Bonded Linqual Retainer의 최신 제작기술 (A New Technique for Fabrication of Bonded Linqual Retainer)

  • Bonded lingual retained are divided into two common types; preformed retainers with attached mash pads, and those that are custom fabricated from standard round ir multi-standed wire. The clinician may encounter problems in the bonding process of both types of retainers because of an inability to accurtely place and temporarily stability the wire in the same position as it was adapted an a guide model. Because of these problems, a new fabrication technique of bonded lingual retainers which increase their accuracy and ease of placement and also increased their tretention was suggested by the author. Using a current model as a guide, this bonded lingual retainer was fabricated from $.028"{\sim}.032"$ standard round wire. The wire was formed with pliers for idel adapation to the lingual surfaces of the involved teeth. Right angle bends were placed in the retainer wire ends and custom "composite" bonding pads were added to the ends of the retainer wire. This bonded lingual retainer with custom "composite" bonding pads is easir to place because of the increased stability and the accuracy of placement is greatly increased. The increased insure that the retainer comforms ideally to the lingual surface of the teeth being retained.

Screening panoramic radiographs in a group of patients visiting a Health Promotion Center

  • Purpose : To report the incidence of radiological findings from screening panoramic radiographs and verify the validity of the panoramic radiography for screening purposes. Materials and Methods : Six thousand one hundred and sixty panoramic radiographs taken from the patients visiting the Health Promotion Center of CNUH were selected for this retrospective study. Panoramic radiographs were examined into the following pathologic conditions : the presence of periodontal bone loss, dental caries, periapical radiolucencies, retained roots, impacted supernumerary teeth, impacted third molars, odontoma, cystic lesions other than radicular cyst, sialoliths, and mixed radiolucent-radiopaque lesions. Number of pathologic conditions and Prevalence values were recorded. Results: The prevalences of pathologic conditions were $72.9\%$ of periodontal bone loss, $32.2\%$ of dental caries, $11.9\%$ of periapical radiolucencies, $10.8\%$ of retained roots, $0.4\%$ of root fracture, $1.0\%$ of impacted supernumerary teeth, $1.0\%$ of impacted third molars, $0.06\%$ of odontoma, $0.08\%$ of cystic lesion other than radicular cyst, $0.2\%$ of prolonged retention of deciduous tooth, $0.1\%$ of sialolith, and $0.04\%$ of mixed radiopaque and radiolucent lesion. Conclusion : Although the panoramic radiograph should not be used to replace intraoral radiographic and clinical examinations, this study showed that many dental pathologic conditions could be detected on panoramic radiographs. The panoramic radiograph might serve as a diagnostic aid in dental health evaluation programs.

Anaphylactic reaction after local lidocaine infiltration for retraction of retained teeth

  • Although allergic reactions are not rare complications in drug use, anaphylaxis or anaphylactoid reactions to some widely used drugs can embarrass clinicians because anaphylaxis is not easily diagnosed at the time of the event and treatment is unfamiliar to many. Lidocaine is a very popular drug in dental procedures and anaphylactoid reaction to it has been rarely reported. Clinicians who use lidocaine daily should, however, be aware of the possibility of anaphylaxis after its use. Once it occurs, anaphylaxis can be fatal, but if it is quickly diagnosed or suspected, treatment is simpler than most clinicians believe. An 86-year-old woman experienced an anaphylactic reaction 30 min after local infiltration of lidocaine for retraction of retained teeth. The dentist called an anesthesiologist for assistance. Fortunately, an anaphylactic reaction was quickly suspected and after subsequent rapid treatment with the administration of fluid and drug therapy, the patient recovered completely.

  • The management of teeth in the line of a mandibular fracture is controversial despite the general agreement that most of these teeth can be preserved. Teeth should be retained if bony attachments are adequate for survival, the tooth is sound and important in maintaining fixation of the fractured segment of bone. Teeth should be removed if they are loose and interfere with the reduction of fragments, are devitalized and potentially a source of wound infection, are damaged beyond their usefulness or may become devital and interfere with healing by becoming infected. However, tooth removal will increase the level of trauma, extend the severity of the wound and require expensive prosthetic treatment. Therefore, it is very important to conserve infected teeth in the line of a mandibular fracture through early primary endodontic treatment (pulp extirpation, canal enlargement and canal opening drainage) and splinting. The basic principles underlying the treatment of pulpless teeth are those underlying general surgery. Therefore, debridement of the infected wound (pulp extirpation and canal enlargement), drainage (canal opening) and gentle treatment of the tissues (occlusal reduction and teeth splinting) are the principles of surgery. This is a representative case report of conservative care by the early endodontic drainage of infected teeth in the line of a mandibular fracture.

