Since mid-1960s the reports from the Surgeon General, the World Health Organization, and other health experts state that there is no risk-free level exposure to smoking and secondhand smoke. Tobacco smoke is made up of more than 7,000 chemicals. Hundreds are toxic, and at least 70 are carcinogens. The chemicals in tobacco smoke reach smoker's lungs quickly every time smoker inhale causing damages immediately. Inhaling even the smallest amount of tobacco smoke can also damage smoker's DNA, which can lead to cancers. Smoking is responsible for more than 87% of lung cancers, but there are a host of other chronic diseases directly related to exposure to tobacco smoke. It's also a major cause of heart disease, stroke, aortic aneurysm, peripheral arterial disease and most of the other diseases. In the United States, each year with more than from 440,000 to 520,000 deaths caused by smoking and exposure to involuntary smoke. They conclude that smoking is the single most important source of preventable morbidity and mortality. The United States of America have about 60-year history of tobacco litigation. Tobacco litigation has been an important tool in tobacco control strategies aimed at limiting the activities of tobacco companies and providing redress to people who have become ill as a result of their use of tobacco products. Tobacco litigation is a kind of tort litigation. Quite often, as in the asbestos and other mass tort litigation episodes, tobacco litigation can play an educational role, warning the public about the magnitude of health risks that might otherwise be less clearly perceived. Tobacco litigation allows smokers, their families or other victims of smoking to sue tobacco companies in order to be compensated for the harm they have suffered. Potential benefits of tobacco litigation include compensation for smoking-related damages, strengthening regulatory activity, publicity, documents disclosure and changing tobacco industry behavior. And also tobacco litigation can limit the political activities of tobacco industry, protect human rights of smokers and non-smokers, increase burden to tobacco price-up and enhance the effects of law and politics in public health.
Increasing sugar intake of population has become a nutritional issue in Korea. Sweet taste perception may be related to behaviors such as eating sweet food including high sugars and total sugar intake. This study aimed to evaluate objective and subjective sweet taste perception and the association among objective sweet taste perception, dietary behaviors related to eating sweet snack food including high sugar, and total sugar intake from the snacks. Participants were 261 healthy female college students (mean age: $21.0{\pm}1.6years$), who were divided into three subgroups based on oral sweet taste evaluation using a sweet taste assessment tool provided by Ministry of Food and Drug Safety: sweet-seeker group (n=139), medium sweet-seeker group (n=54), and unsweet-seeker group (n=68). There was no significant difference in weight and body mass index (BMI) among the three groups; however, the sweet-seeker group had significantly higher sweet taste preference than that of the other groups. Though more people in the sweet-seeker group thought they tended to eat sweet foods than the medium sweet-seeker and unsweet-seeker groups, over half of the sweet-seekers did not think they tended to eat sweet foods. The sweet-seeker group was more likely to eat sweet snacks such as breads, chocolate products, sugar-sweetened milk, and so on than the unsweet-seeker group. Total sugar intake from the selected sweet snacks was 44.4 g for the sweet-seeker group, 34.4 g for the medium sweet-seeker group, and 28.0 g for the unsweet-seeker group with a significant difference. These results indicated the absence of relationship between objective sweet taste perception and the obesity index; however, significant associations were detected among objective sweet taste perception, eating sweet snacks and total sugar intake from the snacks. We also found high disagreement between objective and subjective sweet taste perception of the subjects. The present study provided the novel insight that measuring objective sweet taste perception may be useful for assessing the risk of high sugar consumption and undesirable dietary behaviors.
Benzene is a class 4 hazardous material according to the Act on the Safety Control of Hazardous Substances. This study qualitatively evaluated the damage size of a "toxic" accident and "pool fire" accidents based on benzene in a virtual scenario of a fire and leakage accident during unloading at a port facility. The KORA program was used as an evaluation method, which is supported as a universal program by the National Institute of Chemical Safety. The range of damage effects of a benzene-induced fire and leakage accident was predicted. In the case of toxic damage range, the accident's damage effect range for the "worst case scenario" was reduced by up to 5.11% with a decrease in the size of the leakage hole. In the case of the leakage time, the damage effect range increased to 145.12% with a 10 min leakage time compared to that of a 5 min leakage time and went up to 20 min (212.29%) with a 20 min leakage time. In the case of pool-fire-induced damage, the damage effect range by radiant heat in the "worst case scenario" was 228.8 m in radius from the center of the handling facility. In the "alternative scenario," the damage effect range by radiant heat was reduced by up to 8.26% compared to that in the "worst case scenario" since the size of the leakage hole was decreased by reducing the cross-sectional area of the pipe.

