Characteristics of Grip Strength in People with Osteoarthritis and Rheumatoid Arthritis: Data From the 8th Korean National Health and Nutrition Examination Survey (퇴행성 관절염과 류마티스 관절염 유병자의 악력 특성 -제 8기 국민건강영양조사 자료 분석-)

  • Purpose: The purpose of this study was to investigate the characteristics and grip strength of people with osteoarthritis and rheumatoid arthritis using data from the Korea National Health and Nutrition Examination Survey. Methods: This was a retrospective study that analyzed raw data from the first year of the 8th Korea National Health and Nutrition Examination Survey (2019). The study population was 780 people in total, ranging in age from their teens to their 80s. These were people who had been diagnosed with osteoarthritis and rheumatoid arthritis, and the presence of arthritis in and grip strength of these subjects were determined using the average value of three measurements. Results: According to the National Health and Nutrition Examination Survey, the prevalence of arthritis in Korea was 2.5% in men, 10.7% in women, and 13.1% overall. More women than men had osteoarthritis and rheumatoid arthritis, and the number of people with osteoarthritis increased with age. In this study, of those with osteoarthritis, 13.5% were men and 88% were women of those with rheumatoid arthritis, 19.3% were men and 56.3% were women. The number of patients with osteoarthritis increased with age, and rheumatoid arthritis was more common in older people. Subjects with osteoarthritis had lower grip strength than those without the disease, and the older the age at which rheumatoid arthritis was first diagnosed, the lower the grip strength. Conclusion: Grip strength is lower in patients with osteoarthritis than in those without osteoarthritis, and it is possible to estimate the degree to which muscle strength decreases.

  • Objective : This case report aims to show the effect of Bekhogainsam-tang on Overactive Bladder and Rheumatoid arthritis. The patient was diagnosed with Yangmyung-byung based on Shanghanlun provisions. Methods : According to Diagnostic system based on Shanghanlun provisions, the patient was diagnosed with Yangmyung-byng and has been unusual dietary habits. So she treated with Bekhogainsam-tang decoction for 75 days. We evaluated the results of treatment by Overactive Bladder Symptom Score(OABSS), Modified Overactive Bladder Validated 8-question Screener(OAB-V8), and Rheumatoid Arthritis Pain Score(RAPS). Results : After taking Bekhogainsam-tang decoction for 75 days, the patient's symptoms and OABSS was decreased from 20 to 11, OAB-V8 was from 42 to 20, and RAPS was from 130 to 63. Conclusions : This case report showed an effectiveness of using Bekhogainsam-tang according to Diagnositic system based on Shanghanlun provisions on Overactive Bladder and Rheumatoid Arthritis.

The Effect of Tai Chi Exercise on the Depression and Self Efficacy of the Patients with Rheumatoid Arthritis (타이치 운동이 류마티스 관절염 환자의 우울 및 자기효능감에 미치는 효과)

  • Purpose: This study was attempted to verify the effects of Tai Chi exercise on the depression and self efficacy of the patients with rheumatoid arthritis. Method: It employed a non-equivalent control group pre- and post-test design. The research instruments used in this study were depression and self efficacy. Thirty-two patients in the experimental group carried out 50 minutes of Tai Chi exercise for 12 weeks, and 29 patients in the control group did not. Before and after the experiment, both groups were tested for depression and self efficacy. Collected data were processed using the SPSS/WIN 10.0 program analyzing by the frequency, percentage, $X^2$- test, and t-test. Results: Depression significantly decreased in the experimental group. However the improvement in self efficacy of the rheumatoid arthritis patients was not statistically significant but slight enhancement was. Conclusion: Tai Chi exercise is an effective nursing intervention that can be used for rheumatoid arthritis patients.

