Ecological Risk Assessment of Pharmaceuticals in the Surface Water Near a Pharmaceutical Manufacturing Complex in Korea (제약단지 인접 지역 지표수의 잔류 의약물질 생태위해성평가)

  • Objectives: Limited information is available on the presence and associated ecological risks of pharmaceutical residues in aquatic environments near pharmaceutical manufacturing areas in Korea. In this study, we investigated the current state of pharmaceutical contamination and its associated ecological risks in streams near a pharmaceutical manufacturing complex. Methods: Seven pharmaceuticals (acetaminophen, clarithromycin, diclofenac, diphenhydramine, ibuprofen, mefenamic acid and roxithromycin) were measured in water samples collected from the streams near a pharmaceutical manufacturing complex. A predicted no-effect concentration (PNEC) was derived using either the assessment factor method or species sensitivity distribution method. In addition, a hazard quotient for each pharmaceutical was calculated by dividing its measured environmental concentration by its PNEC. Results: Samples collected downstream from the wastewater treatment plant (WWTP) had higher concentrations of pharmaceuticals than those collected from the reference site (upstream). Moreover, pharmaceutical concentrations were greater in ambient water than in the final effluent from the WWTP, which suggested that non-point sources were contributing to the contamination of the ambient water environment. Some of the target pharmaceuticals exhibited a hazard quotient >1, indicating that their potential ecological effects on the aquatic environment near the pharmaceutical industrial area should not be ignored. Conclusion: This study demonstrated that the pharmaceutical manufacturing area was contaminated with residual drugs, and that there was a possible non-point source near the WWTP effluent discharge area. The results of this study will aid in the development of management plans for pharmaceuticals, particularly in hotspots such as pharmaceutical industrial sites and their vicinities.

Effectiveness of NQ-E index-based individual nutrition counseling for community-care elderly: an intervention study on improving nutritional status, complex chronic diseases, and quality of life (커뮤니티케어 대상 노인의 NQ-E 지수 기반 개별영양상담 효과성: 복합만성질환과 삶의 질 개선에 관한 중재 연구)

  • Objectives: This study sought to assess the effectiveness of community-based nutrition counseling on improving nutritional status, managing complex chronic diseases, and enhancing the quality of life for elderly individuals with chronic conditions, particularly in older adults with high levels of food insecurity and multiple chronic illnesses. Methods: Thirty elderly subjects with diabetes and hypertension who were registered at local Senior Welfare Center received individualized nutrition counseling, based on their Nutrition Quotient for the Elderly (NQ-E) index. Over a 16-week period, they received tailored counseling and underwent various health and nutritional assessments. The final analysis included 28 participants after two dropped out. Data analysis was conducted using the SPSS v28.0. Results: The subjects were over 70, with multiple chronic diseases including diabetes and hypertension and predominantly female. After 16 weeks, significant improvements were observed in the subjects' grip strength, and HbA1c levels, as well as in their NQ-E scores, indicating improved dietary balance and diversity. There were no significant improvements in the 'Moderation' subdomain of the NQ-E index, suggesting that this aspect requires further attention in nutritional counseling. The subjects' nutritional risk scores (NSI) were also significantly decreased, indicating less nutritional risk. Lastly, as measured by the SF-36K, the subjects' quality of life showed significant improvement in several domains including physical role performance and social function. Conclusions: This study demonstrates that tailored nutrition counseling, based on the NQ-E index, can improve elderly health, manage chronic diseases, and enhance quality of life. This approach potentially broadens the scope of community nutritionists' roles within an aging society. However, additional research is necessary to evaluate these interventions' long-term effects and sustainability.

Investigation and Risk Assessment of Heavy Metals Contamination around an Abandoned Metal Mine in Korea

  • Recently, heavy metals contamination of the agricultural soil and crops surrounding mining areas has been identified as one of the most serious environmental problems in South Korea. The Ministry of the Environment in Korea conducted a Preliminary National Environmental Health Survey (PNEHS) in abandoned metal mines in 2007. The priority for a subsequent detailed examination was ranked from the results of PNEHS. The studied mine which was ranked as being of the highest priority is located in the midwestern part of Korea and was operated from 1911 to 1985. In this study, the contamination levels of the heavy metals in the abandoned metal mine were investigated. From the results, the average daily dose (ADD), target hazard quotient (THQ) and target cancer risk of the heavy metals were evaluated. The concentration of arsenic (As) in all of the tailings from the mine was higher than its countermeasure standard of Korea. In particular, the highest concentration of As, 330 mg/kg, was up to 15 times higher than its countermeasure standard. The average concentration of As in agricultural soils was higher than the warning standard of Korea, and higher than its countermeasure standard at six sites. The average concentrations of the analyzed heavy metals in agricultural soil were below the warning standard, but concentrations of cadmium (Cd) and lead (Pb) at 4 sites were higher than its warning standard. The average concentration of As in surface water exceeded the warning standard of Korea. The value of the THQ of As for the tailings was higher than the health protection standard 1. The value of THQ of As for the farmlands was lower than the standard, while the hazard index (HI) of As was higher than the standard. The value of target cancer risk (TCR) of As, $6.44{\times}10^{-4}$, were higher than the health protection standard of a lifetime risk for TCR at $1{\times}10^{-6}$. This suggests that the residents around the metal mines are exposed to As pollution with a carcinogenic risk.

