만성폐쇄성폐질환 외래환자에서 자기효능감증진 호흡재활프로그램의 효과 (The Effects of Self-Efficacy Promoting Pulmonary Rehabilitation Program in Out-Patients with Chronic Obstructive Pulmonary Disease)

  • 배 경: 만성폐쇄성폐질환의 여러 가지 호흡재활프로그램 중에서 자기효능감을 증진시키는 것이 호흡재활치료에 효과적이라는 국외 보고는 있으나 국내에서는 연구가 시도되지 않았다. 저자들은 만성폐쇄성폐질환자에서 자기효능감증진 호흡재활프로그램의 효과를 알아보기 위하여 이 연구를 하였다. 대상 및 방법: 대상은 호흡기내과 외래에서 진료 받는 만성폐쇄성폐질환자 중 GOLD분류에 의한 II,III 병기 환자로 하였으며, 치료군 18명, 대조군 18명으로 선정, 비교하였다. 연구는 무작위할당 대조군 전-후연구로 이루어졌다. 저자들이 개발한 자기효능감증진 호흡재활프로그램을 8주 동안 실시하였으며 치료군은 실시 전, 4주, 8주째는 1:1의 교육과 상담이 이루어졌고 전화상담은 매주 1~2회 이루어졌다. 교육과 상담으로 대리경험, 성취경험 등이 이루어지도록 하였고 매일 가정에서 운동훈련과 이완요법을 수행하도록 하여 성취경험과 정서적 이완이 이루어지도록 하였다. 대조군은 프로그램 실시 전에 1:1의 교육과 상담만 제공하였다. 평가는 자기효능감, 평소 호흡곤란, 운동유발호흡곤란, 운동내구성은 6분 걷기 거리로, 폐기능, 삶의 질 등을 양 군에서 측정 비교하였다. 결 과: 자기효능감증진 호흡재활프로그램은 대조군보다 치료군에서 유의하게 자기효능감이 향상되었다. 평상 시 호흡곤란, 운동 시 호흡곤란, 6분 걷기 거리도 치료군에서 유의하게 향상되었다. 그러나 폐기능은 유의한 차이가 없었다. 자기효능감증진 호흡재활프로그램은 치료군에서 삶의 질도 유의하게 향상되었다.. 결 론: 만성폐쇄성폐질환자에서 자기효능감증진 호흡재활프로그램은 자기효능감, 운동내구성, 삶의 질을 증가시키고 운동유발 호흡곤란 정도를 감소시키는 효과가 있었으며, 자기효능감증진 호흡재활프로그램은 만성폐쇄성폐질환자의 호흡재활치료로 효과적으로 활용될 수 있겠다.

대사질환자의 모려 온열요법이 대사질환 관련 변인과 면역 및 호흡건강 관련 변인에 미치는 영향 (Effect of Oyster Shell Thermal Therapy on Metabolic Disease Risk Factors, Respiratory Health and Immune-Related Variables in Patients with Metabolic Diseases)

  • Purpose : In this study, using Oyster Shell Thermal Therapy for metabolic diseases, we analyzed the effect of immune and inflammation-related variables and respiratory health-related variables of test subjects to verify the effect of improving respiratory health. Methods : In this study, 26 patients with metabolic diseases were divided into an experimental group (N=13) and a control group (N=13). After Oyster Shell Thermal Therapy (four weeks/three times a week/1 hour per time), metabolic disease-related variables and immune and respiratory health-related variables were measured and compared between the two groups. The conclusion of this study is as follows: Results : After the four-week Oyster Shell Thermal Therapy, in terms of changes in the metabolic disease-related variables, the control group exhibited a higher increase in TC and LDL-C levels than the experimental group. In the case of glucose, the experimental group showed a decrease after the experiment (p<.05). After the four-week thermotherapy, a statistically significant interactive effect occurred in natural killer (NK) cells among the immune-related variables. According to the results of a post-experimental analysis, the control group showed a higher decrease in NK cells than the experimental group (p<.05). After the 4-weeks thermotherapy, the experimental group showed a greater increase in maximum oxygen intake of the respiratory health-related variables than the control group. Conclusion : Based on a comprehensive review of the study results, the subjects who underwent the four-week Oyster Shell Thermal Therapy exhibited positive physical changes in metabolic disease-related variables as well as immune and respiratory health-related variables, which demonstrates the effectiveness of Oyster Shell Thermal Therapy on immune and respiratory health. Accordingly, it is recommended to conduct long-term Oyster Shell Thermal Therapy with various models in terms of the size and shape.

