Drug allergy in children: what should we know?

  • The drug allergy "label" may have a lifetime of consequences for a child. Many children with alleged drug allergies are proven to be tolerant to the culprit medication when challenged. The field of drug hypersensitivity is a recently evolving field of research, but studies on its epidemiology and diagnostic tools are lacking in children. Clinical history is significant in the diagnosis and classification of drug hypersensitivity in children. Diagnostic tools have been evaluated in a limited number of children; therefore, the guidelines are mainly in line with those for adults. Here, we review the clinical characteristics, main drugs, risk factors, and diagnosis of drug hypersensitivity to aid in its accurate diagnosis in children.

학령기 아동의 아토피 피부염 유병률과 교실 실내 환경 위험요인에 관한 연구 -통합창원시 지역을 중심으로- (Prevalence Rate and Indoor Risk Factors for Atopic Dermatitis in the School Aged Children in Changwon)

  • The purpose of this study was to investigate the prevalence rate of atopic dermatitis (AD) and indoor risk factors of AD in school-aged children living in Changwon, which is one of the largest industrial cities in Korean. Data were collected from 2,118 children in 12 elementary schools in Changwon city. A cross-sectional questionnaire based on the Korean Version of the ISAAC (International Study of Allergy and Asthma in Childhood) was employed to survey the $1^{st}$ and $4^{th}$ year elementary students from the 12 elementary schools in Changwon. The life time and last 12-month prevalence of itchy eczema were 20.6%; 70.5% in the elementary children. The life time and last 12-month prevalence of AD diagnosis were 28.8%; 12.6% in the elementary children. There was only one elementary school that was abnormal in the levels of particulate matters (PM), and $Co_2$. Multivariate logistic regression analysis revealed that risk factors for AD were BMI, the levels of PM, $Co_2$, CO, $No_2$, $O_3$, Ethylebenzene, and Xylene. This study could be used to manage possible risk factors that are related to the prevalence of AD and develop the strategies for prevention of AD.

사상체질병증 임상진료지침: 예방 및 위험인자 (Clinical Practice Guideline for Sasang Constitutional Medicine: Prevention and Risk Fators of Sasangin Disease)

  • Objectives This research was proposed to present Clinical Practice Guideline (CPG) for Prevention of Sasangin disease pattern of Sasang Constitutional Medicine (SCM) and diseases closely related with Sasang constitution. Each CPG was developed by the national-wide experts committee consisting of SCM professors. Methods At first, we searched the literatures related to SCM such as "Dongeuisusebowon", Textbook of SCM and Clinical Guidebook of SCM. Also we searched the articles related to the studies about risk factors for Sasangin disease pattern of both at home and abroad. Finally, we selected leading risk factors of Sasangin disease pattern and developed CPG for prevention of Sasangin disease pattern of SCM. And then, we searched the literatures related SCM such as "Dongeuisusebowon" and the articles on the correlation between disease and Sasang constitution using case-control studies, observational studies or cross sectional studies of both at home and abroad. Next, we selected diseases closely related with Sasang constitution on the basis of articles including prevalence rate and odds ratio between disease and Sasang constitution and finally developed CPG for these diseases. Results and Conclusions We categorized risk factors of Sasang disease pattern into 2 types: non-modifiable and potentially modifiable. 3 items (age, sex and genetic factors) were classified as non-modifiable risk factors of Sasang disease pattern. 6 items (original symptom, stress, diet and nutrition, physical activity, alcohol and drug misuse) were classified as less well-documented or potentially modifiable risk factors of Sasangin disease pattern. We found out Sasang constitution is more likely to develop some diseases. It was proven that Sasang constitution increase the risk of hypertension, diabetes mellitus, metabolic syndrome, stroke, nonalcoholic fatty liver and obstructive sleep apnea. And there is high probability of Sasang constitution being potential risk factor for obesity, hyperlipidemia, allergy and cancer. Also, we found out Taeeumin is independent risk factor for hypertension, diabetes mellitus, metabolic syndrome, stroke, nonalcoholic fatty liver and obstructive sleep apnea. Therefore we recommend that Taeeumin need to prevent these disease by regular checkups and aggressive management.

우리나라 청소년의 알레르기 질환 유병 영향요인 (Factors Influencing Allergy related Disease among Korean Adolescents)

  • 본 연구의 목적은 우리나라 청소년에서 천식, 알레르기 비염 및 아토피 피부염을 포함하는 알레르기 질환의 유병 영향요인을 파악하고자 하는 것이다. 본 연구는 제10차(2014년)청소년건강행태 온라인조사의 원시자료를 이용하였으며, 만 12-18세 청소년 총 72,060명을 연구대상자로 선정하였다. 알레르기 질환 유병 영향요인을 파악하기 위해 로지스틱 회귀분석 결과 모든 변수가 보정된 상태에서 천식은 성별, 연령, 체질량지수, 주관적 경제상태, 흡연 및 주관적 스트레스 인지가 알레르기 비염은 연령, 도시규모, 주관적 경제상태, 음주 및 주관적 스트레스 인지가 아토피 피부염은 성별, 연령, 주관적 경제상태, 음주 및 주관적 스트레스 인지가 알레르기 질환 유병 영향요인으로 나타났다. 따라서 앞으로 청소년의 알레르기 질환에 악영향을 미치는 흡연과 음주와 같은 생활습관들의 규제정책 강화, 포괄적인 예방교육 및 스트레스 관리가 국가적 측면에서 시행될 필요가 있다.

