Cone-beam computed tomography-based radiographic considerations in impacted lower third molars: Think outside the box

  • Purpose: This study aimed to evaluate the anatomic circle around the impacted lower third molar to show, document, and correlate essential findings that should be included in the routine radiographic assessment protocol as clinically meaningful factors in overall case evaluation and treatment planning. Materials and Methods: Cone-beam computed tomographic images of impacted lower third molars were selected according to specific inclusion criteria. Impacted teeth were classified according to their position before assessment. The adjacent second molars were assessed for distal caries, distal bone loss, and root resorption. The fourth finding was the presence of a retromolar canal distal to the impaction. Communication with the dentist responsible for each case was done to determine whether these findings were detected or undetected by them before communication. Results: Statistically significant correlations were found between impaction position, distal bone loss, and detected distal caries associated with the adjacent second molar. The greatest percentage of undetected findings was found in the evaluation of distal bone status, followed by missed detection of the retromolar canal. Conclusion: The radiographic assessment protocol for impacted third molars should consider a step-by-step evaluation for second molars, and clinicians should be aware of the high prevalence of second molar affection in horizontal and mesioangular impactions. They also should search for the retromolar canal due to its associated clinical considerations.

구강검진에서 임상검사와 비교한 파노라마방사선사진의 유용성 (Efficacy of panoramic radiography as a screening procedure in dental examination compared with clinical evaluation)

  • Purpose : To evaluate the efficacy of panoramic radiography by comparing the results of clinical examination with radiographic findings. Materials and Methods: We studied 190 patients (20 men and 170 women; mean age, 40 years; range, 22 to 68 years) who visited the health promotion center of Korea Medical Science Institute and were examined both clinically and by panoramic radiography. We compared results from both examinations. Treatment options by clinical examination were described as "no treatment indicated", "treatment of dental caries", "removal of calculus", "treatment of periodontal disease", "prothodontic treatment" and "extraction of the third molar". Findings taken from the panoramic radiography were: dental caries, periapical lesion, alveolar bone loss, calculus deposition, retained root, impaction of the third molar, disease of maxillary sinus, bony change of mandibular condyle, etc. Results: The prevalence of panoramic findings were: 37.9% of dental caries, 17.4% of periapical lesions, 44.7% of alveolar bone losses, 62.6% of calculi deposition, 7.9% of retained roots, 26.8% of third molar impactions, 6.3% of diseases of maxillary sinus, 2.1 % of bony changes of mandibular condlye and 35.8% of miscellaneous lesions. Abnormal conditions revealed by panoramic radiography which had not been discovered on clinical examination were: 24.2% of the patients had dental caries, 17.4% had periapical lesions, 7.4% had calculi deposition, 5.3% had retained roots, 15.3% had third molar impactions. The opposite cases were: 5.2% had dental caries, 12.6% had calculi deposition, and 9.5% had third molar impactions. Conclusion: The use of panoramic radiography as a supplement to the clinical examination might be a valuable screening technique.

산 완충용액의 pH 및 유산의 농도가 인공치근우식의 형성에 미치는 영향 (THE INFLUENCE OF pH AND LACTIC ACID CONCENTRATION ON THE FORMATION OF ARTIFICIAL ROOT CARIES IN ACID BUFFER SOLUTION)

  • 치근 우식증은 복합적인 요인에 의해 발생되는 질환으로 고령 인구의 증가로 최근 증가추세에 있으나 아직까지는 병소 깊이, 상아질 탈회의 정도 및 양상, collagen의 파괴 정도 및 수산화인회석 결정 변화에 대한 탈회 완충액의 조직학적 연구는 미비한 실정이다. 본 연구는 우식 형성에 영향을 주는 산 완충용액 내의 pH와 유산의 농도변화에 따른 치근 상아질 우식 병소의 진행에 미치는 변화를 편광현미경을 이용하여 관찰하고 관찰된 우식 병소층의 수산화인회석의 결정 형태 변화를 주사전자현미경으로 관찰하여 탈회 과정을 살펴보고자 세 가지 pH (4.3, 5.0, 5.5)군과 각각의 pH군에 세 가지 유산의 농도 (25mM, 50 mM, 100 mM)를 이용하여 인공치근 우식을 형성한 후 비교 분석하여 다음과 같은 결과를 얻었다. 1. 편광현미경 소견에서 우식 병소의 깊이는 pH보다는 유산의 농도에 의해 더 영향을 많이 받았다. 2. 주사전자현미경 소견에서 유산의 농도가 높아질수록 그리고 pH가 낮아질수록 수산화인회석 결정의 소실이 더 많이 진행되었다. 3. 탈회는 수산화인회석 결정의 변연부가 소실되며 결정 cluster내 결정의 숫자 및 크기가 감소하였고 결정 cluster 사이의 간격이 넓어지는 양상으로 관찰되었다. 4. 표면층에서의 주사전자현미경 관찰 시 유산의 농도가 높아질록 수산화인회석 결정 cluster의 형태는 소실되고 콜라겐 섬유 표면에 수산화인회석 결정의 용해, 재결합된 양상으로 관찰되었다. 5. 탈회 과정에 대한 주사전자현미경 관찰 시 상아질의 탈회는 단순히 탈회만 독립적으로 일어나는 과정이 아닌 탈회와 재광화가 동시에 일어나는 양상으로 관찰되었다. 이상의 결과 산 완충용액 내의 유산의 농도가 높아지고 pH가 낮아질수록 탈회의 속도가 증가하고 탈회의 과정은 수산화인회석 결정 cluster의 표면으로부터 진행되며 시간이 경과함에 따라 수산화인회석 결정의 형태는 원형 또는 타원형에서 불규칙한 형태로 변화되었다.