Splinted or Non-splinted: 다수의 인접한 치아 결손부 수복을 위한 임플란트 보철 (Splinted and non-splinted implant-supported restorations : prosthetic considerations for restoring multiple adjacent teeth)

  • The purpose of this paper was to investigate the significance of splinted and non-splinted implant-supported restorations with an internal connection for multiple consecutively missing teeth. Upon examination of the effects of fixture-abutment connection, the distribution of occlusal load was favorable in splinted implant-prosthesis with an external connection, but effect of strain distribution was not significant in splinted implant-prosthesis with an internal connection. In splinted implant-prostheses for short implants, strain distribution was not affected by the method of retention. For cement-retained prostheses, the effect of strain distribution due to splinting was not significant. In clinical studies, non-splinted prostheses with an internal connection for multiple consecutively missing teeth showed high survival rate, mild marginal bone loss, and stable periodontal condition. However, failure to achieve optimal proximal contact between single-unit prostheses may lead to food impaction, and veneer fracture may be inevitable when the framework provides inadequate support in the proximal region. In conclusion, splinted implant-prosthesis is not an indication in all cases, and clinical consideration of its use should be based on the patient's oral condition, such as location and number of implants, formation of proximal contact, canine guidance, existence of parafunctional habit, and oral hygiene, when multiple consecutively missing teeth are replaced by internal connection type implant.

Locator attachment를 이용한 하악 임플란트 유지 피개의치의 후향적 평가 (Retrospective Evaluation of Implant Retained Overdentures Using Two Implants and Locator Attachments in the Edentulous Mandibles)

  • 이번 연구는 2개의 임플란트 식립 후 로케이터를 이용한 하악 임플란트 유지 피개의치를 한 20명의 환자를 대상으로 임플란트 생존율, 변연골 흡수량, 보철적 합병증을 알아보았다. 진료 기록부를 조사하고 방사선 사진을 계측하여 후향적 임상연구를 하였으며, 임플란트는 95%의 누적 생존율을 보였고, 평균 94주 후 방사선 사진에서 임플란트는 평균 1.21 mm($SD{\pm}0.20mm$)의 골소실을 보였다. 보철적 합병증으로는 피개의치 파절, 의치상 이장 또는 개상, 메일의 탈락 또는 유지력 감소로 인한 메일 교체 등이 있었다. 그러나 의치상 이장 및 개상, 메일 인써트의 간단한 교체로 인해 장기적으로 임플란트 및 임플란트 유지 피개의치가 환자에게 유지되고 있었다. Locator를 이용한 하악 임플란트 유지 피개의치는 최소침습적인 수술, 간단한 보철 과정 및 수리 과정의 장점으로 인해 장기적으로 안정적인 치료법으로 사료된다.

  • Purpose: Prosthodontics for edentulous patients is a treatment technique using implant, which has impactful results in retention and support effects. Methods: As a retention technique, SCRP (screw and cement retained implant prosthesis) has reported in many studies as a beneficial method for both patients and curers, which can reduce errors in process of making abutment and top implant. Results: Prosthesis manufacturing, as polymerization method of hardened resin teeth with thermoplastic resin, is helpful for patients with aesthetic and financial situations regarding residual ridge and interocclusal relationship, also indicates reliable results in both retention and care. Conclusion: Using SCRP technique, we notably obtained a clinical and aesthetic outcome from five implants in anterior tooth, which are half fixable and detachable implants on screw of implant abutment by the technicians at anytime.

  • 치아전위는 비교적 드문 발육장애이다. 적절하게 차단치료된다면 예방이 가능할 뿐 아니라 이후의 교정과 수복과정이 보다 용이할 수 있다. 이를 위한 8-9세 경의 조기 임상검사가 이러한 발육장애의 인지를 위해 추천된다. 일단 전위가 일어난 경우에는, 부정교합을 교정하고 최상의 기능적, 심미적 결과를 얻기 위해 주의깊은 교정적 평가가 요구된다. 이상적인 결과를 얻기 위하여 두 치아를 원래의 위치로 환원시키는 치료가 요구되나, 완전전위되어 두 치아가 맹출되었다면 전위된 상태로 치아를 배열하여도 비교적 양호한 결과를 얻을 수 있었다.

  • The purpose of this study was to evaluate a cases which was treated with magnetic attachments on the patients in edentulous mandible and maxilla. Author made magnetic overdenture for two patients and then clinical examination were used for evaluation of the function and change in retention during 30months. Patients showed difficulty in oral hygiene care at the beginning but they go used to it soon. It was effective in promotion of denture retention and stability and also gave patient emotionally uplifting psychologic state. This study suggest that magnetic attachment was effective for the treatment of patients presenting a few remaining teeth or teeth under severe caries or mobility.

상악 전치부 다발성 수평 치근 파절의 치료: 증례보고 (Treatment of Multiple Horizontal Root Fractures of the Maxillary Anterior: A case report)

  • Objectives: Management of a horizontal root fracture of an anterior teeth is challenging and often requires multiple approaches for improving the functional and esthetic outcomes. This case report describes the treatment and 2-yr follow up of 3 maxillary incisors with horizontal root fracture. Two maxillary central incisors were treated with Mineral Trioxide Aggregate (ProRoot MTA, Dentsply, Tulsa, OK, USA). Left maxillary lateral incisors were treated with endodontic treatment and submerged. During 2-yr of follow-up evaluation, the root-fractured teeth of the present patients were well retained in the arch, showing periodontal healing even after endodontic treatment.
