소아치과 개원의 진료 현황 분석 (THE PRACTICE PATTERN OF PEDIATRIC DENTISTS IN KOREA)

  • 소아치과는 소아와 청소년의 구강상태에 대하여 보존, 교정, 치주, 외과치료 등 다방면의 치료를 복합적으로 시행한다는 점에서 타분야와 구분된다. 초창기의 소아치과학은 유치와 영구치의 우식의 치료, 발치 등에 국한되었으나, 최근에는 교정, 소 수술, 심미, 예방치료 등으로 진료의 범주가 확대되고 있다 2005년 11월 현재 소아와 청소년을 대상으로 하는 개인치과에서 근무하는 치과의사 50명을 대상으로 설문지를 발송하고 회송된 설문지 21매를 토대로 진료현황을 분석한 결과 다음과 같은 결론을 얻었다. 1. 일주일 간 평균적으로 진료하는 환자의 수는 82.4명이었고, 각 환자당 평균적으로 치료받는 치아의 수는 2.35개였다. 2, 전체 치료 중 예방치료가 15.7%, 수복치료가 55.7%, 치수치료가 15.6%, 외과치료가 10.5%, 교정치료가 2.4%를 차지하였다. 3. 수복치료 중 아말감이 3.8%, 글래스 아이오노머가 5.5%, 복합레진이 63.0%, 스테인레스 기성금관이 277%를 차지하였다.

Conservative approach of a symptomatic carious immature permanent tooth using a tricalcium silicate cement (Biodentine): a case report

  • The restorative management of deep carious lesions and the preservation of pulp vitality of immature teeth present real challenges for dental practitioners. New tricalcium silicate cements are of interest in the treatment of such cases. This case describes the immediate management and the follow-up of an extensive carious lesion on an immature second right mandibular premolar. Following anesthesia and rubber dam isolation, the carious lesion was removed and a partial pulpotomy was performed. After obtaining hemostasis, the exposed pulp was covered with a tricalcium silicate cement (Biodentine, Septodont) and a glass ionomer cement (Fuji IX extra, GC Corp.) restoration was placed over the tricalcium silicate cement. A review appointment was arranged after seven days, where the tooth was asymptomatic with the patient reporting no pain during the intervening period. At both 3 and 6 mon follow up, it was noted that the tooth was vital, with normal responses to thermal tests. Radiographic examination of the tooth indicated dentin-bridge formation in the pulp chamber and the continuous root formation. This case report demonstrates a fast tissue response both at the pulpal and root dentin level. The use of tricalcium silicate cement should be considered as a conservative intervention in the treatment of symptomatic immature teeth.

  • 본 증례는 26세 여성환자로 폭식증과 이로 인한 구토 및 위산역류로 상악 전치부에 광범위한 치질 손상과 더불어 산부식으로 인해 상아질 노출로 기능뿐만 아니라 심미적인 문제를 주소로 내원했다. 환자는 단시간 내 심미성 회복을 원하는 상태로 교정 등의 치료는 원치 않았다. 이를 감안해 치료계획은 이미 손상된 치질을 최대한 보존하면서 심미성 및 기능 회복을 목표로 하였다. 상악 전치부의 치관 비율 및 치은 형태 개선을 위해 치은절제술 및 치관연장술을 시행했으며, 전부도재관을 이용해 수복하였다. 회전 이동되어있는 우측 상악 측절치는 치질 삭제 없이 veneer 형태로 제작했다. 산 부식 및 마모에 강한 지르코니아에 도재로 순측을 veneering해 기능 회복뿐만 아니라 심미적인 면에서도 만족할만한 결과를 얻을 수 있었다. 추후 미백치료를 고려해 최종보철물의 색상을 결정했으며, 환자는 색상과 형태에 만족했으며 자신감있는 미소를 회복할 수 있었다.