  • I. Objectives This study was performed to get some information about micromorphology of subsurface lesion of root caries by observing autofluorescence using confocal laser scanning microscope(CLSM) with minimum sample preparation. II. Materials and Methods Half-cut and 1 mm thick sample of human teeth were prepared to produce artificial root carious lesions. Incipient subsurface lesions were produced under optimal pH and saturity. The lesions were observed by polarized microscopy, CLSM, and back-scattered electron microscope(BSE). 치근 우식 수복에 사용되는 심미성 수복물의 변연누출에 관한 연구 (A STUDY ON THE MARGINAL LEAKAGE OF ESTHETIC RESTORATIVE MATERIALS ON ROOT CARIES RESTORATION)

  • The purpose of the study was to evaluate the degree of the marginal leakage of esthetic restorative materials on root caries restoration. 120 cavities were prepared to $90^{\circ}$ butt joint on all margins on the crown and root portion, and divided into 4 groups. The four groups of cavity were filled with Amalgam(Dongmuyung Dental Alloy Co., Ltd, KOREA), Silux$^{(R)}$(3M Co., USA)-Scotch Bond 2$^{(R)}$(3M Co., USA), Silux$^{(R)}$-All Bond$^{(R)}$(BISCO USA), and GC Fuji II$^{(R)}$(G-C Co., JAPAN) respectively. The apical margin of the preparation was finished to leave a flash of restorative material. The coronal margin of the preparation was finished not to leave a flash of restorative material. All specimens were sectioned longitudinally with Isomet Low speed saw(Buether Ltd, USA). The degree of dye penetration was evaluated as the parameter of marginal leakage under the stereoscope. The results were as follows. 1. At the enamel and dentin/cementum margins, the margin were finished to leave a flash of material showed less marginal leakage than that were finished not to leave a flash of material (P<0.001). 2. The enamel margins showed less marginal leakage than the dentin/cementum margins(P<0.001). 3. There was no significant difference in the degree of the marginal leakage between Silux$^{(R)}$-Scotch Bond 2$^{(R)}$ group and Silux$^{(R)}$-All Bond$^{(R)}$ group.

Determining the reliability of diagnosis and treatment using artificial intelligence software with panoramic radiographs

  • Purpose: The objective of this study was to evaluate the accuracy and effectiveness of an artificial intelligence (AI) program in identifying dental conditions using panoramic radiographs(PRs), as well as to assess the appropriateness of its treatment recommendations. Materials and Methods: PRs from 100 patients(representing 4497 teeth) with known clinical examination findings were randomly selected from a university database. Three dentomaxillofacial radiologists and the Diagnocat AI software evaluated these PRs. The evaluations were focused on various dental conditions and treatments, including canal filling, caries, cast post and core, dental calculus, fillings, furcation lesions, implants, lack of interproximal tooth contact, open margins, overhangs, periapical lesions, periodontal bone loss, short fillings, voids in root fillings, overfillings, pontics, root fragments, impacted teeth, artificial crowns, missing teeth, and healthy teeth. Results: The AI demonstrated almost perfect agreement (exceeding 0.81) in most of the assessments when compared to the ground truth. The sensitivity was very high (above 0.8) for the evaluation of healthy teeth, artificial crowns, dental calculus, missing teeth, fillings, lack of interproximal contact, periodontal bone loss, and implants. However, the sensitivity was low for the assessment of caries, periapical lesions, pontic voids in the root canal, and overhangs. Conclusion: Despite the limitations of this study, the synthesized data suggest that AI-based decision support systems can serve as a valuable tool in detecting dental conditions, when used with PR for clinical dental applications.

