만성폐쇄성폐질환자의 가정호흡재활 프로그램 효과에 대한 체계적 고찰: 무작위시험설계 (A Systematic Review of Home based Pulmonary Rehabilitation in COPD Patients: Randomized Controlled Trials)

  • Purpose: The purpose of this study was to review systematically the effects of home based pulmonary rehabilitation in chronic obstructive pulmonary disease (COPD) patients using qualitative synthesis. Methods: Studies designed randomized controlled trials were identified to extract data and to assess the quality of studies in 8 domestic and 3 foreign search engines, and hand search reference reviews. Results: A total of 9 studies met the inclusion criteria. Overall, the methodological quality of the studies ranged from average to poor. Home based pulmonary rehabilitation consisted of exercise, education, and psychosocial interventions. Several exercises such as aerobic, strength, respiratory muscle training, and flexibility were used in home based pulmonary rehabilitation for COPD patients. Upper muscle exercise and walking were the most frequently used exercises. The most common topics for education were 'knowledge of the illness' and 'drug management'. Walking distance test (WDT) and quality of life (QoL) were the most frequently measured as the dependent variables to evaluate rehabilitation outcomes, followed by pulmonary function test (PFT) and dyspnea. WDT was the most effective outcome, followed by dyspnea, QoL and PFT. Conclusion: This study informed about the details of scientific and effective home based rehabilitation programs in COPD patients for future researchers and clinicians. These findings can help expand the recognition and accessibility of home based rehabilitation in COPD patients.

Botulinum Toxin Therapy in a Patient with HHH Syndrome with Gait Disturbance: A Case Report

  • Background : Hyperornithinemia-hyperammonemia-homocitrullinuria (HHH) syndrome is a rare, autosomal recessive metabolic disorder which is caused by genetic mutations that disrupt the urea cycle. It is characterized by variable clinical presentation and the age of onset. Patients may present with gait disturbance and progressive paraplegia and muscle tightness in the lower extremities. The use of botulinum toxin in metabolic disease has rarely been discussed. We describe a case of a 14-year-old-boy with HHH syndrome, who presented with a several - month history of gait disturbance and lower extremity weakness. Case presentation : A 14-year old male had a history of recurrent upper respiratory tract infections, occasional vomiting, loss of appetite, and general weakness, all of which started since he was 10 months old. He was diagnosed with HHH syndrome at one year of age. At the age of 14, he was referred for the assessment and treatment of his gait disturbance and aggravated weakness of the lower extremities. Brain MRI, electrodiagnostic study and blood test were performed to exclude any lesions related to neurologic dysfunction. Botulinum toxin type A were injected into muscles of adductor longus, adductor magnus, lateral and medial hamstring, and lateral and medial gastrocnemius muscle heads under needle electromyography guidance to reduce lower limb spasticity. Intensive physical therapy including gait training and stretching exercise of adductor and calf muscles were also provided. After intensive physical therapy and botulinum toxin injection to reduce lower limb spasticity, he was able to ambulate for 20 meters independently without any walking aids. There were no adverse events after the injection. Conclusion : Botulinum toxin injection is a safe and effective therapy for patients with HHH syndrome who suffer from gait disturbance.

젊은 만성허리통증자의 허리부위 불안정성 검사 양성반응 수와 호흡패턴변화율, 통증 간의 상관관계분석 (Correlation Analysis between Lumbar Instability Test Positive Response Number and Breathing Pattern Change Rate and Pain in Young Peoples with Chronic Low Back Pain)

  • PURPOSE: This study examined the correlation between the visual analog scale (VAS) and the rate of change in the respiration patterns according to the result of Lumbar Instability Tests (LITs) in young people with chronic low back pain (CLBP) METHODS: Thirty-six adults, aged 20-40 years with CLBP, participated in this study. The general characteristics and VAS of the participants were recorded by the subjects themselves and seven structure and functional LITs were conducted. According to the positive response number, the positive group was divided into four groups (group 1: n=8, group 2: n=9, group 3: n=10, group 4: n=9). The breathing pattern change tests were performed in three states: during forced breathing exercise and motor control tests. A total of 13 positive lists were set, each of which was scored by 1 or 2 points according to the severity. After the positive lists were scored, the breathing pattern changing rate (BPCR) and VAS were compared according to the positive response number of LITs, and the correlation between them was analyzed. RESULTS: A strong correlation was observed between the number of positive of LITs and BPCR (r= .863, p= .000) and a moderate correlation between the positive number of LITs and VAS (r= .508, p= .002). In addition, there was a poor correlation between the BPCR and VAS (r= .434, p= .008). CONCLUSION: In young CLBP people, when the structural and functional instability are both present, the changes in the respiratory pattern of the whole body can be varied and broader, and the pain scale also increases.