불안정한 지지면에서의 균형운동이 건강한 성인의 폐활량에 미치는 효과: 사전 연구 (Effect of the Balance Exercise on the Unstable Surfaces for the Vital Capacity in Healthy Adults: A Preliminary Study)

  • Purpose : This study attempts to examine the effect of the balance exercise on the unstable surfaces for the vital capacity in healthy adults. Methods : A total of 13 subjects was randomly divided into a breathing exercise training group (n=7) and a breathing and balance exercise training group (n=6). Changes in vital capacity and respiratory muscle activity were measured before and after the intervention. The intervention was performed three times a week for four weeks. Forced vital capacity (FVC), forced expiratory volume at one second (FEV1) and maximal voluntary ventilation (MVV) were used as measurement tools for the vital capacity test. Electromyography (EMG) was also used to examine respiratory muscle activity. Result : The breathing exercise training group showed significant improvement in terms of FVC, FEV1 and external oblique (EO), and transverse abdominis/internal oblique (TrA/IO) of MVV. The breathing and balance exercise training group showed significant improvement in terms of FVC, MVV and EO, TrA/IO of FVC and rectus abdominis (RA), EO, and TrA/IO of MVV. However, in comparing changes in vital capacity and respiratory muscle activity before and after the training, the breathing exercise training group and the breathing and balance exercise training group showed a significant difference in terms of MVV. Conclusion : This study is as a preliminary study to find out the relation between a balance exercise and a vital capacity, it is considered to require a further study with several revisions of subjects, duration and time for an intervention.

The Comparison of Clinical Variables in Two Classifications: GOLD 2017 Combined Assessment and Spirometric Stage of Chronic Obstructive Pulmonary Disease

  • Background: There are limited number of studies that investigate clinical variables instead of chronic obstructive lung disease (COPD) management according to Global Initiative for Chronic Obstructive Lung Disease (GOLD) 2017 classification. The aim of the study was to investigate whether there was a difference between GOLD 2017 classification and spirometric stage in clinical variables in patients with COPD. The data of 427 male patients with stable COPD were investigated retrospectively. Methods: Patients were allocated into combined assessment of GOLD 2017 and spirometric stage. Age, amount of smoking, pulmonary function, modified Medical Research Council (mMRC), incremental shuttle walk test (ISWT), Hospital Anxiety-Depression Scale (HADS), St. George's Respiratory Questionnaire (SGRQ), body mass index (BMI), and fat free mass index (FFMI) were recorded. Results: Seventy-three (17%) patients were in group A, 103 (24%) constituted group B, 38 (9%) were included in group C, and 213 (50%) comprised group D according to the combined assessment of GOLD 2017. Twenty-three patients (5%) were in stage 1, 95 (22%) were in stage 2, 149 (35%) were in stage 3, and 160 (38%) were in stage 4 according to spirometric stage. According to GOLD 2017, age, amount of smoking, mMRC, BMI, FFMI, SGRQ, HADS, forced vital capacity, forced expiratory volume in 1 second ($FEV_1$), and ISWT were significantly different between groups. Ages, amount of smoking, FFMI, BMI, HADS of group A were different from B and D. Smiliar values of $FEV_1$ were found in A-C and B-D. A and C had smiliar ISWT. According to spirometric stage, BMI, FFMI of stage 4 were statistically different. mMRC, ISWT, and SGRQ of stages 3 and 4 were different from other stages, amongst themselves. $FEV_1$ was correlated with mMRC, SGRQ, anxiety scores, BMI, FFMI, and ISWT. Conclusion: This study showed that the GOLD ABCD classification might not represent the severity of COPD sufficiently well in terms of lung function or exercise capacity. The combination of both spirometric stage and combined assessment of GOLD 2017 is important, especially for estimating clinical variables.

