Objectives: This study aimed to compare the postoperative pain and clinical performance after applying three different intracanal medicaments and root canal sealers. Materials and Methods: Sixty-five patients requiring root canal treatment due to symptomatic apical periodontitis were included in this study. After a glide path preparation by using PathFile, each canal was shaped with ProTaper Next file system. After the canal cleaning and shaping procedure, the canal was dried and each intracanal medicaments were adjusted (Calcipex II, TRC-paste, Metapaste). At the next visit, the patients were requested to answer the absence of the pain after the procedure. Once the patients showed no symptom, the canal was obturated with each corresponded root canal sealers (AH plus, Radic-sealer, ADseal). The patients were recalled after 1 week, 1, 3, and 6 months to check the postoperative pain or unexpected clinical signs. One-way ANOVA and Duncan's post hoc comparison, and Chi-square test were used for statistical analysis to evaluate any differences among tested materials. Results: The average number of visits for intracanal medication was 2.69, 2.65, and 2.61 for Calcipex II, TRC-paste, and Metapaste. There were no statistically differences in post-obturation pain among three groups obturated with different root canal sealers (P > 0.05). Conclusions: Under the limitations of this study, three tested intracanal medicaments and epoxy resin root canal sealers showed clinically acceptable similar results.
Objectives: The aim of this study was to compare root canal volume change and canal transportation by Vortex Blue (VB; Dentsply Tulsa Dental Specialties), ProTaper Next (PTN; Dentsply Maillefer), and ProTaper Universal (PTU; Dentsply Maillefer) nickel-titanium rotary files in curved root canals. Materials and Methods: Thirty canals with $20^{\circ}-45^{\circ}$ of curvature from extracted human molars were used. Root canal instrumentation was performed with VB, PTN, and PTU files up to #30.06, X3, and F3, respectively. Changes in root canal volume before and after the instrumentation, and the amount and direction of canal transportation at 1, 3, and 5 mm from the root apex were measured by using micro-computed tomography. Data of canal volume change were statistically analyzed using one-way analysis of variance and Tukey test, while data of amount and direction of transportation were analyzed using Kruskal-Wallis and Mann-Whitney U test. Results: There were no significant differences among 3 groups in terms of canal volume change (p > 0.05). For the amount of transportation, PTN showed significantly less transportation than PTU at 3 mm level (p = 0.005). VB files showed no significant difference in canal transportation at all 3 levels with either PTN or PTU files. Also, VB files showed unique inward transportation tendency in the apical area. Conclusions: Other than PTN produced less amount of transportation than PTU at 3 mm level, all 3 file systems showed similar level of canal volume change and transportation, and VB file system could prepare the curved canals without significant shaping errors.
The length of root canal has to be measured to cure root canal. Several apex locators were developed to measure the length of root canal. And they were verified by X-ray or micrometer method. But these methods do not consider the non-linear bends of pulp and have ${\pm}0.5mm$ error which is large to measure the length of root canal. The purpose of this study is the research of new method to measure the length of root canal and verifying the apex locator using Micro-CT. The length of root canal of 6 teeth were measured with the apex locator. When the apex locator reads 0.5, 0.6, 0.7, 0.5, 0.9, 1.0mm, the teeth with the file fixed were photographed. The average lengths of root canal of 6 teeth measured by Micro-CT were 0.488, 0.589, 0.680, 0.775, 0.897, 0.992mm.

Variation in root canal morphology, especially in maxillary first molar presents a constant challenge for a clinician in their detection and management. This case report describes the successful root canal treatment of a three rooted right maxillary first molar presenting with three canals each in the mesiobuccal and distobuccal roots and one canal in the palatal root. The clinical detection of this morphologic aberration was made using a dental operating microscope, and the canal configuration was established after correlating and computing the clinical, radiographic and cone-beam computed tomography (CBCT) scan findings. CBCT images confirmed the configuration of the canals in the mesiobuccal and distobuccal roots to be Al-Qudah and Awawdeh type (3-2) and type (3-2-1), respectively, whereas the palatal root had a Vertucci type I canal pattern. This report reaffirms the importance of careful examination of the floor of the pulp chamber with a dental operating microscope and the use of multiangled preoperative radiographs along with advanced diagnostic aids such as CBCT in identification and successful management of aberrant canal morphologies.
