Biological effects of a root conditioning agent for dentin surface modification in vitro

  • Purpose: Connective tissue reattachment to periodontally damaged root surfaces is one of the most important goals of periodontal therapy. The aim of this study was to develop a root conditioning agent that can demineralize and detoxify the infected root surface. Methods: Dentin slices obtained from human teeth were treated with a novel root planing agent for 2 minutes and then washed with phosphate-buffered saline. Smear layer removal and type I collagen exposure were observed by scanning electron microscopy (SEM) and type I collagen immunostaining, respectively. Cell attachment and lipopolysaccharides (LPS) removal demonstrated the efficiency of the root conditioning agent. Results: SEM revealed that the smear layer was entirely removed and the dentinal tubules were opened by the experimental gel. Type I collagen was exposed on the surfaces of the dentin slices treated by the experimental gel, which were compared with dentin treated with other root planing agents. Dentin slices treated with the experimental gel showed the highest number of attached fibroblasts and flattened cell morphology. The agar diffusion assay demonstrated that the experimental gel also has effective antimicrobial activity. Escherichia coli LPS were effectively removed from well plates by the experimental gel. Conclusions: These results demonstrated that this experimental gel is a useful tool for root conditioning of infected root surfaces and can also be applied for detoxification of ailing implant surface threads.

치근면 활택술후 2% minocycline gel의 치료효과 (Effects of 2% minocycline gel as an adjunct to scaling and root planing on the treatment of adult periodontitis)

  • The objective of the present study was to evaluate the clinical and microbiological effects of scaling and root planing combined with local application of 2% minocycline gel to patients with moderate to advanced chronic adult periodontitis. 27 healthy patients with moderate to advanced chronic adult periodontitis were enrolled in the study. The quadrants that had 2 or more teeth with $5{\sim}8mm$ probing pocket depth and radiographic evidence of alveolar bone loss were selected and divided into test side and control side according to the splitmouth design. All patients received standardized oral hygiene instructions at the beginning of the study. Subsequently scaling and root plaining was performed on all remaining teeth until 0 week. The 2% minocycline gel was applied to periodontal pocket at 0, 1, 2, 3week in the test side. The normal saline was irrigated subgingivally for about 30 seconds in the control side. The clinical and microbiological analysis was carried out at 0, 4, 8, and 12weeks. The results of this st udy were as follows; 1.2% minocycline gel delivered subgingivally as an adjunct to scaling and root planing provided benefit in reducing sulcular bleeding index and pocket depth than the use of normal saline. 2. The relative proportion of cocci and non-motile bacteria was increased in the test and control groups with time, and there was no statistically significantdifference between two groups. 3. The proportion of spirochetes was slowly reduced in the control group, but, inthe test group, they were remarkably reduced from the 4th week, and there was a statistically significant difference between two groups. 4. In both groups, the relative proportion of motile rods was notably decreasedat the beginning of the study, and remained until 12th week in the test group,but, in the control group, they were slowly increased from the 4th weekand finally similar to that of the initial examination. In conclusion, local application of 2% minocycline gel may be effective in the clinical and microbiological aspects as an adjunct to scaling and root planing in periodontal disease sites.

수종의 기구를 이용한 치근면 활택술후 치근면의 변화 (Evaluation of the Effect of Various Root Planing Instruments on the Root Surfaces)

  • The purpose of this study was to evaluate the effects of periodontal curet and various rotating instruments on the root surfaces. Thirty-five extracted teeth with advanced periodontal disease were used. They was root planed with periodontal curet, periodontal Perio-Clean bur, periodontal Roto-Perio bur, resin polishing ET bur, and resin polishing diamond fissure bur. To find dentinal tubule orifices on the root surface, tetracycline HCI solution was applied to the one tooth of treated each group. Then, root surfaces were investigated using scanning electron microscope. Amount of loss of cementum was evaluated by loss of tooth substance index. The results were as follows. 1. Groups treated with periodontal curet and Perio-Clean bur showed irregular surface and concavities. Concavities seemed to be lacunae of cementocyte. Other groups treated with Roto-Perio bur, resin polishing ET bur, and resin polishing diamond fissure bur showed partially opened dentinal tubule orifice. 2. Groups treated with periodontal curet and Perio-Clean bur and tetracycline HCl showed irregular surface. No dentinal tubule orifice was seen. Other groups treated with Roto-Perio bur, resin polishing ET bur, and resin polishing diamond fissure bur and tetracycline HCl showed dentinal tubule orifice with various shape and size. 3. Loss of tooth substance indices were compared between groups. There was no statistically difference between periodontal curet and Perio-Clean bur groups. There were statistically differences between periodontal curet and Roto-Perio bur, ET bur, and diamond fissure bur groups. As a result of this study, groups treated with Roto-Perio bur, resin polishing ET bur, and resin polishing diamond fissure bur showed more cementum removed than groups treated with periodontal curet and Perio-Clean bur. Therefore, in a conventional treatment for periodontal disease, it was recommended that periodontal curet or Perio-Clean bur should be used. In a treatment for regeneration of periodontal tissue, it was recommended that Roto-Perio bur, resin polishing ET bur, or resin polishing diamond fissure bur should be used

