  • This paper presented on an environment estimation of cutting fluid which is a mouse model of acute bacterial rhinosinusitis using cutting fluid in grinding and cutting. Above results will be remarked the necessity of friendly environmental cutting skill when it had used in workshop. The results of a mouse experimental using cutting fluid showed that it are occurred to the respiratory organs sickness. Also cutting fluid are occurred to hurtfulness a person when it used to grinding and cutting in workshop. Also, the results of grinding experiments in avitation materials showed that surface roughness are superior to more emulsion type oil than vegetable type oil according to increase of the depth of cut. Grinding force are similar to both emulsion type oil and vegetable type oil.

Nomogram plot for predicting chronic otitis media in Korean adults

  • Nomogram is useful for predicting the prevalence of each patient through the scoring system without a complex formula. Because there are few studies on chronic otitis media (COM) in adults, this study aims to identify the relevant risk factors for COM in Korean adults and to build a nomogram for the risk factors. The Health Interview Survey data subset, derived from the Sixth Korean National Health and Nutrition Examination Survey (KNHANES VI), was used to evaluate the participants. Of the participants, the weighted prevalence of COM was 5.3%. Residence, earphone use, atopic dermatitis, allergic rhinitis, chronic rhinosinusitis, and subjective hearing status were identified as risk factors for COM. Using 6 risk factors, we propose a nomogram for COM, and use AUC to verify the discrimination of the nomogram.

  • Asthma is a chronic inflammatory disease of the airways characterized by varying and recurrent symptoms, reversible airway obstruction, and bronchospasm. In this paper, clinical important studies on asthma published between March 2021 and February 2022 were reviewed. A study on the relationship between asthma and chronic rhinosinusitis, bronchiectasis, and hormone replacement therapy was published. A journal on the usefulness of fractional exhaled nitric oxide for the prediction of severe acute exacerbation was also introduced. Studies on the effect of inhaler, one of the most important treatments for asthma, were published. Studies on the control of severe asthma continued. Phase 2 and 3 studies of new biologics were also published. As the coronavirus disease 2019 (COVID-19) pandemic has been prolonged, many studies have explored the prevalence and mortality of COVID-19 infection in asthma patients.

  • Siewert-Kartagener's syndrome, a type of primary ciliary dyskinesia, is a complex disease comprising situs inversus, rhinosinusitis, and bronchiectasis. Situs inversus totalis is a condition in which all organs in the thoracic and abdominal cavities are reversed. Furthermore, primary ciliary dyskinesia, an autosomal genetic disease, may coexist with situs inversus totalis. Reports on Siewert-Kartagener's syndrome in veterinary medicine are limited. We report a rare case of primary ciliary dyskinesia with Siewert-Kartagener's syndrome in a dog, concurrently infected with canine distemper virus and type-2 adenovirus. This case highlights that situs inversus totalis can cause primary ciliary dyskinesia, and concurrent infections are possible.

Korean Red Ginseng aqueous extract improves markers of mucociliary clearance by stimulating chloride secretion

  • Background: Abnormal chloride (Cl-) transport has a detrimental impact on mucociliary clearance in both cystic fibrosis (CF) and non-CF chronic rhinosinusitis. Ginseng is a medicinal plant noted to have anti-inflammatory and antimicrobial properties. The present study aims to assess the capability of red ginseng aqueous extract (RGAE) to promote transepithelial Cl- secretion in nasal epithelium. Methods: Primary murine nasal septal epithelial (MNSE) [wild-type (WT) and transgenic CFTR-/-], fisher-rat-thyroid (FRT) cells expressing human WT CFTR, and TMEM16A-expressing human embryonic kidney cultures were utilized for the present experiments. Ciliary beat frequency (CBF) and airway surface liquid (ASL) depth measurements were performed using micro-optical coherence tomography (μOCT). Mechanisms underlying transepithelial Cl- transport were determined using pharmacologic manipulation in Ussing chambers and whole-cell patch clamp analysis. Results: RGAE (at 30㎍/mL of ginsenosides) significantly increased Cl- transport [measured as change in short-circuit current (ΔISC = ㎂/㎠)] when compared with control in WT and CFTR-/- MNSE (WT vs control = 49.8±2.6 vs 0.1+/-0.2, CFTR-/- = 33.5±1.5 vs 0.2±0.3, p < 0.0001). In FRT cells, the CFTR-mediated ΔISC attributed to RGAE was small (6.8 ± 2.5 vs control, 0.03 ± 0.01, p < 0.05). In patch clamp, TMEM16A-mediated currents were markedly improved with co-administration of RGAE and uridine 5-triphosphate (8406.3 +/- 807.7 pA) over uridine 5-triphosphate (3524.1 +/- 292.4 pA) or RGAE alone (465.2 +/- 90.7 pA) (p < 0.0001). ASL and CBF were significantly greater with RGAE (6.2+/-0.3 ㎛ vs control, 3.9+/-0.09 ㎛; 10.4+/-0.3 Hz vs control, 7.3 ± 0.2 Hz; p < 0.0001) in MNSE. Conclusion: RGAE augments ASL depth and CBF by stimulating Cl- secretion through CaCC, which suggests therapeutic potential in both CF and non-CF chronic rhinosinusitis.

