마황윤폐탕과 ciprofloxacin의 병용투여가 Streptococcus pneumoniae 호흡기감염에 대한 항균력에 미치는 영향 (Investigation of in vivo Synergism of Mawhangyounpye-tang, a Poly-herbal Formula and Ciprof1oxacin against Streptococcus Pneumoniae Respiratory Infection)

  • Objectives & methods; In order to evaluate the in vivo synergic effect of Mawhangyounpye-tang, a traditional poly-herbal formula used in the treatment of respiratory diseases in Korea, with the quinolone antibiotic ciprofloxacin (CPFX), the viable bacterial number and histopathological changes were monitored after experimental respiratory infection with Streptococcus Pneumoniae ATCC 6303. Results: 1. In CPFX groups, the viable bacterial numbers were significantly decreased compared to that of the control group, and were even more dramatically decreased in concomitant group treated with Mawhangyounpye-tang. 2. In the control group, severe infiltration of inflammatory cells, hemorrhage and hypertrophy of alveolar linings were demonstrated at microscopic levels. However, these abnormal histopathological changes were significantly decreased compared. to that of the control group in CPFX groups, and were even more dramatically decreased in concomitant groups treated with Mawhangyounpye-tang. 3. In CPFX groups, the LSA (Iuminal surface of alveoli $\%$) were significantly increased compared to that of the control group, and more dramatically in concomitant groups treated with Mawhangyounpye-tang. Conclusions: According to these results, it is considered that the in vivo antibacterial activity of CPFX against Streptococcus Pneumoniae ATCC 6303 infection of respiratory tract was dramatically increased by concomitant use of Mawhangyounpye-tang.

Application of respiratory function tests in patients with neurological diseases

  • Breathing is controlled by complex interactions between the central and peripheral nervous systems in conjunction with the respiratory system. Neurological diseases predispose patients to nocturnal desaturation and pneumonia due to respiratory dysfunction, which increases mortality, daytime sleepiness and fatigue, and reduces the quality of life. Respiratory function tests are required to identify respiratory function decline and to consider compensatory management. This review summarizes the characteristics of several respiratory function tests and their applications to neurological diseases.

  • Objective: The purpose of this study was to develop a standard tool for pattern identification of radiation pneumonitis. Methods: Textbooks, published studies, and references with comments about patterns were reviewed. Through the Delphi method, we determined pattern identifications based on advice from a committee of experts composed of 13 Korean respiratory internal medicine professors. Results: Using the Delphi method, four pattern identifications were chosen: Qi Deficiency (氣虛), Yin Deficiency (陰虛), Heat Toxin (熱毒), and Phlegm Dampness (痰濕). The tool was developed in a question-and-answer format with 35 questions. Conclusions: A pattern identification tool that can discriminate the patterns of radiation pneumonitis for standardized diagnosis was developed through expert consultation. Further study of its validity and reliability is necessary.

호흡동조 방사선치료 시 호흡유도시스템의 유용성 평가 (Evaluation of the Usefulness of the Respiratory Guidance System in the Respiratory Gating Radiation Therapy)

  • 목 적: 호흡동조방사선치료(Respiratory Gating Radiation Therapy, RGRT)에서 호흡의 안정성은 매우 중요한 인자이다. 이러한 호흡의 안정을 위해 본인의 호흡주기를 직접 확인할 수 있도록 시청각시스템을 이용한 호흡유도시스템을 개발하였고 이의 유용성을 평가하고자 하였다. 대상 및 방법: 2011년 6월부터 2012년 4월까지 본원에서 호흡동조방사선치료를 받은 7명의 환자를 대상으로 시청각시스템을 이용하지 않는 자유호흡을 먼저 측정하고 자체개발한 호흡유도시스템을 이용한 호흡을 측정하였다. 시청각시스템을 이용한 호흡연습 후에는 치료실내에서의 자가호흡과 시청각시스템을 이용한 호흡을 각각 측정하였다. 측정된 데이터는 호흡주기, 호흡함수의 면적을 구하여 표준편차를 구하였으며, 이를 분석하여 치료전후의 호흡변화를 알아 보았다. 결 과: 자유호흡과 오디오 유도시스템, 시청각 유도시스템의 표준편차는 PTP (peak to peak)가 각각 0.343, 0.148, 0.078이다. 호흡주기는 각각 0.645, 0.345, 0.171이며, 호흡함수의 면적은 각각 2.591, 1.008, 0.877로 나타났다. 전체 환자의 CT실과 치료실에서의 차이를 평균한 값은 PTP가 0.425, 호흡주기가 1.566, 호흡면적이 3.671로 측정되었다. 호흡유도시스템 적용전후의 표준편자는 PTP가 0.265, 호흡주기가 0.474, 호흡면적이 1.714의 차이를 나타내었다. 자유호흡과 시청각유도시스템 적용전후의 값을 T-검정한 결과에서는 PTP, 주기, 호흡함수면적에서 각각 P-value 0.035, 0.009, 0.010의 값을 나타냈다. 결 론: 호흡동조방사선치료에서 호흡조절은 치료의 성패를 좌우할 만큼 중요한 인자이다. 자유호흡이나 청각에 의존한 호흡주기 획득에 비해 시청각 호흡유도 시스템을 이용한 경우에 보다 안정적인 호흡을 얻을 수 있었다. 특히, 치료실에서도 같은 시스템을 이용하여 호흡을 조절함으로써 호흡주기의 재현성이 뛰어났다. 이러한 시스템은 호흡불안정에 의한 치료시간의 지연을 줄이고 좀 더 정확하고 정밀한 치료가 가능하게 되었다.