Objectives: Currently, there is only limited knowledge regarding the hazard of low-level exposure to CMR materials in workplaces. To overcome this limitation, a reference concentration for workers($RfC_w$) from among the risk assessment tools proposed by the US EPA is widely used to set a provisional workplace exposure level(PWEL) for CMR materials for which there are no established Korea Occupational Exposure Limits(KOELs) or subjective chemicals for work environment measurements as regulated by Korea Ministry of Employment and Labor(KMOEL). A simple European calculator of derived no effect level(SECO-DNEL) as proposed by REACH can also be used in place of $RfC_w$ to set the PWEL for chemicals. This study was performed to test the acceptability of using SECO-DNEL as an alternative to $RfC_w$ when setting a PWEL for low-level exposures. Methods: The $RfC_w$ and DNEL for the five CMR materials of dinitrogen oxide, catechol, 2-phenoxy ethanol, carbitol, and carbon black were calculated using the dose-response assessments of the US EPA for $RfC_w$ and REACH guidance for SECO-DNEL, respectively. They were compared using paired t-tests to determine the statistical differences between them. Results: For the five chemicals, the $RfC_w$ were 2.53 ppm, 0.10 ppm, 1.73 ppm, 1.66 ppm, and $0.05mg/m^3$, respectively, while the SECO-DNEL were 2.01 ppm, 0.11 ppm, 1.83 ppm, 1.77 ppm, $0.14mg/m^3$, respectively. There was no statistically significant difference between $RfC_w$ and SECO-DNEL. Conclusions: This study suggests that the SECO-DNEL could be applied in place of $RfC_w$ to set a PWEL for low-level exposure to chemicals, especially CMR materials. To further ensure the reliability of SECO-DNEL as an alternative tool, more chemicals should be applied for calculation and comparison with $RfC_w$.

Objective : The purpose of this study was to analyze occupational therapy intervention on the community mobility for stroke patients, and to provide evidence of intervention in the clinical fields. Methods : A systematic review was executed according to the PRISMA checklist. The accessed database was PubMed, EMBASE, Cochrane Library (CENTRAL), ProQuest Dissertations & thesis (PQDT), RISS, and KoreaMed. We included the articles published from 2005 to September 2018. RoBANS checklist was used to evaluate the quality of the articles. Included articles, totally eight, were categorized according to the type of intervention. Results : The study design of the literature was varied from two-group randomized trial, quasi-experimental study, case-control trial, one group pre-post comparison study, and cross-sectional study. In the evidence level, 6 articles were included in level II (75%). The percentage of low risk of bias in each article ranged from 52.5%~87.5%. Four studies (50%) provided intervention based on virtual reality or virtual environment. The three (37.5%) provided intervention based on the protocol, and the other (12.5%) did wheelchair training. All studies reported significant effects of the intervention. Conclusion : This systematic review provided evidences to use proper intervention in the clinical fields. Various type of studies should be conducted to prove the effect of occupational therapy intervention for community mobility.

Objectives: This study aimed to examine the characteristics of patients according to their nutritional status as assessed by five nutritional screening tools: Patient-Generated Subjective Global Assessment (PG-SGA), NUTRISCORE, Nutritional Risk Index (NRI), Prognostic Nutritional Index (PNI), and Controlling Nutritional Status (CONUT) and to compare the agreement, sensitivity, and specificity of these tools. Methods: A total of 952 gastric cancer patients who underwent gastrectomy and chemotherapy from January 2009 to December 2012 at the Samsung Medical Center were included. We categorized patients into malnourished and normal according to the five nutritional screening tools 1 month after surgery and compared their characteristics. We also calculated the Spearman partial correlation, Cohen's Kappa coefficient, the area under the curve (AUC), sensitivity, and specificity of each pair of screening tools. Results: We observed 86.24% malnutrition based on the PG-SGA and 85.82% based on the NUTRISCORE among gastric cancer patients in our study. When we applied NRI or CONUT, however, the malnutrition levels were less than 30%. Patients with malnutrition as assessed by the PG-SGA, NUTRISCORE, or NRI had lower intakes of energy and protein compared to normal patients. When NRI, PNI, or CONUT were used to identify malnutrition, lower levels of albumin, hemoglobin, total lymphocyte count, total cholesterol, and longer postoperative hospital stays were observed among patients with malnutrition compared to those without malnutrition. We found relatively high agreement between PG-SGA and NUTRISCORE; sensitivity was 90.86% and AUC was 0.78. When we compared NRI and PNI, sensitivity was 99.64% and AUC was 0.97. AUC ranged from 0.50 to 0.67 for comparisons between CONUT and each of the other nutritional screening tools. Conclusions: Our study suggests that PG-SGA and NRI have a relatively high agreement with the NUTRISCORE and PNI, respectively. Further cohort studies are needed to examine whether the nutritional status assessed by PG-SGA, NUTRISCORE, NRI, PNI, and CONUT predicts the gastric cancer prognosis.