상아질 표면 처리에 의한 상아질과 복합레진의 결합에 관한 연구 (STUDIES ON THE BOND BETWEEN COMPOSITE RESIN AND DENTIN TREATED BY DENTIN BONDING AGENTS)

  • The purpose of this study was to compare the shear bond strengths to ground dentin surfaces of four dentinal bonding agents in 193 teeth. Various dentin surfaces treated with four dentin bonding agents were attached with two restorative composite resins. The effectiveness of the bonding were tested by the monitoring the shear bond strength. The shear bond strengths were measured after 2 hours and 24 hours after surface conditioning with four dentin bonding agents. Effects of EDTA, the additive illumination, and sealer treatments without primer on bond strength to dentin surfaces were assessed. In addition the effects of the thickness of specimens ranging from 0.65 mm to 1.95 mm and the ratio of catalyst and base paste on the bond strength of chemical cure composite resin were estimated. The shear bond strength was determined by testing specimens in the Instron universal testing machine (Model No. 1122) at a crosshead speed of 1.0 mm/min. Following condusions were drawn: 1. The highest mean shear bond strengths of chemical cure composite resin to dentin conditioning with dentin bonding agents aged 2 hours were obtained, and then that was decreased with time followed by EDTA treatment. 2. In light cure composite resin, the shear bond strength was increased following dentin conditioning with bonding agents with time, irradiation time and EDTA treatment except in SB group. 3. The thicker the composite resin specimen was, the less the shear bond strength in chemical cure composite resin was. 4. In light cure composite resin, there was a little change in shear bond strength following dentin conditioning with bonding agents. 5. In chemical cure composite resin, the shear bond strength was the highest in the ratio of 1/1 of catalyst and base part. 6. Without a dentin primer, shear bond strength to dentin conditioned only with UB sealer was the highest among four sealers in light cure composite resin.

The Restorative Effect of Gallic Acid on the Experimental Sciatic Nerve Damage Model

  • Objective : Peripheral nerve injuries occur mostly as a result of mechanical trauma. Due to the microvascular deterioration in peripheral nerve damage, it becomes challenging to remove free oxygen radicals. Gallic acid is a powerful antioxidant with anti-inflammatory effects and a free radical scavenger. The purpose of the study is to show that gallic acid contributes to the restorative effect in mechanical nerve damage, considering its antioxidant and anti-inflammatory effects. Methods : Thirty male Sprague Dawley albino mature rats were included in the study. Ten of them constituted the control group, 10 out of 20 rats for which sciatic nerve damage was caused, constituted the saline group, and 10 formed the gallic acid group. Post-treatment motor functions, histological, immunohistochemical, and biochemical parameters of the rats were evaluated. Results : Compared to the surgery+saline group, lower compound muscle action potential (CMAP) latency, higher CMAP amplitude, and higher inclined plane test values were found in the surgery+gallic acid group. Similarly, a higher nerve growth factor (NGF) percentage, a higher number of axons, and a lower percentage of fibrosis scores were observed in the surgery+gallic acid group. Finally, lower tissue malondialdehyde (MDA) and higher heat shock protein-70 (HSP-70) values were determined in the surgery+gallic acid group. Conclusion : Gallic acid positively affects peripheral nerve injury healing due to its anti-inflammatory and antioxidant effects. It has been thought that gallic acid can be used as a supportive treatment in peripheral nerve damage.

The evaluation of maximum bite force in the occlusal rehabilitation of patient with Angle Class III malocclusion: a case report

  • The case report describes the occlusal rehabilitation of a male patient with Angle Class III malocclusion and its effect on maximum bite force. The main complaints of patient were masticatory difficulty and poor esthetic. The patient's expectations from the treatment were a good esthetic and function with a less invasive and relatively promptly way. Therefore, increasing of the occlusal vertical dimension (OVD) and then restoring the maxillary and mandibular teeth was chosen by the patient among the treatment options. At the beginning of treatment maximum bite force of patient was measured. Then an occlusal splint was provided to evaluate the adaptation of the patient to the altered OVD. Full mouth rehabilitation with metal ceramic restorations was made. After the completion of full mouth restoration, bite force measurement was repeated and patient exhibited increased maximum bite force. Full mouth restorative treatment in a patient with Class III malocclusion could be an effective treatment approach to resolve esthetic concern and to improve masticatory function related to maximum bite force.