A standardized method to study immune responses using porcine whole blood

  • Background: Peripheral blood mononuclear cells (PBMCs) are commonly used to assess in vitro immune responses. However, PBMC isolation is a time-consuming procedure, introduces technical variability, and requires a relatively large volume of blood. By contrast, whole blood assay (WBA) is faster, cheaper, maintains more physiological conditions, and requires less sample volume, laboratory training, and equipment. Objectives: Herein, this study aimed to develop a porcine WBA for in vitro evaluation of immune responses. Methods: Heparinized whole blood (WB) was diluted (non-diluted, 1/2, 1/8, and 1/16) in RPMI-1640 media, followed by phorbol myristate acetate and ionomycin. After 24 h, cells were stained for interferon (IFN)-γ secreting T-cells followed by flow cytometry, and the supernatant was analyzed for tumor necrosis factor (TNF)-α. In addition, diluted WB was stimulated by lipopolysaccharide (LPS) and polyinosinic:polycytidylic acid (poly I:C), reference strain KCTC3557 (RS), field isolate (FI), of heat-killed (HK) Streptococcus suis, and porcine reproductive and respiratory syndrome virus (PRRSV). Results: The frequency of IFN-γ+CD3+ T-cells and concentration of TNF-α in the supernatant of WB increased with increasing dilution factor and were optimal at 1/8. WB TNF-α and interleukin (IL)-10 cytokine levels increased significantly following stimulation with LPS or poly I:C. Further, FI and RS induced IL-10 production in WB. Additionally, PRRSV strains increased the frequency of IFN-γ+ CD4-CD8+ cells, and IFN-γ was non-significantly induced in the supernatant of re-stimulated samples. Conclusions: We propose that the WBA is a rapid, reliable, and simple method to evaluate immune responses and WB should be diluted to trigger immune cells.

Gynostemma pentaphyllum extract and its active component gypenoside L improve the exercise performance of treadmill-trained mice

  • BACKGROUND/OBJECTIVES: The effectiveness of natural compounds in improving athletic ability has attracted attention in both sports and research. Gynostemma pentaphyllum (Thunb.) leaves are used to make traditional herbal medicines in Asia. The active components of G. pentaphyllum, dammarane saponins, or gypenosides, possess a range of biological activities. On the other hand, the anti-fatigue effects from G. pentaphyllum extract (GPE) and its effective compound, gypenoside L (GL), remain to be determined. MATERIALS/METHODS: This study examined the effects of GPE on fatigue and exercise performance in ICR mice. GPE was administered orally to mice for 6 weeks, with or without treadmill training. The biochemical analysis in serum, glycogen content, mRNA, and protein expressions of the liver and muscle were analyzed. RESULTS: The ExGPE (exercise with 300 mg/kg body weight/day of GPE) mice decreased the fat mass percentage significantly compared to the ExC mice, while the ExGPE showed the greatest lean mass percentage compared to the ExC group. The administration of GPE improved the exercise endurance and capacity in treadmill-trained mice, increased glucose and triglycerides, and decreased the serum creatine kinase and lactate levels after intensive exercise. The muscle glycogen levels were higher in the ExGPE group than the ExC group. GPE increased the level of mitochondrial biogenesis by enhancing the phosphorylation of peroxisome proliferator-activated receptor-γ coactivator-1α (PGC-1α) protein and the mRNA expression of nuclear respiratory factor 1, mitochondrial DNA, peroxisome proliferator-activated receptor-δ, superoxide dismutase 2, and by decreasing the lactate dehydrogenase B level in the soleus muscle (SOL). GPE also improved PGC-1α activation in the SOL significantly through AMPK/p38 phosphorylation. CONCLUSIONS: These results showed that GPE supplementation enhances exercise performance and has anti-fatigue activity. In addition, the underlying molecular mechanism was elucidated. Therefore, GPE is a promising candidate for developing functional foods and enhancing the exercise capacity and anti-fatigue activity.

특성화 고등학교 용접 실습의 유해인자 노출 실태 비교 분석 (Comparative Analysis of Exposure to Hazardous Factors of Welding Lab Activities in Specialized High School)

  • Objectives: This study aims to identify and analyze the exposure status of welding students in specialized high school welding labratories, compare it with the exposure to welding hazards of industrial workers, and seek to improve the educational environment for youth through domestic and international exposure standards. Methods: This study compares the level of exposure to hazardous factors in a welding laboratory of a vocational high school in Jeollanam-do and a welding process in a general industrial site by measuring the work environment. A 10-question survey was conducted to review the effects of welding hazards on the human body, carcinogenicity information, international (US, UK, France) exposure standards, general characteristics between the two groups, and awareness of occupational health. Results: Exposure to hazardous factors in both groups was below the standards set by MOEL. Specialized high school students were exposed to higher levels than workers, and some hazardous factors exceeded the standards when compared to international exposure standards. During the survey, students were less aware of the hazards of welding, safety and health education, and the need for work environment measurement than workers. Conclusions: For the respiratory protection of students in vocational high school welding labs, it is necessary to create a comfortable training environment. Exposure standards for harmful factors should be strictly applied, such as overseas standards, or exposure should be limited by setting a limit on the number of hours of welding practice per week. In addition, it is necessary to conduct safety and health education for welding students to raise their awareness of the importance of measuring the working environment and wearing appropriate protective equipment.

