The purpose of this study was to compare the root canal shaping ability of four nickel-titanium rotary instruments. Forty eight simulated curved root canals were instrumented in plastic with the Crown-down technique using the $ProTaper^{TM}$ , the ProFile, the $GT^{TM}$, and the Quantec. Canals were instrumented until apical canal were up to size 30 by one operator. Each instrument was used only once and instrumentation time was measured.(omitted)
The purpose of this study was to compare the shaping time of two shaping methods and the leakage of three different obturation techniques. Ninty three canaled human molar teeth were used, which were randomly divided into two groups of forty teeth each and ten control teeth. After working length determination, the one group was prepared crown-down technique using rotary root canal instruments of GT rotary files .12/20, .10/20, .08/20 and .06/20 taper(Maillefer Instrument SA. Switzerland). The other group was instrumented with Gates Glidden burs(#1, #2, and #3) to coronal preparation and GT rotary files .08/20 and .06/30 taper to apical preparation. Shaping time was measured. After root canals were instrumented, they were divided to three subgroups and obturated as follows : Subgroup 1, obturated with single cone method Subgroup 2, obturated with lateral condensation : Subgroup 3, obturated with continuous wave technique. Three subgroups were obturated using non-standardized gutta-percha cone(Diadent, Korea, .06 or .08 taper) and AH-26(Dentsply DeTrey, Germany) as a root canal cement. Ten unobturated teeth served as positive and negative controls. After immersion in 2% methylene blue solution for 1 month, the teeth were washed during 24h. The teeth were demineralized in 10% nitric acid and dehydrated by immersion in 80, 90 and 100% ethyl alcohol. The teeth were finally cleared and stored in 100% methylsalicylate, and apical dye penetration was evaluated under stereomicroscope(Leica M420, LC, U.S.A)at $\times$8.75 magnification. Liner measurement of dye penetration was assessed with the use of digitalized image analysing system (analySIS, GmbH, Germany) The data were analysed statistically using independent T-test and Two-way ANOVA and Tukey test. The result were as follows 1. In canal prepared with GT$^{TM}$ rotary file, shaphing time taked more than the group of using Gates Glidden drill to coronal preparation without statistical significance (p>0.05) 2. The group of single cone obturation using canal preparation of GT$^{TM}$ rotary files showed significantly more apical leakage than those of lateral condensation and continuous wave technique regardless of shaping method (p<0.05). 3 The group of single cone obturation using canal preparation of GT$^{TM}$ rotary files and Gates Glidden drill showed significantly more apical leakage than those of continuous wave technique regardless of shaping method (p<0.05). 4. Regardless of shaping method, The group of continuous wave obturation showed less apical leakage than those of lateral condensation without statistical significance (p>0.05). 5. The group of single cone obturation using canal preparation of GT$^{TM}$ rotary files and Gates Glidden drill showed more apical leakage than the group of lateral condensation using same shaping method with-out statistical significance (p>0.05).
