5급 병소가 있는 치아 유한요소 모델의 수복방법 최적화 (An Optimization of Restoration Method at Class V Lesion of a Finite Element Tooth Model)

  • A general treatment is to restore abfraction lesions with dental filler materials to reduce stress concentration. A material should be selected from various dental products based on long term experiences of dentist or personal preference concerning filler methods. A quantitative criterion is necessary to make an evaluation of the results as dentists decide treatment methods and dental materials relying on their clinical experiences. The purpose of this study is to find an optimal restoration method and material for noncarious cervical lesions using the finite element method. An objective function was defined to minimize the sum of tension or compression stress. Trial-and-error and approximation were used to find an optimal restoration method. An optimal solution was to fill TetricFlow inside the lesion and Z100 in the remaining region. The most desirable thickness ratio of the two filler materials was 0.125 with trial-and-error and it was similar to the results of approximation, 0.121 and 0.132.

개흉술 후 발생한 종격동염의 대흉근-복직근 양경근피판을 이용한 치료 (Pectoralis Major-Rectus Abdominis Bipedicle Muscle Flap in Treatment of Postoperative Mediastinitis)

  • Although the incidence of mediastinal wound infection in patient undergoing median sternotomy for cardiovascular surgery is relatively low(less than 1%), it is not only a devastating and potentially life-threatening complication but also associated morbidity, mortality and cost are unacceptably high. During the past few decades various methods had been applied for the treatment of postoperative mediastinitis. Currently, chest wall reconstruction by using muscle flaps-especially pectoralis major muscle and rectus abdominis muscle are commonly selected for the reconstruction after wide debridement has become widely accepted. We performed bilateral pectoralis major-rectus abdominis muscles in-continuity bipedicle flap to overcome the limit of each flap for reconstruction of sternal defects in 17 patients. We analyzed the results of the surgery. Recurrent infection developed in 17.6% of cases and abdominal herniation was observed in one patient. There was no postoperative hematoma or death. We conclude that this flap is very valuable in reconstruction of the anterior chest wall defect caused by post-sternotomy infection because it provides sufficient volume to fill the entire mediastinum, and the complication rate compares favorably to that of other methods.

Treatment of Pseudoangiomatous Stromal Hyperplasia of the Breast: Implant-Based Reconstruction with a Vascularized Dermal Sling

  • Pseudoangiomatous stromal hyperplasia (PASH) of the breast is a benign mesenchymal lesion with incidental histologic findings. Surgical excision is recommended as the treatment of choice for PASH, although the recurrence rates after excision range from 15% to 22%. A 46-year-old-female presented with a six-month history of bilateral breast enlargement and painful sensation mimicking inflammatory carcinoma. Imaging studies demonstrated innumerable enhancing nodules in both breasts. Due to the growth of the lesions and progressive clinical symptoms, bilateral subcutaneous mastectomy was performed. Grossly, the specimens were round and well-circumscribed, and the histologic examination revealed PASH. After mastectomy, we created a pocket with the pectoralis major muscle and a lower skin flap, which was deepithelized. Anatomical mammary implants were inserted, and the nipple areolar complex was transferred to a new position as a free graft. The aesthetic result was satisfactory after twelve months of follow-up.

CEREC 3D System을 이용한 당일수복 (one-day treatment) 임상증례

  • Application of CAD/CAM is changing the way partial or full veneer all ceramic restoration is made. CAD/CAM systems, which were used mainly in other industries, have been developed and introduced for the dental purposes recently. It produced a flood of information on the CAD/CAM systems. It also influenced the development of restorative materials and all ceramic is substituting the traditional restorative materials of gold, composite resin and metal. Price increase of gold and other raw materials made the all ceramic more appealing. The introduction of a CEREC 3D system was innovative in several ways. Image of the prepared tooth is captured by camera and impression taking is unnecessary. Restoration can be delivered to the patient on one appointment and it will satisfy the demand of busy patients. One-day treatment with direct CAD/CAM system saves time compared to indirect CAD/CAM system. More superior restoration can be produced if lab work such as the adaptability check and shade selection is cooperated with lab technician. Short working time and comparably superior shade compatibility of color block was close to ideal. In the future, restorations with better quality can be fabricated in less time to busy patients thanks to the development of CAD/CAM system and dental materials.

