  • AESOP (Automated Endoscope System for Optimal Positioning)을 이용한 최소 침습적 심장 수술은 고식적인 정중흉골 절개술을 이용한 심장 수술에 비하여 수술 상처가 작고, 수술 후 환자의 회복이 빠르며, 입원 기간을 단축시키는 등의 장점이 알려져 있다. 저자들은 26세 여자 환자에서 좌측 최소 개흉술을 이용한 동맥하형 심실중격 결손증의 교정수술을 성공적으로 시행하였기에 문헌고찰과 함께 보고하는 바이다.

The Feasibility and Future Prospects of Robot-Assisted Surgery in Gastric Cancer: Consensus Comments from the National Evidence-based Collaborating Agency Round-Table Conference

  • To establish an appropriate policy for robotic surgery in Korea, the National Evidence-based Collaborating Agency (NECA) and the Korean Society of Health Policy and Administration held a round-table conference (RTC) to gather opinions through a comprehensive discussion of scientific information in gastric cancer. The NECA RTC is a public discussion forum wherein experts from diverse fields and members of the lay public conduct in-depth discussions on a selected social issue in the health and medical field. For this study, representatives from the medical field, patient groups, industry, the press, and policy makers participated in a discussion focused on the medical and scientific evidence for the use of robotic surgery in gastric cancer. According to the RTC results, robotic surgery showed more favorable results in safety and efficacy than open surgery and it is similar to laparoscopy. When the cost-effectiveness of robotic surgery and laparoscopy is compared, robotic surgery costs are higher but there was no difference between the two of them in terms of effectiveness (pain, quality of life, complications, etc.). In order to resolve the high cost issue of the robotic surgery, a proper policy should be implemented to facilitate the development of a cost-effective model of the robotic surgery equipment. The higher cost of robotic surgery require more evidence of its safety and efficacy as well as the cost-effectiveness issues of this method. Discussions on the national insurance coverage of robotic surgery seems to be necessary in the near future.

Effects of Gait Training Using a Robot for Balance in Total Hip Arthroplasty Patients after Bilateral Avascular Necrosis: A Case Study

  • Purpose: This study sought to investigate the effects of robot-assisted gait training on balance in total hip arthroplasty (THA) patients after bilateral avascular necrosis (AVN). Methods: This case study in two patients utilized an 'A-B-A' single-subject experimental design that included five days of pre-intervention, followed by five days of intervention, and five days of post-intervention. The intervention involved the use of a standing inclined robot (R-bot) for 15 minutes. The outcome measures were evaluated using the Functional Reaching Test (FRT), Time Up to Go (TUG), and the Modified One Leg Standing Test (OLST). Results: Patient 1 showed improvement based on data gathered from baseline A to intervention period B, with results as follows: FRT improved from 27.7 cm to 41.28 cm, OLST LT from 14.03 seconds to 67.37 seconds, OLST RT from 2.94 seconds to 35.97 seconds, and TUG from 12.96 seconds to 7.82 seconds. Patient 2 also showed improvement from baseline A to intervention period B, with results as follows: FRT improved from 17.18 cm to 24.3 cm, OLST LT from 11.53 seconds to 52.01 seconds, OLST RT from 12.99 seconds to 62.19 seconds, and TUG from 27.31 seconds to 12.99 seconds. Conclusion: Based on the results of this study, robotic rehabilitation during the early stages after surgery is effective for promoting balance in patients who have undergone THA due to bilateral AVN.

Novel cystography parameter to predict early recovery from urinary continence after radical prostatectomy for prostate cancer: a retrospective study

  • Background: The purpose of this study was to investigate whether postoperative cystography findings can predict early and longterm recovery from incontinence after radical prostatectomy (RP), compared with the other cystography parameters. Methods: I retrospectively reviewed 118 patients who underwent robot-assisted RP (RARP) for localized prostate cancer at single institution between January 2016 and April 2021. One hundred and seven patients were included in the study. Postoperative cystography was routinely performed 7 days after surgery. The bladder neck to pubic symphysis ratio, vesicourethral angle, and bladder neck anteroposterior length (BNAP) ratio (the bladder neck-posterior margin distances divided by the anteroposterior lengths) were evaluated. Continence was defined as cessation of pad use. The association between these variables and urinary incontinence was also analyzed. Results: The urinary incontinence recovery rates 1, 3, 6, and 12 months after RARP were 43.92%, 66.35%, 87.85%, and 97.19%, respectively. Multivariate logistic regression analysis demonstrated that a lower BNAP ratio and wider vesicourethral angle were significantly associated with continence restoration at 1, 3, and 6 months after surgery. In addition, in terms of days of pad usage, lower BNAP ratio, wider vesicourethral angle, and bladder neck preservation were significantly associated with recovery from urinary incontinence within 12 months as assessed by Cox proportional hazard analysis. Conclusion: This study demonstrated that vesicourethral angle and BNAP ratio were independent predictors of early recovery from post-prostatectomy incontinence. I suggest that both the sagittal and coronal views of postoperative cystography help anticipate early continence restoration after RARP.

