  • PURPOSE: This study was conducted to investigate the effect of the resistance respiratory muscle exercise with transcutaneous electrical nerve stimulation (TENS) on the respiratory muscle tone and pulmonary function of stroke patients. METHODS: Twenty stroke patients were divided into the TENS group (n = 7), placebo TENS group (n = 7), and control group (n = 6), and each intervention was performed on the three groups 5 times a week for 4 weeks. The assessment was carried out by measuring changes in the muscle tone of the latissimus dorsi and abdominal external obliques, and pulmonary function. RESULTS: In this study, the TENS group and the placebo TENS group had significant increases in the paretic side latissimus dorsi muscle tone, forced vital capacity (FVC), forced expiratory volume in 1 second (FEV1), and peak expiratory flow (PEF). There was no significant difference in changes in respiratory muscle tone between the three groups. The pulmonary function was significantly different between the three groups, and it was observed from the results of the post-hoc test that FVC showed a significant increase in the TENS group and the placebo TENS group compared to the control group. CONCLUSION: Through this study, it was found that the respiratory muscle resistance exercise was more effective as a method to increase respiratory muscle tone and pulmonary function in stroke patients than combined transcutaneous electrical nerve stimulation.

Chronic Respiratory Symptoms and Associated Factors among Fruit and Vegetable Workers in Addis Ababa, Ethiopia: A Comparative Cross sectional Study

  • Background: Fruit and vegetable market is an abundant source of bioaerosols. Exposure to organic and inorganic waste and long-term inhalation of bioaerosols during working hours leads to chronic respiratory symptoms. Hence, this study aimed to determine the prevalence of chronic respiratory symptoms and related factors among fruit and vegetable workers compared with the control group in Addis Ababa, Ethiopia. Methods and materials: A comparative cross-sectional study was conducted from 2020 to 2021. Data were entered in EpiData 3.1 and exported to Statistical Package for the Social Sciences (SPSS) version 25. Logistic regressions were computed to depict the data and related factors. The culture method was done to count and compare bacterial and fungal concentrations between fruit and vegetable workers and office workers. Results: In this study, the prevalence of chronic respiratory symptoms (PR = 2.87, 95% confidence interval [CI]: 1.772-4.66) was significantly higher among fruit and vegetable workers (46.7%) than controls (23.4%). Sex (adjusted odds ratio [AOR] = 2.11, 95% CI = 1.12-3.98), educational status (AOR = 1.34, 95% CI = 0.78-2.32), working hours per day (AOR = 3.91, 95% CI = 1.586-9.65), and working department (AOR = 3.20, 95% CI = 0.90-11.40) were associated with chronic respiratory symptoms. Bacterial and fungal concentrations were significantly higher in the air of the vegetable market (276 colony-forming unit) than the air in the workplace of controls (7 colony-forming unit). Conclusion: The fruit and vegetable market workers (greengrocers) had a higher prevalence of chronic respiratory symptoms relative to office workers. Respiratory protective devices should be given to deliver preventive measures.

  • Background : TNF may play an important role(central mediator) in the development of an acute respiratory distress syndrome. Since TNF induced lung injury in the acute respiratory distress syndrome and abnormalities in surfactant function have been described in acute respiratory distress syndrome, the authors investigated the effects of TNF on the regulation of surfactant protein A, B and C mRNA accumulation. Methods : The effects of TNF on gene expression of surfactant protein A, B, and C were analyzed using filter hybridization, 12 and 24 hours after intravenous injection of TNF in rats. Results : 1. The accumulation of SP-A mRNA in the TNF treated group (12 and 24 hours after TNF injection) was significantly decreased by 22.9% and 27.4%, respectively, compared to the control group (P<.025, P<.025). 2. The accumulation of SP-B mRNA in 24 hours after TNF treated group was significantly decreased by 20.5% compared to that of the control group(P<.01). 3. The accumulation of SP-C mRNA in 12 hours after TNF treated group was significantly decreased by 31% the compared to that of the control group(P<.01). Conclusions : These findings indicate the marked inhibitory effects of tumor necrosis factor on surfactant proteins expression in vivo. This finding. in turn, supports the idea of inhibitory effects of tumor necrosis factor on surfactant proteins expression as it relates to pathogenesis of acute respiratory distress syndrome.