Epigenetic Changes in Asthma: Role of DNA CpG Methylation

  • For the past three decades, more than a thousand of genetic studies have been performed to find out the genetic variants responsible for the risk of asthma. Until now, all of the discovered single nucleotide polymorphisms have explained genetic effects less than initially expected. Thus, clarification of environmental factors has been brought up to overcome the 'missing' heritability. The most exciting solution is epigenesis because it intervenes at the junction between the genome and the environment. Epigenesis is an alteration of genetic expression without changes of DNA sequence caused by environmental factors such as nutrients, allergens, cigarette smoke, air pollutants, use of drugs and infectious agents during pre- and post-natal periods and even in adulthood. Three major forms of epigenesis are composed of DNA methylation, histone modifications, and specific microRNA. Recently, several studies have been published on epigenesis in asthma and allergy as a powerful tool for research of genetic heritability in asthma albeit epigenetic changes are at the starting point to obtain the data on specific phenotypes of asthma. In this presentation, we mainly review the potential role of DNA CpG methylation in the risk of asthma and its sub-phenotypes including nonsteroidal anti-inflammatory exacerbated respiratory diseases.

Association of polymorphisms in thromboxane A2 receptor and thromboxane A synthase 1 with cerebral infarction in a Korean population

  • Thromboxane A2 (TBXA2) is a potent vasoconstrictor in cerebral circulation and is a known contributor to the pathogenesis of cerebral infarction. Thromboxane A2 synthase 1 (TBXAS1) and thromboxane A2 receptors (TBXA2R) are key components in TBXA2 function. We examined whether genetic variants in TBXA2R and TBXAS1 are risk factors for cerebral infarction by genotyping 453 Korean patients with noncardiogenic cerebral infarction and 260 controls. A few, specific polymorphisms in the TBXA2R (-3372G>C, +4710T>C and 4839T>C) and TBXAS1 (+16184G>T, +141931A>T and +177729G>A) genes were chosen and investigated. Logistic regression showed the frequencies of TBXAS1+16184G>T and TBXAS1-ht3 were significantly more frequent in cerebral infarction (P = 0.002, OR = 2.75 and P = 0.01, OR = 1.57, respectively), specifically in small-artery occlusion (SAO) type of cerebral infarction (P = 0.0003 and 0.005, respectively). These results suggest specific TBXAS1 gene polymorphisms may be a useful marker for development of cerebral infarction, especially SAO type in Korean population.

성동구내 어린이집 어린이의 알레르기 질환과 허약아의 실태 및 관련성에 대한 보고 (The Relationships between Allergy and Weakness in Children in Seong-Dong District)

  • Objectives The purpose of this study is to find the relationships between the allergy and weakness in children in Seong-Dong district. Methods 802 day care centers children were participated for survey on allergic disease, risk factors, and weakness. From the result, we analyzed the relationships between the aforementioned factors. Results 1. 802 children were participated for the survey. We eliminated survey answers with unclear answers, and selected 792 survey sheets as our final subject of study. The age of majority of subjects(720 children) were between one to six years old, and the others were under the age of one and over seven. 2. The prevalence rate of asthma in a lifetime was 6.69%, allergic rhinitis was 14.87%, atopic dermatitis was 28.82%, allergic conjunctivitis was 6.29%, food allergies was 9.58%, and drug allergies was 1.4%. In 2009, the prevalence rate of allergies during treatments is 6.69% for asthma, 13.41% for allergic rhinitis, 13.64% for atopic dermatitis, 4.13% for allergic conjunctivitis, 5.23% for food allergies, 0.96% for drug allergies. 3. The significant risk factors were types of dwelling, pet with fur, dirty environments without vacuum cleaners, the numbers of times bed covers were washed, frequency of air ventilation, changes in body after moving, family history. 4. The distribution of the number of children with weakness within the five visceras were respiratory weakness, digestive weakness, psychoneurologic weakness, neuromotor weakness, and urogenital weakness in the order of most to the least. More specifically, children who only has respiratory weakness were the majority, while children who have respiratory and digestive weaknesses, children who have respiratory, digestive, and psychoneurologic weaknesses, children who have respiratory and psychoneurologic were followed. 5. With the exception of children with urogenital weakness, Most of the children with respiratory weakness, digestive weakness, psychoneurologic weakness, and neuromotor weakness had been treated for allergies in 2009, while children who were diagnosed with allergic diseases at least once in their lifetime most likely had weak respiratory, digestive or psychoneurologic systems. Conclusions The results of this study showed that there was a correlation between children with weak viscera and children with allergic diseases. However, further research would be necessary.

