Maxillary removable partial overdenture within Locator Root Attachment in a retained root: a case report (소수 잔존 치근에 Locator Root Attachment를 활용한 상악 가철성 국소의치 증례)

  • If maintaining oral few retained root when planning the removable prosthesis, it is possible to obtain a more comfortable and functional result. In this case report, 'Locator Root Attachment' was used to maxillary removable overdenture in patients with a few teeth and retained root. A functionally proper clinical result from attachment after clinical and radiographic analysis was observed in this case.

  • Statment of problem : Little in known about the importance of selection of various double crowns as retainer in determining the outcome of treatment with double crown retained removable partial denture. Purpose : To obtain information about the effects and the results of this treatment modality. Material and methods : This study describes 61 double crown retained removable partial dentures worn by 51 patients from Samsung Medical Center, Seoul a time ranging between 6 and 48 months and evaluate occlusal contacts on the denture teeth, denture movement, incidence of denture relining, denture retention, fracture of dentures and abutments, hygiene, residual ridge inflammatory changes, number of lost abutment,. interruption of denture use. Results : One tenth of all the restorations were relined. Restorations in 21 arches fractured repeatedly for various reasons . There was no apparent interrelationship between fractures and the five groups. Conclusion : Good prognoses of removable partial dentures were shown in all groups(Kennedy Classes I, II, III, combination and few remaining abutment).

  • A review of the literature was presented, illustrating the effect that gingivally retained orthodontic elastic bands have on the periodontal tissues. A case report was presented for treatment of mobile maxillary central incisors secondary to a subgingival elastic band. Following surgical removal of elastic band and periodontal debridement, the teeth were successfully retained with active orthodontic appliances. Until a harmless radiopaque medium can be safely incorporated into elastic bands, extreme caution in their use should be observed. Not only should all attempts be made to anchor the elastic bands to the clinical crown, but the patient should receive through instructions in their placement and removal.

A comparison of the fracture resistances of endodontically treated mandibular premolars restored with endocrowns and glass fiber post-core retained conventional crowns

  • PURPOSE. This in-vitro study aimed to evaluate the fracture resistances and failure modes of endodontically treated mandibular premolars restored with endocrowns and conventional post-core retained crowns. MATERIALS AND METHODS. Thirty mandibular premolars were assigned into three groups (n=10): GI, intact teeth; GE, teeth with endocrowns; GC, teeth with conventional post-core supported crowns. Except for the teeth in group GI, all specimens were cut to 1.5 mm above the cementoenamel junction and endodontically treated. Both endocrowns and conventional crowns were fabricated from lithium-disilicate blocks using a CEREC 3D CAD/CAM unit. All specimens were subjected to thermocycling and then to $45^{\circ}$ oblique compressive load until fracture occurred. The fracture resistance and failure mode of each specimen were recorded. Data were analyzed with one-way ANOVA and LSD Post Hoc Test (${\alpha}=.05$). RESULTS. The fracture resistances of GE and GC were significantly lower than that of GI (P<.01), while no significant difference was found between GE and GC (P=.702). As of the failure mode, most of the specimens in GE and GC were unfavorable while a higher occurrence of favorable failure mode was presented in GI. CONCLUSION. For the restoration of mandibular premolar, endocrown shows no advantage in fracture resistance when compared with the conventional method. Both of the two methods cannot rehabilitate endodontically treated teeth with the same fracture resistances that intact mandibular premolars have.

THE ANOMALIES OF PERMANENT DENTITION IN CLEIDOCRANIAL DYSPLASIA (쇄골두개 이형성증 환아의 치아발육이상)