  • The patient, 19 years old male, visited with complaints of trismus and consults for construction of denture. 4 years ago, he received /sup 60/Coteletherapy of left laryngeal area for treatment of throat tumor. Clinical apperance showed rampant dental caries, multiple root rests, xerostomia, disturbance of mandibular growth and asymmetry of face. Roentgenographic examination disclosed deep cervical caries, destruction of alveolar crest, punched out bone destruction of mandible and maxilla.

한국청년의 구강상태에 대한 방사선학적 연구 (THE STUDY OF ORAL CONDITIONS BY THE FULL MOUTH ROENTGENOGRAMS IN YOUNG ADULTS.)

  • The author examined 515 full mouth roentgenograms stored in the Dept. of Oral Radiology, College of Dentistry, Seoul National University. For evaluating the efficiency of the routine full mouth roentgenogram, each of abnormal conditions such as impacted teeth, missing, caries, crown, filling and apical lesions was observed. The results obtained were as follows; 1. Among 14.420 teeth examined, missing teeth were 174, impacted 16, caries 161 and treated 1,162. 2. The incidence of impacted third molar was close to 18.7 percent and the incidence of missing third molar was about 33.4 percent. 3. Among carious and treated teeth, 178 teeth (1.2%) were needed observation of root apex. And of these teeth, 119 teeth 0.8% were observed with apical lesion. 4. There is a considerable relation between the incidence or size of apical lesion and the accuracy of endodontical treat. 5. Among the teeth with apical lesion, upper and lower first molars were most frequent (about 41%), upper and lower canine were rare (about 1.7%).

근관내 여러 가지 조건에 따른 Root-ZX의 정확도에 관한 연구 (A STUDY ON THE ACCURACY OF THE ROOT-ZX ACCORDING TO THE VARIOUS CONDITIONS OF ROOT CANALS)

  • Currently frequency-dependent type electronic apex locators have been widely used to determine the working length in endodontic treatment. But, accuracy of electronic apex locators is controversial. The purpose of this study was to evaluate the accuracy of Root-ZX(Morita Co., Japan) at different kinds of conditions of root canals compared with the radiographic working length. The 40 extracted human anterior teeth with fully formed apices and without any caries were used. The radiographs were taken for working length with the 0.5mm short of #15 K-file tip just visible at the foramen under the surgical microscope(Carl Zeiss Co. Germany) at 25X. Then the electronic working lengths were determined with Root-ZX at the different kinds of conditions of root canals according to the presence of electrolyte and Crown-down pressureless technique. The results were as follows ; 1. There was no significant statistical difference in working length between radiograph and Root-ZX. 2. There was no significant statistical difference in electronic working length between the canal with electrolyte and without electrolyte. 3. There was no significant statistical difference in electronic working length between the canal without any instrumentation and after Crown-dow pressureless technique. 4. Of the total 40 root canals, 85% in Group I, 92.5% in Group II, 95% in Group III and 95% in Group IV using Root-ZX showed coincidence within 0.5mm accuracy compaing with the radiographic working length. The results showed that the Root-ZX can be use effectively for measuring the working length of root canal after instrumentation with Crown-down pressureless technique regardless of the presence of electrolyte in root canal.

Endocem MTA와 ProRoot MTA를 이용한 부분치수절단술 후 통증 발생에 관한 임상 연구 (Pain Occurrence after Partial Pulpotomy by using Endocem MTA and ProRoot MTA: a Clinical Study)

  • Objectives: This study aimed to compare the postoperative pain and clinical performance after partial pulpotomy by using ProRoot MTA and Endocem MTA. Materials and Methods: Twenty-eight teeth requiring partial pulpotomy due to deep dental caries or traumatic injury were included in this study. After 2mm removal of exposed pulp and bleeding control, the ProRoot MTA or Endocem MTA was randomly adjusted to the exposed site. 1, 2, 4, and 12 weeks after the final restoration, the patients were recalled to check the postoperative pain or another unfavorable signs. Pearson's chi-square test was used for statistical analysis to evaluate any differences among tested materials. Results: 3 of 28 teeth showed postoperative pain and cold positive during follow-up period (10.7%). There were no statistically differences in pain occurrence between two tested materials (P > 0.05). Conclusions: In the limitations of this study, partial pulpotomy by using Endocem MTA showed the advantages of short setting time and lower postoperative pain incidence, allowing one visit treatment.
