Rotation Control of Shoulder Joint During Shoulder Internal Rotation: A Comparative Study of Individuals With and Without Restricted Range of Motion

  • Background: Limitations of shoulder range of motion (ROM), particularly shoulder internal rotation (SIR), are commonly associated with musculoskeletal disorders in both the general population and athletes. The limitation can result in connective tissue lesions such as superior labrum tears and symptoms such as rotator cuff tears and shoulder impingement syndrome. Maintaining the center of rotation of the glenohumeral joint during SIR can be challenging due to the compensatory scapulothoracic movement and anterior displacement of the humeral head. Therefore, observing the path of the instantaneous center of rotation (PICR) using the olecranon as a marker during SIR may provide valuable insights into understanding the dynamics of the shoulder joint. Objects: The aim of the study was to compare the displacement of the olecranon to measure the rotation control of the humeral head during SIR in individuals with and without restricted SIR ROM. Methods: Twenty-four participants with and without restricted SIR ROM participated in this study. The displacement of olecranon was measured during the shoulder internal rotation control test (SIRCT) using a Kinovea (ver. 0.8.15, Kinovea), the 2-dimensional marker tracking analysis system. An independent t-test was used to compare the horizontal and vertical displacement of the olecranon marker between individuals with and without restricted SIR ROM. The statistical significance was set at p < 0.05. Results: Vertical displacement of the olecranon was significantly greater in the restricted SIR group than in the control group (p < 0.05). However, no significant difference was observed in the horizontal displacement of the olecranon (p > 0.05). Conclusion: The findings of this study indicated that individuals with restricted SIR ROM had significantly greater vertical displacement of the olecranon. The results suggest that the limitation of SIR ROM may lead to difficulty in rotation control of the humeral head.

Effects of elevation on shoulder joint motion: comparison of dynamic and static conditions

  • Background: Although visual examination and palpation are used to assess shoulder motion in clinical practice, there is no consensus on shoulder motion under dynamic and static conditions. This study aimed to compare shoulder joint motion under dynamic and static conditions. Methods: The dominant arm of 14 healthy adult males was investigated. Electromagnetic sensors attached to the scapular, thorax, and humerus were used to measure three-dimensional shoulder joint motion under dynamic and static elevation conditions and compare scapular upward rotation and glenohumeral joint elevation in different elevation planes and angles. Results: At 120° of elevation in the scapular and coronal planes, the scapular upward rotation angle was higher in the static condition and the glenohumeral joint elevation angle was higher in the dynamic condition (P<0.05). In scapular plane and coronal plane elevation 90°-120°, the angular change in scapular upward rotation was higher in the static condition and the angular change in scapulohumeral joint elevation was higher in the dynamic condition (P<0.05). No differences were found in shoulder joint motion in the sagittal plane elevation between the dynamic and static conditions. No interaction effects were found between elevation condition and elevation angle in all elevation planes. Conclusions: Differences in shoulder joint motion should be noted when assessing shoulder joint motion in different dynamic and static conditions.

  • Background: The objective of this study was to compare dysmenorrhea with pelvic alignment, hip joint range of motion about 20's female university students. Sixty two female students participated in this study. To investigate the measure of dysmenorrhea, we used modified menstrual distress questionnaire (MMDQ) and numerric rating scale (NRS). The pelvic alignment was assessed by using the palpation meter. The active range of motion (ROM) were assessed by measuring the flexion, extension, abduction, adduction, internal rotation and external rotation. Each measurement was assessed by goniometer. The data were analyzed by calculating independent T-test. Consequently, As the increase of dysmenorrhea, we observed significantly the increase of pelvic anterior tilt in right and left pelvic (p<.05). Otherwise, as the increase of dysmenorrhea, range of motion of hip joint was significantly limited in right and left external rotation (p<.05). Thus, this study provides young female with valuable information about dysmenorrhea.

  • To establish of standard technique of nano-length measurement in 2D plane, new AFM system has been designed. In this system, measurement uncertainty is dominantly affected by the Abbe error of XY scanning stage. No linear stage is perfectly straight; in other words, every scanning stage is subject to tilting, pitch and yaw motion. In this paper, an AFM system with minimum offset of XY sensing is designed. And XY scanning stage is designed to minimize rotation angle because Abbe errors occur through the multiply of offset and rotation angle. To minimize the rotation angle optimal design has performed by maximizing the stiffness ratio of motion direction to the parasitic motion direction of each stage. This paper describes the design scheme of full AFM system, especially about XY stage. Full range of fabricated XY scanner is $100um\times{100um}$. And tilting, pitch and yaw motion are measured by autocollimator to evaluate the performance of XY stage. Using this AFM system, 3um pitch specimen was measured. As a result, the uncertainty of total system has been evaluated.

