진폐증환자에서의 임파구 분획 및 싸이토카인의 역할 (The Role of Lymphocyte Compartment and Cytokine in Coal Workers Pneumoconiosis)

  • 연구배경 : 진폐증은 주로 분진흡입에 의해 폐실질에 섬유화를 일으키는 질환으로 만성염증에 의해 폐포내에 대식세포를 축적시키는 특정이 있으며, 크게 단순진폐증(SP)과 진행성 종괴성 폐섬유증(PMF)으로 구분된다. 본 연구의 목적은 진폐증 환자에서 질환의 심한 정도를 ILO 분류에 따라 SP, PMF로 구분한 후, 감염이나 기타 기저질환이 없는 상태에서 혈중 임파구 분획 및 싸이토카인의 농도를 비교한 것이다. 방 법 : 태백 중앙 병원과 영동 병원의 진폐증 환자 중 정밀 검사를 시행하기 위해 병원을 방문한 단순진폐증 환자와 입원 중이거나 정밀검사를 시행한 진행성 종괴성 폐섬유증 환자, 대조군을 대상으로 하였고, 대조군(C;10), 단순진폐증 (S;19), 진행성 종괴성 폐섬유증 (P;30명)에서 혈중 임파구 분획을, C(10), S(14), P(14명)에서 싸이토카인 (IFN-${\gamma}$, TNF-${\alpha}$, IL-6, IL-4))의 농도를 비교하고, 특히 흡연력이나 연령, 폐기능에 의해 변할 수 있는 싸이토카인의 변화를 동시에 고려하여 진폐증의 심한 정도와의 연관성을 관찰하였다. 결 과 : C, S, P 3 군간의 B-임파구, T, $T_H$, Ts, 및 NK-세포를 측정하였을 때 대조군과 단순진폐증환자 사이에서 T, $T_H$는 통계학적으로 의미있게 증가하였으며(P<.01) $T_H$는 두 군간에 의미있게 감소하였으나(P<.01), S, P 두 군 사이에는 특별한 변화가 없었다. 세 군 사이의 싸이토카인 분비 양상은 통계학적인 의의를 발견할 수 없었다. 결 론 : 혈청 임파구 분획은 C군과 비교하여 S군에서 T, $T_H$는 통계학적으로 의미 있게 증가하였으며 Ts는 감소하는 경향을 나타내어 진폐증의 진행 초기에 이들이 연관성이 있다고 추측되어진다.

  • 목 적:스트레스와 성격적 요인이 수면에 여러가지 다양한 영향을 미친다는 것은 최근까지 많이 연구되어 왔다. 그러나 스트레스와 성격요인 그리고 수면 이 세가지 요소간의 연관성과 서로간의 영향에 대한 연구는 없었던 것이 사실이다. 본 연구는 스트레스와 성격요인이 수면에 어떠한 영향을 주는지, 또한 이들 사이에 어떠한 연관성을 가지는지를 연구하고자 하였다. 방 법:건강한 남녀대학생 174명을 대상으로 스트레스와 수면상태, 성격 특성 등을 평가하기 위하여 스트레스 평가 척도(K-DSI)와 평소 수면평가지(modified PSQI), 성격요인검사(16 PF)를 작성하도록 하였다. 이 밖에 주관적인 우울, 불안의 정도도 조사하였다. 이들의 서로간의 연관성을 밝히기 위한 통계방법으로 기술분석, 피어슨 상관분석, 위계적 다중회귀분석, 경로분석 등을 이용하였으며 SPSS ver. 10.0과 AMOS 4.0을 사용하였다. 결 과:스트레스는 비회복적 수면으로 인한 증상들(기상 시 증상, 주간 증상 등)에 유의한 영향을 주는 것으로 나타났다. 성격요인 중에서는 죄책감 요인(O 요인), 자아강도 요인(C 요인)과 불안감 요인(Q4 요인)이 비회복적 수면으로 인한 증상들과 높은 상관관계를 보이는 것으로 나타났다. 우울과 불안 총점과 스트레스, 성격 사이에는 매우 높은 상관관계가 있는 것으로 나타났으며, 경로분석 상에서는 스트레스와 성격요인이 감정반응을 매개로 하여 수면에 영향을 미치는 것으로 나타났다. 결 론:본 연구를 통하여 스트레스가 다른 수면의 요소들보다 비회복 수면으로 인한 이차증상들에 크게 영향을 미친다는 것을 알 수 있었으며, 이는 수면의 회복 기능 효과를 감소시키기 때문인 것으로 여겨진다. 또한 자아강도가 약하고 죄책감이 많으며 높은 불안감을 가진 성격이 스트레스에 취약하며, 다양한 성격특성과 그에 따른 대처방식의 차이, 여러가지 스트레스 사건, 그리고 이에 대한 감정반응들이 복합적으로 수면에 영향을 미치는 것으로 생각된다.

