급성산-염기 균형장해때의 국소 혈류량 변화 (Local blood flow in acute respiratory and metabolic acid-base distrubances in dog)

  • The influences of acute respiratory and metabolic acid-base disturbances on the carotid, renal and coronary blood flow were measured in dogs. Respiratory acidosis was induced by artificial respiration with 8% CO2 -02 gas mixture and respiratory alkalosis was induced by hyperventilation under the control of respirator. Metabolic acidosis and metabolic alkalosis were induced by intravenous infusion of 0.3N hydrochloric acid and 0.6M sodium bicarbonate solution. To observe the effect of hyperkalemia, isotonic potassium chloride solution was infused. CVI electromagnetic flowmeter probes were placed on the left common carotid artery, left renal artery and left circumflex coronary artery. Each flow was recorded on polygraph. 1. The carotid blood flow showed rapid showed rapid and marked increase in acute respiratory acidosis. Even in the cases when arterial blood pressure was lowered during the state of respiratory acidosis, carotid blood flow increased. By the infusion of hydrochloric acid, carotid blood flow increased slowly and returned to the previous label after discontinuation of the infusion. Carotid blood flow also increased by the infusion of large amount of sodium bicarbonate, but it might be the combined effect of expansion of extracellular fluid and compensatory elevation of carbon dioxide tension. 2.The renal blood flow remained unchanged during the acute acid-base disturbances, suggesting effective autoregulation. Renal blood flow, however, increased very slowly when the infusion of potassium chloride continued for a long period. 3.Although less marked than the carotid blood flow, the coronary blood flow increased in the acute respiratory and metabolic acidosis. In asphyxiated condition, coronary blood flow increased most markedly and this might be the combined effect of hypoxia, hypercapnea, and lowering of pH. In summary, the carotid blowflow showed more marked change in the acute respiratory and metabolic acidosis than the renal and coronary blood flow. Respiratory and metabolic components of acid-base disturbances may influence the local blood flow concomitantly, there being more differences in the individual responses, but respiratory component manifested more rapid and marked effect than metabolic component.

Resuscitation from a pH of 6.5: A Case Report and Review of Pathophysiology and Management of Extreme Acidosis from Hypovolemic Shock after Trauma

  • Extreme acidosis is a life-threatening physiological state that causes disturbances in the cardiovascular, pulmonary, immune, and hematological systems. Trauma patients commonly present to the operating room (OR) in hypovolemic shock, leading to tissue hypoperfusion and the development of acute metabolic acidosis with or without a respiratory component. It is often believed that trauma patients presenting to the OR in severe metabolic acidosis (pH <7.0) will have a nearly universal mortality rate despite aggressive resuscitation and damage control. The current literature does not include reports of successful resuscitations from a lower pH, which may lead providers to assume that a good outcome is not possible. However, here we describe a case of successful resuscitation from an initial pH of 6.5 with survival to discharge home 95 days after admission with almost full recovery. We describe the effects of acute acidosis on the respiratory and cardiovascular systems and hemostasis. Finally, we discuss the pillars of management in patients with extreme acute acidosis due to hemorrhage: transfusion, treatment of hyperkalemia, and consideration of buffering acidosis with bicarbonate and hyperventilation.

대사성 산증, 기도저항 변화 및 미주신경 절단이 구호흡 발생에 미치는 영향 (THE INFLUENCE OF METABOLIC ACIDOSIS, AIRWAY RESISTANCE AND VAGOTOMY ON THE DEVELOPMENT OF MOUTH BREATHING)

  • Respiration is one of the most important functions which are carried out in stomatognathic system. When nasal orifice is obstructed or the resistance of upper airway is increased mouth breathing is initiated. Mouth breathing is regarded as an important etiologic factor of dentofacial anomalies. This experiment was performed to observe the influences of metabolic acidosis, tracheal resistance and vagotomy on mouth breathing. After rabbits were anesthetized with sodium pentobarbital, a pair of wire electrode was inserted into mylohyoid muscle, anterior belly of digastric muscle and dilator naris muscle to record EMG activity. Femoral vein and artery were cannulated for infusion of 0.3N HCl and collection of blood sample to determine the blood pH, and tracheal intubation was done to control airway resistance. Mouth breathing was induced by metabolic acidosis. Increase of the airway resistance through tracheal cannula intensified the activity of dilator naris, mylohyoid and digastric muscle. The higher the resistance, the larger the EMG amplitude. After bilateral vagotomy, respiratory volume and inspiatory time were increased and the activities of dilator naris, mylohyoid and digastric muscle were strengthened. It was concluded that the muscle activity related to mouth breathing was induced by metabolic acidosis and increase of tracheal tube resistance.