Association of gingival biotype with the results of scaling and root planing

  • Purpose: The concept of gingival biotype has been used as a predictor of periodontal therapy outcomes since the 1980s. In the present study, prospective and controlled experiments were performed to compare periodontal pocket depth (PPD) reduction and gingival shrinkage (GSH) after scaling and root planing (SRP) according to gingival biotype. Methods: Twenty-five patients diagnosed with chronic periodontitis participated in the present study. The PPD and GSH of the labial side of the maxillary anterior teeth (from the right canine to the left canine) were evaluated at baseline and 3 months after SRP. Changes in the PPD following SRP were classified into 4 groups according to the gingival thickness and initial PPD. Two more groups representing normal gingival crevices were added in evaluation of the GSH. The results were statistically analyzed using the independent t-test. Results: In the end, 16 patients participated in the present study. With regard to PPD reduction, there were no significant differences according to gingival biotype (P>0.05). Likewise, sites with a PPD of over 3 mm failed to show any significant differences in the GSH (P>0.05). However, among the sites with a PPD of under 3 mm, those with the thin gingival biotype showed more GSH (P<0.05). Conclusions: PPD changes after SRP were not affected by gingival biotype with either shallow or deep periodontal pockets. GSH also showed equal outcomes in all the groups without normal gingival crevices. The results of SRP seem not to differ according to gingival biotype.

급속진행형치주염에서 치석제거술과 국소 약물송달제재의 병용효과 (THE COMBINED EFFECT OF A LOCAL MINOCYCLINE DELIVERY SYSTEM AND MECHANICAL DEBRIDEMENT IN RADIDLY PROGRESSIVE PERIODONTITIS)

  • Rapidly progressive periodontitis is known to be usually associated with systemic problems and improved with antibiotic therapy. Recent experiments in which bioresorbable polycaprolactone was polymerized with minocycline has shown that the system released effective antibiotic concentration during the 7 days' period. This clinical trial was to compare the efficacy of a minocycline film(poly-caprolactone+polyglycol+10% minocycline) insertion plus supragingival scaling(MS) or subgingival scaling & root planing(MSRP) with the scaling(S) or subgingival scaling & root planing alone(SRP), at improving the periodontal condition in RPP. Fifteen patients were examined for plaque accumulation, gingival inflammation, probing depth and attachment loss at baseline, then 1, 2, 4 and 8 weeks after 4 treatment regimens were randomly undergone in 4 comparable sites(PD>5mm, LA>3mm) in each subject. Results revealed statistically significant treatement effect with a reduction in a probing depth in SRP(2.0mm), MS(1.8mm), and MSRP(2.1mm). There was no significant reduction in the supragingival scaling alone group(0.6mm). Similarly, attachment levels were significantly improved in the SRP(1.5mm), MS(2.0mm) and MSRP(2.0mm) groups. Net % BOP reduction at 8 week compared to baseline was 6.7% (S), 26.7% (MS), 26.7% (SRP), and 33.3% (MSRP). MSRP produced the greatest improvement in BOP at 8 week. This data suggests that a subgingival minocycline delivery system as a adjunct to scaling alone or scaling & root planing may produce significant clinical benefits over scaling alone in rapidly progressive periodontitis patients.

