  • Objective: The purpose of this retrospective study was to evaluate the appropriateness of various follicle-stimulating hormone (FSH) starting doses in expected normal responders based on the nomogram developed by La Marca et al. Methods: A total of 117 first in vitro fertilization cycles performed from 2011 to 2017 were selected. All women were expected normal responders and used a recombinant FSH and flexible gonadotropin-releasing hormone antagonist protocol. The FSH starting dose was empirically determined (150, 225, or 300 IU). The FSH starting dose indicated by La Marca's nomogram was determined using female age and serum $anti-M{\ddot{u}}llerian$ hormone or basal FSH levels. If the administered dose was exactly the same as the proposed dose, the cycle was assigned to the concordant group (34 cycles). If not, it was assigned to the discordant group (83 cycles). Optimal ovarian response was defined as a total of 8-14 oocytes, hypo-response as < 8 oocytes, and hyper-response as > 14 oocytes. Results: Between the concordant and discordant group, ovarian response (optimal, 32.4% vs. 27.7%; hypo-response, 55.9% vs. 54.2%; and hyper-response, 11.8% vs. 18.1%) and the number of total or mature oocytes were similar. Ovarian hyperstimulation syndrome was rare in both groups (0% vs. 1.2%). The implantation rate, clinical pregnancy rate, miscarriage rate, and live birth rate were all similar. Conclusion: The use of the proposed FSH starting dose determined using La Marca's nomogram did not enhance the optimal ovarian response rate or pregnancy rate in expected normal responders. Individualization of the FSH starting dose by La Marca's nomogram appears to have no distinct advantages over empiric choice of the dose in expected normal responders.

  • Objective: To investigate correlations between adenosine triphosphate chemotherapy response assay (ATP-CRA) and clinical outcomes after ATP-CRA-based chemotherapy for drug selection in patients receiving intravesical chemotherapy to prevent recurrence of superficial bladder cancer after surgery. Methods: The chemosensitivities of 12 anticancer drugs were evaluated, including 5-Fu ADM, and EPI, using ATP-CRA and primary tumor cell culture in 54 patients. In addition, a further 58 patients were treated according to clinical experience. Differences in post-chemotherapeutical effects between drug sensitivity assay and experience groups were compared. Results: The evaluable rate of the test was 96.3%, the clinical effective rate was 80.8%, the sensitivity rate was 97.6% (41/42), the specificity was 20%, the total predicting accuracy was 74.3%, the positive predictive value was 83.7% (41/49), the negative predictive value was 66.7% (2/3); in the drug sensitivity test group, the clinical effective rate was 80.8%, the experience group response rate was 63.8%, with a significant difference in clinical effects between the ATP-based sensitivity and experience groups (${\chi}^2$=7.0153, P<0.01). Conclusion: ATP-CRA is a stable, accurate and potentially practical chemosensitivity test providing a predictor of chemotherapeutic response in patients with superficial bladder cancer.

  • Site response analysis is widely used in estimating local seismic site effects. The soil behavior in the analysis is assumed to be Independent of the rate of the seismic loading laboratory results, however, indicate that cohesive soil behavior is greatly influenced by the rate of loading. A new equivalent linear analysis method is developed that accounts for the rate-dependence of soil behavior and used to perform a series of one dimensional site response analyses. Results indicate that while rate-dependent shear modulus has limited influence on computed site response, rate-dependent soil damping greatly filters out high frequency components of the ground motion and thus results in lower response.

  • A Large Eddy Simulation (LES) of the flow in an inducer is carried out under flow rate oscillations. The present study focuses on the dynamic response of the backflow and the unsteady pressure performance to the flow rate fluctuations under non-cavitation conditions. The amplitude of angular momentum fluctuation evaluated by LES is larger than that evaluated by RANS. However, the phase delay of backflow is nearly the same as RANS calculation. The pressure performance curve exhibits a closed curve caused by the inertia effect associated with the flow rate fluctuations. Compared with simplified one dimensional evaluation of the inertia component, the component obtained by LES is smaller. The negative slope of averaged performance curve becomes larger under unsteady conditions. From the conservations of angular momentum and energy, an expression useful for the evaluation of unsteady pressure rise was obtained. The examination of each term of this expression show that the apparent decrease of inertia effects is caused by the response delay of Euler's head and that the increase of negative slope is caused by the delay of inertial term associated with the delay of backflow response. These results are qualitatively confirmed by experiments.