수혈 관련 급성 폐손상이 동반된 외상환자에서 체외막 산화기의 적용 경험 (Application of Extracorporeal Membranous Oxygenation in Trauma Patient with Possible Transfusion Related Acute Lung Injury (TRALI))

  • The case of a patient with a transfusion-related acute lung injury (TRALI) to whom extracorporeal membrane oxygenation (ECMO) had been applied is reported. A 55-year-old male injured with liver laceration (grade 3) without chest injury after car accident. He received lots of blood transfusion and underwent damage control abdominal surgery. In the immediate postoperative period, he suffered from severe hypoxia and respiratory acidosis despite of vigorous management such as 100% oxygen with mechanical ventilation, high PEEP and muscle relaxant. Finally, ECMO was applied to the patients as a last resort. Aggressive treatment with ECMO improved the oxygenation and reduced the acidosis. Unfortunately, the patient died of liver failure and infection. TRALI is a part of acute respiratory distress syndrome (ARDS). The use of ECMO for TRALI induced severe hypoxemia might be a useful option for providing time to allow the injured lung to recover.

Incidental finding of hemolymph nodes in a Holstein cow (Bos taurus taurus) with coccidiosis

  • This case report is about hemolymph nodes found in a dairy cow whose function is still not fully elucidated. A 4-month Holstein cow presented severe respiratory symptoms and hematochezia for a while with respiratory acidosis and metabolic alkalosis. Coccidiosis was diagnosed and treated immediately, but the cow died from respiratory acidosis and metabolic alkalosis. At necropsy, no abnormal appearance in thoracic and peritoneal organs was observed, but hemolymph nodes were observed being multifocally stuck on omasum serosa and the subcutaneous fascia of abdominal region, and the larger dark red lymph nodes were found along the omasum great curvature. Microscopically, lymphoid depletion and lymphadenitis in the lymph nodes were examined to point systemic infection, and in the hemolymph node, multifocally demarcated pale lesions with macrophage infiltration and fibrin deposition nearby subcapsular sinus. In subcapsular sinus of the hemolymph node, rod to linear gram-negative bacteria were found. Through this study, we might conclude that the hemolymph node is involved in pathogen phagocytosis.

흰쥐 급성 대사성 알칼리증과 호흡성 알칼리증 모델에서 혈액 전해질 및 대사산물 (Blood Electrolytes and Metabolites in Rat Model of Acute Metabolic and Respiratory Alkalosis)

  • 산염기 불균형과 수반되는 치료과정에서 혈액 이온들은 변동되어 질 수 있다. 생리적 그리고 생물학적 활성을 가진 순환 이온화 $Mg^{2+}$ 측정이 수의 임상분야에도 적용되고 있다. 실험동물모델에서 급성 대사성 알칼리증 및 호흡성 알칼리증에 수반하는 혈액 이온화 $Mg^{2+}$ 변동을 관찰하였다. 대사성 알칼리증은 $NaHCO_3$ 정맥투여로 그리고 호흡성 알칼리증은 과호흡에 의해 유도하였다. 혈액 이온화 $Mg^{2+}$$NaHCO_3$에 의해 유도된 대사성 알칼리증에서는 가역적인 감소를 보인 반면, 과호흡에 의해 유도된 호흡성 알칼리증에서는 비가역적인 증가를 보였다. 따라서 산염기 불균형 치료에 있어 혈액 이온화 $Mg^{2+}$의 잠재적 변동 가능성을 고려하여야 한다고 판단된다.

  • 미토콘드리아 질환은 단일 장기에서부터 여러 장기에 걸쳐 침범할 수 있다는 임상 증상의 광범위한 이질성이 특징이다. 안검하수, 색소 망막 퇴화, 외안근 마미, 시신경 위축 등과 같은 다양한 안구 증상이 미토콘드리아 질환에서 함께 나타날 수 있지만, 진행성 양안 백내장은 미토콘드리아 질환의 안과적 증상에서 매우 드물다. 저자들은 미토콘드리아 호흡 연쇄 복합체 결핍 환자에서 흔치 않은 안구 발현 현상인 진행성 양안 백내장 침범 사례를 경험하였기에 보고하는 바이다.