The purpose of this study was to evaluate the antimicrobial effect of root canal cements such as AH-26, Sealapex, Canals and Apatite root sealers(Type I, II, III) and to determine the efficacy of their activities. S.mutans(AHT), S.sanguis(NCTC 9811) and B.gingivalis(381) were streaked on blood agar and the PVC tybes filled with root canal cements were applied on. Then the microorganisms were cultured for 48 hours, anaerobically. B.cereus(KCTC 1012) was streaked on nutrient agar, PVC tubes were applied on and were cultured for 48 hours, aerobically. The inhibition zones of root canal cements were measured with vernier caliper. The data statistically analyzed, and the results were as followed. 1. Apatite root sealers(Type I, II, III) showed no inhibition zones. 2. AH-26, Sealapex and Canals had inhibition zones with varying degrees. The inhibition zone of AH-26 was greatest and followed by Canals and Sealapex(P<0.01). 3. As time goes by after mixing the root canal cements, AH-26, Canals and Sealapex showed significantly reducing inhibition zones(P<0.01). 4. There were the least inhibition zones of all the root canal cements on S.mutans and followed in such order as; B.gingivalis, S.sanguis and B.cereus(P<0.01).
본 연구의 목적은 근관 충전재로서 MTA(Endoseal, Endoseal MTA)의 밀폐 효과와 효율을 평가하는 것이다. 총 106개의 발거된 단근치가 사용되었으며, 충전된 실러에 따라 AH군(AH-26), PR군(ProRoot MTA), ES군(Endoseal), EM군(Endoseal MTA)으로 나누어졌고, 모든 충전시 소요 시간이 측정되었다. 그 군들은 다시 A 하위군, B 하위군으로 나누어 근관벽과의 밀폐성과 상아세관내 실러의 침투도를 각각 평가하였다. 결과는 근관 충전용 MTA는 AH-26에 비해 근관벽과의 밀폐성과 상아세관 침투도는 떨어졌으며, ProRoot MTA와 비교시 근관벽과의 밀폐성은 차이가 없었지만, 상아세관 침투도는 높았다. 작업 시간은 근관 충전용 MTA가 ProRoot MTA와 AH-26에 비해 짧았다. 결론적으로 근관 충전용 MTA는 레진계 실러보다 근관 밀폐 효과는 떨어지지만 MTA가 필요한 근관 충전시 효과적인 대안이 될 수 있다.
Objectives: This study aimed to determine the effects of 1-rooted mandibular second molar (MnSM) teeth on root canal anatomy complexities of the mandibular central incisor (MnCI), mandibular lateral incisor (MnLI), mandibular canine (MnCn), mandibular first premolar (MnFP), mandibular second premolar (MnSP), and mandibular first molar (MnFM) teeth. Materials and Methods: Cone-beam computed tomography images of 600 patients with full lower dentition were examined. Individuals with 1-rooted MnSMs were determined, and the complexity of root canal anatomy of other teeth was compared with individuals without 1-rooted MnSMs (Group-1; subjects with at least one 1-rooted MnSM, Group-2; subjects with more than a single root in both MnSMs). A second canal in MnCIs, MnLIs, MnCns, MnFPs, and MnSPs indicated a complicated root canal. The presence of a third root in MnFMs was recorded as complicated. Results: The prevalence of 1-rooted MnSMs was 12.2%, with the C-shaped root type being the most prevalent (9%). There were fewer complicated root canals in MnCIs (p = 0.02), MnLIs (p < 0.001), and MnFPs (p < 0.001) in Group 1. The other teeth showed no difference between the groups (p > 0.05). According to logistic regression analysis, 1-rooted right MnSMs had a negative effect on having complex canal systems of MnLIs and MnFPs. Left MnSMs were explanatory variables on left MnLIs and both MnFPs. Conclusions: In individuals with single-rooted MnSMs, a less complicated root canal system was observed in all teeth except the MnFMs.