The Effects of Simultaneous Pulmonary Rehabilitation during Thoracic Radiotherapy in the Treatment of Malignant Diseases

  • Background: Radiotherapy is a common treatment option for lung or esophageal cancer, particularly when surgery is not feasible for patients with poor lung function. However, radiotherapy can affect pulmonary function and thereby induce pneumonitis or pneumonia, which can be fatal in patients with respiratory impairment. The purpose of this study is to evaluate if reductions in pulmonary function after radiotherapy can be minimized through simultaneous pulmonary rehabilitation (PR). Methods: In this matched case control study, we retrospectively analyzed patients who had undergone radiotherapy for thoracic malignant disease between January 2018 and June 2019. We analyzed results from pulmonary function tests and 6-minute walking tests (6MWT) conducted within the six months before and after radiotherapy treatment. Results: In total, results from 144 patients were analyzed, with 11 of the patients receiving PR and radiotherapy simultaneously. Of the 133 patients in the control group, 33 were matched with 11 patients in the PR group. Changes in forced expiratory volume in one second (FEV1) and FEV1/forced vital capacity were significantly different between the PR group and the matched control group (240 mL vs. -10 mL, p=0.017 and 5.5% vs. 1.0%, p=0.038, respectively). The median distance of 6MWT in the PR group also increased significantly, from 407.5 m to 493.0 m after radiotherapy (p=0.017). Conclusion: Simultaneous PR improved pulmonary function, particularly in measures of FEV1, and exercise capacity for patients with lung or esophageal cancer even after radiotherapy treatment. These findings may provide an important base of knowledge for further large population studies with long-term follow-up analysis in the identification of the PR's effects during thoracic radiotherapy.

입원 호흡재활 프로그램이 만성 폐질환자의 호흡곤란, 운동능력과 건강관련 삶의 질에 미치는 효과 (Effects of an Inpatient Pulmonary Rehabilitation Program on Dyspnea, Exercise Capacity, and Health Related Quality of Life for Patients with Chronic Lung Disease)

  • Purpose: The purpose of this study was to examine the effects of an inpatient pulmonary rehabilitation program on dyspnea, exercise capacity, and health related quality of life in inpatients with chronic lung disease. Method: This quasi experimental study was designed with a nonequivalent control group pre-post test time series. Twenty three patients were assigned to the experimental group and nineteen to the control group. The inpatient pulmonary rehabilitation program was composed of upper and lower extremity exercise, breathing retraining, inspiratory muscle training, education, relaxation and telephone contacts. This program consisted of 4 sessions with inpatients and 4 weeks at home after discharge. The control group was given a home based pulmonary rehabilitation program at the time of discharge. The outcomes were measured by the Borg score, 6MWD and the Chronic Respiratory Disease Questionnaire(CRQ). Results: There was a statistically significant difference in dyspnea between the experimental group and control group, but not among time sequence, or interaction between groups and time sequence. Also significant improvements in exercise capacity and health related quality of life were found only in the experimental group. Conclusions: An Inpatient pulmonary rehabilitation program may be a useful intervention to reduce dyspnea, and increase exercise capacity and health related quality of life for chronic lung disease patients.

Use of real-time ultrasound imaging for biofeedback of diaphragm motion during normal breathing in healthy subjects

  • Objective: To determine if the provision of visual biofeedback using real-time rehabilitative ultrasound imaging (RUSI) enhances the acquisition and retention of diaphragm muscle recruitment during exercise. Design: Two group pretest posttest design. Methods: Thirty healthy subjects were randomly assigned to the verbal feedback group (VG, n=15) or the visual and verbal feedback group (VVG, n=15). The VG performed breathing exercises 10 times with verbal feedback, and the VVG also performed breathing exercises 10 times with verbal feedback and visual feedback with the use of RUSI to measure changes in diaphragm thickness (DT). For DT, the mid-axillary lines between ribs 8 and 9 on both sides were measured in standing, and then the chest wall was perpendicularly illuminated using a linear transducer with the patients in supine to observe the region between rib 8 and 9 and to obtain 2-dimensional images. DT was measured as the distance between the two parallel lines that appeared bright in the middle of the pleura and the peritoneum. After one week, three repetitions (follow-up session) were performed to confirm retention effects. Intra- and between- group percent changes in diaphragm muscle thickness were assessed. Results: In the VVG, the intervention value had a medium effect size compared to the baseline value, but the follow-up value decreased to a small effect size. In the between-group comparisons, during the intervention session, the VVG showed no significant effect on percent change of DT but had a medium effect size compared to the VG (p=0.050, Cohen's d=0.764). During the follow-up session, retention effect did not persist (p=0.311, Cohen's d=0.381). Conclusions: RUSI can be used to provide visual biofeedback and improve performance and retention in the ability to activate the diaphragm muscle in healthy subjects. Future research needs to establish a protocol for respiratory intervention to maintain the effect of diaphragmatic breathing training using RUSI with visual feedback.