Results: Malnutrition was observed in 86.24% of patients based on the PG-SGA and 85.82% based on the NUTRISCORE. When NRI or CONUT were applied, the proportions of malnutrition were < 30%. Patients with malnutrition had lower intakes of energy and protein than normal patients when assessed using the PG-SGA, NUTRISCORE, or NRI. Lower levels of albumin, hemoglobin, total lymphocyte count, and total cholesterol and longer postoperative hospital stays were observed among patients with malnutrition compared to normal patients when NRI, PNI, or CONUT were applied. Relatively high agreement for NUTRISCORE relative to PG-SGA was found; the sensitivity was 90.86%, and the AUC was 0.78. When NRI, PNI, and CONUT were compared, the sensitivities were 23.72% for PNI relative to NRI, 44.53% for CONUT relative to NRI, and 90.91% for CONUT relative to PNI. The AUCs were 0.95 for NRI relative to PNI and 0.91 for CONUT relative to PNI. Conclusions: NUTRISCORE had a high sensitivity compared to PG-SGA, and CONUT had a high sensitivity compared to PNI. NRI had a high specificity compared to PNI. This relatively high sensitivity and specificity resulted in 77.00% agreement between PNI and CONUT and 77.94% agreement between NRI and PNI. Further cohort studies will be needed to determine if the nutritional status assessed by PG-SGA, NUTRISCORE, NRI, PNI, and CONUT predicts the gastric cancer prognosis.

A realistic lifetime seismic-reliability based approach is unavoidable to perform Life-Cycle Cost (LCC)-effective optimum design, maintenance, and retrofitting of structures against seismic risk. So far, though a number of researchers have proposed the LCC-based seismic design and retrofitting methodologies, most researchers have only focused on the methodological point. Accordingly, in most works, they have not been quantitatively considered critical factors such as the effects of seismic retrofit, maintenance, and environmental stressors on lifetime seismic reliability assessment of deteriorating structures. Thus, in this study, a systemic lifetime seismic reliability analysis methodology is proposed and a program HPYER-DRAIN2DX-DS is developed to perform the desired lifetime seismic reliability analysis. To demonstrate the applicability of the program, it is applied to an example bridge with or without seismic retrofit and maintenance strategies. From the numerical investigation, it may be positively stated that HYPER-DRAIN2DX-DS can be utilized as a useful numerical tool for LCC-effective optimum seismic design, maintenance, and retrofitting of bridges.

Climate change is significantly affecting coastal areas, and its impacts are expected to intensify. Recent studies on climate change adaptation and risk assessment in coastal regions increasingly integrate the concepts of recovery resilience and vulnerability. The aim of this study is to develop a measurement model for coastal hazard recovery resilience in the context of climate change adaptation. Before constructing the measurement model, a comprehensive literature review was conducted on coastal hazard recovery resilience, establishing a conceptual framework that included operational definitions for vulnerability and recovery resilience, along with several feedback mechanisms. The measurement model for coastal hazard recovery resilience comprised four metrics (MRV, LRV, RTSPV, and ND) and a Coastal Resilience Index (CRI). The developed indices were applied to domestic coastal erosion cases, and regional analyses were performed based on the index grades. The results revealed that the four recovery resilience metrics provided insights into the diverse characteristics of coastal erosion recovery resilience at each location. Mapping the composite indices of coastal resilience indicated that the areas along the East Sea exhibited relatively lower coastal erosion recovery resilience than the West and South Sea regions. The developed recovery resilience measurement model can serve as a tool for discussions on post-adaptation strategies and is applicable for determining policy priorities among different vulnerable regional groups.

The goal of international development projects is to assist sustainable development in recipient countries through foreign aid from donor countries. However, despite the need for both countries to negotiate and work together from the initial stages to maintain sustainability after the project, clear guidelines or standards have not been established. Additionally, despite the need for donor countries, which are relatively advanced, to understand the situation of recipient countries, many projects are donor-centric and fail to prioritize the value of sustainability. Therefore, this study extracted economic, social, and environmental risks that threaten sustainability through literature review and proposed a sustainability framework based on these criteria. To validate framework, actual international development cases conducted by advanced donor countries such as Australia, the United States, and Japan, in collaboration with South Korea, were analyzed by applying content analysis with the reports, which covers the overall contents from the planning stage to the operation stage. Analysis of sustainability perspectives focused on economy, society and the environment, advanced donor countries emphasized (1) the importance of pre-assessment, (2) the need for coordination with the local population and communities despite the existence of donor-specific values, and (3) addressing economic considerations such as pre-operational and maintenance costs, social communication with the local population, and environmental considerations starting from the initial stages of construction regarding the treatment of pollutants as values to be improved. Compared to other advanced donor countries, the Republic of Korea should also focus on consultation with local residents to achieve social integration, and improve sustainability by deployment the managers in local sites for better negotiation. The proposed framework in this study will serve as a tool to enhance communication among the countries and the locals, with the expectation of increasing project efficiency and sustainability.