The purpose of this study is to compare the shape of the apical regions of root canals after instrumentation by various enlarging instruments. 120 extracted, single-rooted human teeth were seperated into 4 experimental groups. Each group provided 30 teeth for experimental use. Group 1 root canals instrumented by the hand-operated Reamer. Group 2 root canals instrumented by the hand-operated H-file. Group 3 root canals instrumented by the hand-operated K-file. Group 4 root canals instrumented by the automated Giromatic file. The results were as follows: 1. The degree of the roundness of the apical regions of root canal walls: 1) Hand-operated instruments were superior to the automated Giromatic file. 2) The Reamer was the most effective instrument among hand-operated instruments 3) There was little difference between the H-file and the K-file. 2. The degree of the smoothness of the apical regions of root canal walls: 1) Root canal walls instrumented by the hand-operated Reamer, H-file and, K-file were generally smooth, and there was little difference among them. 2) Root canal walls instrumented by the automated Giromatic file showed many irregular canal walls. 3. The existance of organic debris in the apical regions of root canals: All organic debris is not removed from root canals, and there was little difference between hand-operated instruments and the automated Giromatic instrument in removing organic debris.
Ninety four human mandibular third molars were chosen to study the anatomy of the root canal. The experimental teeth were injected with china ink, decalcified, cleared and used in study, in vitro, to determine the number of root, the number of root canals, canals per root, frequency and location of transverse anastomoses, frequency and location of lateral canals and frequency of the apical deltas. The results were as follows: 1. Most of the teeth showed two canals, but 17.0% of the teeth were found to have one canal, 17.0% of them three canals, 3.2% of them four canals and l.1% of them five canals. 2. In so far as observing one canal per root, 17.0% of the teeth were found to have one canal in single-rooted tooth, 48.9% of them in mesial root and 58.5% of them in distal root. 3. In roots with two or three canals, the separated apical foramen appeared in 55.6% in single-rooted tooth, 64.3% in mesial side and 80.0% in distal side, and the common apical foramen appeared in 44.4% in single-rooted tooth, 35.7% in mesial side and 20.0% in distal side. 4. Of the two root canals in one root, 19.1% of the canals were found to have transverse anastomoses and were usually located in the apical third of the root. 5. 63.8% of 94 teeth were found to have lateral canals, and ramifications were mainly located in the apical third of the root.
This study compared the shaping ability of nickel-titanium rotary files with different rake angle and radial land. The nickel-titanium files used in this study were Profile(Dentsply, Maillefer, Ballaigues, Switzerland), Hero 642(Micromega, Besancon, France) and K3 (SybronEndo, Glendora, Ca, USA) file. Resin blocks substituted for root canals. 36 resin blocks were divided into 3 groups with 12 canals each The time for canal preparation was recorded. The images of pre- and postoperative resin canal were scanned and those were superimposed Amounts of canal deviation, total canal widths, inner canal widths, and outer canal widths were measured at apical 1, 2 ,3, 4, 5, 6, and 7mm levels. The amount of canal deviation was the smallest in Profile group , and the time far canal preparation was the shortest in Hero 642 group. K3 group resulted in competent characteristics in both measurements. Positive rake angle seemed to result in fast shaping of root canal and radial land guide the instrument in center of the canals and around curvatures. Radial land also tended to reduce the sense of screwing into the root canal. The proper selection of the nickel-titanium fie based on the knowledge about file design is needed for the safer, simpler and faster root canal therapy.