  • Objective : This study shows the clinical effects on Bee-venom acupuncture and Burning acupuncture therapy for a patient who was diagnosed as Rheumatoid arthritis with ankle pain. Methods : This study was carried out a patient who was diagnosed as Rheumatoid arthritis with ankle pain. The patient was treated by Bee-venom acupuncture, Burning acupuncture therapy. To evaluate the effect of treatment, measured grade of pain by Visual Analogue Scale(VAS). Result and Conclusion: The ankle pain was reduced from VAS 10 to VAS 2 and the patient could walk on foot. In this study, Bee-venom acupuncture, Burning acupuncture therapy was effective in arthralgia of Rheumatoid arthritis.

  • Objective: Improvement of symptoms in patients with rheumatoid arthritis using OCNT Methods: The patient was a Korean woman in her 50s who was experiencing foot pain due to rheumatoid arthritis and was experiencing discomfort in her daily life. OCNT was applied to the patient for approximately 6 months. Results: After starting OCNT, the patient's joint pain and chest pain gradually decreased, and after about six months, she no longer felt discomfort due to these symptoms. Conclusion: For rheumatoid arthritis patients suffering from joint pain and chest pain, OCNT can help relieve symptoms.

  • Purpose: The purpose of this study was to investigate depression and activity of daily living of patients with rheumatoid arthritis by their gender. Methods: The subjects for this study were 184 patients registered in S University Hospital. and the period of data collection was from May 1, 2012 to August 20, 2012. The data were analyzed with t-test, ANOVA, Mann-Whitney U test, Kruskal-Wallis H test, and Pearson's correlation. Results: For depression, there were significant differences by marriage and religion in male patients and by education in female patients. For activity of daily living, there were significant differences by marriage, age and education in female patients. Depression was negatively correlated with activity of daily living in patients with rheumatoid arthritis. Conclusion: Managing depression and activity of daily living were important for both male and female patients with rheumatoid arthritis. To promote the quality of life of patients with rheumatoid arthritis, nursing intervention strategies must be developed for decreasing depression or improving their activity of daily living.

A Case of Live Leech(Hirudo Medicinalis) Therapy with Herbal Medication on Regional Rheumatoid Arthritis (한약투여 및 의료용 거머리 요법을 병행 시술 후 관절 경직 및 부종이 호전된 국소 류마티즘 환자 증례 1례)

  • Objectives : The purpose of this study is to report the effect of live leech(hirudo medicinalis) therapy with oriental medication on the patient with regional rheumatoid arthritis. The patient had resistant to DMARDs so she was in the debilitating condition and suffered from edema, pain, stiffness of joint and purpura before live leech(hirudo medicinalis) therapy. Methods : The regional rheumatoid arthritis patient had edema, pain, stiffness of joint and purpura on right foot. We treated the patient with live leech(hirudo medicinalis) therapy and oriental medicine. Results : The symptoms of regional rheumatoid arthritis was improved through live leech(hirudo medicinalis) therapy and oriental medicine. The patient maintains her condition with oriental medicine treatment after stopped all DMARDs such as MTX(methotrexate) and NSAIDs. After this treatment the patient's walking difficulty has been improved. Conclusions : This study shows that live leech(hirudo medicinalis) therapy and oriental medicine can elevate the regional rheumatoid arthritis patient's quality of life with continuous health care and treatment for major problem. For more accurate studies, further studies would be needed with more cases.

  • For the study of the temporomandibular joint in rheumatoid arthritis 30 patients were selected who were diagnosed as rheumatoid arthritis through the clinical, radiographic examination and laboratory findings. Temporomandibular joint involvement was evaluated through the clinical, radiographic examination. The results were as follows; 1. TMJ was involved in 15 patients of 30 patients with rheumatoid arthritis. (50% involvement). 2. Duration of rheumatoid arthritis was more longer in patients with TMJ involvement than in patients without TMJ involvement. 3. Osseous changes in TMJ were in order of frequency erosion, flattening, osteophyte, sclerosis, deformity, and most common involved site was mandibular condyle. 4. Most common positional change of condyle was forward position in centric occlusion, and restricted movement of condyle in 1inch mouth opening. 5. TMJ involvement of rheumatoid arthritis was almost bilateral. 6. Main symptoms of TMJ were pain, stiffness, tenderness, limitation of mouth opening, crepitation 7. There was not the case of ankylosis. 8. There was statistically insignificant correlation between mandibular deviation and TMJ involvement, but some cases showed severe deviation on mouth opening.