  • Cleidocranial Dysplasia(CCD) is an autosomal dominant human bone disease characterized by abnormal clavicles, patent sutures and fontanelles, and dental anomalies. Among dental anomalies, it is characterized that permanent dentition is severly disturbed due to multiple supernumerary teeth and abnormalities of tooth morphology. A eight-year-old female patient diagnosed as cleidocranial dysplasia visited in our hospital. Upon clinical oral exam, retained deciduous teeth, constriction of dental arch, anterior cross bite, and multiple dental caries were observed. In the dental panoramic radiograph, retained deciduous teeth and multiple supernumerary teeth in the maxilla and the mandible were found. In the cephalometric radiograph, open sutures and wormian bones were seen. In the chest P-A view absence of clavicles was observed. The cleidocranial dysplasia patients have eruption problems in permanent dentition both in regions with and without supernumerary teeth. The severely delayed or arrested eruption of permanent teeth has been ascribed to various factors : 1) The presence of multiple supernumerary teeth, 2) malformed roots with lack of cellular cementum, 3) the jaw bone being too dense, and 4) abnormal resorption of bone and primary teeth. Formation and maturation of primary teeth in cleidocranial dysplasia are normal, whereas the permanent dentition has various anomalies. Therefore, dentists should understand the development of dentition in cleidocranial dysplasia, and treat them in proper time.

  • Oligodontia is defined as a congenital tooth agenesis with the absence of six or more permanent teeth. This clinical report describes a patient with non-syndromic partial oligodontia, with retained deciduous teeth and the absence of 16 permanent teeth. Anterior esthetic problems were caused by interarch tooth size discrepancy, interdental space, aberrant tooth dimensions, and the absence of centric contacts of the anterior teeth. Prosthetic restoration after orthodontic and implant treatment was performed with a multi-disciplinary team approach. Favorable functional and esthetic results were obtained using a definitive prosthesis.

  • Conical crown-retained denture (CCRD) has been used as a very effective treatment method in cases with few remaining teeth with heterogeneous prognosis. However, in spite of many advantages of CCRD, high technical failure rate was a problem to be considered. Incorrect path of insertion and excessive retention were thought to be the main cause of technical failure and to result from laboratory procedure with a coping misfit and/or a coping transfer error. In order to prevent this error, secure anchoring of inner coping and re-examination and milling of convergence angle were recommended on the master model from pick-up impression.

  • The author studied the gingival responses to some dental cements in the gingival sulcus around artificial crowns. Abutment preparation for full veneer crown was performed in the canines of the two dogs. The location of cervical margins was about 0.5mm. below the gingival crest. Niranium metal crowns were constructed for the teeth, and cemented with zinc phosphate cement or polycarboxy late cement. In the experimental groups the retained cements in the gingival sulucus were not removed, and in the control groups the cements were removed completely after cementation. The dogs were sacrificed at 3 weeks and 5 weeks respectively after cementation. The gingival responses to these cements were examined histologically. The findings were as follows. 1. There was severe inflammation in the gingiva where the cements had been retained in the gingival sulcus around artificial crowns. 2. There was more severe inflammation in the gingiva which had contacted with zinc phosphate cement than in the gingiva with polycarboxylate cement. 3. There was mild inflammation in the gingiva around the margins of Niranium crowns. 4. The retained cement around the margin of restoration should be completely removed after cementation.

Prosthetic treatment for patient with congenital bilateral cleft lip and palate to close oro-nasal communication using maxillary double crown and clasp retained removable denture (선천성 양측성 구순구개열 환자의 구비강 연결 폐쇄를 위한 상악 이중관과 고리 유지형 국소의치를 이용한 수복 증례)

  • Patients with cleft lip and palate have several problems such as oro-nasal communication, dental cross-bite caused by the insufficient growth of maxilla, poor pronunciation and esthetic problem. Removable denture with maxillary double crown near cleft palate and with clasp in the posterior teeth was used to solve the problems of the patient with cleft lip and plate. Double crowns make up for the insufficient mucosal support caused by the mucosa and alveolar bone loss in cleft lip and palate. Double crowns also allow easy repair of denture in case of abutment teeth extraction. In this case, 55-year-old female patient had bilateral cleft lip and palate and few remaining teeth on anterior maxilla. Prosthetic treatment was done for patient with bilateral cleft lip and palate to close oro-nasal communication using maxillary double crown and clasp retained removable denture. As a result, oro-nasal communication was effectively closed and the cross-bite was easily corrected by double crown and clasp retained denture. In addition, pronunciation and appearance were also improved.