두부 전방전위 자세가 젊은 성인들의 호흡기능에 미치는 영향 (Effect of Forward Head Posture on Respiratory Function in Young Adults)

  • Purpose: Forward head posture is a typical symptom in people who use computers for long periods of time. Respiration is a complex function involving co-operation of muscular, skeletal, and nervous systems. Abnormal posture can have a negative effect on respiratory function. The purpose of this study was to investigate the relationship between forward head posture and respiratory function in young adults. Methods: Forty-six healthy subjects participated in this study. Craniovertebral angle was measured for assessment of the forward head posture. The respiratory function of all subjects was evaluated by measuring forced vital capacity (FVC), forced expiratory volume at one second (FEV1), forced expiratory volume at one second/forced vital capacity (FEV1/FVC) ratio, and peak expiratory flow (PEF). The baseline of forward head posture was less than 49 degrees. Results: : Significant differences for predicted FVC and FEV1 were observed between the two groups, however, no statistically significant differences in FEV1/FVC ratio and PEF were observed between the two groups. Conclusion: Results of this study demonstrate that forward head posture has a negative effect on respiratory function in young adults.

  • Purpose: The purpose of the study was to assess the effects of proprioceptive neuromuscular facilitation (PNF) training and respiratory muscle endurance training on pulmonary function and activity in chronic stroke patients. Methods: The participants were 25 chronic stroke patients. They were assigned to two groups: a PNF and respiratory muscle training group (experimental group; n = 12) and a conservative training group (control group; n = 13). The experimental group completed 50 minutes (30 minutes of conventional physical therapy, 10 minutes of PNF training, and 10 minutes of respiratory muscle endurance training). The control group also completed 50 minutes (30 minutes of conventional physical therapy and 20 minutes on a full-body workout machine). Pulmonary function and activity were measured before and after the intervention, using Cosmed to analyze pulmonary function and 6MWT as clinical evaluation indicators. Results: Both groups showed significant within-group differences on all tests before and after the intervention; the experimental group showed greater improvement on all tests. Conclusion: The findings confirm that PNF training and respiratory muscle endurance training have a positive effect on pulmonary function and activity index in chronic stroke patients.

호흡운동 프로그램과 수중운동 프로그램이 아급성기 뇌졸중 환자의 폐기능에 미치는 영향 비교 (Comparison between the Effects of Combined Conventional Exercise Therapy Plus Respiratory Exercise Program with Combined Conventional Exercise Therapy Plus Aqua Exercise Program on the Pulmonary Function of Subacute Stroke Patients )

  • PURPOSE: This study examined the effects of combined conventional exercise therapy plus respiratory exercise program with combined conventional exercise therapy plus aqua exercise program in the pulmonary function of subacute stroke patients. METHODS: The respiratory exercise program group underwent inspiration and expiration training using the Threshold IMT and Threshold PEP three days per week for four weeks. The aquatic exercise program group had aquatic aerobics, halliwick, and bad ragaz ring training three days per week for four weeks. Before and after the experiment, Pony fx was used to examine the FVC, FEV1, FEV1/FVC, VC, and MVV. RESULTS: Significant improvement was found after the experiment in the FVC. FEV1, VC, MVV, excepting FEV1/FVC, of the pulmonary function in the aquatic exercise program group. After the experiment, significant improvement was found in FVC, FEV1, FEV1/FVC, VC, and MVV of pulmonary function in the respiratory rehabilitation therapy group. No significant difference in FVC, FEV1, FEV1/FVC, VC, and MVV of pulmonary function was observed in the inter-group comparison. CONCLUSION: Significant improvement was found after the experiment in both the aquatic exercise program group and the respiratory exercise program group. No difference in pulmonary function was noted in the inter-group comparison. Therefore, combining general exercise therapy and an aquatic or respiratory exercise program is expected to be effective for the pulmonary function of acute stroke patients. These results are expected to provide basic data to help research intervention of aquatic and respiratory exercise programs for subacute stroke patients.