스트레칭 굴곡 신전이 두경부에 미치는 영향 (Effects of Flexion-Extension of Stretching on Craniocervical)

  • Purpose : To identify the effects of flexion-extension of stretching on the functional improvement of patients with neck myofascial pain syndrome. Methods : the present research investigated 30 patients with neck myofascial syndrome, dividing them into a group doing flexion-extension of stretching. This study examined degree of recovery from neck pain by comparing their neck myofascial pain syndrome before and after the treatment, and compared to find difference in the degree of recovery from myofascial pain syndrome. Results : The results are as follows. For the flexion of stretching, 1. For the visual analogue scale (VAS) decreased significantly for six weeks treatment, 2. For the flexion decreased significantly for six weeks treatment. and the range of motion of cervical vertebrae increased significantly(p>.05). 3. For Stretching, range of motion left rotation indicated significant difference after pre test and after two week but no significant difference after six week. 4. For the left rotation decreased significantly for six weeks treatment. and the range of motion of cervical vertebrae increased significantly(p>.05). 5. For the right rotation decreased significantly for six weeks treatment. and the range of motion of cervical vertebrae increased significantly(p>.05). Conclusion : This study suggest that flexion-extension of stretching have an effect on the functional improvement of patients with neck myofascial pain syndrome.

Thoracic Hyperkyphosis affects Scapular Orientation and Trunk Motion During Unconstrained Arm Elevation

  • Background: Shoulder function is achieved by the coordinated movements of the scapula, humerus, and thoracic spine, and shoulder disorders can be associated with altered scapular kinematics. The trunk plays an important role as the kinematic chain during arm elevation. Objects: The purpose of this study was to determine the effects of thoracic hyperkyphosis on scapular orientation and trunk motion. Methods: Thirty-one subjects (15 in the ideal thorax group and 16 in the thoracic hyperkyphosis group) performed right-arm abduction and adduction movements in an unconstrained plane. The scapular orientation and trunk motion were recorded using a motion analysis system. Results: Those subjects with thoracic hyperkyphosis displayed greater scapular posterior tilting at a $120^{\circ}$ shoulder elevation, greater scapular internal rotation throughout the arm raising phase, and greater trunk axial rotation at the upper ranges of the shoulder elevation, compared to those subjects with an ideal thorax (p<.05). Conclusion: Thoracic hyperkyphosis can cause scapular instability, greater trunk rotation and greater scapular posterior tilting, and may contribute to preventing the achievement of a full range of humeral abductions in an unconstrained plane.

유방암 절제술 환자의 관절가동술과 전기자극을 융합한 중재가 어깨 가쪽돌림과 통증에 미치는 영향 (The Effect of Joint Mobilization with Electrotherapy interventions on External Rotation and Pain in Mastectomy Patients)

  • 본 연구는 관절가동술 후 두 가지 유형의 전기치료 방법을 각각 적용하였을 때 유방암 환자의 어깨 가쪽돌림 가동범위와 통증에 미치는 효과를 알아보기 위해 실시하였다. 유방절제술을 시행한 유방암 환자 30명을 STMG (관절가동술+비침습적 무통증 신호 전기치료) 15명과 TENMG (관절가동술+경피신경전기자극) 15명으로 나누었다. 중재는 4주간 주 3회, 1회당 1시간씩 실시하였다. 측정은 어깨 가쪽 돌림 관절가동범위(Range of motion, ROM), 통증(Visual analog scale)을 측정하였다. 중재 전·후 두 군 모두 어깨 가쪽돌림 관절가동범위, 통증에서 유의한 차이가 있었다(p<.05). STMG는 어깨 가쪽 돌림 관절가동범위를 제외하고 TENMG보다 통증 감소에서 더욱 효과적이었다(p<.05). 유방암 절제술 환자의 통증을 경감시키기 위해서는 STMG이 TENMG보다 더욱 효과가 있다는 것을 확인할 수 있었다. 다양한 차이를 확인하기 위해 대조군을 더하고, 더 많은 대상자가 필요할 것으로 사료된다.