I. Objectives The purpose of this study was to compare the shaping ability between the single length technique performed with Mtwo instruments (VDW, Munich, Germany) and the crown-down technique using K3 (SybronEndo, West Collins, CA, USA) and RaCe (FKG, La Chaux-de-Fonds, Switzerland) instruments. II. Materials & Methods Forty five curved canals in resin blocks were equally divided in to three groups. Group 1 (Mtwo) was instrumented used the full length of canal according to the manufacturer's instructions. The simulated canals was prepared to an instrument size of 35, 0.04 taper canal terminus. In group 2 (Race) and group 3 (K3) was instrumented in a crown-down manner and prepared to an instrument size of 30, 0.06 taper canal terminus. Pre- and post-instrumentation images were scanned and assessment of canal shape was completed with a computer image analysis program. Material removal was measured at 7 measuring points, beginning 1mm from the end point of preparation. Differenced of centering ratio were statistically analyzed using One-way ANOVA followed by Duncan's test. II. Results & Conclusion There was no significant difference on 1, 2, 3 and 7mm measuring point. At 4 and 5 measuring point, significant difference showed between the Mtow instruments and other two instruments. (p<0.05)
During cleaning and shaping of narrow and curved canals, it is very difficult or nearly impossible to maintain the original canal shape. Procedural accidents such as, ledge, zipping, perforation, and instrument breakage are frequently occurred and even may lead to failure of endodontic therapy. To prevent these kinds of accidents, various instrumentation techniques and materials have been introduced. Recently some nickel titanium (NiTi) files are introduced and it is reported that These NiTi files created rounder preparations with less transportation than conventional instruments in curved canals. This study compared the change of the canal curvature and procedural accidents after instrumentation produced by stainless steel K-flexo file, and NiTi rotary files (Profile 29 and Quantec 2000). Thirty narrow and curved canals (25-45 degree) of extracted human molars were randomly divided into three groups. In group 1, canals were instrumented using a step-back and watch-winding/pull motion with K-flexo files. In group 2, canals were prepared with Profile 29. Group 3, canals were prepared with Quantec 2000 files. Before and after preparation of canals, periapical radiographs were taken and scanned. The change of canal curvature were measured using Photoshop 4.0 program and the incidence of procedural accidents were also evaluated. The results were as follows: 1. All group showed some loss of canal curvature after instrumentation. 2. Average loss of canal curvature was $6.70{\pm}5.31$ degree for group 1, $3.80{\pm}2.57$ degree for group 2, and $5.40{\pm}4.83$ degree for group 3. All group There was significant change in curvature between before and after instrumentation (p<0.05). But there was no statistical difference amoung 3 groups. 3. In group I, there were no procedural accidents, such as ledging, perforation, or instrument fracture. In group 2, two cases of ledge and one case of instrument fracture were produced Goup 3, each one case of ledge, perforation and instrument fracture were occurred. Whthin the limits of above results, It seems that NiTi rotary instrumentation is not All Mighty and if we use uncarefully, it is more dangerous to produce some procedural accidents than conventional hand files. But more studies should be taken to evaluate the exact effects of NiTi rotary instrumentations.

Objectives: This study aimed to evaluate and compare the efficacy of the S1 reciprocating system and the D-Race retreatment rotary system for filling material removal and the apical extrusion of debris. Materials and Methods: Sixty-four freshly extracted maxillary canines were shaped with size 10 and size 15 K-files, instrumented using ProTaper Gold under irrigation with 2.5% sodium hypochlorite (NaOCl), obturated according to the principle of thermo-mechanical condensation with gutta-percha and zinc oxide eugenol sealer, and allowed to set for 3 weeks at 37℃. Subsequently, the teeth were divided into a control group (n = 4), the D-Race rotary instrument group (n = 30), and the S1 reciprocating instrument group (n = 30). After classical retreatment, the canals were subjected to a complementary approach with the XP-Endo Shaper. Desocclusol was used as a solvent, and irrigation with 2.5% NaOCl was performed. Each group was divided into subgroups according to the timing of radiographic readings. The images were imported into a software program to measure the remaining filling material, the apical extrusion, and the root canal space. The data were statistically analyzed using the Z-test and JASP graphics software. Results: No significant differences were found between the D-Race and S1 groups for primary retreatment; however, using a complementary cleaning method increased the removal of remnant filling (p < 0.05). Conclusions: Classical removal of canal filling material may not be sufficient for root canal disinfection, although a complementary finishing approach improved the results. Nevertheless, all systems left some debris and caused apical extrusion.