SARS-CoV-2 parental vaccination and risk of multisystem inflammatory syndrome in children: a single-center retrospective study

  • Purpose: Severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) usually causes a mild disease in children and the most serious consequence is multisystem inflammatory syndrome in children (MIS-C). Currently, there are no data about the protective role of vaccination performed by parents on children regarding the development of MIS-C. The aim of our study is to establish whether parental vaccination is related to MIS-C and the protective value of SARS-CoV-2 vaccination performed by parents against the occurrence of MIS-C in their children. Materials and Methods: Our retrospective single center study included 124 patients aged 1 month to 18 years admitted to emergency department from April 2020 to March 2022 for coronavirus disease 2019 disease. Results: Parental vaccination was negatively correlated with the development of MIS-C: 4% of patients with both parents vaccinated developed MIS-C, while patients with no parent vaccinated to have developed MIS-C were 20%. Conclusion: Parental vaccination could be an important factor influencing the course of the disease and reduces the probability that a child would develop MIS-C by 83% if both parents vaccinated.

호흡재활치료 전후 $^{31}P$ 자기공명분석법을 이용한 골격근대사의 변화에 관한 연구 (Assessment of Effect of Pulmonary Rehabilitation on Skeletal Muscle Metabolism by $^{31}P$ Magnetic Resonance Spectroscopy)

  • 연구배경 : 만성폐쇄성폐질환 환자의 치료에 널리 적용되고 호흡재활치료는 폐기능을 호전시키지는 못하지만 호흡곤란 등의 증상과 운동능력을 호전시키는 것으로 알려져 있다. 그러나 이런 운동능력 개선의 기전은 여러 각도에서 해석되고 있다. 저자들은 $^{31}P$ MRS를 이용하여 만성 폐질환 환자들을 호흡재활치료 전후 전박근의 대사 변화를 관찰함으로써, 호흡재활치료 후 운동능력호전에 골격근 대사개선을 기여할 가능성을 조사하였다. 방 법 : 총 9명의 만성 폐질환을 갖고 있는 남자 환지들을 대상으로 하였고 이들의 평균 연령은 $58{\pm}11$세였으며, 이들의 기저 질환은 만성폐쇄성폐질환 8예 및 폐유육종증 1예였다. 호흡재활치료는 근육강화운동, 답차운동(treadmill walking), 자전거 운동(stationary bicycle riding) 및 상지 운동력측정계(arm ergometer)를 이용한 상지운동으로 구성했으며, 호흡재활치료 전후로 폐기능 검사, 운동부하 검사, 상하지의 지구력 측정 및 6분 보행거리 검사를 실시하였다. $^{31}P$ MRS검사를 치료 전후 전박근을 대상으로 안정시, 운동시 및 20분간의 회복시에 시행하여 세포내 산소성 인산화 능력을 반영하는 Pi/PCr의 비와 pHi(intracellular pH)를 구하여 비교하였다. 결 과 : 호흡재활치료 후 환자의 폐기능과 가스 교환의 호전은 없었으나, 운동지구력 및 보행능력은 현저한 호전을 보였으며 최대산소섭취량은 증가하는 경향을 보였고, 동 운동량에서의 분당환기량은 감소하는 경향을 나타내었다. $^{31}P$ MRS를 이용하여 재활치료 전후의 골격근 대사를 비교해 본 결과, 치료 후 운동시 및 극심한 피로상태에서의 pHi는 유의하게 높았고 산소성 인산화 과정을 반영하는 지표인 Pi/PCr는 감소하는 경향을 보였으나 안정시 및 회복기의 골격근 대사과정은 변화가 없었다. 결 론 : 이상으로 만성 폐질환 환자에서 6주간의 호흡재활치료는 운동지구력 및 보행 호전시켰으며 이러한 운동능력 호전에는 골격근 대사의 개선으로 초래된 골격근 세포내의 산성화 지연으로 인한 환기량의 감소가 기여할 사료되었다.