동위원소 이뇨 요관그람을 이용한 소아 요관폐쇄의 평가 (Assessment of Hydroureteronephrosis in Children Using Diuretic Radionuclide Ureterography)

  • 수신증 또는 요관수류수신증으로 이뇨 신장스캔을 시행한 80명의 환아중 이노신장스캔영화영상에서 10분 이상의 뚜렷한 지속적인 요관 영상을 보이는 12명의 16예의 신장요관계를 대상으로 요관 관심영역에 의한 이뇨요관그람 요관곡선과 신장 관심 영역에 의한 이뇨 레노그람 신장곡선의 배뇨 반감시간을 포함한 각각의 정량적 지표를 비교 분석하여 다음의 결과를 얻었다. 1) 이뇨 신장스캔영화영상에서 10분 이상의 뚜렷한 지속적 요관 영상을 보이는 경우인 스캔상 요관 이상(요관확장)의 진단률을 방사선학적 검사에 의한 결과와 비교하였을 때, 예민도와 특이도는 각각 88%와 99%이었다. 2) 이뇨 요관그람 요관곡선상 이뇨 요관배뇨 반감시간의 비폐쇄의 폐쇄에 대한 경계치를 10분 이하로 하였을 때 요관 폐쇄 여부를 판별한 예민도와 특이도는 각각 86% (7/8) 이었다. 3) 이뇨 요관그람 요관곡선상 이뇨 요관배뇨 반감시간(Ut1/2)과 이뇨 레노그람 신장곡선상 이뇨 신장배노 반감시간(Kt1/2)간에 유의한 상관 관계를 보였다(r=0,70: p<0.01). 결론적으로 소아의 폐쇄성 요로질환에서 이뇨 신장스캔상 10분 이상의 지속적 요관 영상을 보이는 경우 이뇨요관그람 요관곡선을 이뇨 레노그람 신장곡선과 함께 분석하여 요관 폐쇄의 유무와 정확한 부위를 평가하여 치료 방침을 결정하는 것이 필수적일 것으로 사료된다.

황사기간 중 천식 환자에서 대기 중 미세먼지($PM_{10}$)가 최대호기 유속과 호흡기 증상에 미치는 영향 (Effects of Ambient Particulate Matter($PM_{10}$) on Peak Expiratory Flow and Respiratory Symptoms in Subjects with Bronchial Asthma During Yellow Sand Period)