  • Early extra-uterine development of the newborn infant has the potential for clarifying important questions of nature and care taking environment. In this study, tactile and auditory stimulation is used on .a group of infants to determine the effects of their response. Infants were tested by EKG at 24∼48 hrs after birth ana all they are free from complication. Stimuli presented were-touch and sound (40dB, 70dB and 40∼70dB). The result of the study were as follows: 1) The heart rate was seen to depend on their sleep-wake state. (light sleep ; 125/min, eyes: open & motor activity increase ; 151/min crying ; 185/min). 2) The heart rate affected by tactile & auditory stimulus. The heart rate response was acceleration above the prestimulus level. (tactile, 231min increase, auditory ; 16∼21/min increase), and heart rate change was seen to depend on prestimulus rate. (γ=.73) 3) There was a decrease in heart rate when they were soothe. (touch ; 24.8sec, 21/min decrease, pacifier ; 14.5sec, 26/min decrease) and significant difference in time between touch and pacifier (t= 1.76, p<0.05). This result support the evaluation of newborn infant's sensory abilitics, and has even begin to influence changes in nursing care. But systemic monitoring of newborn infants response would. further define.

  • Purpose: The aim of this study was to examine the effects of a yoga program on perceived stress, stress response and heart rate variability in professionals. Methods: The research design was a quasi-experimental intervention study. The participants were 47 adults with professional jobs consisting of an experimental group with 23 adults and a control group with 24 adults. The duration of the yoga program was eight weeks from July 4 to August 30, 2012. Results: There were significant decreases in the stress response score after the yoga program. No significant changes in perceived stress and heart rate variability were observed between the experimental group and control group. However, there were significant decreases in perceived stress, stress response scale, and heart rate variability for some participants whose stress levels were high. Conclusion: An eight-week-long yoga program could be considered as an intervention to decrease the stress response of professionals even though it might not reduce their perceived stress and heart rate variability. To further analyze the effects of a yoga program in professionals, changes in intervention strength (such as longer periods and frequency) and the development of physiological measures, which would show the co-relationship between perceived stress and physiological response, are necessary in future studies.

  • This paper presents a new uniaxial material model for rate-sensitive analysis addressing both the transient and steady-state responses. The new model adopts visco-plastic theory for the rate-sensitive response, and employs a three-parameter representation of the overstress as a function of the strain-rate. The third parameter is introduced in the new model to control its transient response characteristics, and to provide flexibility in fitting test data on the variation of overstress with strain-rate. Since the governing visco-plastic differential equation cannot be integrated analytically due to its inherent nonlinearity, a new single-step numerical integration procedure is proposed, which leads to high levels of accuracy almost independent of the size of the integration time-step. The new model is implemented within the nonlinear analysis program ADAPTIC, which is used to provide several verification examples and comparison with other experimental and numerical results. The companion paper extends the three-parameter model to trilinear static stress-strain relationships for steel and concrete, and presents application examples of the proposed models.

  • The response time represents how fast a system responds to a given disturbance at the system boundary. Flow restricting devices for controlling transients can result in a decrease in the peakm pressure, but may change response time. Response lag in a hydraulic system leads to inefficient working cycle and operator discomfort. The experiments were conducted in order to get information on the parameters which exert appreciable influence on the response time. The experimental apparatu including a hydraulic actuator, orifice and a hydraulic pump was an idealization of a bucket hydraulic shifting system. Experimental results show that the response time depends on operating pressure and flow rate. The effects of orifice type and size on the response time are quantified.