보건계열 대학생의 중동호흡기증후군에 대한 지식, 태도 및 예방행위 실천도와의 관계 (Correlation between the Preventive Behaviors on Middle East Respiratory Syndrome and the Knowledge, Attitude, and Compliance of Medically Inclined College Students)

  • 본 연구는 보건계열 대학생의 메르스에 대한 지식, 태도 및 예방행위 실천도를 파악하고, 메르스 및 호흡기 감염예방과 교육을 위한 기초자료를 제공하고자 하였다. 자료수집은 2015년 6월 8일부터 6월 13일까지 전주시 소재 일개대학교 보건계열 대학생 251명을 대상으로 설문조사하였다. 연구 대상자는 치위생과 46.6%, 임상병리과 30.7%, 응급구조과 22.7%로, 메르스 감염예방 교육은 69.7%가 받았다. 메르스에 대한 지식은 메르스 감염예방 교육을 받은 경우(t=3.457, p=0.001), 여학생이 높았으며(t=-2.945, p=0.005), 메르스에 대한 태도는 치위생과(F=8.048, p<0.001), 3학년 학생(F=3.978, p=0.020)이 높았다. 메르스에 관한 지식, 태도 및 예방행위 실천도, 감염에 대한 두려움간의 상관관계는 지식(r=0.133, p=0.036)이 높고, 태도(r=0.158, p=0.012)가 긍정적이고, 감염에 대한 두려움(r=0.312, p<0.001)이 높을수록 예방행위 실천도가 높았다. 이상의 연구결과를 종합해볼 때 보건계열 대학생이 신종감염병인 메르스 감염을 예방하기 위해서는 메르스에 관한 지식을 향상시키고, 의료기관에서 메르스 감염관리에 대해 긍정적인 태도를 갖도록 하여 메르스 감염 예방행위를 실천할 수 있도록 해야할 것이다. 또한 의료기관 임상실습 전 신종감염병을 포함한 감염병에 대한 감염원인, 전파 기전, 예방법 등의 감염관리 교육 프로그램을 구체적으로 개발하여야 할 것이다.

  • Background: In practicing qigong, People must achieve three Points : adjust their Posture, control their breathing and have a peace of mind. That is, Cho-Sin [調身] , Cho-Sik [調息] , Cho-Sim [調心] . Slow respiration is the important pattern of respiration to improve the human health. However, unsuitable breathing training have been occurred to mental disorder such as insomnia, anorexia etc. So, we think that the breathing training to consider the individual variations are desired. Objectives: We performed this study to examine the physiological effects of controlled respiration on the normal range of frequency domain electroencephalogram(EEC) in healthy subjects Also, to study examine individual variations according to the physiological effects between controlled respiration and Han-Yeol [寒熱] , respiration period, gender and age-related groups on the EEC in healthy subjects. Methods: When the subjects controlled the time of breathing (inspiration and expiration time) consciously, compared with natural respiration, and that their physiological phenomena are measured by EEC. In this research we used breathing time as in a qigong training (The Six-Word Excise) and observed physiological phenomena of the controlled natural respiration period with the ratio of seven to three(longer inspiration) and three to seven(longer expiration) . We determined, heat-cold score by Han-Yeol [寒熱] questionnaire, average of natural respiration period, according to decade, EEC of 140 healthy subjects (14 to 68 years old; 38 males, 102 females) by means of alpha, beta spectral relative power. Results: 1) In Controlled respiration compared with the natural respiration, ${\alpha}\;I\;(Fp2)\;and\;{\beta}$ I (Fpl, Fp2, F3, F4) decreased on the EEC. 2) In controlled respiration compared with the natural respiration, ${\beta}$ I (Fpl, Fp2, F3, F4) increased with cold group, ${\alpha}/{\beta}$(F3) decreased with heat group, ${\alpha}$ I (Fp2)increased with cold group in longer inspiration. But by means of compound effects, ${\alpha}$ II(F3) increased with cold group in longer inspiration, the other side ${\alpha}$ I (F3) decreased with heat group in controlled respiration on the EEC. 3) In controlled respiration compared with the natural respiration, ${\alpha}$ I (Fp2) decreased with decreased-respiratory-rate(D.R.R.) group, ${\beta}$ I (Fpl, Fp2, F3, F4) increased with D.R.R. and D.R.R. groups, ${\alpha}/{\beta}$(F3) decreased with D.R.R. group. But by means of compound effects, in controlled respiration compared with the natural respiration, ${\alpha}/{\beta}$(F3) decreased with D.R.R. group on the EEG. 4) In controlled respiration compared with the natural respiration, ${\beta}$ I (Fpl, F3, F4) increased with female cup, ${\beta}$ I (Fp2) increased with male and female groups, ${\alpha}/{\beta}$(F3) decreased with male group. But by means of compound effects, in controlled respiration compared with the natural respiration, ${\alpha}$ I (Fp2) increased with female group on the EEC. 5) Compared with the natural respiration, in longer expiration ${\alpha}$ I (Fp2) increased in their forties group, in longer inspiration ${\alpha}$ I (Fp2) increased in their fifties group. But by means of compound effects, in controlled respiration compared with the natural respiration, ${\beta}$ I (Fpl) decreased in teens group on the EEG.
