전동식 니켈 티타늄 파일의 표면 결함 및 단면 형태가 반복응력 하에서 피로 파절에 미치는 영향 (EFFECT OF SURFACE DEFECTS AND CROSS-SECTIONAL CONFIGURATION ON THE FATIGUE FRACTURE OF NITI ROTARY FILES UNDER CYCLIC LOADING)

  • The purpose of this in vitro study was to evaluate the effect of surface defects and cross-sectional configuration of NiTi rotary files on the fatigue life under cyclic loading. Three NiTi rotary files ($K3^{TM},{\;}ProFile^{\circledR},{\;}and{\;}HERO{\;}642^{\circledR}$) with #30/.04 taper were evaluated. Each rotary file was divided into 2 subgroups : control (no surface defects) and experimental group (artificial surface defects), A total of six groups of each 10 were tested. The NiTi rotary files were rotated at 300rpm using the apparatus which simulated curved canal (40 degree of curvature) until they fracture. The number of cycles to fracture was calculated and the fractured surfaces were observed with a scanning electron microscope. The data were analyzed statistically. The results showed that experimental groups with surface defects had lower number of cycles to fracture than control group but there was only a statistical significance between control and experimental group in the $K3^{TM}$ (p<0.05), There was no strong correlation between the cross-sectional configuration area and fracture resistance under experimental conditions. Several of fractured files demonstrated characteristic patterns of brittle fracture consistent with the propagation of pre-existing cracks. This data indicate that surface defects of NiTi rotary files may significantly decrease fatigue life and it may be one possible factor for early fracture of NiTi rotary files in clinical practice.

수종의 엔진구동형 Nickel-Titanium file을 이용한 근관형성 방법이 근관만곡도 유지능력에 미치는 영향 (EFFECT OF VARIOUS CANAL PREPARATION TECHNIQUES USING ROTARY NICKEL-TITANIUM FILES ON THE MAINTENANCE OF CANAL CURVATURE)

  • There are increasing usage of Nickel-Titanium rotary files in modern clinical endodontic treatment because it is effective and faster than hand filing due to reduced step. This study was conducted to evaluate the effect of canal preparations using 3 different rotary Nickel-Titanium files that has different cross sectional shape and taper on the maintenance of canal curvature. Simulated resin block were instrumented with Profile(Dentsply, USA), GT rotary files(Dentsply, USA), Hero 642(Micro-Mega France), and Pro-Taper(Dentsply, USA). The image of Pre-instrumentation and Post-instrumentation were acquired using digital camera and overspreaded in the computer. Then the total differences of canal diameter, deviation at the outer portion of curvature, deviation at the inner portion of curvature, movement of center of the canal and the centering ratio at the pre-determined level from the apex were measured. Results were statistically analyzed by means of ANOVA, followed by Scheffe test at a significance level of 0.05. The results were as follows; 1. Deviation at the outer portion of curvature, deviation at the inner portion of curvature were showed largest in Pro-Taper so also did in the total differences of canal diameter(p<0.05). 2. All the groups showed movements of center Profile combined with GT rotary files and Hero 642 has no difference but Pro-Taper showed the most deviation(p<0.05). 3. At the 1, 2, 3mm level from the apex movements of center directed toward the outer portion of curvature, but in 4, 5 mm level directed toward the inner portion of curvature(p<0.05). As a results of this study, it could be concluded that combined use of other Nickel-Titanium rotary files is strongly recommended when use Pro-Taper file because it could be remove too much canal structure and also made more deviation of canal curvature than others.

Cyclic fatigue resistance, torsional resistance, and metallurgical characteristics of M3 Rotary and M3 Pro Gold NiTi files

  • Objectives: To evaluate the mechanical properties and metallurgical characteristics of the M3 Rotary and M3 Pro Gold files (United Dental). Materials and Methods: One hundred and sixty new M3 Rotary and M3 Pro Gold files (sizes 20/0.04 and 25/0.04) were used. Torque and angle of rotation at failure (n = 20) were measured according to ISO 3630-1. Cyclic fatigue resistance was tested by measuring the number of cycles to failure in an artificial stainless steel canal ($60^{\circ}$ angle of curvature and a 5-mm radius). The metallurgical characteristics were investigated by differential scanning calorimetry. Data were analyzed using analysis of variance and the Student-Newman-Keuls test. Results: Comparing the same size of the 2 different instruments, cyclic fatigue resistance was significantly higher in the M3 Pro Gold files than in the M3 Rotary files (p < 0.001). No significant difference was observed between the files in the maximum torque load, while a significantly higher angular rotation to fracture was observed for M3 Pro Gold (p < 0.05). In the DSC analysis, the M3 Pro Gold files showed one prominent peak on the heating curve and 2 prominent peaks on the cooling curve. In contrast, the M3 Rotary files showed 1 small peak on the heating curve and 1 small peak on the cooling curve. Conclusions: The M3 Pro Gold files showed greater flexibility and angular rotation than the M3 Rotary files, without decrement of their torque resistance. The superior flexibility of M3 Pro Gold files can be attributed to their martensite phase.

