Systematic Analysis of Icotinib Treatment for Patients with Non-Small Cell Lung Cancer

  • Purpose: This analysis was conducted to evaluate the efficacy and safety of icotinib based regimens in treating patients with non-small cell lung cancer (NSCLC). Methods: Clinical studies evaluating the efficacy and safety of icotinib-based regimens with regard to response and safety for patients with NSCLC were identified using a predefined search strategy. Pooled response rates of treatment were calculated. Results: With icotinib-based regimens, 7 clinical studies which including 5,985 Chinese patients with NSCLC were considered eligible for inclusion. The pooled analysis suggested that, in all patients, the positive reponse rate was 30.1% (1,803/5,985) with icotinib-based regimens. Mild skin itching, rashes and diarrhea were the main side effects. No grade III or IV renal or liver toxicity was observed. No treatment-related death occurred in patients treated with icotinib-based regimens. Conclusions: This evidence based analysis suggests that icotinib based regimens are associated with mild response rate and acceptable toxicity for treating Chinese patients with NSCLC.

Cantharidin Combined with Chemotherapy for Chinese Patients with Metastatic Colorectal Cancer

  • Background: This systematic analysis was conducted to evaluate the efficacy and safety of cantharidin combined with chemotherapy in treating Chinese patients with metastatic colorectal cancer. Methods: Clinical studies evaluating the efficacy and safety of cantharidin combined with chemotherapy on response and safety for Chinese patients with colorectal cancer were identified using a predefined search strategy. Pooled response rate (RR) of treatment were calculated.Results: When cantharidin combined with chemotherapy, 4 clinical studies which included 155 patients with advanced colorectal cancer were considered eligible for inclusion. The systematic analysis suggested that, in all patients, pooled RR was 46.5% (72/155) in cantharidin combined regimens. Major adverse effects were neutropenia, leukopenia, fatigue, and anemia with cantharidin combined treatment; no treatment related deaths occurred. Conclusion: This systematic analysis suggests that cantharidin combined regimens are associated with high response rate and accepted toxicity in treating Chinese patients with metastatic colorectal cancer suggesting that randomized clinical trials are now warranted.

A Novel WBAN MAC protocol with Improved Energy Consumption and Data Rate

  • Wireless Body Area Networks (WBANs) are introduced as an enabling technology in tele-health for patient monitoring. Designing an efficient Medium Access Control (MAC) protocol is the main challenge in WBANs because of their various applications and strict requirements such as low level of energy consumption, low transmission delay, the wide range of data rates and prioritizing emergency data. In this paper, we propose a new MAC protocol to provide different requirements of WBANs targeted for medical applications. The proposed MAC provides an efficient emergency response mechanism by considering the correlation between medical signals. It also reduces the power consumption of nodes by minimizing contention access, reducing the probability of the collision and using an efficient synchronization algorithm. In addition, the proposed MAC protocol increases the data rate of the nodes by allocating the resources according to the condition of the network. Analytical and simulation results show that the proposed MAC protocol outperforms IEEE 802.15.4 MAC protocol in terms of power consumption level as well as the average response delay. Also, the comparison results of the proposed MAC with IEEE 802.15.6 MAC protocol show a tradeoff between average response delay and medical data rate.

Sensitivity Analysis of Dynamic Response by Change in Excitation Force and Cross-sectional Shape for Damped Vibration of Cantilever Beam (가진력과 단면형상 변화에 따른 외팔보 감쇠 진동의 민감도 해석)

  • This paper describes the time rate of change of dynamic response of a cantilever beam inserted with a damping element, such as bonding, which is excited under a general force at various locations. A sensitivity analysis was performed in a finite element model to show that two types of second-order algebraic governing equations were used to predict the rate of change of dynamic displacement: one is related to the modal coordinate linked to a physical coordinate, and the other to the design parameter of the time rate of change of displacement. The sensitivity differential equation formulation includes more complicated terms compared with that of the undamped cantilever beam. The sensitivities of the dynamic response were observed by changing the location of the excitation force, displacement extraction, and cross-sectional area of the beam. The analytical results obtained by this suggested theory showed a relatively good agreement when compared with those obtained using the commercial finite element program. The suggested analysis procedure enables the prediction of the response sensitivity for any finite element model of the dynamic system.

Single nucleotide polymorphism of GSTP1 and pathological complete response in locally advanced rectal cancer patients treated with neoadjuvant concomitant radiochemotherapy

  • Purpose: Standard treatment for locally advanced rectal cancer consists of neoadjuvant radiochemotherapy with concomitant fluoropyrimidine or oxaliplatin and surgery with curative intent. Pathological complete response has shown to be predictive for better outcome and survival; nevertheless there are no biological or genetic factors predictive for response to treatment. We explored the correlation between the single nucleotide polymorphisms (SNPs) GSTP1 (A313G) and XRCC1 (G28152A), and the pathological complete response and survival after neoadjuvant radiochemotherapy in locally advanced rectal cancer patients. Materials and Methods: Genotypes GSTP1 (A313G) and XRCC1 (G28152A) were determined by pyrosequencing technology in 80 patients affected by locally advanced rectal cancer. Results: The overall rate of pathological complete response in our study population was 18.75%. Patients homozygous AA for GSTP1 (A313G) presented a rate of pathological complete response of 26.6% as compared to 8.5% of the AG+GG population (p = 0.04). The heterozygous comparison (AA vs. AG) showed a significant difference in the rate of pathological complete response (26.6% vs. 6.8%; p = 0.034). GSTP1 AA+AG patients presented a 5- and 8-year cancer-specific survival longer than GSTP1 GG patients (87.7% and 83.3% vs. 44.4% and 44.4%, respectively) (p = 0.014). Overall survival showed only a trend toward significance in favor of the haplotypes GSTP1 AA+AG. No significant correlations were found for XRCC1 (G28152A). Conclusion: Our results suggest that GSTP1 (A313G) may predict a higher rate of pathological complete response after neoadjuvant radiochemotherapy and a better outcome, and should be considered in a more extensive analysis with the aim of personalization of radiation treatment.

