A review of analysis methods for secondary outcomes in case-control studies

  • The main goal of a case-control study is to learn the association between various risk factors and a primary outcome (e.g., disease status). Particularly recently, it is also quite common to perform secondary analyses of the case-control data in order to understand certain associations between the risk factors of the primary outcome. It has been repeatedly documented with case-control data, association studies of the risk factors that ignore the case-control sampling scheme can produce highly biased estimates of the population effects. In this article, we review the issues of the naive secondary analyses that do not account for the biased sampling scheme, and also the various methods that have been proposed to account for the case-control ascertainment. We additionally compare the results of many of the discussed methods in an example examining the association of a particular genetic variant with smoking behavior, where the data were obtained from a lung cancer case-control study.

A retrospective study on related factors affecting the survival rate of dental implants

  • PURPOSE. The aim of this retrospective study is to analyze the relationship between local factors and survival rate of dental implant which had been installed and restored in Seoul Veterans Hospital dental center for past 10 years. And when the relationship is found out, it could be helpful to predict the prognosis of dental implants. MATERIALS AND METHODS. A retrospective study of patients receiving root-shaped screw-type dental implants placed from January 2000 to December 2009 was conducted. 6385 implants were placed in 3755 patients. The following data were collected from the dental records and radiographs: patient's age, gender, implant type and surface, length, diameter, location of implant placement, bone quality, prosthesis type. The correlations between these data and survival rate were analyzed. Statistical analysis was performed with the use of Kaplan-Meier analysis, Chi-square test and odds ratio. RESULTS. In all, 6385 implants were placed in 3755 patients (3120 male, 635 female; mean age $65{\pm}10.58$ years). 108 implants failed and the cumulative survival rate was 96.33%. There were significant differences in age, implant type and surface, length, location and prosthesis type (P<.05). No significant differences were found in relation to the following factors: gender, diameter and bone quality (P>.05). CONCLUSION. Related factors such as age, implant type, length, location and prosthesis type had a significant effect on the implant survival.

안면골 골절의 발생 인자에 대한 통계학적 분석 (Statistical Analysis of Factors Associated with Facial Bone Fractures)

  • Purpose: Statistical analysis of facial bone fractures has been performed in various papers. However, reports on risk factors for facial bone fractures are rare. In order to prevent facial bone fractures, it is important to determine the risk factors for their occurrence. This study seeks to perform a statistical analysis on and identify the risk factors associated with facial bone fractures. Methods: A retrospective study was performed to assess facial bone fractures in patients presenting from October 2009 to January 2011 through a chart review. The data collected included age, gender, etiology, and alcohol consumption. Data was analyzed using multinomial logistic regression analysis. The significance level was set at p<0.05 and SAS ver. 9.2 was used. Results: A total of 489 patients were analyzed. The patients' age ranged from 2 to 85 years (mean age, $31.8{\pm}15.4$ years). The ratio of men to women was 5.0:1. The predominant group was age below 19 years old (30.9%). The main causes of facial bone fractures were assaults (37.8%), falls (27.2%), and sport accidents (19.5%). On multinomial logistic regression analysis, age, especially in the teen group was associated with assaults (p<0.05) resulting in facial bone fractures. Alcohol consumption was significantly associated with assaults and falls (p<0.05) leading to facial bone fractures. Conclusion: Facial bone fracture is a challenging problem, because of its high incidence and financial cost. The findings of this study indicate that more effective policies aimed at reducing alcohol intake and teenage violence are needed.

Melanoma in Iran: a Retrospective 10-Year Study

  • Background: Melanoma, the most life-threatening type of skin cancer, is a malignant tumor initiating in melanocytes that rapidly metastasizes and causes death. Materials and Methods: In this retrospective study, samples were selected from patients' information files in our Cancer Institute in Tehran with a designed checklist. A total of 322 files were found from 2003 until 2012. Then the raw data were transferred to Statistical Package for Social Sciences (SPSS) software version 16 and additional analysis was performed by Students t-test. The important variables were considered according to the available information from history of pathology including age, gender, occupation, stage and location of tumor. Results: Our data showed that incidence of melanoma has been different in the studied 10-year period according to age. Also, incidence of melanoma was higher in men than women. It was more common in lower limbs. More commonly housewives among women and farmers among men were affected by melanoma. Conclusions: Taken together the descriptive data clarified general aspects of this disease for further screening and interventions.