During preparation of narrow curved canals, procedural accidents such as, ledge, zipping, and transportation are frequently encountered and may lead to failure of endodontic therapy. To reduce these procedural errors and efficiently manage curved canals, various modifications in instrumentation technique and the design and flexibility of instruments have been advocated. This study compared the maintenance of the original canal curvature, cross sectional canal shape, and preparation time during instrumentation with stainless steel hand (K-Flexo) file, and nickel-titanium rotary files (Profile and Lightspeed). Thirty resin blocks with simulated curved canals of 20~25 degrees were used and divided into three groups of 10 each. In group 1, canals were instrumented using a quarter turn/pull technique with K-Flexofiles. Group 2 canals were prepared with rotary NiTi Profiles. Group 3 was prepared with rotary NiTi Lightspeed instrument. Before and after instrumentation, all canals were scanned using stereo microcope, FlexCam camera, and Photoshop 3.0 computer program. The results were as follows : 1. All groups showed some loss of canal curvature after instrumentation. Average loss of canal curvature was 8.6 degrees for K-Flexofile, 7.7 degrees for Profile, and 5.8 degrees for Lightspeed. Lightspeed exhibited significantly less curvature loss than K-Flexofile (p<0.05). 2. At the apical 1-mm level, Profile produced significantly rounder canals than Lightspeed (p<0.05). At the 3-mm level, Profile and Lightspeed exhibited significantly rounder canals than K-Flexofile (p<0.05). 3. Preparation with Lightspeed was significantly faster than Profile and K-Flexofile, and Profile was faster than K-Flexofile (p<0.05). 4. There was no significant difference in incidence of zipping between the hand K-Flexofile and rotary NiTi (Profile and Lightspeed) instruments. Most of apical canals were slightly widened near the apical foramen. As a results of this study, rotary NiTi instruments are superior to the K-Flexofile in regard to the maintenance of original canal curvature, cross-sectional shape and preparation time. But more investigations and studies should be needed to evaluate the ideal canal instrumentation.

The aim of this study was to compare the effects of three Ni-Ti instruments on leaning ability by evaluating the volumetric and morphological changes in the apical 6mm of the root canals before and after preparation, using three-dimensionally reconstructed root canals of extracted human teeth. Forty-five teeth were used in this study. They were opened the chambers and removed the all pulp remnants ultrasonically. Subsequently, the canal wall was coated with silver paste and prepared using ProTaper, ProFile and GT rotary files according to the manufacturers instructions. Before and after root canal preparation, all the specimens were scanned with micro computed tomography and examined the differences in dentine volume removed, canal straightening, the proportion of the unchanged area and canal transportation. Quantitative analysis revealed that instrumentation increased in canal volume ranging between 0.081 and $1.866{\;}\textrm{mm}^3$. On average, the large apical preparation produced by ProTaper demonstrated smaller proportions of unchanged surface areas compared to the two other instruments in small canals. But in large canals like maxillary central incisor, the preparation of ProTaper instruments was not enough. ProTaper instrument was tended to increase more in canal volume as compared with the other two instruments but unchanged area was no significant difference. These results showed that three instruments had similar preparation ability and micro computed tomography in combination with the coated wall of root canal using silver paste is a nondestructive and valuable tool to study root canal geometry and changes after preparations in detail.
The purpose of this study was to compare the centering ratio and reduction of canal curvature according to the preparation sizes of #30, #40 and #50 using three rotary NiTi instruments which have different shaft tapers. Seventy-two simulated root canals in clear resin blocks (Endo Training Bloc; Dentsply Maillefer, Ballaigues, Switzerland) were divided as following 3 groups according to the file system; the 24 canal blocks prepared with each of ProTaper Universal system (Group P), LightSpeed eXtra system (Group L), and K3 (Group K). The pre- and post-instrumented root canals were scanned and superimposed to evaluate and calculate the centering ratio and reduction of canal curvature. Mean scores of each group were statistically analyzed using one-way ANOV A and Duncan's multiple range test for post-hoc comparison. The results were as followings: 1. Group L showed better centering ratio, followed by K and P. And all experimental groups generally showed increasing tendency of centering ratio as the apical size was increasing from #30 to #50, except at 1 mm level of group P where showed reducing tendency of centering ratio. The smaller the ratio, the better the instrument remained centered in the canal. 2. Group P showed more decrease of canal curvature at all apical shaping size (p < 0.05). Under the conditions of this study, the shaft design could affect the quality of canal shaping and the smooth taperless flexible (LightSpeed) shaft design was capable of preparing canals with good morphological characteristics in curved canals.
