A Case Report on Multiple Rib Fracture Improved with Daewhangchija-paste Adhesive treatment and Herb-medicine treatment. (대황치자고 첩부법과 한약 치료를 병용한 다발성 늑골 골절 환자 증례보고 1례)

  • Objectives: The purpose of this study is to report the improvement of multiple rib fracture after korean medical treatment; adhesive treatment and herb-medicine treatment. Methods: A patient with multiple rib fracture was treated with Daewhangchija-paste(大黃梔子膏) adhesive and herb-medicine treatment. Verbal numeric rating scale, medical examination by interview and rib series x-ray were used to assess progress of treatment. And we took pictures of left flank after attaching Daewhangchija-paste to observe the changes of the skin colors. Results: Rib series x-ray taken after 2 months of treatment revealed hard callus which added stability against external force on rib cage. After taking off Daewhangchija-paste, left plank skin turned into blue, green and yellow. And the pain level(VNRS) of left plank decreased from 10 to 0.5 for 4 months. Conclusions: Pain reduction on trauma site and improvement of general health condition were observed during combination treatment of Daewhangchija-paste adhesive and herb-medicine.

EFFECTS OF RIB ARRANGEMENTS AND ROTATION ON HEAT TRANSFER IN A ROTATING TWO-PASS DUCT (회전덕트에서 요철 배열 및 회전수 변화에 따른 열전달 특성)

  • The present study investigates heat/mass transfer characteristics in a rotating two-pass duct for smooth and ribbed surfaces. The duct has an aspect ratio of 0.5 and a hydraulic diameter of 26.67 mm. 70-angled rib turbulators are attached on the leading and trailing sides of the duct in parallel and cross arrangements. The pitch-to-rib height ratio is 7.5 and the rib height-to-hydraulic diameter ratio is 0.075. The Reynolds number based on the hydraulic diameter is constant at 10,000 and the rotation number ranges from 0.0 to 0.2 Detailed local heat/mass transfer coefficients are measured using a naphthalene sublimation technique. The results show that the secondary flows generated by the $180^{\circ}-turn$, rib turbulators, and duct rotation affect the wall heat/mass transfer distribution significantly, As the duct rotates, the rotaion-induced Coriolis force deflects the main flow and results in differences on the heat/mass transfer distribution between the leading and trailing surfaces. Its effects become more dominant as the rotaion number increases. Discussions are presented describing how the rib configuration and the rotaion speed affect the flow patterns and local heat/mass transfer in the duct.

Heat Transfer Characteristics in a Leading Edge Cooling Channel of a Turbine Blade with Various Rib Arrangements (터빈 기익 선단부에 설치된 냉각유로에서의 요철 배열에 따른 열전달 특성)

  • The present study investigates the heat transfer characteristics of a triangular channel. Three different rib configurations are tested. The ribs are installed on two sides of the channel. The rib height (e) to channel hydraulic diameter is 0.079 and the rib-to-rib pitch (p) is 8 times of the rib height. The rotation number ranges from 0.0 to 0.1 while the Reynolds number is fixed at 10,000. The copper blocks with heaters are installed on the channel walls to measure the regionally averaged heat transfer coefficients. For the stationary $45^{\circ}$ and $135^{\circ}$ ribbed channels, a pair of counter rotating vortices is induced by the angled rib arrangements, and high heat transfer coefficients are obtained on the regions near the inner wall for the $45^{\circ}$ ribbed channel and near the leading edge for the $90^{\circ}$ ribbed channel. The heat transfer coefficients of angled ribbed channels are changed little with rotation, whereas those of the transverse ribbed channel are changed significantly with rotation.

  • Twelfth rib syndrome is thought to be due to intercostal nerve irritation by a mobile twelfth rib, and presents with upper abdominal pain, or low thoracic pain. This syndrome appears to be a fairly common entity and diagnosis is based on clinical findings. Patients with twelfth rib syndrome can be misdiagnosed when it has been overlooked. We report a case of a 34-year-old male along with a presentation of twelfth rib syndrome. One patient was transferred from urologic clinic to pain clinic due to right flank pain and admitted. The patient had direct tenderness on twelfth rib area and direct tenderness was reproducible. Pain increased when the patient flexed laterally, rotated trunk. There were no specific abnormal findings in laboratory test, electrocardiogram, and radiologic examination. After intercostal nerve block and epidural block, pain decreased and the patient was discharged.

  • A primary giant cell tumor of the rib is very rare. The most common site of a giant cell tumor arising from the rib is the posterior arc. A giant cell tumor arising from the anterior arc of the rib is extremely rare. The treatment of a giant cell tumor of the rib is not well defined. Generally, a complete surgical resection is performed in a patient with a primary giant cell tumor of the rib. We report a case of a giant cell tumor arising from the anterior arc of the rib that was treated with a wide excision and chest wall reconstruction.

