A CASE REPORT OF ORBITAL FLOOR RECONSTRUCTION WITH TEMPORAL MUSCLE-CORONOID PROCESS FLAP (측두근-오훼돌기 피판을 이용한 안와저의 재건 예)

  • The maxillary squamous cell carcinoma is major part of maxillary malignant tumor. The treatment of maxillary malignancy tumor is the maxillectomy in combination with radiation therapy and chemorherapy. When tumor invasion is occured to the orbit, orbital exenteration is required. But if the periosteum of the orbital floor is intact, the orbit can be preserved. There are many orbital floor reconstruction materials for the prevention of ptosis of the orbital content. The patients on this paper were diagnosised as squamous cell carcinoma on maxilla, we performed the partial maxillectomy including the orbital floor, and we used temporalis muscle-coronoid process flap for the reconstruction of the orbital floor after partial maxillectomy and obtained good esthetic and functional results, as followed. 1. We obtained sufficient flap width for defect of orbital floor. 2. It permits good blood supply and no necessary other donor site. 3. It gives a solid base for the support the globe and the orbital floor. 4. It gives minimal postoperative morphorogical defect and functional disturbance.

Danshensu Isolated from Prunella vulgaris var. Lilacina Attenuates MK-801-induced Prepulse Inhibition Deficits in Mice (하고초 추출물로부터 분리된 Danshensu의 MK-801으로 유도된 사전자극 억제 손상의 회복에 대한 작용)

  • Schizophrenia is a severe psychiatric disorder and characterized by positive symptom (i.e., delusions, hallucinations), negative symptom (i.e., lack of emotion, social withdrawal), and cognitive impairment. Previously, we reported that the ethanolic extract of Prunella vulgaris var. lilacina attenuated the MK-801-induced schizophrenia-like behaviors such as prepulse inhibition (PPI) deficits and cognitive impairment in mice. The aim of the present study was to investigate whether danshensu isolated from P. vulgaris var. lilacina attenuates MK-801-induced sensorimotor gating dysfunction (PPI deficits), hyperlocomotion, and memory impairment in mice. Acute administration of danshensu (1, 3, or 10 mg/kg) significantly ameliorated the MK-801-induced PPI deficits in the acoustic startle response test. We also observed that the impaired recognition memory induced by MK-801 was attenuated by danshensu (1 mg/kg) in the novel object recognition test. However, danshensu failed to reverse the MK-801-induced hyperlocomotion in the open-field test. Collectively, the present results indicate danshensu would be an active agent for treating neuropsychiatric disorders such as schizophrenia.

Single Case Report of a Stage IV Geriatric Gallbladder Cancer Patient treated with Korean Medicine Who Could Not Receive Conventional Therapies (항암 치료가 불가한 고령의 4기 담낭암 환자 한방치험 1례)

  • Background : This case is to report a gallbladder cancer patient treated with Korean Medicine who could not receive conventional cancer therapies such as operation and chemotherapy due to her underlying diseases, old age and poor performance. Method and Results : An eighty-year-old female patient was diagnosed with stage IV gallbladder cancer on Feb. 2014 and she already had hypertension, diabetes and Parkinsonism. Comprehensive Korean Medicine (KM) treatments instead of operation and chemotherapy were administrated due to her poor condition and old age. KM treatments were done focused on improving symptoms and quality of life combined with best supportive care. During KM treatments her abdominal pain was generally managed and she survived for 22 months. Conclusion : Further studies should be performed in the future to clarify the survival benefit and symptom management of comprehensive KM treatments.

  • In pediatric dentistry, chloral hydrate is habitually selected for sedation of uncooperative children. Although chloral hydrate has been used for decades, various adverse effects are reported and necessity for new alternative drugs has increased. Dexmedetomidine was approved by FDA for sedation at intensive care units (ICU) in 1999. Compared to conventional sedative drugs, dexmedetomidine has not only analgesic and sedative effects but also it barely suppresses the respiratory system. Due to these characteristics, dexmedetomidine is known as safe sedative drug for children and elderly patients. Furthermore, approved by KFDA in 2010 in Korea, the frequency of sedation using dexmedetomidine is increasing. However, due to its intravenous administration method, it was difficult to apply in pediatric dentistry. Recently, intranasal administration method was introduced which might be a new possible alternative of oral sedation. In this study, we compare the mechanisms, pros and cons of chloral hydrate and dexmedetomidine, introducing new possibilities.