  • 배 경 : 황사는 주로 $10{\mu}m$이하의 미세먼지($PM_{10}$)로서 $PM_{10}$의 증가는 황사현상의 증가와 매우 밀접한 관련이 있다. $PM_{10}$은 그 크기로 보아 호흡기에 침착이 가능하고 따라서 호흡기 증상 및 폐기능에 영향을 미칠 수 있올 것으로 생각되나 아직 이에 대한 연구는 없었다. 이에 연구자들은 천식으로 확진된 환자를 대상으로 황사 기간 중에 $PM_{10}$을 포함한 대기오염물질이 호흡기증상 및 폐기능에 미치는 영향을 알아 보고자 하였다. 대상 및 방법 : 인천지역 거주자이고 천식으로 확진된 환자를 대상으로 2002년 3월 2일부터 6월 14일까지 매일 최소 2회 이상의 최대호기유속 측정, 속효성 기관지 확장제 사용회수, 야간 증상, 증상 점수, 설외활동량 등을 기록하였다. 또한 대기오염지수인 $PM_{10}$, $SO_2$, $NO_2$, $O_3$, 황사발생일, 기온 및 습도를 측정하여 이들과 최대호기유속 및 변동률, 증상과의 관련성, 야간 증상 및 속효성 기관지확장제 사용회수 등의 연관성을 분석하였다. 결 과 : 대상환자는 남자 31 명, 여자 33명으로 평균 연령은 $46.1{\pm}14.1$세, FEV1은 $2.2{\pm}0.9L$, FEV1%는 $71.5{\pm}24.9%$ 이었다. 연구 기간 중 황사 발생일은 총 14일이었으며, $PM_{10}$수치는 황사발생일에 $188.5{\pm}163.0{\mu}g/m^3$, 비황사발생일에 $60.1{\pm}19.9{\mu}g/m^3$로 황사 발생기간 동안에 높게 측정되었다(p<005). 일일 평균 $PM_{10}$이 높은 날에는 대상 환자의 오전 및 오후 최대 호기유속이 낮아지는 경향을 보였으며(p<0.05), 최대 호기유속 변화율이 증가하였고(p<0.05), 야간에 천식으로 인해 잠을 깨는 횟수가 증가하였다(p<0.05). 반면, 황사 기간 중에 $SO_2$, $NO_2$, $O_3$ 등 다른 대기오염지수와 증상 및 최대호기유속의 변화는 상관성이 없었다(p>0.05). CO는 황사와 관계없이 천식환자에서 최대 호기유속 변동률 및 평균 최대호기유속에 영향을 미치고 있었다(p<0.05). 결 론 : 황사 기간 중 증가하는 미세먼지 농도는 기관지 천식 환자의 호흡기 증상의 악화 및 폐기능의 저하를 초래할 수 있다.

엔지니어링 노임단가 산출기준 개선방안과 적정 노임단가 추정 (Improvement in Calculating Engineer Standard Wage Rate and Its Appropriate Level Computation)

  • 본 연구의 목적은 엔지니어링 노임단가의 산출기준 개선방안을 마련하고 이를 모델화하여 적정한 노임단가 수준을 산정하는데 있다. 이를 위해 엔지니어링 노임단가 산출기준의 타당성 검토와 더불어 광범위한 엔지니어링 산업 실태조사가 실시되었다. 실태조사는 5,879개 모집단을 층화하여 추출된 표본 1,000개의 기업을 대상으로 실시되었고 이중 유효하게 응답한 748개 기업의 설문지가 분석에 사용되었다. 본 연구가 제시한 엔지니어링 노임단가 산출기준의 개선방안 및 산출모델은 다음과 같다. ① 엔지니어링 대가 산정 시 적용되는 노임단가는 평균임금이 아닌 원청 임금으로 산정하는 것이 합리적인 것으로 분석되었다. 원청노임단가는 '평균 기술자임금÷ [1-하청금액 수주비중×(1-하도급률)]'의 산식에 의해 추정되었다. ② 실태조사결과 엔지니어링산업의 1개월 근로일수는 99 % 신뢰구간에서 20.35일~20.54일로 현행기준(22일)과 차이가 컸다. 또한 노임단가 산출기준 법령을 검토한 결과 2022년 이후부터는 현행 22일에서 근로기준법에서 정한 휴일을 계산하여 근로일수를 산정하는 것이 법령에 부합되는 것으로 나타났다. ③ 엔지니어링 대가 산정 시 임금조사와 노임단가 적용시점 간의 시간차이는 정부지침을 준용할 경우 과거 특정기간 노임단가 상승률로 보정하여 사용할 수 있는 것으로 검토되었다. ④ 분석결과 현행 엔지니어링 노임단가는 하도급 거래구조의 미 반영(4.1 %), 근로일수의 과다 계상(6.8 %~7.8 %), 과거의 임금적용(2.6 %)으로 적정 노임단가보다 13.5~14.5 % 낮았다. 본 연구에서 제시된 모델은 적정한 엔지니어링 대가를 산정할 수 있을 뿐만 아니라 유사 분야의 노임단가 산정 시 유용한 틀로 사용될 수 있어 정책 활용도가 높을 것으로 기대된다.

  • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70$\pm$1.32mmHg/min)compared to CF dialyzers(4.32$\pm$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The CF 15-11 HFDs which failed APD test did not show changes in the UFR compared to normal dialyzers indicating that APD is a more sensitive test than UFR test to evaluate the integrity of the fibers. 30527 T00401030527 ^x For quantitative measurement of reflected light from a clinical diagnostic strip, a prototype old reflectance photometer was designed. The strip loader and cassette were made to obtain more accurate reflectance parameters. The strip was illuminated at 45˚c through optical fiber and the intensity of reflected light was determined at rectanguLat angle using a photodiode. The kubelka-munk coefficient and reflection optical density were determined ar four different wavelengths(500, 550, 570 and 610nm) for blood glucose strip. The mice were divided into two groups. One group was injected with Photofrin II, intravenously (2.5mg/kg body weight). The other group received no Photofrin II. Mice from both groups were irradialed for approximately 15 minutes at 100, 300, or 500 mW/cm2 with the argon (488nm/514.5 nm), dye(628nm) and gold vapor (pulsed 628 nm) laser light. A photosensitizer behaved as an added absorber. Under our experimental conditions, the presence of Photolfrin II increased surface temperature by at least 40% and the temperature rise due to 300 mW/cm2 irradiation exceeded values for hyperthermia. Light and temperature distributions with depth were estimated by a computer model. The model demonstrated the influence of wavelength on the thermal process and proved to be a valuable tool to investigate internal temperature rise. 30536 T00401030536 ^x We investigated the structural geometry of thirty-eight Korean femurs. The purpose of this study is to identify major geometrical differences between Korean femurs 3nd others that we believe belong to Caucasians so that we would be able to get insights into the femoral component design that fits Asians including Koreans. We utilized computerized tomography (CT) images of femurs extracted from cadavers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. The results suggested that the femoral component suitable for Asians should be different from the currently-used components designed and manufactured mostly by European or American companies. 30537 T00401030537 ^x It is well known that nonlinear propagation characteristics of the wave in the tissue may give very useful information for the medical diagnoisis. In this paper, a new method to detect nonlinear propagation characteristics of the internal vibration in the tissue for the low frequency mechanical vibration by using bispectral analysis is proposed. In the method, low frequency vibration of f0( = 100Hz) is applied on the surface of the object, and the waveform of the internal vibration x (t) is measured from Doppler frequency modulation of silmultaneously transmitted probing ultrasonic waves. Then, the bispectra of the signal x (t) at the frequencies (f0, f0) and (f0, 2f0) are calculated to estimate the nonlinear propagation characteristics as their magnitude ratio, w here since bispectrum is free from the gaussian additive noise we can get the value with high S/N. Basic experimental system is constructed by using 3.0 MHz probing ultrasonic waves and the several experiments are carried out for some phantoms. Our department has developed an information system for routine hematology using down-sized computer system. We have used an IBM 486 compatible PC with 16MB main memory, 210 MB hard disk drive, 9 RS-232C port and 24 pin dot printer. The operating system and database management system were SCO UNIX and SCO foxbase, respectively. For program development, we used Xbase language provided by SCO foxbase. The C language was used for interface purpose. To make the system use friendly, pull-down menu was used. The system connected to our hospital information system via application program interface (API), so the information related to patient and request details is automatically transmitted to our computer. Our system interfaced with fwd complete blood count analyzers(Sysmex NE-8000 and Coulter STKS) for unidirectional data tansmission from analyzer to computer.   • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70${\pm}$1.32mmHg/min)compared to CF dialyzers(4.32${\pm}$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The purpose of this study is to identify major geometrical differences between Korean femurs 3nd others that we believe belong to Caucasians so that we would be able to get insights into the femoral component design that fits Asians including Koreans. We utilized computerized tomography (CT) images of femurs extracted from cadavers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. Then, the bispectra of the signal x (t) at the frequencies (f0, f0) and (f0, 2f0) are calculated to estimate the nonlinear propagation characteristics as their magnitude ratio, w here since bispectrum is free from the gaussian additive noise we can get the value with high S/N.