An Inquiry to the Causal Perceptions & Emotions of Rheumatoid Arthritis Patients (류마티스 관절염 환자의 원인지각에 대한 연구 - Q방법론적 접근 -)

  • This study was undertaken to find out the causal perception of rheumatoid arthritis patients, and to understand the typology. The Q-population consisted of 236 statements of causal perception were collected. Thirty eight Q-samples of causal perception were selected. The P-sample for this study were made up of 28 first visiting female rheumatoid arthritis patients from a rheumatoid arthritis specialty hospital. Each respondent responded Q-set of causal perception according to 9-point scale. The result of Q-sorting were coded and analyzed using QUANL PC program. 1) Typological Observation on Causal Perception (1) Physical Fatigue Type : Type 1 perceived that the illness occurred due to excessive work requiring physical labor or strain that had occurred from not resting after excessive physical labor, therefore, thinking the origin of the illness was from physical strain. (2) Physical origin Type : Type 2 perceived that the major cause for the illness is not only excessive physical labour but also fecundity and old age. (3) Causality to Environment Type : Type 3 perceived that rheumatoid arthritis occurred from injury to the joints or bad and humid weather. (4) Conscience of Guilty Type : Type 4 consisted of people with guilty conscience for lack of religious commitment. They perceived that the illness was a punishment from God for not praying or because of bad luck. (5) Rationally Perceiving Type : People who belong in type 5 perceived the cause of illness in light of scientific facts such as genetics, unbalanced diet or lack of exercise. (6) Psychological Stress Type : People who belong in type 6 believed that excessive stress was the cause of the illness. 2) Emotions of Rheumatoid arthritis patients Rheumatoid arthritis patients' positive emotions included determination, courage, coping, acceptance, hope, and adoption ; and their negative emotions were prostration, worry, stupor, conflicts, grievance, giving-up, resignation, depression, loss, solitariness, fear, anxiety, avoidance, anger and loneliness. Rheumatoid arthritis patients experience different level of emotions from their suffering experience from the severe pains. Rheumatoid arthritis patients also experience negative emotions when they could not perform self-care and lose their self-esteem from painful suffering ; however, they regain positive emotions when they recover from pain with the use of drugs, physical therapy or exercise. Their emotional states are closely connected to level of and presence of pain.

Anti-inflammatory Effect of Hong Hua Za Pharmacupuncture on Rheumatoid Arthritis that Caused by Lipopolysaccharide in Mouse (홍화자약침(紅花子藥鍼)의 항염증능이 백서의 LPS로 유발된 류마티스성 관절염에 미치는 영향)

  • Objectives : This study is to evaluate Effect of Hong Hua Za acupuncture anti-inflammatory on rheumatoid arthritis that caused by Lipopolysaccharide in mouse. Methods : Hong Hua Za(Carthami Tinctorii Fructus) acupuncture(HA) was prepared and tested for therapeutic potential of rheumatoid arthritis, by measuring the inhibition of inducible nitric oxide synthase (iNOS) expression, nitric oxide(NO) production in vitro experiment, by measuring Tumpr Necrosis Factor-$\alpha$(TNF-$\alpha$) & Nuclear Factor kappa B(NF-$\kappa$B) p65 activation, synovial hyperplasia, angiogenesis and fibrosis in synovial membrane of knee joint of mice. Results : HA showed therapeutic effects on Lipopolysaccharide(LPS)-induced rheumatoid arthritis. HA inhibited iNOS mRNA and NO in RAW264.7cell of LPS-induced rheumatoid arthritis in a dose dependent manner. HA also showed significant inhibition of TNF-$\alpha$ & NF-$\kappa$B p65, activation, synovial hyperplasia, angiogenesis and fibrosis in synovial membrane of knee joint of mice. Conclusions : These results suggest that HA has an therapeutic effects on LPS induced-rheumatoid arthritis by inhibiting TNF-$\alpha$ activation.