노벨 스트레칭과 관절가동술이 어깨 후방 뻣뻣함으로 인한 어깨관절 안쪽돌림 결핍 환자의 관절가동범위와 봉우리-위팔뼈 거리에 미치는 영향 (The Effect of Novel Stretching on the Range of Motion and Acromio-Humeral Distance in Patients with Glenohumeral Internal Rotation Deficits with Posterior Shoulder Tightness)

  • Background: The cross-body and the sleeper stretches have been used to improve posterior shoulder tightness (PST). However, such stretching techniques may not always help achieve scapular stability and may even aggravate patient's symptoms. Therefore, a new stretching technique (passive glenohumeral internal rotation with bridging) was developed as a more effective method that may allow for greater scapular stability without aggravating the symptoms. Thus, this study aimed to examine and compare this novel stretching technique to determine its effect on the range of motion (ROM) and acromio-humeral distance (AHD) in patients with glenohumeral internal rotation deficits (GIRD) with posterior shoulder tightness. Methods: A total of 30 symptomatic patients with asymmetrical GIRD were randomly assigned to two groups: the novel stretching group (n=15) and the joint mobilization group (n=15). The intervention was conducted twice a week for a total of four weeks. The ROM of shoulder internal rotation was measured by a goniometer and the AHD was evaluated by an X-ray before and after the intervention. Results: Both the treatments improved ROM and AHD in patients with GIRD (p<.05). However, the improvements in internal rotation ROM and AHD in the novel stretching group were significantly greater than that of the subjects in the mobilization group. There was a significant difference between the two groups (p<.05). Conclusion: These results show that both novel stretching and joint mobilization improved ROM and AHD in patients with GIRD with PST. However, novel stretching was more effective than joint mobilization.

The Reliability and Validity of the Digital Goniometer and Smart Phone to Determine Trunk Active Range of Motion in Stroke Patients

  • Background: Trunk movements are an important factor in activities of daily living; however, these movements can be impaired by stroke. It is difficult to quantify and measure the active range of motion (AROM) of the trunk in patients with stroke. Objects: To determine the reliability and validity of measurements using a digital goniometer (DG) and smart phone (SP) applications for trunk rotation and lateral flexion in stroke patients. Methods: This is an observational study, in which twenty participants were clinically diagnosed with stroke. Trunk rotation and lateral flexion AROM were assessed using the DG and SP applications (Compass and Clinometer). Intrarater reliability was determined using intraclass correlation coefficients (ICCs) with 95% confidence intervals. Pearson correlation coefficient was used to determine the validity of the DG and SP in AROM measurement. The level of agreement between the two instruments was shown by Bland-Altman plot and 95% limit of agreement (LoA) was calculated. Results: The intrarater reliability (rotation with DG: 0.96-0.98, SP: 0.98; lateral flexion with DG: 0.97-0.98, SP: 0.96) was excellent. A strong and significant correlation was found between DG and SP (rotation hemiplegic side: r = 0.95; non-hemiplegic side: r = 0.90; lateral flexion hemiplegic side: r = 0.88; non-hemiplegic side: r = 0.78). The level of agreement between the two instruments was rotation (hemiplegic side: 23.02° [LoA 17.41°, -5.61°]; non-hemiplegic side: 31.68° [LoA 23.87°, -7.81°]) and lateral flexion (hemiplegic side: 20.94° [LoA 17.48°, -3.46°]; non-hemiplegic side: 27.12° [LoA 18.44°, -8.68°]). Conclusion: Both DG and SP applications can be used as reliable methods for measuring trunk rotation and lateral flexion in patients with stroke. Although, considering the level of clinical agreement, DG and SP could not be used interchangeably for measurements.

A Theoretical Study on Free Gyroscopic Compass

  • The authors aim to establish the theory necessary for developing the free gyroscopic compass and focus on mainly two points. One is to suggest north-finding principle by the angular velocity of the earth's rotation, and the other is to suggest orthogonal coordinate transformations of the motion rate of the spin axis, which transforms the components of motion rate in the free gyro frame into those in the platform frame and that this transformed rate is, in turn, transformed into the NED(north-east-down) navigation frame. Subsequently, ship's heading is obtained by using the fore-aft and athwartship components of the motion rate of the spin axis in the NED frame. In addition it was found how to solve the transformation matrix necessary for transforming each frame.