Efficacy of non-surgical treatment accompanied by professional toothbrushing in the treatment of chronic periodontitis in patients with type 2 diabetes mellitus: a randomized controlled clinical trial

  • Purpose: The present study aimed to evaluate the clinical benefit of additional toothbrushing accompanying non-surgical periodontal treatment on oral and general health in patients with type 2 diabetes mellitus (T2DM). Methods: We conducted a doubled-blind randomized controlled trial in 60 T2DM patients between June 2013 and June 2014. The patients were randomly assigned to the scaling and root planing (SRP) group; the scaling and root planing with additional toothbrushing (SRPAT) group, in which additional toothbrushing was performed by toothpick methods; or the control group. Microbiological and oral examinations were performed for up to 12 weeks following treatment. Non-surgical treatment was conducted in the experimental groups. The SRP group received scaling and root planing and the SRPAT group received additional toothbrushing with the Watanabe method once a week from the first visit through the fifth visit. The primary outcomes were changes in haemoglobin A1c (or glycated haemoglobin; HbA1c) levels, serum endotoxin levels, and interleukin-1 beta levels. Periodontal health status was measured by periodontal pocket depth, the calculus index, and bleeding on probing (BOP). Results: Both the SRP and SRPAT groups showed improvements in periodontal health and HbA1c, but the SRPAT group showed significantly less BOP than the SRP group. Furthermore, only the SRPAT group showed a statistically significant decrease in serum endotoxin levels. Conclusions: Non-surgical periodontal treatment was effective in improving HbA1c and serum endotoxin levels in T2DM patients. Furthermore, non-surgical treatment with additional tooth brushing had a more favourable effect on gingival bleeding management. Effective professional intraoral tooth brushing instruction using the modified plaque score: a randomized clinical trial

  • Purpose: The purpose of this study was to evaluate the efficacy of the modified plaque score (MPS) for assessing the oral hygiene status of periodontitis patients. Methods: A total of 116 patients were included in this study. After evaluation of the $L{\ddot{o}}e$ and Silness gingival index (GI), Silness and $L{\ddot{o}}e$ plaque index (PlI), O'Leary plaque control record (PCR), and MPS, patients were randomly assigned to either a conventional tooth brushing instruction (C-TBI) group (n=56) or a professional intraoral tooth brushing instruction (P-TBI) group (n=60). The MPS and clinical parameters were re-evaluated after scaling and a series of root planing. The convergent validity of MPS with the PlI and PCR was assessed. The measurement time for MPS and PCR was compared according to the proficiency of the examiner. Results: After root planing, the GI, PlI, PCR, and MPS improved from their respective baseline values in both groups. Three different plaque indices including the MPS, showed significant differences between the C-TBI group and the P-TBI group after root planing. The MPS showed significant concurrence with the PCR and PlI. The mean time for PCR measurement was $2.76{\pm}0.71$ times longer than that for MPS measurement after 2 weeks of training. Conclusions: MPS seems to be a practical plaque scoring system compared with the PlI and PCR. These findings suggest that repetitive plaque control combined with an easily applicable plaque index (MPS) may facilitate more effective oral hygiene education and improved periodontal health.

구연산과 섬유소의 치근면 도포가 성견 치주조직 재생에 미치는 영향 (THE EFFECTS OF TOPICAL CITRIC ACID AND FIBRIN APPLICATIONS ON THE PERIODONTAL REGENERATION OF EXPERIMENTAL DEFECTS IN DOGS)

  • 성견 견치의 인위적 골결손부에 치근활택술 시행 후, 섬유소 처리, 구연산 탈회, 섬유소 처리및 구연산 탈회가 치주조직 재생에 미치는 영향을 관찰하기 위하여 치근장축 방향으로 절편을 1일, 3일, 7일, 14일, 21일 간격으로 제작하여 광학현미경으로 관찰한 결과 다음과 같은 결론을 얻었다. 술후 1일째 대조군을 제외한 섬유소 단독도포군, 구연산 단독도포군 및 섬유소-구연산 병용도포군에서 섬유소양이 대차없이 밀집되어 나타났으며, 술후 3일째부터 대조군과 섬유소 단독도포군에서 상피의 하방성장이 관찰되기 시작하였고 구연산 단독도포군과 섬유소-구연산 병용도포군에서는 상피의 하방성장이 거의 관찰되지 않았다. 술후 1주째부터 각 군의 섬유소가 부분적으로 교원섬유로 대치되기 시작하였으며 2주후 각 군간에 대차없이 대부분의 섬유소기 교원섬유로 대치되는 양상을 보였으며 , 술후 3주째에 구연산 단독도포군과 섬유소-구연산 병용도포군에서 부분적으로 교원섬유의 규칙적인 배열상이 관찰되었고 대조군 및 섬유소 단독도포군에서는 거의 관찰되지 않았다. 구연산 단독도포군과 섬유소-구연산 병용도포군에서는 술후 2주째부터 치근흡수가 관찰되었으며 3주째에는 병 용도포군에 비해 구연산 단독도포군에서 더 많은 치근흡수가 관찰되었다.