Design Optimization of Three-Dimensional Channel Roughened by Oblique Ribs Using Response Surface Method (반응면 기법을 이용한 경사진 리브가 부착된 삼차원 열전달유로의 최적설계)

  • A numerical optimization has been carried out to determine the shape of the three-dimensional channel with oblique ribs attached on both walls to enhance turbulent heat transfer. The response surface based optimization is used as an optimization technique with Reynolds-averaged Navier-Stokes analysis of fluid flow and heat transfer. Shear stress transport (SST) turbulence model is used as a turbulence closure. Numerical results fur heat transfer rate show good agreements with experimental data. four dimensionless variables such as, rib pitch-to-rib height ratio, rib height-to-channel height ratio, streamwise rib distance on opposite wall to rib pitch ratio, and the attack angle of the rib are chosen as design variables. The objective function is defined as a linear combination of heat-transfer and friction-loss related coefficients with a weighting factor. D-optimal method is used to determine the training points as a means of design of experiment. Sensitivity of the objective parameters to each design variable has been analyzed. And, optimal values of the design variables have been obtained in a range of the weighting factor.

Heat Transfer Enhancement in a Divergent Passage with 30° Inclined Ribs (30° 경사 리브가 있는 확대 채널 통로 내의 열전달 증가)

  • The effect of different rib geometries such as V-shaped continuous (case A), parallel broken (case B), and V-shaped broken (case C) ribs on local heat transfer distributions and pressure drops in a divergent channel with $30^{\circ}$ inclined ribs on one wall or two walls was investigated for Reynolds numbers from 22,000 to 75,000. Top and bottom walls were insulated; two side walls were uniformly heated in the divergent channel. Heated walls were composed of 10 isolated coper sections and length-to-outlet hydraulic diameter ratio of 10. Rib height-to-outlet hydraulic diameter ratio was 0.1, and rib pitch-to-height ratio equaled 10. Results revealed that V-shaped continuous rib (case A) produced approximately 1.4 times higher average Nussselt number than in the parallel broken rib (case B), and V-shaped broken rib (case C) in the channel with two ribbed walls at Re = 54,000.

Effects of Rib Cage Joint Mobilization Combined with Diaphragmatic Breathing Exercise on the Pulmonary Function and Chest Circumference in Patients with Stroke

  • Background: Patients with stroke have core muscle weakness and limited rib cage movement, resulting in restrictive lung disease. Objectives: To examine the comparison of effects of rib cage joint mobilization combined with diaphragmatic breathing exercise and diaphragmatic breathing exercise on the pulmonary function and chest circumference in patients with stroke. Design: A cluster randomized controlled trial. Methods: Twenty-four patients were randomly assigned to an experimental group (rib cage joint mobilization combined with diaphragmatic breathing exercise group) and control group (diaphragmatic breathing exercise group). Patients in the experimental group underwent rib cage joint mobilization for 15 min and diaphragmatic breathing exercise for 15 min. The control group underwent diaphragmatic breathing exercise for 30 min. Both groups underwent exercise thrice a week for 4 weeks. The pulmonary function and chest circumference were measured using the MicroLab spirometer and a tape measure, respectively. Results: After the intervention, the pulmonary function and chest circumference significantly improved in both groups. These improvements were significantly higher in the experimental group than those in the control group. Conclusion: Rib cage joint mobilization combined with diaphragmatic breathing exercise improves pulmonary function and chest circumference in patients with stroke.

Analytical Study on the Characteristic of Fatigue Behavior in Connection Parts of Orthotropic Steel Decks with Retrofitted Structural Details in Longitudinal Rib (세로리브 내부 보강상세에 따른 강바닥판 연결부의 피로거동 특성에 관한 해석적 연구)

  • In steel deck bridges suffering directly on wheel load according to the number of serviced years, the occurrence of fatigue cracks increases in structural details, which includes the cross section parts of the longitudinal rib and transversal rib, and so on. Through the control method for these fatigue cracks the increased thickness of the steel deck plate or the application of retrofit detail to the inside of the longitudinal rib was observed to be effective. This study suggests structural details for the retrofitted and non-retrofitted longitudinal rib. The target details in this study are the connection parts of the lo ngitudinal and transversal rib, and the slit parts of transverse rib where fatigue cracks were frequently reported in previous studies. In the analyses, detailed structural analyses were performed as parameters, which include the shape, change of size and attached position. From the results the stress reduction in the target details was observed to be larger in the retroffited details. Also, the improvement of fatigue strength is more effective in the retrofitted details with the vertical rib than the bulkhead plate.