Emerging Role of Robot-assisted Gastrectomy: Analysis of Consecutive 200 Cases

  • Purpose: Robotic surgery for gastric cancer is a promising alternative to laparoscopic surgery, but the data are limited. We aimed to evaluate whether gaining experience in robotic gastrectomy could improve surgical outcomes in the treatment of gastric cancer. Materials and Methods: Two hundred and seven consecutive cases of patients with clinical stage I gastric cancer who underwent robotic surgery at the National Cancer Center of Korea between February 2009 and February 2012 were retrospectively reviewed. Surgical outcomes were analyzed and compared between the initial 100 and later 100 cases. Results: Seven patients required conversion to open surgery and were excluded from further analysis. The mean operating time for all patients was 248.8 minutes, and mean length of hospitalization was 8.0 days. Twenty patients developed postoperative complications. Thirteen were managed conservatively, while 6 had major complications requiring invasive procedures. One mortality occurred owing to myocardial infarction. Operating time was significantly shorter in the latter 100 cases than in the initial 100 cases (269.9 versus 233.5 minutes, P<0.001). The number of retrieved lymph nodes was significantly greater in the latter cases (35.9 versus 39.9, P=0.032). The hospital stay of patients with complications was significantly longer in the initial cases than in the latter cases (16 versus 7 days, P=0.005). Conclusions: Increased experience with the robotic procedure for gastric cancer was associated with improved outcomes, especially in operating time, lymph node retrieval, and shortened hospital stay of complicated patients. Further development of surgical techniques and technology might enhance the role of robotic surgery for gastric cancer.

Successful Robotic Gastrectomy Does Not Require Extensive Laparoscopic Experience

  • Purpose: We evaluated the learning curve and short-term surgical outcomes of robot-assisted distal gastrectomy (RADG) performed by a single surgeon experienced in open, but not laparoscopic, gastrectomy. We aimed to verify the feasibility of performing RADG without extensive laparoscopic experience. Materials and Methods: Between July 2012 and December 2016, 60 RADG procedures were performed by a single surgeon using the da $Vinci^{(R)}$ Surgical System (Intuitive Surgical). Patient characteristics, the length of the learning curve, surgical parameters, and short-term postoperative outcomes were analyzed and compared before and after the learning curve had been overcome. Results: The duration of surgery rapidly decreased from the first to the fourth case; after 25 procedures, the duration of surgery was stabilized, suggesting that the learning curve had been overcome. Cases were divided into 2 groups: 25 cases before the learning curve had been overcome (early cases) and 35 later cases. The mean duration of surgery was 420.8 minutes for the initial cases and 281.7 minutes for the later cases (P<0.001). The console time was significantly shorter during the later cases (168.6 minutes) than during the early cases (247.1 minutes) (P<0.001). Although the volume of blood loss during surgery declined over time, there was no significant difference between the early and later cases. No other postoperative outcomes differed between the 2 groups. Pathology reports revealed the presence of mucosal invasion in 58 patients and submucosal invasion in 2 patients. Conclusions: RADG can be performed safely with acceptable surgical outcomes by experts in open gastrectomy.

승모판 성형술에 있어 최초 침습적 수술방식과 고전적 정중 흉골 절개술을 통한 접근방식의 비교 (Comparison of Mitral Valve Repair between a Minimally Invasive Approach and a Conventional Sternotomy Approach)