Subepithelial neutrophil infiltration as a predictor of the surgical outcome of chronic rhinosinusitis with nasal polyps

  • Background: Neutrophils present as major inflammatory cells in refractory chronic rhinosinusitis with nasal polyps (CRSwNP), regardless of the endotype. However, their role in the pathophysiology of CRSwNP remains poorly understood. We investigated factors predicting the surgical outcomes of CRSwNP patients with focus on neutrophilic localization. Methods: We employed machine-learning methods such as the decision tree and random forest models to predict the surgical outcomes of CRSwNP. Immunofluorescence analysis was conducted to detect human neutrophil elastase (HNE), Bcl-2, and Ki-67 in NP tissues. We counted the immunofluorescence-positive cells and divided them into three groups based on the infiltrated area, namely, epithelial, subepithelial, and perivascular groups. Results: On machine learning, the decision tree algorithm demonstrated that the number of subepithelial HNE-positive cells, Lund-Mackay (LM) scores, and endotype (eosinophilic or non-eosinophilic) were the most important predictors of surgical outcomes in CRSwNP patients. Additionally, the random forest algorithm showed that, after ranking the mean decrease in the Gini index or the accuracy of each factor, the top three ranking factors associated with surgical outcomes were the LM score, age, and number of subepithelial HNE-positive cells. In terms of cellular proliferation, immunofluorescence analysis revealed that Ki-67/HNE-double positive and Bcl-2/HNE-double positive cells were significantly increased in the subepithelial area in refractory CRSwNP. Conclusion: Our machine-learning approach and immunofluorescence analysis demonstrated that subepithelial neutrophils in NP tissues had a high expression of Ki-67 and could serve as a cellular biomarker for predicting surgical outcomes in CRSwNP patients.

Chronic osteitic rhinosinusitis as a manifestation of cystic fibrosis: A case report

  • A 28-year-old male patient with a history of cystic fibrosis (CF) was referred to the University of Connecticut School of Dental Medicine for an evaluation of a cystic lesion in the right maxilla using cone-beam computed tomography (CBCT). CF is an autosomal recessive disease characterized by an abnormal production of viscous mucus, affecting the mucociliary clearance. The CBCT scan revealed a large cystic lesion in the right maxilla extending from the right maxillary second molar to the midline in the region of the right central incisor with a significant buccal expansion. Further evaluation revealed complete opacification of the paranasal sinuses with medial bulging of the lateral maxillary sinus walls. The maxillary and sphenoid sinuses also appeared hypoplastic. The peculiar finding seen in this case was the presence of marked sclerosis and an increase in the thickness of the adjacent bony framework. This report aimed to describe the common sinonasal findings associated with CF and its underlying pathophysiology.

비폐색에 대한 심의혈(心醫穴)의 임상 효과 (Effect of Sim-eui Point on Allergic Rhinitis, Rhinosinusitis, and Other Causes of Nasal Obstruction)

  • 목적 : 이 연구는 알레르기 비염 또는 비부비동염으로 인한 비폐색증에 대한 침치료의 효과를 입증하고, 신혈인 심의혈의 임상적 효과를 알아보고자 시도하였다. 방법 : 2009년 9월 22일부터 11월 3일까지 2회 이상 경희의료원 동서비염클리닉에 내원하여 침 치료를 받은 비염환자를 대상으로, 무작위배정방법에 따라 실험군 또는 대조군으로 배정하였다. 실험군은 routine 혈위인 신회 상성 신정 양두유 양찬죽 양지창에 심의혈을 추가하였고, 대조군은 routine 혈위에 양영향을 추가하였다. 유침 시간은 각 군에서 15분간으로 하였다. 유효성 평가 도구로는 SNOT-20를 사용하여 비염의 전반적 증상 점수 변화를 측정하였고, VAS를 사용 하여 비폐색 완화 효과의 정도를 측정하였다. 평가시기는 치료 전후로 하였고, 통계분석은 치료 전후 SNOT-20 차이의 경우 Wilcoxon Signed Ranks Test로, VAS 비교는 Mann-Whitney Test를 이용하여 검정하였다.

비부비동염의 한의학적 문헌고찰 (A Literature Study of Korean Medicine for Sinusitis)

  • Objectives : The objective of this study is to analyze the research of Korean medicine for the treatment of sinusitis. Methods : We searched domestic journals for this analysis with the title 'Sinusitis, Rhinosinusitis, Biyeon(鼻淵)'. We searched 'KISS, OASIS, Korean traditional knowledge portal'. Results : Total numbers of searched articles are 103. We selected 14 papers; 5 systematic reviews, 9 clinical trials. Systematic reviews deal with cause, treatment, herbal medicine and acu-points of sinusitis. Clinical trials deal with treatment cases of Korean medicine which are failed to treatment of western medicine which is antibiotics and surgery. Conclusions : Positive effect of Korean medicine on sinusitis is reported but cases are less and research cases are limited. We conclude that well-designed clinical studies are needed.

Novel approach to the maxillary sinusitis after sinus graft

  • Background: Postoperative infection occurs when bone graft material is dislodged into the maxillary sinus cavity and most of the patients are often uncomfortable with the drainage and irrigation procedures to eradicate the infection. In this case report, we share a technique in treating patients with such condition. Material and methods: A 47-year-old patient was referred after sinus elevation using the crestal socket osteotome, bone graft, and implant insertion at a local clinic. Clinical and radiographic findings confirmed the diagnosis of right maxillary sinusitis. A surgical and medical treatment regimen was applied. Results: By using this technique for irrigation, we were able to achieve successful results, and the patient was satisfied due to less discomfort during the irrigation process. Conclusion: This method is a patient-friendly technique for sinus irrigation. It is not only limited to sinus grafted patients, but also maxillary sinusitis patients in any other type of odontogenic infection.
