생쥐의 호흡기 감염에 대한 퀴놀론계 항생제 LB20304a의 치료 효과 (In vivo Efficacy of LB20304a against Experimental Respiratory Tract Infection in Mice)

  • The therapeutic activity of LB20304a was examined on experimental respiratory tract infection (RTI) caused by Klebsiella pneumoniae DT-S in mice. A single oral dose of LB 20304a (1.2mg/mouse) showed a rapid bacteriocidal activity in lung tissue at 4,7 and 24h after administration of drug. The in vivo activity of LB20304a was comparable to that of ciprofloxacin against K. pneumoniae infection, although in vitro MIC of LB20304a was four-fold higher than that of ciprofloxacin.

외래 호흡기계 질환에서 항생제 사용에 대한 후향적 평가방안 (Retrospective Drug Utilization Review of Antibiotics for Respiratory Tract Infection(RTI) in Ambulatory Outpatient Care)

  • As respiratory tract infections (RTI) account for about 60% of all antibiotic prescriptions in outpatient care setting, there are significant concerns about emerging resistance that are largely due to the excessive or inappropriate use of antibacterial agents for viral respiratory infections. This study was aimed to develop retrospective drug utilization review (DUR) program of antibiotics for RTIs using Delphi methods. Retrospective DUR criteria of antibiotics for RTIs were identified based on clinical practice guidelines and opinion of experts. Expert panel members were clinical doctors and pharmacists and Delphi method was applied by survey on 16 members of panels. The claim data from Korean Health Insurance Review & Assessment (HIRA) were used to examine trends in outpatient antibiotic prescription between Janunary to December of 2008. As results, Quality index for RTI was assessed for the claim type, antibiotics use of quantity, duration, number and cost. Antibiotic prescription rate for RTIs, Defined Daily Dose (DDD), and duration of antibiotics use were more recognized as significant quality index by experts' opinion. Use of first line agents suggested by guidelines was low and duration of antibiotics use was shorter compared to the recommendations. Antibiotics were over prescribed for RITs. However, dose and duration of antibiotics were under-used.

주사제용 세파로스포린계 항생제 LB10522의 in vitro 및 in vivo 항균력 (In Vitro and in Vivo Antibacterial Activities of a New Parenteral Cephalosporin, LB10522)

  • The in vitro antibacterial activities of LB10522, a new catechol-substituted cephalosporin, were compared with those of cefpirome, ceftazidime, ceftriaxone, and cefoperaz one against clinical isolates and laboratory standard anaerobes. LB10522 had broad spectrum antibacterial activities against both gram-positive and gram-negative microorganisms. It was most active against gram-positve bacteria among the reference cephalosporins tested. Against gram-negative strains such as the family Enterobacteriaceae, LB10522 showed an activity comparable to that of cefpirome. But LB10522 was more potent than ceftazidime, ceftriaxone and cefoperazone. In particular, Pseudomonas aeruginosa was highly susceptible to LB10522, which was 32-fold and 64-fold more active than ceftazidime and cefpirome, respectively. Against anaerobic strains, the activity of LB10522 was similar to those of reference compounds. LB10522 exhibited potent therapeutic activities against experimental local infections in mice. The therapeutic effect of LB10522 against urinary tract infection (UTI) caused by P. aeruginosa 1912E in mice was superior to that of cefpirome. Against experimental respiratory tract infection (RTI) caused by K. pneumoniae DT-S in mice, LB10522 was as effective as cefpirome. The in vivo efficacy of LB10522 was correlated well with its in vitro activity.

비타민 D와 소아 호흡기 감염의 위험성: 무작위 대조 연구에 대한 체계적 문헌고찰 및 메타분석 (Vitamin D and Risk of Respiratory Tract Infections in Children: A Systematic Review and Meta-analysis of Randomized Controlled Trials)

  • 목적: 최근 관찰연구들에서 낮은 혈중 25(OH)D 농도가 호흡기 감염과 관련이 있다는 결과들이 보고되고 있다. 하지만, 소아의 무작위 대조 연구에서 비타민 D의 투여가 호흡기 감염을 예방하는 효과에 대해서는 아직 논란의 여지가 있다. 이번 연구는 비타민 D 보충 요법이 호흡기 감염 예방에 미치는 영향을 체계적 문헌고찰과 메타분석을 통하여 알아보고자 한다. 방법: MEDLINE, EMBASE, the Cochrane Central Register of Controlled Trial을 이용하여 소아의 호흡기 감염 예방을 위해 비타민 D 보충을 실험한 무작위 대조 연구들이 분석 대상에 포함되었다. 문헌의 비뚤림 위험도를 평가하기 위해 코크란 연합의 질 평가 방법(Cochrane Collaboration's tool for assessing the risk of bias)을 사용하였다. 연구별로 상대위험도와 95% 신뢰구간을 추출한 후, Review Manager 5.3을 이용하여 메타분석을 시행하였다. 결과: 총 7편의 무작위 대조 연구들이 메타분석 대상에 포함되었다. 임의효과모형을 사용하여 산출된 전체 비타민 D 보충군에서 상대적 위험도는 0.82 (95% CI: 0.69-0.98)이었고, 이질성에 대한 $I^2=62%$ 이었다. 연구 간의 이질성 원인을 파악하기 위해 시행한 하위 집단 분석에서 추적 관찰 기간이 1년 미만인 경우, 대상군의 나이가 5세 이상인 경우, 대상군이 환자인 경우, 비타민 D 투여 요법이 매일 요법인 경우 이질성이 감소하였다. 깔때기 그림에서 출판 비뚤림의 가능성을 시사하는 비대칭 소견을 보였다. 결론: 메타분석 결과는 소아에서 비타민 D 보충요법이 호흡기 감염을 예방하는 데 효과가 있을 수 있음을 보여준다. 하지만, 이번 연구에서는 분석대상에 포함된 연구들이 적고, 연구들 간에 이질성이 존재하며, 출판 비뚤림이 존재할 가능성이 있어 결과를 주의해서 해석할 필요가 있다.