  • Objectives This study aims to overview traditional Chinese medicine (TCM) treatment guidelines for severe acute respiratory syndrome (SARS) and Middle East respiratory syndrome (MERS) in order to facilitate the use of Korean medicine (KM) treatment in pandemic diseases. Methods We compared the characteristics between SARS and MERS, and overviewed the Traditional Chinese Medicine treatment guidelines for SARS and MERS. We assessed the efficacy of simultaneous administration of herbal medicine and Western medicine on SARS by studying Cochranes 2012's Systematic Review Studies. Results and Conclusions Based on wen bing (warm diseases) of KM as well as TCM, KM treatment can be an alternative for pandemic diseases such as SARS and MERS considering the Korean's characteristic environment.

Inhibitory Effect of Respective Herbs in Cheonggugamrosu on Oral Malodor Using Malodor Modeling of the Salivary Sediment System (타액침전물모델을 이용한 청구감로수 구성약물의 구취억제작용)

  • Objective: This study assessed five respective herbs in Cheonggugamrosu (CGGRS) for ability to inhibit malodor in vitro. Methods: We used malodor modeling of the salivary sediment system. Incubation mixtures were prepared with sediment at 16.7% (v/v), amino acids (cysteine and tryptophan) at 6mM and either herb extract at 1% (w/v) or water as control. Mixtures were incubated at $37^{\circ}C$ and odor was monitored organoleptically and for volatile sulfur compounds with the Halimeter. Indole/skatole was determined by Kovac's colorimetric method. Results: Two of the herbs, Caryophilli Flos and Glycyrrhizae Radix, had inhibitory effect on malodor generation. Both of them significantly reduced VSC, organoleptic odor and indole/skatole formation (p<0.05). Conclusions: We found Caryophilli Flos and Glycyrrhizae Radix played a main role in CGGRS. Both herbs have potential as effective anti-malodor agents and this suggests they are worthy of further exploration.

Review of Clinical Cancer Research Methodology of Botanical Agents (한약 및 천연물의 항암 임상연구 방법론에 대한 고찰)

  • Objectives : The cancer incidence and cancer burden is increasing. In addition, the use of botanical agents in cancer care is increasing. This article aims to review a research strategy for botanical agents. Methods : The clinical studies of anticancer botanical agents and the papers about clinical research methodology of botanical agents were reviewed. Results : In phase I study, safety confirmation, optimal dose determination and drug interaction study are important. Most botanical agents have low toxicity and some have non-monotone dose response. Therefore, dose-response curve must be evaluated separately from the dose-toxicity curve to determine optimal dose. Although anticancer botanical agents can't shrink tumor size rapidly, they do extend survival. So, in phase II study, response should be evaluated by the survival. Conclusions : Clinical research of botanical agents in cancer is different from traditional methods and strategies. Considering the characteristics of botanical agents and experimental mechanism is necessary in conducting botanical based clinical trials.

  • This study has been carried out to investigate the cause symptom and treatment of wind-coughing ( 風嗽 ) by referring to 38 literatures. The results were obtained as follows; 1. The factors causing wind-coughing ( 風嗽 ) is wind-air. The 1st factors are mistaking of transport lung-air ( 肺氣不宣 ) and cleaning lung (肺失淸肅). 2. The symptom of wind-coughing ( 風嗽 ) is as follows. Bi Saek Sung Jung ( 鼻塞聲重 ), Gu Gun Hu Yang ( 口乾喉痒 ), A Mi Kung I Hae ( 語未竟而咳 ), Maek Bu ( 脈浮 ), Jeang Han Jang Yul ( 憎寒壯熱 ), Ja Han OU Pung ( 自汗惡風 ), Bun Jo ( 煩躁 ), Bi Ryu Chung Chae ( 鼻流淸?), Ya Jung 11 Jyung ( 夜重日輕 ). 3. The treatment-method of wind-coughing ( 風嗽 ) is as follow. Bal San Bul Ga Ha ( 發散 不可下 ), So Pung San Han ( 疏風散寒 ), Chung Yul ( 淸熱 ), Sun Pae Ji Hae ( 宣肺止咳 ). 4. The treatment-herbs of wind-coughing ( 風嗽 ) is as follow. Kwan Dong Hwa San ( 款冬花散 ), Gun Bi Cho San ( 金沸草散 ), Sam Yo Tang ( 三拗湯 ), Ji Hae San ( 止嗽散 ), Hang So San ( 杏蘇散), Sang Kuk Yeam ( 桑菊飮).