근관형성을 하는 과정에서 기구가 파절될 가능성은 언제나 존재한다. File을 비롯하여 lentulo spiral, Gates-Glidden (GG) bur 또는 여타의 이물질도 근관 내에 남겨질 수 있다. 근래에는 수동 file에 비해 많은 장점을 가지고 있는 회전식 nickel-titanium (NiTi) file의 사용이 보편화 되고 있으나 기구의 높은 파절 경향이 문제가 되고 있다. NiTi file의 파절은 매우 빠른 시간내에 일어나고 예측이 불가능하며 임상가에게 정신적 stress를 가져다. 주기 때문에 임상가로 하여금 niTi file의 사용을 주저하게 만들기도 한다. 근관 내 기구가 파절되면 제거하는 것이 가장 최선의 해결 방법이다. 초음파 기기와 미세 현미경의 사용으로 파절된 기구의 제거율이 높아지고 있지만 제거하는 것이 쉬운 일만은 아니며 항상 제거가 가능한 것도 아니다. 그러므로 예방이 최선의 방법이라 할 수 있다. 본 증례에서는 근관 내 file이 파절된 경우에 있어 여러 가지 대처 방안에 대해 보고하고자 한다.
근관형성을 위해 표준화된 근관형성기구에 비해 taper가 큰 기구들이 사용되고 있으며 이를 이용하여 형성된 근관은 taper가 큰 근관의 모양을 갖는다. 근관충전방법에 있어 근관의 크기에 적합한 1차적 gutta-percha cone을 선택할 필요가 있다. 본 연구에서는 .04 및 .06 taper의 전동화일로 형성된 근관에 열연화충전법을 위해 가장 적합한 1차 cone을 선택하기 위한 지침 마련의 목적으로, 근단공의 크기와 근관의 taper에 따른 gutta-percha cone의 근단부 근관 내 적 합도를 평가하였다. ProFile$^{(R)}$ .04 taper와 .06 taper를 이용하여 Crown down 방법으로 60개의 모형근관을 형성하였다. 표준화 gutta-percha cone, Dia-Pro ISO-.04$^{TM}$ 및 .06 gutta-percha cone, MF, F, FM 및 M 크기의 비표준화 gutta-percha cone의 근관 내 적합도는 치근단 5mm의 근관면적에 대한 gutta-percha cone의 점유 면적비(%)로 하였다. .04 taper, 25번 크기의 근관에서는 F, MF 크기의 비표준화 cone이 표준화 cone과 Dia-Pro 180-.04$^{TM}$ 보다 우수한 근관적합도를 나타내었고(p<0.05), 30번 크기의 근관에서는 F, Dia-Pro ISO-.04$^{TM}$, FM 크기의 gutta-percha cone 모두 표준화 cone보다 우수한 근관적합도를 나타내었으나(p<0.05), 35번 크기의 근관에서는 모든 gutta-percha cone 사이에 유의한 차이를 나타내지 않았다(p>0.05) .06 taper, 25번 크기의 근관에서는 사용된 비표준화 cone 모두가 표준화 cone, Dia-Pro ISO-.06$^{TM}$ 보다 나은 근관적합도를 나타내었고(P<0.05), 30번 크기의 근관에서는 표준화 cone을 제외한 나머지 gutta-percha cone에서 유의한 차이를 발견할 수 없었다. 35번 크기의 근관에는 M 크기의 비표준화 cone이 가장 우수한 근관적합도를 보이고 있는 것으로 나타났으며, FM과 Dia-Pro ISO-.06$^{TM}$ 사이에서는 유의한 차이가 나타나지 않았다(p>0.05).