고령 편마비 환자에 대한 호흡운동 적용의 효과 (The Efficacy of Respiratory Exercise Programs in the Elderly Persons with Hemiplegia)

  • Objective : Respiratory muscle weakness and decreased chest mobility has been suggested to result from the deconditioning that accompanied activity level in chronic elderly stokes. The benefits of respiratory exercise programmes on exercise capacity and muscle strength in hemiplegia. This study aimed to determine the effects of selective inspiratory and expiratory muscles training and chest mobility exercise on patients with strokes to establish if an improved exercise capacity can be obtained in patients that are not limited in their daily activities. Methods & Intervention : Twelve patients were assigned to the intensive respiratory exercise group participated in a measures design that evaluated the subjects with pre-treatment and post-treatment. Thirteen subjects who were assigned to a control group received training with breathing exercise and resistance exercise of skeletal muscles. The subjects performed spirometry then undertook a 6-week programme of respiratory muscle and chest mobility training. Training for the two groups was carried out 2 times a week for 6 weeks. Measurements and Results : Spirometry(Forced Vital Capacity: FVC and Closed Circuit Spiromety: CCS) and thoracic mobility were measured before and after the 6 weeks. The experimental group improved significantly compared to control group in FVC, $FEV_1$, MVV, IRV and ERV, and upper chest wall expansion(p<0.05). No significant improvement was seen in thoracic mobility or lung function in control group(p>0.05). Conclusion : The major findings in this study were that a intensive 6week exercise programme of resistive breathing and chest mobility in patients with hemiplegia led to an increase in lung capacity. The resistive breathing exercise programme used here resulted in a significant increase in the chest excursion during breathing.

Detection rate and clinical impact of respiratory viruses in children with Kawasaki disease

  • Purpose: The purpose of this prospective case-control study was to survey the detection rate of respiratory viruses in children with Kawasaki disease (KD) by using multiplex reverse transcriptase-polymerase chain reaction (RT-PCR), and to investigate the clinical implications of the prevalence of respiratory viruses during the acute phase of KD. Methods: RT-PCR assays were carried out to screen for the presence of respiratory syncytial virus A and B, adenovirus, rhinovirus, parainfluenza viruses 1 to 4, influenza virus A and B, metapneumovirus, bocavirus, coronavirus OC43/229E and NL63, and enterovirus in nasopharyngeal secretions of 55 KD patients and 78 control subjects. Results: Virus detection rates in KD patients and control subjects were 32.7% and 30.8%, respectively (P=0.811). However, there was no significant association between the presence of any of the 15 viruses and the incidence of KD. Comparisons between the 18 patients with positive RT-PCR results and the other 37 KD patients revealed no significant differences in terms of clinical findings (including the prevalence of incomplete presentation of the disease) and coronary artery diameter. Conclusion: A positive RT-PCR for currently epidemic respiratory viruses should not be used as an evidence against the diagnosis of KD. These viruses were not associated with the incomplete presentation of KD and coronary artery dilatation.

만성 뇌졸중 환자에게 PNF 내려치기와 들어올리기 패턴을 이용한 운동이 호흡기능에 미치는 영향 (Effects of Exercise using PNF Chopping and Lifting Pattern on the Respiratory Function of Chronic Stroke Patients)

  • PURPOSE: This study examined the changes in the respiratory function when PNF chopping and lifting patterns were used in chronic stroke patients METHOD: The subjects were 30 patients diagnosed with chronic stroke. The respiratory function (FVC, FEV1, and FEV1/FVC) were measured by spirometry. Thirty subjects were divided randomly into an experimental group to which 15 PNF chopping and lifting were applied and a control group to which chest breathing exercise was performed. The intervention was conducted three times a week for six 6 weeks. To examine the effects of intervention, the pre- and post-intervention values for each group were compared using a paired t-test. An independent t-test was used to compare the differences in the values of changes pre- and post-intervention in the two groups. Statistical significance was set to .05. RESULTS: Satistically significant differences in FVC and FEV1 were observed in both the experimental group and control group according to the intervention (p < .05). A statistically significant difference was found in FVC and FEV1 compared to values of changes pre- and post-intervnetion between the experimental and control groups. There was no difference in FEV1/FVC. CONCLUSION: The results suggest that PNF chopping and lifting can be applied as an excellent respiratory intervention program compared to general chest breathing exercises to improve respiratory function in stroke patients.