환자고유의 호흡 패턴을 적용한 호흡 연습장치 개발 및 유용성 평가 (Development of Respiratory Training System Using Individual Characteristic Guiding Waveform)

  • 본 연구에서는 환자 고유의 호흡 패턴을 적용하여 호흡의 규칙성을 향상 시킬 수 있는 호흡 연습장치(respiratory training system)를 개발하여, 호흡에 의한 움직임이 고려된 4D-RT (4-dimension radiation therapy) 또는 4D-CT (4-dimension computed tomography) 수행 시 효율성과 정확성을 높이고자 했다. 개발한 호흡연습장치는 푸리에 급수(Fourier series)를 기반으로 환자 고유의 호흡패턴을 만들어 환자에게 편안한 호흡 유도를 제공한다. 호흡연습장치를 사용했을 때 호흡의 규칙성 향상 정도를 알아보기 위하여 5명의 지원자를 대상으로 실험을 진행하였다. 10개의 자유호흡신호를 획득하여 실험 대상자의 고유한 호흡패턴(guiding waveform)을 만들고, 자유호흡(free breathing)을 3분 동안 시행한 후, 고유한 호흡패턴을 이용하여 호흡을 유도하는 신호모니터-호흡(guide breathing)을 3분 동안 시행하여 데이터를 획득하였다. 획득된 자유호흡과 신호모니터-호흡의 데이터를 이용하여 호흡크기(displacement)와 호흡주기(period)의 변동성을 Root mean square error (RMSE)를 적용하여 정량적으로 비교 분석하였다. 호흡의 변동성을 분석한 결과 신호모니터-호흡은 자유호흡과 비교하여 호흡크기의 경우 최대 40%, 호흡주기의 경우 최대 76%까지 RMSE 값이 감소하였으며, 모든 지원자들의 데이터를 분석한 결과 평균적으로 호흡주기의 경우 RMSE 값이 55% 감소되었고, 호흡크기의 경우 33% 감소하였다. 본 연구에서 개발한 호흡연습장치는 실험대상자의 고유한 호흡패턴을 이용하여 규칙적인 호흡을 유도했기 때문에 피실험자는 큰 노력 없이도 신호모니터-호흡을 따라 할 수 있었다. 따라서 규칙적인 호흡을 오랜 시간 지속시킬 수 있다는 측면에서 장점을 가질 수 있으며, 4D RT, 4D CT를 시행 할 경우 규칙적인 호흡을 통해 효율성과 정확성을 향상 시킬 수 있다.

Sweet Bee Venom 시술이 Rat의 호흡기계에 미치는 영향 (Effects of Sweet Bee Venom on the respiratory system in Rats)

  • Objectives: This study was performed to analyse the effects of Sweet Bee Venom(SBV-purified melittin supported by G&V Co., the major component of honey bee venom) on the respiratory system in rats. Methods: All experiments were conducted at Biotoxtech Company, a non-clinical studies authorized institution, under the regulations of Good Laboratory Practice(GLP). Male rats of 5 weeks old were chosen for this study and after confirming condition of rats was stable, SBV was administered in thigh muscle of rats in 0.175, 0.35 and 0.7 mg/kg dosage. And checked the effects of SBV on the respiratory system using the whole body plethysmography. And respiratory rate, tidal volume and minute volume of rats were checked after administered SBV (melittin). Results: 1. In the measurement of respiratory rate, there were not observed any significant differences compared with control group. 2. In the measurement of tidal volume, there was not observed any significant differences compared with control group. 3. In the measurement of minute volume, 0.35mg/kg dosage group showed significant differences compared with control group. But we estimated that this result was caused by individual differences. Conclusions: Above findings suggest that SBV seems to be safe treatment in the respiratory system of rats. And further studies on the subject should be conducted to yield more concrete evidences.

항생제 치료에도 반복되는 기관지확장증 환자의 기관지폐렴에 대한 한양방 복합 치험 1례 (Recurrent Bronchopneumonia in Bronchiectasis, Despite Antibiotic Treatment: A Case Report on Combined Treatment with Korean and Western Medicine)

  • Background: Bronchiectasis is a chronic respiratory condition leading to recurrent respiratory infections. Despite the use of antibiotics and other standard treatments, managing bronchiectasis remains challenging due to the frequent recurrence of airway infections and concerns about antimicrobial resistance. Given these challenges, traditional Korean medicine (TKM) has gained attention due to its potential to reduce the frequency of respiratory infections, possibly minimizing the need for antibiotics. Case report: A 59-year-old female with bronchiectasis experienced recurrent pneumonia and was treated with antibiotics for over 2 weeks without any significant improvement in clinical symptoms. She received comprehensive Korean medicine treatment, including herbal medicine (Sikyungbanha-tang combined with Bigwabojungikki-tang-gami), acupuncture, and Chuna manual therapy, for pulmonary rehabilitation. Post-treatment, clinically meaningful improvements were observed in symptoms, serum C-reactive protein (CRP) levels, and bronchopneumonic lesions on chest X-rays. Conclusion: This case suggests that complex traditional Korean medicine treatments for recurrent chronic airway inflammation due to bronchiectasis can lead to clinically significant improvements in symptoms and help to prevent recurrence.

  • Objective: This study reports on traditional Korean medicine therapy used for pneumonia among elderly patients.Method: Two patients diagnosed with pneumonia were treated with herbal medicine and acupuncture, as well as cupping along the back. We checked chest X-rays, coughing and sputum on the visual analogue scale (VAS), and lab evaluations in order to evaluate the effectiveness of the treatment.Results: Following treatment with traditional Korean medicine therapy, VAS scores for coughing and sputum decreased and the chest X-rays and inflammation markers improved.Conclusion: Traditional Korean medicine therapy treatment appears to be effective for treating pneumonia in elderly patients.
