastroparesis and non-gastroparetic FD by 1.0 of power ratio. Healthy persons were used as reference. Postprandial normogastria of gastroparesis decreased and that of non-gastroparetic functional dyspepsia increased compared before and after meals. Suggested causes of onset were the meat diets that most patients were on in the gastroparesis group, and that drugs that patients in the non-gastroparetic FD group were taking. Distribution of main symptom was equally regular in patients with gastroparesis, but patients with non-gastroparetic FD showed mainly upper abdominal pain and fullness. Values for the gastroparesis group were double those for the non-gastroparesis FD group in frequency of upper GI endosopic examination. Results suggest that effective application of eastern traditional medical treatment requires that first a clear distinction be made in state of gastric dysmotility and clinical characteristics of gastroparesis and non-gastroparesis FD when treating dyspepsia.

Effects of Traditional Korean Medical Therapy on Gastric Myoelectrical Activity in Patient with Functional Dyspepsia (한방치료가 기능성 소화불량증 환자의 위전도 소견에 미치는 효과)

  • Background & Object: The aim of this study was to investigate the effects of Traditional Korean medical therapy on gastric myoelectrical activity in patients with functional dyspepsia. Methods: 29 patients(male 10, female 19) were divided into two groups; Inpatient group(N=10, male 1, female 9) and outpatient group(N=19, male 9, female 10). 10 healthy people participated in this study as normal controls. Gastric motility was recorded and analyzed by electrogastrography(EGG) during fasting and postprandial periods. Hospital treatment included electroacupuncture on ST36, acupuncture, and herbal medicine. Outpatients were treated with herbal medicine and acupuncture. Inpatients underwent hospital treatment for two weeks, and after discharge they were treated as outpatients for six weeks. In inpatient group, EGG was recorded before and after hospital treatment and it was recorded at six weeks after discharge. In outpatient group, EGG was recorded before starting treatment and it was recorded once a every six weeks for twelve weeks. Results: There was no significant difference in EGG parameters between before and after therapy in outpatient group. In inpatient group, postprandial normal slow wave increased after hospital treatment(from $33.94{\pm}14.13$ to $62.63{\pm}20.18$, p<0.05) and postprandial tachygastria decreased(from $43.53{\pm}13.99$ to $28.30{\pm}16.63$, p<0.05) and arrhythmia decreased(from $20.35{\pm}15.91$ to $5.67{\pm}6.80$, p<0.05). Conclusions: Results suggest that hospital treatment through electroacupuncture on ST36, acupuncture, and herbal medicine is useful to improve gastric myoelectrical activity in Patients with Functional Dyspepsia.

  • Background & Object: The aim of this study was to investigate the association of degree of fatigue and gastric motility, measured by EGG, with skin sympathetic tone or cardiovascular reactivity in patients with functional dyspepsia. Methods: Subjects were 56 patients with Functional dyspepsia and eight healthy people. Degree of fatigue was assessed by questionnaires consisting of subjective complaints of fatigue. Skin sympathetic tone was measured by Ryodoraku Score and Cardiovascular Reactivity was checked by Pulse diagnostic apparatus. Gastric motility was estimated by EGG. First, all patients were divided into two groups by Ryodoraku Score $40{\mu}A$(below and above). Second, they were subdivided into two groups by Cardiovascular Reactivity(decreased and increased or not decreased). Estimates were made on the extent differences of degree of fatigue or state of gastric motility in each group. Results: 1. Fatigue scores was significantly higher in females and in the Ryodoraku-Score-below-$40{\mu}A$ group. It was higher in the decreased cardiovascular reactivity group than the increased group, but to no significanct extent. Also, gastric motility was better in the Ryodoraku-Score-above-$40{\mu}A$, group than in the below group. Conclusions: These results suggest that degree of fatigue and gastric motility are associated with skin sympathetic tone, but not associated with cardiovascular reactivity, and that $40{\mu}A$ is a useful cutoff point in Ryodoraku Score for assessing degree of fatigue in functional dyspepsia patients.

Analysis of symptom pattern through comprehensive diagnosis of Qui Xui Shui in patients with functional dyspepsia (기혈수변증(氣血水辨證)에 의한 기능성 소화불량 환자의 변증유형 분석)

  • Background & Object : It is well known that functional dyspepsia is one of the most common diseases. While many dyspepsia patients have been helped with oriental medical therapy, there has not been a study based on the concepts of oriental medicine. The aim of this study was to perform a fundamental epidemiological survey and to analyse the symptom pattern of functional dyspepsia. Methods : 86 patients(27 males, 59 females) diagnosed with functional dyspepsia in Kyunghee Oriental Medical Center from May to December 2002 were involved in this investigation. The disease characteristics of functional dyspepsia(based on Rome criteria II) and the Qui Xue Shui diagnostic procedure were investigated by questionnaire and physical examination. The total score and composition ratio of each comprehensive diagnosis were calculated from the symptom score described in the questionnaire. Results : The total score from the Qui Xue Shui diagnostic procedure was found to be influenced by the number of functional dyspepsia symptoms(p=0.026) and the patient's own cognition of their current dyspeptic situation(p=0.006), in addition to digestive ability and general congnition(p=0.006), and was not associated with sex, duration of dyspeptic symptoms in one year, or the total illness period, In the composition ratio of the Qui Xue Shui diagnostic procedure, only Qui-yu(p=0.048) diagnosis was accurate regarding the number of symptoms in one year, total illness period, or the patients' own congnition of their current dyspeptic situation, But the value of the composition ratio among the total factors involved was most similar to that of sex. Conclusions : Thus, it is shown here that the total score of using the Qui Xue Shui diagnostic procedure is mainly dependent on dyspeptic symptoms and the patinets' own cognition of their current dyspeptic situation, and that regular symptom patterns in the Qui Xue Shui diagnostic procedure exist in dyspeptic patients.

  • Background & Object : The aim of this study was to investigate the usefulness of electrogastrography on differential diagnosis of deficiency or excess Condition in patients with functional dyspepsia Methods : Selected symptoms for diagnosis of deficiency and excess in functional dyspepsia have been reviewed in some literatures, notably within Oriental medicine, Donguibogam and Gogeumdoseojipseong-uibujeolrok. This is what was investigated in this study. 93 patients (male 32, female 61) were divided into three groups; Non-pain group, Pre-treatment pain group (chief complain was abdominal pain at the first medical examination), and Mid-treatment pain group(they had no pain at the time of first medical examination, but showed abdominal pain within two weeks). 10 healthy people participated as normal controls. Gastric motility were recorded and analyzed using electrogastrography during fasting and postprandial period. In assessment, effectiveness was divided into validity, sensitivity and specificity. Results : Epigastric pain reflected the tendency for excess condition in comprehensive diagnosis, which was linked with postprandial arrhythmia in electrogastrography(p=0.001). Postprandial arrhythmia detected abdominal pain with a validity of 78.6%, a sensitivity of 79.2%, and a specificity of 78%(p<0.001). Conclusions : Results 1Tom Postprandial arrhythmia in electrogastrography support that the index of excess condition in the comprehensive diagnosis of symptoms is useful for patients with functional dyspepsia.