하천수질 오염요소 분석을 근거로 금강수계의 우선정비 대상하천 선정을 위한 집단화 기법적용 (Application of Grouping Method to select Priority Restoration Streams in Geumgang Watershed based on Analysis of Pollution Factors)

  • River-water quality has been greatly improved during past several decades with the extraordinary expansion for the wastewater treatment capacities by the government. Research aims to select the priority restoration streams based on the chronicle data for tributaries in Geumgang watershed as the main stream area in the Chungchungnamdo province. The quality of BOD, phosphorus and percent of sewered population on 15 branch streams were compared by the grouping methods. The results of group D streams by category I that exceed 3.0 mg/L for BOD and 0.1 mg/L for phosphorus were Seuksung, Ganggyung and Bangchuk stream. The results of group D streams by category II that exceed 3.0 mg/L for BOD and less than 63.5 % of average percent of sewered population were Ganggyung, Gilsan, Bangchuk and Seuksung stream. The final results of selected streams drawn by the chronicle data which exceeded the standard quality and lower than the average percent of sewered population were Seoksung, Gangeyung and Bangchuk stream. The pollution of rivers in the down streams were more serious than in the upper streams. Their watersheds have to be improved river water quality, especially to extend sewer systems as well as wastewater treatment facilities.

  • 강동경희치대병원 보철과에서는 치료계획단계에서 마지막 결과를 미리 형상화하고 그것에 입각하여 치료를 진행하는 Top-Down 개념의 치료접근법을 2006년부터 실행하고 있다. 이번 증례도 그 개념에 입각하여 임플란트 수술에 앞서 미리 최종 보철치료의 방법과 결과를 형상화시켜 놓고 치료를 진행한 실례이다. 임플란트 보철의 여러 방법 중 시멘트 유지방식은 임플란트의 기울기에 구속받지 않고 간단하게 보철물 제작이 가능하나, 접착 후 치은 연하의 잉여 시멘트를 완전하게 제거하기 어렵고, 필요할 때 탈착이 불편하다는 단점이 있다. 따라서 전치부 임플란트 보철수복 시 SB-locking method의 유지방식을 이용하여 시멘트 유지방식의 문제를 해결하면서 심미적인 임플란트 보철수복이 가능했던 증례를 보고하고자 한다.

역 L 형 절골술과 Liou 신연기를 이용한 넓은 치조열의 교정 (Correction of a Wide Alveolar Cleft with Reverse L osteotomy and Liou Alveolar Distractor)

  • Purpose: A successful surgical treatment for a wide alveolar cleft with bone graft is difficult to achieve due to several factors such as the limitation of gingivoperiosteal flap, the presence of large scar tissues, and the poor blood circulation. To overcome these problems, alveolar distraction osteogenesis using Liou alveolar distraction device was applied. We analyzed the consequences of this surgical treatment. Method: Between 2006 January and 2007 August, we have conducted analysis on the methods and consequences of Liou alveolar distraction osteogenesis for 6 patients. The age of patients was 12 years and 6 months in average. The follow up period was 19 months in average. The Reverse L osteotomy followed by the placement of the Liou alveolar distraction device was performed. After serial distraction, the distractor was removed after 5 months of the process of osteogenesis, and the result was analyzed using the computed tomography and the x-ray films of the alveolar bone and the teeth. Results: The alveolar cleft with 12.5 mm in average width was filled with 8.5 mm of newly formed bone tissue in average width after 5 months of osteogenesis. Among the 6 cases, 5 required the additional bone graft and 1 case only required the gingivoperioplasty. The newly formed bone tissues did not show any signs of bone resorption. However, a considerable degree of teeth displacement was shown. Conclusion: For the alveolar cleft too wide to be reconstructed by a general bone graft, it is strongly recommended to perform the reverse L osteotomy of the cleft side with Liou alveolar distraction device to initiate the alveolar osteogenesis. However, the migrated teeth showed some degree of relapse, thus, the orthodontic treatment is essential following the distraction osteogenesis treatment.