Risk Assessment for Aquatic Organisms of Pesticides Detected In Water Phase of Six Major Rivers in Korea (주요 하천수역에서 검출된 농약의 수서생물에 대한 위해성 평가)

  • Risk assessments of pesticides detected in six major rivers during peak season were estimated for algae, Daphnia, and fish using hazard quotient (HQ) indexes. The eight pesticides (isoprothiolane, hexaconazole, diazinon, chlorpyrifos, prothiofos, alachlor, butachlor, molinate) were detected within the range of 0.027~12.871 ${\mu}g/L$. Detection frequency of isoprothiolate was estimated to be high at 67.5%, and those of the others varied from 15.0 to 37.5%, Hazard Quotients (HQ) indexes varied by freshwater organisms (algae, Daphnia, and fish). Overall, the ecological risk probability due to exposure of pesticides detected in major rivers did not reveal based on HQ indexes below 1.0. Particularly, butachlor and molinate for algae, chlorpyrifos, diazinon, prothiofos for Daphnia, and chlorpyrifos for fish acted as dominant contributors in increasing the ecological risk in six major rivers. This implied that integrated ecological risk assessment is required using various biological species, reflecting toxicity sensitivity. This study may provide the essential data in establishing the priority for pesticides management in major rivers, Korea.

Randomized controlled clinical trial of 2 types of hydroxyapatite-coated implants on moderate periodontitis patients

  • Purpose: The aim of this study was to compare and analyze the peri-implant tissue conditions and prospective clinical outcomes associated with 2 types of hydroxyapatite (HA)-coated implants: (1) fully HA-coated implants and (2) partially HA-coated implants with resorbable blast medium on the coronal portion of the threads. Methods: Forty-four partially edentulous patients were randomly assigned to undergo the placement of 62 HA-coated implants, and were classified as the control group (partially HA-coated, n=30) and the test group (fully HA-coated, n=32). All patients had chronic periodontitis with moderate crestal bone loss around the edentulous area. The stability and clinical outcomes of the implants were evaluated using the primary and secondary implant stability quotient (ISQ), as well as radiographic, mobility, and peri-implant soft tissue assessments around the implants. The Wilcoxon signed-rank test and the Mann-Whitney test were used to evaluate differences between and within the 2 groups, with P values <0.05 considered to indicate statistical significance. Results: The fully HA-coated implants displayed good retention of crestal bone, and insignificant differences were found in annual marginal bone loss between the 2 types of HA-coated implants (P>0.05). No significant differences were found in the survival rate (group I, 100%; group II, 100%) or the success rate (group I, 93.3%; group II, 93.8%). The fully HA-coated implants also did not significantly increase the risk of peri-implantitis (P>0.05). Conclusions: The fully HA-coated implants did not lead to an increased risk of peri-implantitis and showed good retention of the crestal bone, as well as good survival and success rates. Our study suggests that fully HA-coated implants could become a reliable treatment alternative for edentulous posterior sites and are capable of providing good retention of the crestal bone.

Ecotoxicological Risk Assessment for Acetaminophen in Kyongahn River

  • Acetaminophen (paracetamol), generally used as a pain reducing agent, has good analgesic efficacy in toothaches and headaches, but is of little use in inflammatory and visceral pain. This study was performed to analyze the level of acetaminophen in the Kyongahn river and to investigate the ecological risks of target compounds. Sampling sites were Haesil, Soopyo, Wangsan, Kyongahn, Jiwol, Kwangdong, Paldang and they were analyzed in June and August, 2005. Acute toxicity of acetaminophen wwas evaluated for Daphnia magna. From the ecotoxicological results, environmental risk assessments were performed for acetaminophen residues in Kyongahn river to predict their potential adverse effect. Acetaminophen was detected at Kyonahn river, $0.439{\mu}g/l$). The toxic concentration of acetaminophen calculated with 48-h $LC_{50}$ values as 16.9 mg/l. These results indicated that acetaminophen had no significant ecotoxicological impact on short-term acute exposure.