A retrospective study of mandibular fractures in children

  • Objectives: The incidence and patterns of mandibular fractures vary by country and population age. This retrospective study evaluated the etiologies and patterns of mandibular fractures in children. Materials and Methods: The clinical records of 89 children (45 males and 44 females) aged 0 to 12 years who presented with mandibular fractures from July 2012 to June 2016 were retrospectively reviewed. The sex, patient age, site of fracture, etiology of trauma, and monthly variations of the fractures were recorded. Descriptive statistics, the z-test and chi-square test were used for statistical analysis and the P-value was set at <0.05. Results: Eighty-nine children (male-to-female ratio 1.02:1) sustained 131 mandibular fractures. Within the study sample, the 6 to 9 year age group most frequently experienced fractures (47.3%). Falls and road traffic accidents (RTA) were the two most common etiological factors that accounted for 44.9% and 24.7% of cases. The condylar fracture was the most common anatomical location (38.9%) followed by the angle (20.6%), parasymphysis (18.3%), body (15.3%), and symphysis (5.3%). A single fracture (51.7%) was more common than multiple fractures (48.3%). The month-wise distribution of mandibular fractures was fairly constant. Conclusion: The condylar region was the most common anatomic site for fractures; in addition, a fall and RTA were the major etiological factors for mandibular fractures. A single fracture was observed in 51.7% of patients while multiple fracture lines accounted for 48.3% of cases.

회전근개 파열 수술 후 환자에 대한 한의학적 치료 효과 : 후향적 관찰연구 (Effects of Korean Medicine on Post-rotator Cuff Surgery Patients : A Retrospective Observational Study)

  • The aim of this study was to investigate the effect of Korean medicine treatments on pain reduction and functional improvement of patients after rotator cuff surgery. We studied 10 patients who had admitted to Dunsan Korean Medical hospital after have being diagnosed as rotator cuff tear and got a surgery at other hospital from April 30, 2014 to April 30, 2019. This study was conducted as retrospective study which analyzes patient's medical records. Statistical analysis was performed using the IBM SPSS statistics 24 program. We used Numeric Rating Scale to evaluate pain reduction, Range of Motion to evaluate shoulder functional improvement and Five-point likert scale to evaluate the patient's satisfaction. The result showed that Korean medicine treatments statistically significantly reduced pain on the patients after rotator cuff surgery. Although there were improvement of Range of Motion, it wasn't statistically significant. In conclusion, we found that the Korean medicine treatments showed a positive effect on pain reduction of patients after rotator cuff surgery and got a high satisfaction.

한방병원에 내원한 척추측만증 환자의 특성에 대한 후향적 연구 (Retrospective Study on the Characteristics of Patients with Scoliosis at the Korean Medicine Hospital)

  • Objectives This study was conducted to characterize scoliosis patients visiting Korean medicine hospital and to analyze the demands and factors affecting discomfort. Methods This retrospective study analyzed 33 scoliosis patients who visited Korean medicine hospital from March, 2021 to October, 2021. The data analysis consisted of three factors: (1) demographic characteristics, (2) characteristics of demands on Korean medicine (reasons for choosing Korean medical treatment, preferred treatment methods, most uncomfortable part, treatment priorities) and (3) discomfort factors (treatment experiences, diagnosed age and Cobb's angle). Statistical analyses were performed and a p-value≤0.05 was considered to be statistically significant. Results 43.75% of the patients chose 'effectiveness' for the reason why they preferred Korean medicine treatment. 'Chuna treatment' was the most preferred treatment method. The patients chose 'lower back' for the most uncomfortable part and 'pain' for the highest priority of improvement. The Cobb's angle of included patients was 16.02±7.65° and it is not much differ to average of Cobb's angle in Korean. Discomfort was more severe in the patients with treatment-experienced than treatment-naive. The score of discomfort in appearance and psychological were higher in the patients diagnosed in childhood or adolescent period than who were diagnosed after adult. Classification based on Cobb's angle showed no statistical difference. Conclusions Not only Cobb's angle but also other clinical factors should be considered for effective treatment in scoliosis. Also, It is necessary to pay attention to adult scoliosis patients.