Shaping ability of Ni-Ti Rotary files in combination with GT Rotary Ni-Ti file

  • The objective of this study was to compare the apical shaping ability of the ProFile .04 taper, Lightspeed and HERO 642 rotary instruments and to assess the combined effect of GT Rotary files to Lightspeed and HERO 642 instruments. Forty-eight resin simulated root canal blocks divided into 4 groups with 12 canals. Forty-eight blocks were prepared by one operator with GT Rotary files using a high torque handpiece at 300 rpm. Thirty-six blocks in the three experimental groups were prepared with ProFile 04 taper file, Lightspeed file, HERO 642 file.(omitted)

  • Endodontic rotary NiTi(Nickel-Titanium) files have several advantages, but they also have some problems. Fracture of instrument is the one of the problems. Cyclic fatigue fracture may not give a previous sign(ex, bending or distortion of the files) before it happen. It is affected by various factors. This study investigated time which takes for cyclic fatigue fracture to happen and fracture patterns with regard to different NiTi files and pecking motion.(omitted)

Cyclic fatigue resistance of M-Pro and RaCe Ni-Ti rotary endodontic instruments in artificial curved canals: a comparative in vitro study

  • Objectives: To compare the flexural cyclic fatigue resistance and the length of the fractured segments (FLs) of recently introduced M-Pro rotary files with that of RaCe rotary files in curved canals and to evaluate the fracture surface by scanning electron microscopy (SEM). Materials and Methods: Thirty-six endodontic files with the same tip size and taper (size 25, 0.06 taper) were used. The samples were classified into 2 groups (n = 18): the M-Pro group (M-Pro IMD) and the RaCe group (FKG). A custom-made simulated canal model was fabricated to evaluate the total number of cycles to failure and the FL. SEM was used to examine the fracture surfaces of the fragmented segments. The data were statistically analyzed and comparisons between the 2 groups for normally distributed numerical variables were carried out using the independent Student's t-test. A p value less than 0.05 was considered to indicate statistical significance. Results: The M-Pro group showed significantly higher resistance to flexural cyclic fatigue than the RaCe group (p < 0.05), but there was no significant difference in the FLs between the 2 groups (p ≥ 0.05). Conclusions: Thermal treatment of nickel-titanium instruments can improve the flexural cyclic fatigue resistance of rotary endodontic files, and the M-Pro rotary system seems to be a promising rotary endodontic file.

GT rotary file을 이용한 근관성형법과 충전방법의 비교 (COMPARISON OF CANAL SHAPING METHODS WITH GTTM ROTARY FILE AND CONDENSATION METHODS)

  • The purpose of this study was to compare the shaping time of two shaping methods and the leakage of three different obturation techniques. Ninty three canaled human molar teeth were used, which were randomly divided into two groups of forty teeth each and ten control teeth. After working length determination, the one group was prepared crown-down technique using rotary root canal instruments of GT rotary files .12/20, .10/20, .08/20 and .06/20 taper(Maillefer Instrument SA. Switzerland). The other group was instrumented with Gates Glidden burs(#1, #2, and #3) to coronal preparation and GT rotary files .08/20 and .06/30 taper to apical preparation. Shaping time was measured. After root canals were instrumented, they were divided to three subgroups and obturated as follows : Subgroup 1, obturated with single cone method Subgroup 2, obturated with lateral condensation : Subgroup 3, obturated with continuous wave technique. Three subgroups were obturated using non-standardized gutta-percha cone(Diadent, Korea, .06 or .08 taper) and AH-26(Dentsply DeTrey, Germany) as a root canal cement. Ten unobturated teeth served as positive and negative controls. After immersion in 2% methylene blue solution for 1 month, the teeth were washed during 24h. The teeth were demineralized in 10% nitric acid and dehydrated by immersion in 80, 90 and 100% ethyl alcohol. The teeth were finally cleared and stored in 100% methylsalicylate, and apical dye penetration was evaluated under stereomicroscope(Leica M420, LC, U.S.A)at $\times$8.75 magnification. Liner measurement of dye penetration was assessed with the use of digitalized image analysing system (analySIS, GmbH, Germany) The data were analysed statistically using independent T-test and Two-way ANOVA and Tukey test. The result were as follows 1. In canal prepared with GT$^{TM}$ rotary file, shaphing time taked more than the group of using Gates Glidden drill to coronal preparation without statistical significance (p>0.05) 2. The group of single cone obturation using canal preparation of GT$^{TM}$ rotary files showed significantly more apical leakage than those of lateral condensation and continuous wave technique regardless of shaping method (p<0.05). 3 The group of single cone obturation using canal preparation of GT$^{TM}$ rotary files and Gates Glidden drill showed significantly more apical leakage than those of continuous wave technique regardless of shaping method (p<0.05). 4. Regardless of shaping method, The group of continuous wave obturation showed less apical leakage than those of lateral condensation without statistical significance (p>0.05). 5. The group of single cone obturation using canal preparation of GT$^{TM}$ rotary files and Gates Glidden drill showed more apical leakage than the group of lateral condensation using same shaping method with-out statistical significance (p>0.05).