답압(踏壓)으로 훼손(毁損)된 임간나지(林間裸地)의 임상식생복원(林床植生復元)에 관한 연구(硏究)(I) -임상식생복원(林床植生復元)에 미치는 파종(播種), 시비(施肥) 및 표토처리효과 (表土處理效果)- (Studies on Restoration of Forest-Floor Vegetation Devastated by Recreational Trampling (I) -Seeding, Fertilizing and Soil Surface Treatment Effect on Restoration of Forest-Floor Vegetation-)

  • 본(本) 연구(硏究)는 자연성(自然性)을 유지(維持)하고 자연생태계(自然生態系)를 보전(保全)하면서 동시에 산림휴양(山林休養)에 이용(利用)하려는 산림지역(山林地域)에서 이용객(利用客)들의 답압(踏壓)에 의하여 훼손(毁損)된 임상식생(林床植生)을 복원(復元)할 수 있는 방법을 구명(究明)하기 위하여 파종(播種), 시비(施肥) 및 표토처리(表土處理)의 요인실험(要因實驗)을 임간나지(林間裸地)에서 4년 동안 (1987~1990) 실시하였다. 경기도(京畿道) 안양시(安養市) 관악산(冠岳山) 수목원(樹木園)에서 임상식생복원실험(林床植生復元實驗)을 위하여 분할구배치법(分割區配置法)(주구(主區): 시비(施肥), 세구(細區): 표토처리(表土處理)${\times}$파종(播種)), 난괴법(亂塊法)(시비(施肥)${\times}$파종(播種))에 의한 요인실험(要因實驗)을 3반복(反復)으로 실시한 바 다음과 같은 연구결과(硏究結果)를 얻었다. 표토(表土)의 연화(軟化)를 위한 굴혈처리후(堀穴處理後) 70% 피복도(被覆度)를 나타내는 볏짚거적 피복처리(被覆處理)가 임간나지(林間裸地)에서 파종(播種)한 종자(種子)의 발아(發芽), 활착(活着) 및 생장(生長)에 미치는 효과(效果)가 가장 크게 나타났으며, 특히 볏짚거적 피복처리(被覆處理)는 표토침식지(表土浸蝕地)의 표토안정(表土安定) 및 치수활착(稚樹活着)에 미치는 효과가 양호하였고, 삽으로 파서 뒤엎은 완전표토연화처리(完全表土軟化處理)는 식생천이계열상(植生遷移系列上) 후기(後期)에 속하는 수종(樹種)들의 발아(發芽), 활착(活着) 및 초기생장(初期生長)에 특히 효과적(效果的)인 것으로 나타났다. 다년간(多年間) 계속적인 답압(踏壓)으로 임상표토(林床表土)의 토괴경도(土壞硬度)가 높아지고, 또 자생식물(自生植物)들의 종자(種子)가 잔류(殘留)할 수 있는 표토(表土)가 유실(流失)된 임간나지(林間裸地)에서 조기(早期)에 임상식생(林床植生)을 복원(復元)할 때에는 식생천이계열상(植生遷移系列上) 초기(初期)에서 중기계열(中期系列)에 속하는 자생수종(自生樹種)들을 파종(播種)하는 것이 효과적(效果的)인 것으로 나타났다. 자생식물(自生植物)들의 종자(種子)가 잔류(殘留)할 수 있는 표토(表土)가 유실(流失)된 임간나지(林間裸地)에서 임상식생복원(林床植生復元)(착생개체수(着生個體數)와 수관면적(樹冠面積))에는 파종(播種), 표토굴혈연화(表土掘穴軟化) 및 볏짚거적피복처리시(被覆處理時)에 약 3년이 소요되었다. 표사연화처리(表士軟化處理)는 처리 후 약 2년 동안 표토연화효과(表土軟化效果)가 지속되었으며 또 파종(播種) 및 표토처리(表土處理)는 활착개체수(活着個體數) 증대(增大)를 통해 표토연화(表土軟化), 낙엽퇴고정(落葉堆固定) 등 표토환경개선(表土環境改善)에 영향(影響)을 미치었다. 실험지표토(實驗地表土)의 열악(劣惡)한 물리적조건(物理的條件) 즉, 낮은 보수능(保水能)과 과건피해(過乾被害), 표토침식(表土浸蝕)으로 인한 치수(稚樹)들의 뿌리 노출과 고사(枯死) 및 양료유실(養料流失) 등으로 임상시비효과(林床施肥效果)는 대체적으로 나타나지 않았다. 그러나, 질소(窒素)와 인산시비(燐酸施肥)는 식생천이계열(植生遷移系列) 후기수종(後期樹種)들의 치수활착율(稚樹活着率)을 증진(增進)시키는 데는 효과적(效果的)이었다.