Comparative analysis of proximal humerus fracture management in elderly patients: complications of open reduction and internal fixation by shoulder surgeons and non-shoulder surgeons-a retrospective study

  • Background: Open reduction and internal fixation (ORIF) with a locking plate is a popular surgical treatment for proximal humeral fractures (PHF). This study aimed to assess the occurrence of complications in elderly patients with PHF treated surgically using ORIF with a locking plate and to investigate the potential differences between patients treated by shoulder surgeons and non-shoulder surgeons. Methods: A retrospective study was conducted using a single-center database to identify patients aged ≥70 years who underwent ORIF for PHF between January 1, 2011, and December 31, 2021. Data on the Neer classification, follow-up, occurrence of avascular necrosis of the humeral head, implant failure, and revision surgery were also collected. Statistical analyses were performed to calculate the overall frequency of complications according to the Neer classification. Results: The rates of implant failure, avascular osteonecrosis, and revision surgery were 15.7%, 4.8%, and 15.7%, respectively. Complications were more common in patients with Neer three- and four-part fractures. Although the difference between surgeries performed by shoulder surgeons and non-shoulder surgeons did not reach statistical significance, the rate of complications and the need for revision surgery were nearly two-fold higher in the latter group. Conclusions: PHF are highly prevalent in the elderly population. However, the ORIF surgical approach, as demonstrated in this study, is associated with a considerable rate of complications. Surgeries performed by non-shoulder surgeons had a higher rate of complications and a more frequent need for revision surgery. Future studies comparing surgical treatments and their respective complication rates are crucial to determine the optimal therapeutic options. Level of evidence: III.

교통사고로 유발된 급성 경항통 환자에 대한 고방 치료와 후세방 치료의 효과 비교: 후향적 연구 (A Comparative Study on the Effect of Herb Medication Based on Go-bang Treatment, Huse-bang Treatment for Patients with Acute Neck Pain Caused by Traffic Accidents: A Retrospective Study)

  • Objectives The purpose of this study was to compare the effect of herb medication based on 'Go-bang' treatment, 'Huse-bang' treatment for patients with acute neck pain caused by traffic accidents. Methods We studied 40 patients with acute neck pain after traffic accidents who had admitted to Department of Korean Medicine Rehabilitation, College of Korean Medicine, Daejeon University from November 25, 2018 to November 25, 2019. 20 patients had received Herb Medication Based on 'Go-bang' and 20 patients had received herb medication based on 'Huse-bang' three times a day. The study was conducted as a retrospective observation study which analyze the patient's medical records. We used visual analogue scale (VAS) to evaluate pain reduction and neck disability index (NDI) to evaluate function improvement twice (hospitalization day and 5 days later). Statistical analysis was performed using the IBM SPSS statistics 25 program. Result The VAS scores, NDI scores decreased statistically significantly after treatment in all patients. Both 'Go-bang' and 'Huse-bang'groups showed a statistically significant VAS reduction and improvement of NDI on the fifth day of hospitalization. In the 'Huse-bang' group, there was a statistically significant improvement of NDI than in the 'Go-bang' group. Conclusion We found that both types of herbal-medication were significantly effective on acute neck pain and neck disability after traffic accident. Further study is needed about the comparison of effectiveness between Go-bang' and 'Huse-bang'groups.

Effect of initial placement level and wall thickness on maintenance of the marginal bone level in implants with a conical implant-abutment interface: a 5-year retrospective study

  • Purpose: Implant wall thickness and the height of the implant-abutment interface are known as factors that affect the distribution of stress on the marginal bone around the implant. The goal of this study was to evaluate the long-term effects of supracrestal implant placement and implant wall thickness on maintenance of the marginal bone level. Methods: In this retrospective study, 101 patients with a single implant were divided into the following 4 groups according to the thickness of the implant wall and the initial implant placement level immediately after surgery: 0.75 mm wall thickness, epicrestal position; 0.95 mm wall thickness, epicrestal position; 0.75 mm wall thickness, supracrestal position; 0.95 mm wall thickness, supracrestal position. The marginal bone level change was assessed 1 day after implant placement, immediately after functional loading, and 1 to 5 years after prosthesis delivery. To compare the marginal bone level change, repeated-measures analysis of variance was used to evaluate the statistical significance of differences within groups and between groups over time. Pearson correlation coefficients were also calculated to analyze the correlation between implant placement level and bone loss. Results: Statistically significant differences in bone loss among the 4 groups (P<0.01) and within each group over time (P<0.01) were observed. There was no significant difference between the groups with a wall thickness of 0.75 mm and 0.95 mm. In a multiple comparison, the groups with a supracrestal placement level showed greater bone loss than the epicrestal placement groups. In addition, a significant correlation between implant placement level and marginal bone loss was observed. Conclusions: The degree of bone resorption was significantly higher for implants with a supracrestal placement compared to those with an epicrestal placement.