Effects of Respiratory Muscle Training on the Respiratory Function, Balance, and Activities of Daily Living in Patients with Stroke

  • Purpose: This study evaluated the effects of respiratory muscle training on respiratory function, balance, and activities of daily living (ADL) in patients with stroke. Methods: The study included 21 patients with stroke, divided into the experimental group and control group. Both groups underwent traditional physical therapy once a day for 30 minutes, five times weekly for 6 weeks. The experimental group underwent additional respiratory muscle training once a day, five times a week during the study period. Maximal inspiratory pressure (MIP), maximal expiratory pressure (MEP), Berg balance scale (BBS), and Korean-modified Barthel index (K-MBI) were measured before and after the study period. For statistical analysis, paired t-test was used to compare the difference between the pre and post values. Independent t-test was used compare the differences between groups. Results: Both groups had significantly improved MIP, MEP, BBS, and K-MBI scores after the study period. The experimental group had significantly improved MIP, MEP, BBS, and K-MBI scores. Conclusion: These results suggest that respiratory muscle training improves respiratory function, balance, and ADL in patients with stroke.

호흡재활훈련이 경수손상환자의 호흡기능에 미치는 효과 (The Effects of Respiratory Rehabilitation Training on Respiratory Functions of Cervical Spinal Cord Injury Patients)

  • Purpose: This study was to investigate the effects of respiratory rehabilitation training on the respiratory functions of hospitalized cervical spinal cord injury patients. Methods: One group pre and post test design was used. Subjects were 20 cervical spinal cord injury inpatients of the national rehabilitation center. Training program consisted of air cumulation training, manual assisted coughing training, and abdominal breathing. Trained rehabilitation nurse implemented 20 minutes program twice a day for 4 weeks. Respiratory function was measured as peak coughing flow rate, and perceived respiratory difficulty after activity on wheel chair for 30 minutes and during speaking and singing. Perceived respiratory difficulty was measured with modified Borg scale. Also content analysis was done with the result of open ended question about subjective feeling about training. All variables were measured 3 times before, 2weeks and 4 weeks after the program. Results: Peak coughing flow rate significantly improved after compared to before training. Also all three perceived respiratory difficulty variables decreased significantly after training. In the content analysis, 'it's easier to cough up phlegm' was the most frequent answered subjective feeling. 'Sound at speaking and coughing became louder', 'respiratory volume increased', and 'comfortable chest feeling' were frequent answered subjective feeling, in order. Conclusion: Although it is preliminary since no control group, respiratory rehabilitation training was found to be effective to improve respiratory function in terms of peak coughing flow rate, perceived respiratory difficulty, and subjective feeling. It is necessary further systemic research to investigate the effects of respiratory rehabilitation training.

  • Swine respiratory diseases have induced severe economic losses in swine industry worldwide. Several methods have been developed and applied to prevent and control the disease. However, those are still problematic in swine industry. Recently, the use of egg yolk antibodies with several advantages was introduced and applied to control diseases in animal as well as human. As the first step of the use of egg yolk antibodies in the control of the swine respiratory diseases, we investigated the immunogens of the causative agensts of the diseases and immune response in egg yolk of hens immunized with them. Bacterial antigens prepared from Bordetella bronchiseptica, Pasteurella multocida 3A and 4D, and Actinobacillus pleuropneumaniae serotype 2 and 5 were analyzed by sodium dodecyl sulfate-polyacrylamide gel electrophoresis, Western blot and toxicity test in mice. The antigens were injected into laying hens in order to produce antibodies against them in egg yolk. After chickens were immunized three times in 2 weeks interval, the profile of antibody production was examined by ELISA. The production of antibody in egg yolk was started in 2 weeks after the first injection, reached peak in 6-8 weeks and maintained until 12 weeks. Of two adjuvants used in this study, ISA70 was more effective than aluminum hydroxide gel in enhancing immunogenecity, laying rates and safety in hens. These results suggested that egg yolk antibodies could be a good source for production of antibodies specific to pathogenic bacteria inducing respiratory diseases of swine.