Accidental injury of the inferior alveolar nerve due to the extrusion of calcium hydroxide in endodontic treatment: a case report

  • During clinical endodontic treatment, we often find radiopaque filling material beyond the root apex. Accidental extrusion of calcium hydroxide could cause the injury of inferior alveolar nerve, such as paresthesia or continuous inflammatory response. This case report presents the extrusion of calcium hydroxide and treatment procedures including surgical intervention. A 48 yr old female patient experienced Calcipex II extrusion in to the inferior alveolar canal on left mandibular area during endodontic treatment. After completion of endodontic treatment on left mandibular first molar, surgical intervention was planned under general anesthesia. After cortical bone osteotomy and debridement, neuroma resection and neurorrhaphy was performed, and prognosis was observed. But no improvement in sensory nerve was seen following surgical intervention after 20 mon. A clinician should be aware of extrusion of intracanal medicaments and the possibility of damage on inferior alveolar canal. Injectable type of calcium hydroxide should be applied with care for preventing nerve injury. The alternative delivery method such as lentulo spiral was suggested on the posterior mandibular molar.

The use of platelet rich plasma in the treatment of immature tooth with periapical lesion: a case report

  • This study describes the treatment of an immature permanent tooth with periapical lesion which was treated with regenerative approach using platelet rich plasma (PRP). The root canal of immature human permanent tooth with periapical lesion was gently debrided of necrotic tissue and disinfected with 2.5% NaOCl, and then medicated with triple antibiotic paste comprised of ciprofloxacin, metronidazole, and tetracycline. When the tooth was asymptomatic, PRP and mineral trioxide aggregate (MTA) were placed into the root canal. Six months after PRP treatment, radiographical examination revealed resolution of the radiolucency and progressive thickening of the root wall and apical closure. Our findings suggest that PRP can be used for the treatment of immature permanent teeth with periapical lesion, as part of a regenerative endodontic treatment procedure.

Healing outcomes of root canal treatment for C-shaped mandibular second molars: a retrospective analysis

  • Objectives: This study aimed to evaluate the healing rate of non-surgical endodontic treatment between C-shaped and non-C-shaped mandibular second molars. Materials and Methods: Clinical records and radiological images of patients who had undergone endodontic treatment on mandibular second molars between 2007 and 2014 were screened. The periapical index scoring system was applied to compare healing outcomes. Information about preoperative and postoperative factors as well as the demographic data of the patients was acquired and evaluated using chi-square and multinomial logistic regression tests. Results: The total healing rate was 68.4%. Healing rates for the mandibular second molar were 70.9% in C-shaped canals (n = 79) and 66.6% in non-C-shaped ones (n = 117). The difference was not statistically significant. Conclusions: The presence of a C-shaped canal in the mandibular second molar did not have a significantly negative effect on healing after treatment. Instead, proper pulpal diagnosis and final restoration were indicated as having significantly greater influence on the healing outcomes of C-shaped and non-C-shaped canals, respectively.

YouTube as an information source for instrument separation in root canal treatment

  • Objectives: The reliability and educational quality of videos on YouTube for patients seeking information regarding instrument separation in root canal treatment were evaluated. Materials and Methods: YouTube was searched for videos on instrument separation in root canal treatment. Video content was scored based on reliability in terms of 3 categories (etiology, procedure, and prognosis) and based on video flow, quality, and educational usefulness using the Global Quality Score (GQS). Descriptive statistics were obtained and the data were analyzed using analysis of variance and the Kruskal-Wallis test. Results: The highest mean completeness scores were obtained for videos published by dentists or specialists (1.48 ± 1.06). There was no statistically significant difference among sources of upload in terms of content completeness. The highest mean GQS was found for videos published by dentists or specialists (1.82 ± 0.96), although there was no statistically significant correlation between GQS and the source of upload. Conclusions: Videos on YouTube have incomplete and low-quality content for patients who are concerned about instrument separation during endodontic treatment, or who experience this complication during endodontic treatment.