Risk Assessment for Non-Cancer Effects of Volatile Organic Compounds in Children's Products (어린이용품에 함유된 휘발성유기화합물의 비발암 위해성평가)

  • Background: This study was conducted to assess health risks in regard to exposure by children to volatile organic compounds (VOCs) in children's products. Methods: Ten VOCs were measured by head-space gas chromatography in children's products, including toys, oil pastels, sign pens, furniture, ball pools, and playmats. We estimated the average daily dose (ADD) via inhalation during the use of these children's products and calculated hazard quotient (HQ) by dividing ADD by reference dose of VOCs. Results: Among the measured VOCs, five compounds were identified in children's products: benzene, ethylbenzene, styrene, toluene, and xylene. The detection rates of VOCs in toys, ball pools, furniture, playmats, sign pens, and oil pastels were 85%, 100%, 100%, 30%, 100%, and 60%, respectively. The maximum levels of VOCs were 0.18 mg benzene/kg in toys, 5.92 mg toluene/kg in playmats, 10.37 mg ethylbenzene/kg in ball pools, 24.85 mg xylene/kg in toys, and 118.29 mg styrene/kg in ball pools. From exposure levels of VOCs in the children's products HQs were calculated within a range of $5.71{\times}10^{-10}$ to $4.77{\times}10^{-4}$. The HQ of xylene was the highest for children aged 0-6 playing on the playmats. However, the HQ via inhalation exposure to VOCs in individual products did not exceed 1.00. Conclusion: Based on the results, it was concluded that the use of these children's products do not pose health risks to children.

Analysis of Related Factors and Regional Variation of Mortality in Seoul (서울특별시 사망률 변이 및 관련 특성 분석)

  • Background: Health is affected by various local factors. This study aims to investigate the age-standardized mortality variation of Seoul as well as the characteristics of the factors related to the mortality variation. Methods: The Korea Community Health Survey data, Seoul Survey data, Seoul statistics, and e-regional indicators of the National Statistical Office were used. To investigate the basic boroughs standardized mortality variation in Seoul, external quotient, coefficient of variation (CV), and systematic component of variation (SCV) values were suggested; correlation analysis and multiple regression analysis were conducted to investigate the characteristics related to standardized mortality rate. Results: The highest and the lowest standardized mortality rate of Seoul by boroughs had as much as 1.4 times difference; a low level of variation was shown in CV by 8.2; and was shown in SCV by 79. As a result of the multiple regression analysis of the factors that affect standardized mortality variation, the higher the rate of householders with college or higher, the lower the standardized mortality rate, and the higher the high-risk drinking rate, the higher the standardized mortality rate. Of the two, the rate of householder with a degree equivalent or higher than college was shown to have the biggest impact, followed by high-risk drinking rate. Conclusion: We found a variation in age-standardized mortality rate of boroughs in Seoul. The results suggest that policy makers should take into account socioeconomic environmental characteristics of community in developing community-based health promotion rather than focusing on lifestyle changes of residents.

Concentrations and Composition Profiles of Perfluoroalkyl Substances (PFASs) in Coastal Environments from Gunsan, Korea: Assessment of Exposure to PFASs through Seafood Consumption (군산연안 다매체 환경에서 과불화화합물(PFASs)의 농도분포 및 수산물 섭취를 통한 인체위해도 평가)

  • Concentrations of perfluoroalkyl substances (PFASs) were measured in seawater, sediment, and biota collected from Gunsan coast, Korea to investigate their occurrence, distribution, and risk of exposure to humans through seafood consumption. The total concentrations of PFASs in seawater, sediment, and biota ranged from 5.97 to 74.9 ng/L, 0.01 to 13.3 ng/g dry weight, and 0.02 to 5.73 ng/g wet weight, respectively. Predominant PFAS compounds differed across matrices, indicating that the distribution of PFASs in multiple environmental samples is governed by their carbon-chain length. The concentrations of PFASs in seawater were significantly negatively correlated with salinity (P<0.01), suggesting terrestrial input (including land-used PFASs) as the major source of PFAS contamination in coastal environments. The estimated daily intakes (EDIs) of perfluorooctanoic acid (PFOA) and perfluorooctane sulfonate (PFOS) through seafood consumption were 0.05 and 0.06 ng/kg body weight/day, respectively. The EDIs of PFOA and PFOS measured in this study were lower those the proposed by the United States Environmental Protection Agency and Canada guidelines, indicating limited health risk for Korean population from PFASs through consumption of seafood from Gunsan coastal environment.

Indoor exposure to hazardous air pollutants and volatile organic compounds in low-income houses in Lagos, Nigeria

  • This study investigated exposure to air pollutants in rooms in low-income houses at Shomolu (R1), Mafoluku (R2) and Mushin (R3) in Lagos state. The concentrations of most measured exceeded limits of Illinois Department of Public Health (IDPH) for indoor air quality. Air quality index (AQI) in rooms studied was unhealthy for sensitive people in terms of CO, unhealthy in terms of $SO_2$ and very unhealthy in terms of $NO_2$ while moderate air quality was obtained in terms of $PM_{10}$ in most rooms. High concentrations of carbontetrachloride, formaldehyde and xylene measured could have been responsible for some of the health complaints of the occupants. Factor analysis shows that cooking with kerosene, use of gasoline generator and insecticide were the major contributors to indoor air pollution in these rooms. Therefore, there is need to urgently tackle poverty as all affected by these pollutants were poor who live in substandard houses without kitchens.