만성폐쇄성폐질환 환자의 고탄산혈증성 호흡부전 : 치료 반응 및 기도삽관의 결정인자 (COPD Patients with Hypercapnic Respiratory Failure : Response to Therapy and Determinant of Intubation)

  • 연구배경 : 만성폐쇄성폐질환 환자에서 고탄산혈증성 호흡부전에서 기계호흡 및 약물치료 등에 대한 반응과 치료경과 및 기계호흡을 결정하는 중요한 인자를 알아보고자 하였다. 방 법 : 만성폐쇄성폐질환의 고탄산혈증성 호흡부전으로 본원 호흡기내과에 입원하였던 환자를 내과치료군과 기도삽관군으로 나누어 임상적 특정 및 동맥혈가스소견 등의 검사소견을 비교하고, 호흡부전의 회복시기 및 경과, 기계호흡을 결정하는 인자, 기도삽관 시점, 그리고 기도삽관 기간 등을 비교하였다. 결 과 : 만성폐쇄성폐질환 환자에서 고탄산혈증성 호흡부전의 횟수는 132회였고, 기도삽관을 하였던 경우는 49회(37%)였고 내과적 치료에 반응을 보인 경우는 83회(63%)였다. 기도삽관군은 내과치료군에 비해 백혈구수치, BUN 및 APACHE II 점수가 높아서 질환의 중증도가 심하였고, 호흡성산혈증 및 고탄산혈증이 심하였다. pH가 7.2이하일때 기도삽관율은 87%로 매우 높았고, 기도삽관하기까지의 시간도 $2{\pm}3$시간으로 매우 짧았으며, pH 7.31-7.35일때 기도삽관율은 20%로 낮았고 기도삽관하기까지의 기간은 $97{\pm}121$시간으로 가장 길었다. 내과치료군에서 83%가 3일이내에 고탄산혈증성 호흡부전에서 회복되었다. 기계호흡군은 내원하여 24시간내에 75%에서 기도삽관이 시행되었다. 기도삽관을 결정하는 중요한 인자중 호흡성산혈증이 중요하며, 기도삽관전의 pH 평균의 범위는 7.04에서 7.24로서 pH 7.24를 기점으로 기도삽관이 결정되는 것으로 생각된다. 결 론 : 만성폐쇄성폐질환 환자의 고탄산혈증성 호흡부전에서 병의 중증도가 심하고, 호흡성 산혈증의 정도 및 고탄산혈증의 정도가 심할 때 기계호흡이 필요함을 알 수 있었고 그중 pH 7.24 이하의 산혈증이 특히 기도삽관을 결정하는 중요한 시점이라 생각되었다. 그리고 고탄산혈증성 호흡부전의 치료에 대한 반응 및 회복시기에 대한 정보가 앞으로 치료에 대한 지침이 될 것으로 생각된다.

많은 양을 음독한 급성 amitraz 살충제 중독 1례 (A Case of Amitraz Insecticide Intoxication after Ingestion of Large Amount)

  • Amitraz is used as farm-animal insecticide. Its side effects in humans are related to its pharmacological activity on alpha 2-adrenergic receptors. The case describes a previously healthy 46-year-old woman who intentionally ingested approximately 250mL of liquid amitraz. She presented with vomiting, altered mental status, miosis, dry mouth, hypopnea, metabolic and respiratory acidosis, hypotension, hypothermia, polyuria, metabolic acidosis, elevated serum aminotransferase and abdominal distension. Supportive treatments including mechanical ventilation, hydration, dopamine infusion, bicarbonate infusion and gastric decompression resulted in improvement. By hospital day 3, she recovered with resolution of abdominal distension. It is paramount to recognize amitraz poisoning when a pesticide-intoxicated patient presets with signs and symptoms consistent with organophosphate intoxicated patients but with greater alpha 2-adrenergic related symptoms such as decreased bowel motility and xerostomia.

소라 섭취 후 테트라민 중독에 의한 호흡부전 1례 (Respiratory Failure following Tetramine poisoning after Ingestion of Sea Snail: A Case Report)

  • Tetramine toxicity due to sea snail ingestion is generally mild and has a good prognosis. Tetramine toxicity acts on the acetylcholine receptor, affecting the neuromuscular junction and autonomic nervous system. A 78-year-old female patient visited the emergency room with vomiting and dyspnea after eating sea snails. At the time of admission, the vital signs recorded were 140/80 mmHg-105/min-24/min-36.5℃, and 90% oxygen saturation. Arterial blood test revealed hypercapnia (pCO2 58.2 mmHg) and respiratory acidosis (pH 7.213, HCO3- 22.5 mmol/L), whereas other blood tests showed no specific findings. Due to decreased consciousness and hypoxia, endotracheal intubation and mechanical ventilation were administered to the patient. Successful weaning was accomplished after 12 hrs, and the patient was discharged without any further complications. Although tetramine toxicity rarely results in acute respiratory failure due to paralysis of the respiratory muscle, caution is required whilst treating the patient.