레진모형근관에서 엔진구동형 및 수동형 ProTaper 파일로 근관성형 후 근관형태 변화에 대한 비교연구 (A COMPARATIVE STUDY OF THE CANAL CONFIGURATION AFTER SHAPING BY PROTAPER ROTARY AND HAND FILES IN RESIN SIMULATED CANALS)

  • 본 연구는 형태 및 재질이 동일한 엔진구동형 ProTaper파일과 수동형 ProTaper파일을 이용하여 레진모형근관을 성형한 후 근관형태 변화를 비교 분석하고자 시행되었다. 본 연구에서는 레진모형근관으로 총 40개의 J자와 S자의 근관 형태가 재현된 ENDO-TRAINING BLOC을 사용하였다. 근관 성형 기구로는 엔진구동형 Ni-Ti 파일로 $ProTaper^{TM}$, 수동형 Ni-Ti 파일로 $ProTaper^{(R)}$ For Hand Use를 사용하였다. 사용된 레진모형근관과 파일의 종류에 따라 10개씩 4개의 그룹으로 나누어 근관성형을 시행하였다. 근관 성형 전$\cdot$후 이미지를 스캐너 (Color scanner, UMAX Technologies, Inc., USA)를 이용하여 얻은 후. Photoshop 7.0 프로그램 (Adobe System Inc., USA)을 이용하여 이미지를 중첩하였다. 이미지 분석 프로그램 (Image-$Pro^{(R)}$ Plus, Media Cybernetic, USA)을 이용하여 치근단 쪽에서부터 0, 1, 2, 3, 4, 5, 6 및 7 mm 수준에서 근관 성형에 따른 근관의 내측 및 외측 폭경의 변화량, 근관 총폭경의 변화량, 근관 중심축의 변위량을 측정하였다. 또한 근관 성형 시간을 기록하였다. 두 기구 사이의 유의성 검정을 위해 독립 표본 t-검정을 시행하여, 근관 성형 시 수동형 ProTaper 파일이 엔진구동형 ProTaper 파일에 비해 근관 중심축의 변위가 유의하게 덜 일어나며 근관의 원래 형태를 더욱 잘 유지할 수 있지만, 근관 성형시간은 길어질 수 있다는 결론을 얻었다.

A survey of experience-based preference of Nickel-Titanium rotary files and incidence of fracture among general dentists

  • Objectives: The purpose was to investigate the preference and usage technique of NiTi rotary instruments and to retrieve data on the frequency of re-use and the estimated incidence of file separation in the clinical practice among general dentists. Materials and Methods: A survey was disseminated via e-mail and on-site to 673 general dentists. The correlation between the operator's experience or preferred technique and frequency of re-use or incidence of file fracture was assessed. Results: A total of 348 dentists (51.7%) responded. The most frequently used NiTi instruments was ProFile (39.8%) followed by ProTaper. The most preferred preparation technique was crown-down (44.6%). 54.3% of the respondents re-used NiTi files more than 10 times. There was a significant correlation between experience with NiTi files and the number of re-uses (p = 0.0025). 54.6% of the respondents estimated experiencing file separation less than 5 times per year. The frequency of separation was significantly correlated with the instrumentation technique (p = 0.0003). Conclusions: A large number of general dentists in Korea prefer to re-use NiTi rotary files. As their experience with NiTi files increased, the number of re-uses increased, while the frequency of breakage decreased. Operators who adopt the hybrid technique showed less tendency of separation even with the increased number of re-use.

근관치료 기구의 기계 형태적 특성과 이에 따른 임상적 영향 고찰 (Mechanical and geometric features of endodontic instruments and its clinical effect)

  • 서론: 이 문헌의 목적은 Nickel-titanium (NiTi) 전동 파일의 기계적 형태적 양상과 이에 의한 임상적 연관성을 고찰하는 것이다. NiTi 전동 파일은 다양한 고유의 형태로 시장에 소개되었고 경쟁 상품에 비해서 근관 성형에 더 나은 능력을 가졌다고 주장하고 있다. 본론: 이 문헌에서는 NiTi 전동 파일의 형태(예. 팁, 테이퍼, helical angle 등)와 파일의 임상 적용 성과 사이의 가능한 상관관계를 다음과 같이 다룬다; - NiTi 전동 파일의 파절 양상 - 비활성 파일팁과 glide path - 파일 경사도와 임상 효과 - 파일 횡단면적과 임상 효과 - 열처리와 표면 특성 - Screw-in 효과와 치근 상아질의 유지 - Screw-in 효과를 줄이기 위한 고안 결론: 이상의 내용을 바탕으로, 임상가는 다양한 NiTi 전동 기구를 사용함에 있어 임상 상황에 적절한 장점을 고려하여 선택하고 사용할 수 있는 도움을 받을 수 있을 것이다.