Probiotics as a Potential Immunomodulating Pharmabiotics in Allergic Diseases: Current Status and Future Prospects

  • The prevalence of allergic disorders has dramatically increased over the past decade, particularly in developed countries. Apart from gastrointestinal disorders, neoplasia, genital and dermatological diseases etc., dysregulation of gut microbiota (dysbiosis) has also been found to be associated with increased risk of allergies. Probiotics are increasingly being employed to correct dysbiosis and, in turn, to modulate allergic diseases. However, several factors like strain variations and effector metabolites or component of them in a bacterial species can affect the efficacy of those as probiotics. On the other hand, host variations like geographical locations, food habits etc. could also affect the expected results from probiotic usage. Thus, there is a glaring deficiency in our approach to establish probiotics as an irrefutable treatment avenue for suitable disorders. In this review, we explicate on the reported probiotics and their effects on certain allergic diseases like atopic dermatitis, food allergy and asthma to establish their utility. We propose possible measures like elucidation of effector molecules and functional mechanisms of probiotics towards establishing probiotics for therapeutic use. Certain probiotics studies have led to very alarming outcomes which could have been precluded, had effective guidelines been in place. Thus, we also propose ways to secure the safety of probiotics. Overall, our efforts tend to propose necessary discovery and quality assurance guidelines for developing probiotics as potential immunomodulatory 'Pharmabiotics.'

서울시 일부 아동의 식품알레르기 위험요인 : 식품 섭취와 환경적 요인을 중심으로 (Risk factors for food allergy among children in Seoul: focusing on dietary habits and environmental factors)

  • 서울시 일부 아동 대상의 식품알레르기 유병률과 위험 요인 및 다른 알레르기 질환과의 연관성 조사결과 식품알레르기 의사 진단 유병률은 14.2%, 알레르기 증상을 한번이라도 경험한 아동의 유병률은 20.4%, 최근 12개월 내 증상 경험 유병률은 17.4%이었다. 식품알레르기를 한번이라도 경험한 아동 612명을 대상으로 알레르기 경험 특성을 도출한 결과 대부분 식품 경구 섭취를 통해 발생하였고 증상 유발식품은 난류, 복숭아, 우유, 땅콩, 새우 등이 있었다. 식품알레르기 증상 경험 유병과 12개월 내 주요 알레르기 유발 의심식품 섭취의 위험요인으로는 곡류, 감자류, 패스트푸드, 견과류, 우유 등이 유의한 영향이 있었다. 식품 섭취의 경우 식품알레르기 증상을 경험한 아동일 경우 견과류, 우유 및 유제품, 난류의 섭취 빈도가 낮을수록 위험도가 유의하게 증가하였다. 환경적 위험요인으로 연령, 부모의 알레르기 질환력, 항생제 복용요인이 도출되었으며, 기타 알레르기 질환과도 관련이 있었다. 최근 식품 알레르기의 또 다른 환경적 위험요인으로 다른 연구들에서도 언급되는 요인이기에 이와 관련한 지속적인 연구가 필요할 것이다. 결과적으로 식품알레르기의 경험이 있는 아동은 다른 알레르기 질환으로 이환될 가능성이 상대적으로 높으며, 이에 따라 규칙적인 식품 섭취와 조기진단을 통하여 유발식품을 명확히 알고 제한할 수 있어야 할 것이다. 궁극적으로 이 연구 결과를 통해 서울에 거주하는 아동의 유병률과 증상경험의 특성을 파악하여 효과적인 중재방안을 도출할 기초자료로의 의미가 있을 것이다.

청소년 알레르기성 질환의 복합성과 중증도가 자살 생각에 미치는 영향 (Effect of Allergy Related Disease on Suicide Ideation among Adolescents in Korea)

  • Background & Objectives: There were increasing evidence about the relationship between allergy related disease such as asthma, atopic dermatitis and allergic rhinitis and suicide ideation. However little was known about the concrete relatedness between severity and comorbidity of allergy related disease with suicide ideation. The objective of this study was to investigate the cases of the prevalence of suicide ideation among adolescents with allergy related disease such as asthma, atopic dermatitis and allergic rhinitis, and examine the association between allergy related disease and suicidal ideation among adolescents in South Korea. Methods: Data was based on Korean Youth Risk Behavior Web-based Survey(2014) which was a cross-sectional study containing 34,874 Korean middle and high school students who diagnosed with allergy related disease. We used the weights, strata and primary sampling unit information provided by the public use dataset to compute descriptive statistics and logistic regressions. Computations were done with SPSS version 20.0. Results: 19.9%, 15.6%, 13.8% of adolescents who suffered from one, two and three of allergy related diseases respectively reported having been thought of suicide ideation. Socio-demographic factors were adjusted as control variables. Students with greater severity of disease were more likely to have suicide ideation. Odds ratio for students who were absent one to three days from school because of allergies was 1.96(95% CI 1.51-2.46), and odds ratio for those who were absent more than four days from school was 3.60(95% CI 2.46-5.28). Conclusions: Given that adolescents' severity and comorbidity of allergy related disease were clearly associated with suicide ideation, suicide prevention programs for adolescents with allergy related disease should be improved by strategic approaches towards the severity and comorbidity of disease.