  • 배경: 최소 침습적 심장 수술은 전통적인 정중 흉골 절개술에 비해 수술 후 조기 회복 및 우수한 미용적 효과 등의 장점이 있지만 승모판막 성형술과 같이 복잡한 개심술에서의 성적은 아직 국내에 보고된 바 없다. 본 연구에서는 승모판 폐쇄 부전증의 환자군에서 로봇을 이용한 최소 침습적 승모판 성형수술의 유용성에 관해 정중 흉골 절개술과 비교하여 알아보고자 하였다. 대상 및 방법: 1993년 12월부터 2005년 12월까지 승모판막 폐쇄 부전으로 승모판막 성형술을 시행받은 520명의 환자를 연구대상으로 하였다. 이 중 정중 흉골 절개술을 통하여 승모판 성형술을 시행한 군을 S군(n=432), 최소 침습적 우측 개흉술을 이용한 환자군을 M군(n=88)으로 하여 임상 결과를 비교하였다. 우측 개흉술시에는 AESOP (Automated Endoscopic System for Optimal Positioning) 3000이라는 로봇을 이용하여 5 cm 이하의 절개로 수술을 시행하였다. 결과: 양 군 간의 나이, 성별, 수술 전 좌심실 구출률 등은 양 군 간의 차이는 없었다. S군에서 2예의 조기 사망이 있었으나 M군에서는 수술 사망예가 없었다. M군이 대동맥 차단 시간과 심폐기 가동 시간은 S군에 비해 유의하게 길었지만, 중환자실 재원기간과 수술 후 재원 일수는 S군에 비해 유의하게 짧았다. 제한된 절개에도 M군에서 승모판막 성형술식에 사용된 수술 술기는 제한이 없었으며, 술 후 의미있는 잔존 승모판막 폐쇄부전증의 빈도는 양 군에서 유사하였다 결론: 최소 침습적 우측 개흉술을 통한 승모판막 성형술을 시행하더라도 조기 성적은 차이가 없었다. 향후 최소 침습적 승모판막 성형술의 중-장기 성적의 관찰을 요하며 본 연구를 바탕으로 승모판막 성형술의 일차적인 접근 방법으로 최소 침습적 개흉술이 보다 폭넓게 시행될 수 있을 것으로 기대한다.

수술용 내시경 로봇(AESOP)을 이용한 최소 침습적 개심술과 동 기간에 시행된 전통적인 개심술의 결과에 대한 비교 (Comparison of the Operative Results of Performing Endoscopic Robot Assisted Minimally Invasive Surgery Versus Conventional Cardiac Surgery)

  • 배경: 내시경 장비와 수술 로봇의 발달로 작은 절개를 이용한 최소 침습적 심장수술이 점점 증가하고 있으나 이에 대한 수술 결과를 고식적인 수술 방법과 비교, 분석한 국내 보고는 드문 편이다. 대상 및 방법: 2005년 12월부터 2007년 6월까지 수술용 내시경 로봇(AESOP2000)을 이용하여 심장수술을 받은 48명(A군)과 동 기간에 정중흉골절개로 수술을 받은 50명(B군)을 후향적으로 조사하여 수술 시간, 수술 결과, 술 후 통증, 회복 정도 등을 객관적으로 비교, 분석하였다. 결과: 수술 사망은 없었으며 술 후 합병증 발생률도 두 군간에 큰 차이는 없었다. 평균 수술시간($292.7{\pm}61.7$분, $264.0{\pm}47.9$분; p=0.01), 체외순환시간($(128.4{\pm}37.6$분, $101.7{\pm}32.5$분; p<0.01)은 A군에서 더 길었으나 대동맥차단시간 ($82.1{\pm}35.0$분, $87.8{\pm}113.5$분; p=0.74), 평균 인공호흡기 사용시간($18.0{\pm}18.4$시, $19.7{\pm}9.7$시; p=0.57)은 차이가 없었으며 중환자실 평균 입원시간($53.2{\pm}40.2$시, $72.8{\pm}42.1$시; p=0.02) 및 평균 재원기간($9.7{\pm}7.2$일, $14.8{\pm}11.9$일; p=0.01)은 A군에서 더 짧았다. 입원기간 중 수혈을 받은 환자는 B군에서 더 많았으며 평균 수혈양도 B군에서 더 많았으나 통계적 유의성은 없었다(p=0.06). 술 후 1주에서 4주까지 통증 점수는 A군에서 의미 있게 적었으며 일상생활의 수행 능력을 측정하는 듀크 활동상태지수 (Duke activity status Index)에 따른 기능 점수(functional status score)는 A군에서 현저하게 높았다. 술 후 심초음파 결과 승모판 성형술을 시행한 환자의 역류 정도($0.7{\pm}1.0,\;0.9{\pm}0.9$; p=0.60)와 삼첨판 성형술을 시행한 환자의 역류 정도($1.0{\pm}0.9,\;1.1{\pm}1.0$; p=0.89)는 두 군간에 통계적으로 유의한 차이가 없었다. 판막주위유출은 각각 1명의 환자에게 있었으며 그 외 판막 관련 합병증은 없었다. 결론: 본 연구 결과 로봇을 이용한 치소 침습적 심장수술은 수술 후 통증 감소, 환자의 빠른 회복, 미용적인 효과, 짧은 입원기간 등의 장점뿐만 아니라 수술 성적도 고식적 수술 방법에 비해 떨어지지 않음이 확인되었다.