Capturing the Short-run and Long-run Causal Behavior of Philippine Stock Market Volatility under Vector Error Correction Environment

  • This study investigates the short-run and long-run causal behavior of the Philippine stock market index volatility under vector error correction environment. The variables were tested first for stationarity and then long-run equilibrium relationship. Moreover, an impulse response function was estimated to examine the extent of innovations in the independent variables in explaining the Philippine stock market index volatility. The results reveal that the volatility of the Philippine stock market index exhibit long-run equilibrium relationship with Peso-Dollar exchange rate, London Interbank Offered Rate, and crude oil prices. The short-run dynamics-based VECM estimates indicate that in the short-run, increases (i.e., depreciation) in Peso-Dollar exchange rate cause PSEI volatility to increase. As for the London Interbank Offered Rate, it causes increases in PSEI volatility in the short-run. The adjustment coefficients used with the long-run dynamics validates the presence of unidirectional causal long-run relationship from Peso-Dollar exchange rate, London Interbank Offered Rate, and crude oil prices to PSEI volatility, and bidirectional causal long-run relationship between PSEI volatility and London Interbank Offered Rate. The impulse response functions developed within the VECM framework demonstrate the positive and negative reactions of PSEI volatility to unanticipated Peso-Dollar exchange rate, London Interbank Offered Rate, and crude oil price shocks.

  • This paper presents sliding mode control(SMC) method using target variation rate of Lypunov Function. SMC keeps the response of structure in sliding surface where structure is stable. It can design both linear controller and bang-bang controller. Linear control of previous research, however, can not make most of the performance of controller, because it is designed to satisfy the condition that the variation rate of Lyapunov function is minus. Also, incase of bang-bang controller, unnecessary large control force is generated. Presented method can utilize the capacity of controller efficiently by prescribing the target variation rate of Lyapunov function. Numerical simulation results indicate that the presented control methods can reduce the peak response larger than linear control, and it has control performance equivalent to bang-bang control.

A Direct Injection-mixing Total-flow-control Boom Sprayer System (주입식 총유량 자동제어방식 분관 방제기의 개발)

  • A direct injection sprayer was designed using the concepts of injection mixing and total flow control, flowrate-based system compensating for the variation of forwarding speed. A metered rate, proportionally to the actual diluent flow rate, of a tracer chemical was injected directly into the diluent stream. The injection of chemical may improve the precision and safety of chemical application process. The control system was evaluated for the variables of the control interval, tolerances and sensitivities of flow regulation valve and injection pump. Performance of the system was assessed as that the response time of flow rate, response time of injection rate, absolute steady state error, and the coefficient of variance(C.V.) of concentration were 8.5 and -0.53 seconds, 0.067 lpm(0.8%) and 3.15%, respectively, at optimal parameters of control interval of 1.0 sec, fast sensitivity of flow regulation valve, medium sensitivity of injection pump and medium tolerance of flow rate. Performance of the system can be improved by increasing the sensitivity of flow regulating valve and employing a high resolution velocimeter, such as Doppler radar.

Optimization of Shroud Shape and Fan Location for Increasing Exhaust Flow Rate of Air Conditioner Outdoor Unit (실외기 토출 유량 증대를 위한 Shroud 형상 및 휀 위치 최적 설계)

  • This paper presents a numerical evaluation of the flow rate of air conditioner outdoor unit by investigating the effects of fan location and shroud shape. To determine optimal design parameters, we investigated the exhaust flow rate by changing shroud height, fan height, fan guide height, and fan width. The 3rd order central composite design was performed to select three most important parameters affecting the exhaust flow rate. According to the result of response surface method, the exhaust flow rate of the optimum model increased by 6.25% compared to that of the base model.

  • The study estimates the Structured VAR and the Dynamic Stochastic General Equilibrium Model for the Vietnamese economy based on the new Keynesian model for small and open economies, with the output gap, inflation, policy interest rate, the Vietnamese exchange rate, and the inflation and interest rate in the United States. The paper aims to clarify the impulse response of the macro variables through their shocks. It offers to model the SVAR and DSGE processes, as well as describe why and how interest rate policy is important in the impulse response of macro variables like the output gap and inflation process. The study supports the central role of monetary policy by giving empirical evidence for the new Keynesian theory, according to which an interest rate shock causes the output gap to widen and inflation to decrease. Finally, the application of the DSGE model is becoming more and more popular in the State Bank of Viet Nam to improve its policy planning, analyzing, and forecasting policy towards sustainable and stable growth.
